[Clinical studies of cefotiam in the field of otorhinolaryngology].

Cefotiam (CTM) was intravenously given to 20 patients with infections in the field of otorhinolaryngology. Daily doses of CTM were 2 to 4 g divided into 2. Clinical responses were excellent in 11 patients, good in 8 and fair in 1, the rate of satisfactory clinical responses being 95% (19/20). Ninety-one % (10/11) of Gram-positive cocci isolated from the patients were eradicated by the CTM treatment, while 60% (6/10) of Gram-negative bacilli were persisted. As for the side effects, eruption and elevation of GOT and GPT were observed in 1 patient.

[Detectability of stress-induced ischemic lesion in previous myocardial infarction using 201T1 myocardial single photon emission computed tomography].

The purpose of this study was to evaluate the detectability of stress-induced ischemic lesion in patients with previous myocardial infarction using single photon emission computed tomography (SPECT) producing thallium-201 (T1-201) myocardial perfusion imagings (MPI). Seventy patients underwent stress SPECT by symptom-limited graded bicycle ergometer exercise using a dual-headed rotating gamma camera (Toshiba GCA70A) equipped with a computer system (GMS90). After intravenous administration of 2.5 mCi of T1-201, stress SPECT data at 10 minutes and delayed SPECT data at 3 hours after the injection were collected in the 64 X 64 matrix form covering 360 degrees directions by camera sweep of 180 degrees in 6 minutes, which were immediately followed by conventional planar imagings (PL). Transaxial tomographic image reconstruction was performed by convolution method using a Shepp-Logan's filter. Thereafter, sagittal and coronal tomographic images were reconstructed for about 2 minutes. Image interpretation was assessed visually. The results were as follows: Sensitivity and specificity in detecting the affected vessel with more than 75% stenosis by segmental analysis of myocardial images were higher by SPECT than by PL (LAD 89% and 65%, LCX 68% and 56%, RCA 89% and 76% in sensitivity and LAD 94% and LCX 75%, 92% and 94%, RCA 81% and 59% in specificity, respectively). Sensitivity in detecting both single (82%) and multivessel disease (76%) was fairly high. Detectability of stress-induced ischemia (i.e. occurrence of a new defect in patients with previous myocardial infarction and ST-segment depression in ECG) was significantly higher in SPECT (67%) than in PL (39%, p less than 0.005) and in ECG (39%, p less than 0.005). A perfusion defect in the extensive anterior wall, marked left ventricular dilatation and the widening of the angle toward the apex composed of septal and anterolateral walls in transaxial images were the findings characteristic of anterior myocardial infarction with severe dyskinesis. We conclude that stress SPECT is a useful noninvasive technique for the documentation of the number of vessels affected and severe wall motion abnormality of the LV and for the detection of stress-induced ischemia in previous myocardial infarction.

[Assessment of serum ferritin, CEA, beta 2-MG and PAP determinations in patients with urinary tract malignancies].

Ferritin, carcinoembryonic antigen (CEA), beta 2-microglobulin (beta 2-MG) and prostatic acid phosphatase (PAP) levels in serum from 77 patients with cancer (6 with renal adenocarcinoma, 9 with renal pelvic and ureteral cancer, 29 with bladder cancer and 33 with prostatic cancer) at various stages were clinically evaluated for their significance as a parameter of urinary tract malignancies. Although, ferritin, CEA and beta 2-MG levels in the poorly-differentiated and advanced stage groups of renal adenocarcinoma, renal pelvic and ureteral cancer, and bladder cancer were higher than those in the well-differentiated and early stage groups, those in most cases were within normal ranges. These proteins were not considered suitable for the screening test. Ferritin and beta 2-MG levels increased with advancement of the performance status (P.S.) proposed by Koyama and Saito; however, the latter was affected greatly by renal impairment. In prostatic cancer, PAP and ferritin levels were remarkably high in the poorly-differentiated group (PAP mean +/- S.E.: 57.6 +/- 22.5 ng/ml, ferritin 883 +/- 319 ng/ml) and the advanced stage group (27.2 +/- 10.5 ng/ml, 398 +/- 152 ng/ml) compared to the well-differentiated group (7.87 +/- 3.61 ng/ml, 88.5 +/- 25.8 ng/ml) and the early stage group (2.24 +/- 0.54 ng/ml, 186 +/- 91.7 ng/ml). PAP and ferritin levels of the untreated cases were positive in 10 out of 18 cases (55.6%) and 7 out of 18 cases (38.9%), respectively, and those of the relapsing cases were positive in 4 out of 7 cases (57.1%) and 6 out of 7 cases (85.7%), respectively. However, CEA and beta 2-MG levels were negative in most cases. Furthermore, increments of PAP and ferritin levels, especially that of the ferritin level, were significantly related to advancement of P.S., and high ferritin levels were obtained in all cases of P.S. 3 and 4. Therefore, determination of PAP and ferritin seems to be useful in monitoring prostatic cancer, and the latter to be useful in early detection of relapsing cases.

Antihypertensive effect of nifedipine and its relationship to severity of hypertension.

The antihypertensive effect of nifedipine (Adalat), a Ca2+-antagonistic drug, was studied in patients with mild or severe hypertension. In both short- and long-term trials, nifedipine exerted a strong hypotensive effect, more pronounced in patients with severe than in cases with mild hypertension. The results of our short-term study showed a positive correlation between the maximum fall in blood pressure induced by nifedipine and pretreatment systolic blood pressure values or the total severity index score of hypertension. In our long-term trial, we also observed a positive correlation between maximum reduction of blood pressure--induced by nifedipine--and pretreatment values. However, no correlation between reduction of blood pressure and total severity index score of hypertension could be established. Our results indicate that nifedipine could be effective in the management of severe hypertension. The hypotensive action of nifedipine, at least partly due to its Ca2+ influx blocking action, suggests that etiologically hypertension may be connected with an abnormal calcium metabolism of the cardiovascular muscle cells.

[ECG features of hypertrophy in hypertrophic cardiomyopathy, with special reference to asymmetric septal hypertrophy].

Thirty-eight cases of hypertrophic cardiomyopathy associated with asymmetric septal hypertrophy (ASH) were analyzed in order to correlate the electrocardiographic findings with the distribution of hypertrophic portions identified by two-dimensional echocardiograms. The electrocardiographic features which characterize the selected cases with ASH in this study were abnormal Q waves or regression of R waves in the left precordial leads (six obstructive type and six non-obstructive type) and a pattern of left ventricular hypertrophy (LVH) (31 cases including five cases with abnormal Q waves) (Table 1). There was no significant relation between the voltage of a R wave in V1 and the degree of septal thickness. From the electrocardiographic features mentioned above, the patients were divided into two groups, i.e., 1) the patients with abnormal Q waves (12 cases) and 2) those with LVH without abnormal Q waves (26 cases). The difference in the pattern of distribution of hypertrophy was evaluated based on the presence or absence of the abnormal Q waves. The results were as follows: Although there were no differences in the degree of septal thickness in both groups, the posterior wall in the group with LVH was significantly thicker. The group with LVH showed diffuse hypertrophy in the left ventricular free wall as well as the septum. Moreover, the short-axis view of two-dimensional echocardiograms revealed that hypertrophy in the septum was diffuse in both anterior and posterior portions.(ABSTRACT TRUNCATED AT 250 WORDS)
