Right adrenal pheochromocytoma with anterolateral displacement of the inferior vena cava: skin incision and approach.

We report a case of a right adrenal pheochromocytoma with prominent anterolateral displacement of the inferior vena cava. A right ipsilateral anterior subcostal incision extending from the xiphoid process to the 11th intercostal space allowed for wide upward retraction of the right costal arch. This incision is useful for easy mobilization of the liver and manipulation of the inferior vena cava without thoracotomy.

[Radiotherapy of nasopharyngeal carcinoma].

The results of irradiation on fifty seven patients with nasopharyngeal carcinoma, treated from 1964 to 1985 at the Shinshu University Hospital, were studied. Forty-two percent of the patients had T3-4 primaries and 68% had N1-3 regional nodes; 86% had Stages III-IV of the disease. Of the 57 patients, 32 had squamous cell carcinomas, and 25 had lymphoepitheliomas. The overall 5-year survival rate was found to be 57%. Primary size (T) and histology influenced the survival of patients; the 5-year survival rate was 70% for T1-2 primary and 39% for T3-4 primary, and 39% for squamous cell carcinoma and 80% for lymphoepithelioma.

A case of Cushing's syndrome associated with possible adrenomedullary hyperplasia.

A 57-year-old male, who had been suffered from hypertension and diabetes mellitus for 10 years, was admitted to the hospital because of thirst, lassitude and muscle wasting. On admission, his urinary excretion of 17-OHCS and plasma cortisol levels were elevated without diurnal variations. Plasma ACTH levels were found to be very low with repeated determinations. Dexamethasone suppression test, 2 mg 4 times a day orally for 2 days, showed no changes in plasma cortisol levels and only a mild reduction in urinary 17-OHCS excretion. Estimation of urinary catecholamines showed an increase only in norepinephrine. Abdominal computerized tomography and radionuclide scanning of adrenal glands with 131I-adosterol demonstrated a well-defined adrenal mass in the left side without apparent changes in the right side. 131I-metaiodobenzylguanidine scintigraphy was negative. At surgery, his left adrenal medulla was found to be hypertrophic in addition to the cortical tumor. The left adrenal gland was also removed. After surgery, excretion of urinary catecholamines fell to nearly the normal range and he was discharged without insulin and antihypertensive drugs. Microscopically, the cortical tumor is an adenoma consisting of lipid laden cells and eosinophilic compact cells. Medullary cells were distinctly hyperplastic in appearance and many of the cells were extensively vacuolated, suggesting an active functional status. The present report describes a patient with Cushing's syndrome who showed increased urinary catecholamine excretion due to the possible coexistence of adrenal medullary hyperplasia. As far as we know, this is the first case of Cushing's syndrome with this abnormality.

The effects of intravenous miconazole on fungal keratitis.

Two patients, a 44-year-old man and a 53-year-old man, with fungal keratitis were successfully treated with intravenously infused miconazole, a synthetic imidazole derivative. In these cases healing and regression of the corneal ulcer began four or five days after initiation of miconazole treatment. The clinical evidence of corneal infection disappeared completely after four weeks and visual acuities improved markedly (from 20/500 to 20/20 in Case 1 and from 20/400 to 20/40 in Case 2). Aside from a transient increase of serum triglycerides and beta-lipoproteins, the drug appeared to be well tolerated.

Diffusion coefficient of 36Cl in silver chloride.

Diffusion coefficient of 36Cl in silver chloride was revealed to be about 1 x 10(-10)-4 x 10(-11) cm2 . s-1 in the range of 400 degrees C-200 degrees C by means of radioactive tracer measurements. Activation energy for diffusion was calculated about 0.13 eV. These results were different from those obtained by Maurer and Compton.

Thermal effect of photolysis in 203Hg, 35S and 131I labeled mercury (II) iodide thiocyanate powders.

203Hg, 35S or 131I labeled mercury (II) iodide thiocyanate (HgISCN) powders were prepared, respectively. When the powders were exposed to sunlight, some parts of the crystals of the powders were decomposed and the decomposed atoms diffused toward crystal surface. This diffusion velocity was accelerated by thermal treatment of the darkened powders. The velocity is larger in order of 35S, 203Hg, 131I. Decomposed products consist of colloidal mercury, mercury iodide, mercury sulfide, sulfur dioxide and iodine. Mechanism of photochromism of HgISCN was discussed.
