[Brain metastasis from primary pericardial mesothelioma. Case report].

A 50-year-old female was admitted because of nausea, vomiting, and cerebellar ataxia. Computed tomography scan revealed an enhanced mass accompanied with a cyst in the right cerebellar hemisphere. The mass situated in the subcortical region was removed. Histologically, highly vascular tumor cells lined the cavities. Postoperative radio- and chemotherapy were administered and the clinical symptoms improved gradually. Two months later, the patient complained of dyspnea. Chest X-ray on second admission demonstrated cardiomegaly. Hemorrhagic pericardial effusion amounting to 1000 ml was aspirated by pericardial puncture. Papillary clusters of tumor cells were demonstrated in the pericardial effusion. The patient died of cardiac failure. At necropsy solid tumors were located in the heart, lung, left inguinal region, and cerebellum. Histological diagnosis was mesothelioma arising from the heart. Primary pericardial mesotheliomas are rare; approximately 106 cases have been reported. Pericardial mesothelioma frequently spreads to the adjacent pleura and mediastinum, but distant metastases are extremely rare because patients with pericardial mesothelioma tend to die early due to cardiac failure or cardiac tamponade.

Effects of myringotomy and orally administered drugs on viscosity and elasticity of middle ear effusions from children with otitis media with effusion.

Clinical and rheological studies were performed on 42 children with otitis media with effusion. Twenty-one patients received S-carboxymethylcysteine (group A), and the other 21 patients were administered Kampo medicine (group B), a Japanese traditional herbal medicine, for 4 weeks following myringotomy. At the end of the 4-week treatment period, 47.2% of the ears in group A and 40.5% in group B were regarded as being free from middle ear effusion (MEE). For rheologically thick MEEs, where the viscosity (eta') and elasticity (G') are much greater than the optimal values for mucociliary transport, post-treatment values of eta' for group A decreased significantly, but those of G' did not, in comparison with pre-treatment values. No such changes in either eta' or G' were observed in group B. Results suggests that the combined therapy for myringotomy and oral SCMC is effective for improving the rheological properties of thick MEE in children.

Failure to visualize bilateral adrenal glands in a patient with primary aldosteronism.

Failure to visualize either adrenal gland by adrenal imaging was experienced in a patient with biochemical evidence of primary aldosteronism. Visualization of the adrenal glands was not possible after the administration of ACTH-Z (1 mg/day) for 3 days. The administration of ACTH and dexamethasone elicited an increase in blood pressure. Although the reason for this failure to visualize the adrenal glands by scanning and for the rise in blood pressure brought on by ACTH and dexamethasone remain unclear, we think that the case herein reported is a rare type of primary aldosteronism.

[Bioelectric properties of excised rabbit nasal mucosa].

The water flow across the respiratory epithelia is an important determinant of the efficiency of mucociliary clearance. Bulk water flow has been shown to be coupled to net ion flux. We studied ion transport across rabbit nasal mucosa by measuring bioelectric properties using Ussing chambers. Results were summarized as follows. (1) Compared with tracheal mucosa, nasal mucosa exhibited lower potential difference (p less than 0.01), lower short-circuit current (p less than 0.05), and higher conductance (p less than 0.01). (2) Ouabain 10(-4)M inhibited short-circuit current when added to the submucosal bath of Ussing chambers, and amiloride decreased short-circuit current to about 40% when added to the mucosal bath. (3) When the bubbling of the solution was changed from 95%O2, 5%CO2 to 100%N2, short-circuit current remarkably decreased. (4) A significant positive correlation existed between temperature ranging from 33 degrees C to 41 degrees C and short-circuit current (r = 0.46, p less than 0.02).

Detection of hepatitis C virus cDNA sequence by the polymerase chain reaction in hepatocellular carcinoma tissues.

We found the presence of hepatitis C virus (HCV) infection in liver tissues of hepatocellular carcinoma (HCC) patients who had antibodies to HCV but no serological markers for hepatitis B virus infection by the sensitive reverse transcription/polymerase chain reaction (R/PCR) method. The primers used were derived from the non-structural (NS) 3 and/or the structural (C/E) region. Amplified cDNA sequences of HCV were detected in either cancerous or non-cancerous portion of liver tissues from four out of eight HCC patients with primers of NS3 region. Similar but less efficient results were obtained with primers of C/E region. These results indicate that HCV persists in the liver tissue of HCC. A possible role of persistent infection of HCV for the development of HCC is discussed.

[Characterization of immunoreactive hCG beta-subunit in cultured fluids of the cell lines derived from gynecologic malignant tumors].

We characterize the hCG beta-like materials in cultured fluids from 12 different cell lines derived from gynecologic malignant tumors (SKG-1, -2, -3a and -3b cervical squamous carcinoma, SNG-M and -2 endometrial adenocarcinoma, SKN uterine sarcoma, RTSG ovarian undifferentiated adenocarcinoma, RMUG ovarian mucinous adenocarcinoma, RKN ovarian sarcoma, RMG ovarian clear cell carcinoma and NJG gestational choriocarcinoma) by three kinds of enzyme immunoassay (EIA) which were specific for whole hCG, free hCG beta and beta-core fragment, respectively. Of eleven nontrophoblastic cell lines, nine secreted hCG beta-like immunoreactive substance. The above-mentioned three EIAs for each fractionated specimens with gel chromatography on Sephadex G-100 revealed that the immunoreactivity in the SKG-2 and RTSG cultured fluids were totally attributable to free hCG beta but neither to whole hCG nor beta-core fragment. On the other hand, the NJG choriocarcinoma cell line secreted both whole hCG and free hCG beta, no beta-core fragment could be detected in the cultured fluid. The present results suggested that ectopically produced hCG beta-like material may represent the free hCG beta molecule, and that the beta-core fragment may not be a cellular secretory product.

[Microbiological and chemical analyses of indoor swimming pools and virucidal effect of chlorine in these waters].

Virological and bacteriological examinations and chemical analysis were made of 24 water samples from 6 public indoor swimming pools in Aichi Prefecture. In 3 of these swimming pools, sodium hypochlorite was used as a disinfectant and in the remainder trichloroisocyanurates. All samples were negative for adenovirus per 20 liters. Coliform group was detected in one sample with 26.7 mg/l cyanuric acid. Total plate counts ranged from 0 to 1 per ml in the swimming pools treated with sodium hypochlorite and 0 to 51 in those with trichloroisocyanurates. Turbidity, pH and potassium permanganate concentrations in the samples were within allowable limits. Seven of 24 samples did not meet the recommended value of 0.4 mg/l for free chlorine residuals. Ammonium nitrogen was detected in one pool water with trichloroisocyanurates. In 11 of 12 water samples of swimming pools using sodium hypochlorite, poliovirus 1 (2 x 10(4) PFU/0.2 ml) was inactivated within 1 min under the condition of 1.0 mg/l free available chlorine and 25 degrees C. In 11 of 12 water samples of 3 swimming pools using trichloroisocyanurates, poliovirus type 1 survived after 2 min contact while in 5 samples poliovirus type 1 survived after 5 min contact. This shows that the risk of viral infection is greater in swimming pool water treated with chlorinated isocyanurics than that with sodium hypochlorite. The operator of the swimming pool should pay special attention to the control of water quality and free residual chlorine value.

Characterization of alkaline response induced by cholinergic agents in the rat duodenum: involvement of M2 receptors and the calcium-dependent process.

Duodenal pH, potential difference and acid-neutralizing capacity (HCO3- output) were measured in anesthetized rats, in an attempt to characterize these responses induced by cholinergic agents. When the proximal duodenum (1.7 cm) was perfused at a flow rate of 1 ml/min with saline adjusted to pH 4.5, the pH, potential difference and HCO3- output were 5.2 to 5.5, -3 to -5 mV (mucosa negative) and 1.5 to 1.8 muEq/10 min, respectively. Both carbachol (4 micrograms/kg, i.v.) and bethanechol (100 micrograms/kg, i.v.) significantly elevated all these parameters at the doses that stimulated gastric acid secretion; the net HCO3- output caused by these agents was about 35% of that produced by prostaglandin E2 (300 micrograms/kg, i.v.). These responses induced by carbachol were significantly inhibited by pretreatment with verapamil (0.2 mg/kg, i.v.), a Ca channel blocker, and atropine (0.1 mg/kg, i.v.), a nonselective M1 and M2 antagonist, while they remained unaltered in the presence of pirenzepine (1 mg/kg, i.v.), a selective M1 antagonist, or indomethacin (5 mg/kg, s.c.). However, the effects of prostaglandin E2 on duodenal pH, potential difference and HCO3- output were not significantly affected by any of these agents. These results suggest that the cholinergic agents stimulate HCO3- output in the duodenum, probably mediated by M2 receptors and through the Ca-dependent and electrogenic processes. Endogenous prostaglandins may not be involved in this mechanism.

[Analysis of peripheral lymphocyte subsets and changes due to erythromycin therapy in patients with diffuse panbronchiolitis].

It is well known that patients with diffuse panbronchiolitis (DPB) have various immunological abnormalities. The authors evaluated the peripheral lymphocyte subsets using two-color analysis by FACS. Convening percentages of activated CD3+, CD8+, and CD4+ cells in DPB patients, all, especially that of CD8+ cell, were greater in number before therapy compared to healthy controls. After erythromycin therapy, all three parameters decreased in all patients. Therapy with other antibiotics did not obtain any effects. These results suggested that patients with DPB had some immunological hyperreactivity and that erythromycin was effective in these patients by suppressing this activity. From these results, together with the fact that DPB is a type of sino-bronchial syndrome, the possible pathogenesis of DPB was thought to be related to some immunological defect and hyperreactivity in the respiratory system.

[Cytochemical study of Mg2(+)-ATPase and ALPase activity in human meningiomas].

Ultracytochemical features of microvessels and tumor cells of the human meningiomas were examined by light and electron microscopy with special reference to the distribution of Mg2(+)-ATPase and alkaline phosphatase (ALPase) activity on the walls of the vessels and tumor cell surfaces. Materials used were 4 cases of meningiomas, 2 of which were meningotheliomatous type, one fibroblastic type and one malignant meningioma respectively. For ultracytochemistry, specimens were quickly fixed in an ice-cold 0.1 M cacodylate buffer containing 8% sucrose (pH 7.2) for one hour and transferred to a substrate solution for detection of Mg2(+)-ATPase and ALPase. The preparations were incubated at 37 degrees C for 15-30 min in the medium described by Mayahara et al. for ALPase and for 15-20 min in the medium described by Wachstein and Meisel. The control samples were incubated in a medium containing 1 mM Bromotetramisole for ALPase and also in a substrate free medium for Mg2(+)-ATPase. At the light microscopy, Mg2(+)-ATPase and ALPase activities appeared to be mainly restricted to the capillary wall and around or in the tumor cell nest showing whorl formation. Both enzyme activities were negative in the control study. By electron microscopy, reaction products representing Mg2(+)-ATPase activity were distributed in the basolateral plasma membrane of the endothelial cells on the surface of the pericytes and on the surface of the tumor cells. Reaction products of ALPase activity located mainly on the abluminal surfaces of the endothelial cells and in some specimen on both luminal and abluminal surfaces of those cells. Intense reaction products were distributed evenly on all round surfaces of the tumor cells.(ABSTRACT TRUNCATED AT 250 WORDS)

[Skullbase-penetrating injuries caused by umbrella tips: case reports].

Two cases of skullbase-penetrating injuries caused by umbrella tips are reported. Case 1: 24-year-old male. Admitted with disturbance of consciousness, left hemiparesis, nasal bleeding, and laceration of left lower eyelid because of having been stabbed by an umbrella tip. Pupils and fundi revealed no definite findings. Plain skull X-ray showed turbid ethmoid sinus and fracture of planum sphenoidale. Cranial CT showed right putaminal hematoma with intraventricular hemorrhage and pneumocephalus. Increased ICP necessitated surgery two days after the injury. Dural laceration of planum sphnoidale, laceration of left optic nerve, right rectal gyrus contusion and rebleeding from the right lenticulostriate branch were observed. Dural plasty and removal of hematoma with external decompression were carried out. He had a good postoperative course, but left visual loss and left hemiparesis remained. Case 2: 29-year-old male. Admitted with excoriation of his right nostril because of having been stabbed by an umbrella tip, severe headache, and nasal discharge. Oculomotor palsy was observed as well as CSF rhinorrhea and meningeal irritability. Plain skull X-ray showed niveau in sphenoidal sinus, pneumocephalus, and fracture of sella turcica. His complaint disappeared after conservative therapy. We reviewed the literature and found only 4 similar cases. The skullbase, because of its anatomical character, is likely to be penetrated in orbital and periorbital injury caused by umbrella tips. Cases which include disturbance of consciousness have a poor prognosis. We hope the fact that umbrella tips can easily become life-threatening objects will come to the attention of the general public so that similar cases may be avoided.

[Acute myeloblastic leukemia associated with 46, XY, del(5)(q22)].

A 38-year-old male admitted to the Internal Medicine of Surugadai Nihon University Hospital, complaining of general fatigue and throat pain. The laboratory examinations revealed leukocytosis (83, 900/microliters) and an appearance of myeloblasts (90.2%) in the peripheral blood. The nucleated cell count was 56 x 10(4)/microliters with 85.5% myeloblasts in bone marrow. He was diagnosed as acute myeloblastic leukemia (AML). Though he received two courses of combination chemotherapy with daunorubicin, BH-AC, 6 MP and prednisone, one course of combination with mitoxantrone, etoposide and cytosine arabinoside and one course of combination with aclarubicin cytosine arabinoside and prednisone, he could not achieved remission. A chromosome analysis revealed 46, XY del(5)(q22). The amount of DNA fragments hybridized to 4.5 Kb v-fms probe in blastoid cells was approximately a half amount of normal persons. It is not defined the relationship between the decrease of fms and leukemia in this case. He was diagnosed de novo AML, since he had not been received the therapy with potential mutagenic and carcinogenic agents and had not been exposed the irradiation on his works.

[Therapeutic efficacy of miconazole on deep-seated fungal infections in the respiratory tract system].

We evaluated the therapeutic efficacy of miconazole (MCZ, Florid-F inj.), a new antifungal agent for parenteral use, in deep-seated fungal infections of respiratory tract system. A daily dose of 400-1,800 mg of MCZ was given intravenously for 12-38 days (mean: 23.4 days) to 7 patients: 2 patients with pulmonary aspergillosis, 1 patient with bronchial aspergillosis, 1 patient with pulmonary candidiasis and 3 patients with candidemia. One additional patient with pulmonary aspergillosis received three instillations of 20 mg of MCZ into the thoracic cavity. The clinical effects were excellent in 1, good in 4 and poor in 3 patients. The efficacy rate was 100% in 5 cases with respiratory fungal infections but 3 cases with candidemia did not respond well to the treatment. Four strains each of Aspergillus sp. and Candida sp. were identified as causative organisms. Seven of the 8 strains were eradicated by administration of MCZ. Side effects observed were irritation and heat in a leg in 1 patient, hyperlipoidemia in 2 patients and eosinophilia in 1 patient. The adverse reactions disappeared after the completion of the therapy. From the above results, we conclude that MCZ is one of the most useful antifungal agents for parenteral use as a first choice on deep-seated fungal infections in the respiratory tract.

[Primary pulmonary lymphoma diagnosed by transbronchial biopsy and bronchoalveolar lavage findings].

A 45 year-old male was admitted to Tokyo University Hospital because of a submandibular tumor. Biopsy specimen of the tumor showed medium-sized non-Hodgkin's lymphoma of follicular type and immunoperoxidase staining of frozen sections demonstrated an overwhelming predominance of B lymphocytes with IgM, lambda chain. In the meanwhile, chest X-ray taken on admission showed an ill-defined consolidation with a tumor-like appearance in the right middle lung field. Transbronchial biopsy of this lesion revealed massive infiltrations of small and medium sized lymphocytes, having the same markers as those of submandibular tumor (IgM, lambda chain) and an analysis of bronchoalveolar lavage showed a significant increase of CD19-positive B lymphocytes. Reviewing of check-up X-ray films showed the lung lesion to have preceded the submandibular tumor and to have increased its size in several years. On the basis of the similarity of histological and immunohistochemical findings between pulmonary and submandibular tumor, and considering the time course of the appearance of these tumors, we concluded that these tumors were of the same histological nature and had originated in the lung and metastasized to submandibular gland. This is a relatively rare case of pulmonary lymphoma metastasizing to the submandibular gland, in which transbronchial biopsy specimen and analysis of lymphocytes in bronchoalveolar lavage were helpful in establishing the diagnosis.
