In order to detect the fibrotic response in lung-tissue, concentrations of type III procollagen N-terminal peptide (P-III-P) were examined in sera and bronchoalveolar lavage fluids from rabbits with bleomycin-induced pulmonary fibrosis, and in sera from patients with various respiratory diseases, using a radioimmunoassay method. In the bleomycin-induced pulmonary fibrosis, a significant (p less than or equal to 0.05) rise in P-III-P levels was observed in the lavage fluid at 7 days, and in the sera at 21 days after bleomycin treatment, with histological confirmation of pulmonary fibrosis. Many of the patients with pulmonary fibrotic changes had significantly elevated serum P-III-P level. In lung cancer patients, most of the post-radiation serum samples were found to have P-III-P levels greater than or equal to 18 ng/ml. These results suggest that the increase in P-III-P levels is the reflection of fibrotic changes in the lungs and that the clinical assessment of serum P-III-P level could be important in the early detection of pulmonary fibrosis.
A case of mediastinal immature teratoma was treated with a multimodality approach that included surgery, chemotherapy, and radiotherapy. Serum alpha-fetoprotein at admission was elevated and changed in parallel with the therapy and tumor growth. The patient died of sudden respiratory failure 18 months after admission. Mediastinal immature teratoma with elevated serum levels of alpha-fetoprotein carries a poor prognosis, as does endodermal sinus tumor. We concluded that this condition should most probably be treated with a combination of intensive chemotherapy, surgical resection, and radiotherapy.
Palytoxin (PTX), C129H223N3O54, isolated from marine coelenterates of Palythoa tuberculosa, caused a first rapid contraction followed by the slow phasic contraction of guinea-pig vas deferens. In the presence of ouabain (10(-5) M), PTX (10(-8) M) failed to cause the first contraction; however, the second contraction was potentiated. In the presence of phentolamine (10(-6) M), the second contraction was inhibited selectively. When ouabain was applied to the muscle in the presence of phentolamine, both first and second contractile responses to PTX were abolished. When the muscle was exposed to the potassium-depleted solution, the first contractile response to PTX was rather potentiated. PTX caused the release of norepinephrine from the muscle. Exposure of the muscle to ouabain (10(-5) M) markedly increased the PTX-induced release. It is indicated that the first and second contractile responses to PTX have entirely different properties. The second response is due to a release of norepinephrine from nerves and was potentiated by ouabain through the increase in the norepinephrine release, whereas the first response was not due to the norepinephrine release but presumably to a direct action on smooth muscle cell and was inhibited by ouabain. The mechanism of the action of PTX was discussed in the relation with Na,K-ATPase.

A 65-year-old woman with cystadenocarcinoma of the liver is reported, and 21 Japanese patients cited in the previous literature are reviewed. In our case, CT and US demonstrated a huge solid tumor with calcification in the right lobe of the liver. Angiograms showed a hypovascular tumor with peripheral irregular neovascularity. ERCP showed compression of the extrahepatic bile duct. Right lobectomy of the liver was performed. The pathological diagnosis was cystadenocarcinoma of the liver. In our case, it was difficult to diagnose cystadenocarcinoma because the cystic mass had viscious contents and necrotic tissues, so CT and US showed findings of a solid tumor.

A 36-year-old housewife in the U.S.A. was diagnosed as having gastric cancer with meningeal carcinomatosis and admitted to our hospital in September, 1982. She had severe headache, nausea, vomiting, diplopia and neck stiffness. She was treated by intrathecal chemotherapy using methotrexate, cytosine arabinoside and prednisolone, and by systemic chemotherapy using adriamycin and ftorafur, resulting in complete disappearance of cancer cells from the cerebrospinal fluid and partial response for the primary tumor. She lived for more than 1 year following the first symptoms of her disease and for 10 months following the initiation of chemotherapy. This case suggested the usefulness of employing an intrathecal chemotherapy using methotrexate and cytosine arabinoside with simultaneous systemic chemotherapy for meningeal carcinomatosis of gastric cancer.
