Stimulatory and inhibitory effects of forskolin on adenylate cyclase in rat normal hepatocytes and hepatoma cells.

Forskolin synergistically potentiated adenosine 3',5'-cyclic monophosphate formation by prostaglandin E1 (PGE1) in rat normal hepatocytes freshly prepared by collagenase digestion and rat ascites hepatoma AH66 cells, but dose-dependently inhibited the accumulation by PGE1 in AH66F cells. Forskolin activated adenylate cyclase in a dose-dependent manner in homogenates of all cell lines. In normal hepatocytes and AH66 cells, simultaneous addition of forskolin and other adenylate cyclase activators [isoproterenol (IPN), PGE1, guanosine 5'-triphosphate sodium salt (GTP), 5'-guanylylimidodiphosphate sodium salt (Gpp (NH)p), NaF, cholera toxin, islet activating protein and MnCl2] gave greater than additive responses. On the other hand, in AH66F cells, the effect of forskolin on adenylate cyclase was hardly influenced by GTP, but forskolin diminished the activities induced by high concentrations of GTP to that by the diterpene alone. Forskolin also significantly inhibited the PGE1-stimulated and the guanine nucleotide binding regulatory protein-stimulated activities. Because AH66F cells were insensitive to IPN, the combination with forskolin and IPN gave similar activity to that obtained with the diterpene alone. The effect of forskolin on the activation by manganese ion was neither synergistic nor inhibitory but was additive in AH66F cells. These results suggest that forskolin promotes the interaction between the stimulatory guanine nucleotide binding regulatory protein and the catalytic unit in normal hepatocytes and AH66 cells, but in AH66F cells forskolin interferes with the coupling of the two components of adenylate cyclase.

Analysis of standing posture in patients with bilateral loss of labyrinthine excitability.

The forward-backward and right-left sways in the upright standing posture were examined using stabilometry and analysis with a 5-dimensional feedback model. The forward-backward sway was found to have a fast, hidden periodicity, composed of sways due to a loss of the labyrinthine righting reflex upon the head and interaction between the head and knee sways. Furthermore, on the basis of our previous investigation, the forward-backward sway was found to depend on the increased spinal reflexes caused by a loss of labyrinthine inhibitory control upon the antigravity muscles. The right-left sway is slow and irregular, being mainly produced by disturbance of the head righting reflex.

Cross-reactivity between human and canine helper and suppressor T cell antigens using monoclonal antibodies RPA-T4 and HuLy-m8.

The murine monoclonal antibodies RPA-T4 and HuLy-m8, specific for a framework determinant of human helper/inducer and suppressor/cytotoxic T cell antigens, cross-reacted with canine cell membrane molecules recognizing a biomolecular complex (50,000 to 55,000 daltons) similar to that described in humans. We investigated the distribution of these helper and suppressor T cell-like antigens on canine peripheral blood lymphocytes. With complement-mediated lymphocytotoxicity, 34% and 35% of the canine lymphocytes expressed the helper T cell-like antigens and the suppressor-like T cell antigens, respectively. When the lymphocytes were treated with RPA-T4 and HuLy-m8, the respective helper and suppressor function was significantly inhibited.

Effect of ibudilast on mucociliary transport using frog palate.

For the determination of mucociliary transport, we have used a method to measure mucociliary transport force (MCTF), which was defined as the tension developed by the mucus flow and ciliary motility of a frog palate between a force transducer and a glass boat on the palate. 3-isobutyryl-2-isopropylpyrazolo [1,5-a] pyridine (ibudilast) is supposed to increase intracellular cyclic adenosine monophosphate (cAMP) level by the inhibition of cAMP phosphodiesterase and to potentiate the effect of prostacyclin. The effect of ibudilast on MCTF was investigated for 15 min after the drug application. Ibudilast induced a significant, dose-related increase in MCTF at concentrations of higher than 10(-7) M. Therefore, these results indicate that ibudilast, an anti-allergic agent, activates mucociliary transport in respiratory tract.

[A case of malignant pleural mesothelioma with infectious bronchogenic cyst].

A 47-year-old man was admitted with a cough on January 4, 1986. A chest X-ray film showed a mass shadow in the left lower lung, which was revealed to be a bronchogenic cyst by CT scanning and ultrasonography. Thoracotomy was performed on March 3, 1986 because cytologic tests on the fluid in the cyst suggested malignancy. A cyst, two tumors on the diaphragm and pleural thickening were revealed. Microscopic examination showed a benign bronchogenic cyst and a mixed-type malignant mesothelioma. In spite of chemotherapy (ADR, Cis-DPP, 5-fluorouracil) and immunotherapy (OK-432, PSK), the pleural thickening progressed, as was demonstrated by CT scanning and ultrasonography. Although cardiac tamponade due to invasion by the malignant mesothelioma developed, this was improved by cardiocentesis. The patient died of pneumonia on March 28, 1987. We studied the concentration of mineral fibers in lung and tumor tissues of this case by Energy Dispersive X-ray Analyser because asbestos or non-asbestos inorganic fibers might cause malignant mesothelioma. This case of malignant pleural mesothelioma accompanied by a bronchogenic cyst is very rare.

[Application of HPLC to measurement of plasma histamine in bronchial asthma].

We measured the plasma histamine concentration in patients with bronchial asthma by the post-column derivatization method developed by Yamatodani. The mean fasting plasma concentrations of histamine determined for bronchial asthma and control groups (male and female) were 1.93 +/- 0.29 (male) and 1.37 +/- 0.15 (female) pmol/ml and 4.05 +/- 0.69 (male) and 5.19 +/- 0.35 (female) pmol/ml, respectively. The histamine concentrations in the bronchial asthma group were significantly lower than those in the control group, although no significant sex-related difference was found. On the other hand, the histamine concentrations in the patients with bronchial asthma after provocation with histamine were significantly higher than when the patients were in a quiescent stable. In this study we can get the behavior of the slight changes in bronchial asthma by considering the finding for the patients, sampling trials and selection of the determination method.

[Increased neutrophils in bronchoalveolar lavage fluid from a patient with developing adult respiratory distress syndrome].

A 16-year-old, 28-week pregnant woman was admitted to our hospital with multiple bone fractures caused by a traffic accident. She had massive blood transfusion because of anemia in her laboratory findings and ritodrine hydrochloride was administered because of the fear of threatened abortion. She developed a cough with bloody sputum on the 4th day after admission, and developed pulmonary insufficiency with PaO2 41.0 torr and presented bilateral diffuse infiltrates on chest roentgenograms on the next day. Swan-Ganz catheterization revealed normal pulmonary capillary wedge pressure and analysis of the lavage fluid from the patient showed an increase in the percentage of neutrophils (40.0%) and the existence of leukotriene B4 which is known to be the most potent chemokinetic and chemotactic agent for neutrophils. Her condition was considered to be permeability edema developing adult respiratory distress syndrome (ARDS) and 1 g/day of methylprednisolone was administered intravenously for 3 days, which brought about remarkable improvement of her respiratory failure. This report suggests that analysis of the lavage fluid may provide useful information for the early diagnosis of ARDS and the indications of corticosteroid treatment.

[Pulmonary tuberculosis and mycotic infection--clinical and serological diagnosis].

The antibody activities against Aspergillus fumigatus and Candida albicans by indirect hemagglutination (IHA) and counterimmunoelectrophoresis (CIE) were examined twice at a month's interval in 251 sera from 169 male and 82 female patients admitted to the Higashi Nagoya National Hospital for pulmonary diseases. The patient population was composed of 226 patients with active or cured pulmonary tuberculosis including 25 patients complicated with pulmonary aspergillosis and 25 other lung diseases. In our tests, antigens used were crude supernatants of culture-broth which are the generous gifts of Torii Pharmaceutical Company. The antibody activity against Aspergillus by IHA was positive in 2.9% of the sera in the first and in 0.9% in the second test and was positive against Candida by IHA in 44.9% in the first and in 44.0% in the second test (Table 1). As regards patients with pulmonary tuberculosis, the population whose serological reaction was positive against Candida did not increase with age (Table 2). Two results of the antibody activities against Aspergillus and Candida by CIE were the same. The CIE results were positive in 19.5% against Aspergillus and in 16.3% against Candida (Table 3). In CIE test, 4.3% were positive against both Aspergillus and Candida. The causes of high positive rate in IHA against Candida may be due to (1) difference in the antibody, namely, antibody measured by IHA mainly composed of IgM and that by CIE composed of IgG and (2) difference in the virulence between Candida and Aspergillus.(ABSTRACT TRUNCATED AT 250 WORDS)

Expression of interleukin-2 receptor (IL-2R) in feline peripheral blood lymphocytes.

A monoclonal anti-interleukin-2 receptor (IL-2R) antibody has been identified as a putative antibody against the human IL-2R. In the present study, anti-Tac antibody CD-25 was used to determine cell expressing IL-2 receptor in feline peripheral blood lymphocytes by means of direct immunofluorescence tests and complement-mediated lymphocytotoxicity tests. With complement-mediated lymphocytotoxicity, approximately 18% of feline peripheral blood lymphocytes expressed the receptors. By the direct immunofluorescence test, we found approximately 22% of IL-2R positive cells in lymphocytes of feline peripheral-blood.

[Follicular lymphoma in Japan: retrospective study on natural history and therapeutic results in 50 cases collected from 4 institutions].

Clinical data of the 50 patients with nodal follicular lymphoma were collected from 4 institutions in Japan and were analysed retrospectively. A frequency of follicular lymphoma was 9.3% in the 364 patients with nodal lymphoma registered between 1981-1986. Twenty-one patients (42%) were classified as follicular medium-sized cell type (F-medium; identical with follicular small cleaved cell type by Working Formulation criteria), 12 (24%) as follicular mixed (F-mixed), and 17 (34%) as follicular large (F-large). There were 31 (62%) patients with Stage III-IV disease involving bone marrow in 6 patients, liver in 2, bone marrow and liver in 2, stomach in 1. Overall five- and ten-year survival rates were 65% and 32%, respectively, in the 49 patients whose median follow-up period was 3 years, ranging from 2 to 11 years. Adriamycin-based combination chemotherapy [ADM(+) therapy] produced 10 (100%) complete response (CR) of 10 patients with F-large lymphoma and 8 (80%) of them were free from relapse. But, in the patients with F-medium lymphoma, ADM(+) therapy produced one (17%) CR of 6 patients and their survivals were not superior to those treated with ADM(-) therapy, single agents or radiotherapy alone. There was no difference in the natural history or therapeutic results between the patients in Japan and United States, although the frequency of follicular lymphoma was extremely lower in Japan. ADM(+) therapy can be accepted as the first-line chemotherapy for the patients with F-large lymphoma, but the best modality of treatment should be investigated for the patients with F-medium lymphoma.

[Extended operations in cancer--gastric cancer].

Extended operations of combined resection of surrounding organs and lymphadenectomy of the para-aortic lymph nodes for advanced cancer of the stomach were reported. Combined pancreatoduodenectomy for advanced cancer of lower portion was carried out 5.2% (172/3317). 5 year survival rate was 7% (10/141). Nine 5-year survival cases showed direct invasion to pancreas, so this combined resection is significant for such cases. Left upper abdominal evisceration for advanced cancer of upper and middle portion was carried out in 128 cases (1980-1986). 5-year survival rate was 9.4% (3/32). We expect that this extended procedure shows the progress of surgical results on Borrmann 4 type cancer. Para-aortic lymph node dissection was performed in 400 cases during 10 years (1978-87). The rate of lymph node metastases was 31%. Among them, 5-year survival 5 cases survived over 5 years after operation. For advanced cases with lymph node metastases, lymphadenectomy around the abdominal aorta should be performed more aggressively.

[The histological type and lymphatic permeation of diffuse infiltrative carcinoma of the stomach].

Although diffuse infiltrative carcinomas of the stomach are mostly composed of anaplastic/mucocellular type, some cases show tubular/trabeculoacinar carcinoma in a large or small part of the tumor. We have subdivided diffuse infiltrative carcinoma into two subtypes, mixed type and pure type, from the viewpoint of the carcinoma's histological structure and lymphatic permeation (LP). The former, or mixed type, includes a tubular/trabeculoacinar element in at least 10% of the tumor tissue, and shows a high tendency for LP. On the other hand, the latter, or pure type, consists almost exclusively of anaplastic/mucocellular carcinoma with a low tendency for LP. No difference in the frequency of LP between males and females was found in the pure type carcinomas, but a significantly higher frequency and grade of LP was observed in males in the mixed type carcinomas. Further, even in the pure type, the LP was rather frequent in people 60 or more years old. Thus, lymphatic permeation seems to be influenced not only by the histological type of the carcinoma but also by sex and age.

Protein binding characteristics of a new bronchodilator, 1-methyl-3-propylxanthine (MPX), in different species.

The protein binding of a new bronchodilator, 1-methyl-3-propylxanthine (MPX), in different animal species sera (human, dog, rabbit, rat and mouse) was investigated in vitro by ultrafiltration method. Interspecies differences in the binding affinity and binding capacity for MPX were observed. The dissociation constant (Kd1) for the high affinity binding site of human serum was approximately 17 times higher than that of rat serum. Those of rat serum and rabbit serum were nearly equal. On the other hand, the binding indices (n1p1/kd1) of human serum and rat serum were comparable. The binding of MPX to various proteins such as human serum albumin (HSA), alpha 1-acid glycoprotein (AGP) and isolated human lipoproteins was also investigated. The binding of MPX to HSA was lower than that of human serum protein but the number of specific binding sites (n1) in human serum was in agreement with that in HSA. The binding curves of MPX to AGP and lipoproteins were almost linear and the binding percentages were less than 7% in both cases. These data suggest that MPX is exclusively bound to albumin and that AGP and lipoproteins are of little importance in the protein binding of MPX. The present data appear to be useful for predicting the role of protein binding on the pharmacokinetics of MPX in different animal species.

Portal and mesenteric venous thrombosis as a complication of endoscopic sclerotherapy.

We report on the occurrence of portal and mesenteric venous thrombosis in two patients who underwent endoscopic injection sclerotherapy (EIS) for bleeding esophageal varices. One patient was diagnosed at autopsy, and portal and mesenteric venous thrombosis was not suspected during life. The second patient was suspected on ultrasound examination and was diagnosed by angiography. In the second case, therapy was effective and symptoms disappeared. He was treated by a distal splenorenal shunt after he had stabilized for 1 month. During the operation, no evidence of mesenteric infarction was found. The venous phase of the superior mesenteric arteriogram 1 month after surgery revealed an organized thrombosis in the right intrahepatic portal branches. No other thrombi were seen. This patient is the first reported who has survived portal and mesenteric venous thrombosis after sclerotherapy.

Severe hemolysis after distal splenorenal shunt.

We report a case of severe hemolysis after distal splenorenal shunt. Hemolysis was suddenly recognized at the time splenorenal venous anastomosis was completed, and it continued on the operative day and the first postoperative day. After the shunt, the intraoperative splenic venous blood flow was approximately double its previous level, as measured by electromagnetic flowmeter. Angiographic partial splenic embolization was performed on the second postoperative day, and then hemolysis stopped suddenly. This suggested that hemolysis after distal splenorenal shunt was related to increased splenic blood flow and acceleration of erythrocyte destruction.

Simultaneous assay for individual sulphated 3 alpha- and beta-hydroxysteroids in serum using high-performance liquid chromatography combined with 3 alpha- and beta-hydroxysteroid dehydrogenases immobilized on one column.

A high-performance liquid chromatographic method for the simultaneous determination of individual sulphated 3 alpha- and beta-hydroxysteroids in serum using 3 alpha- and beta-hydroxysteroid dehydrogenases (3 alpha-HSD and beta-HSD, respectively) immobilized on one column and a fluorimeter to detect the resulting NAD+ to NADH transformation is described. Individual sulphated 3 alpha- and beta-hydroxysteroids in serum are extracted with ethanol, solvolysed with sulphuric acid in ethyl acetate and then separated by high-performance liquid chromatography. The hydroxysteroids thus separated are subsequently mixed with NAD+ and then passed through the column in which the following catalytic reaction occurs: (formula, see text) The detection limits are as low as 0.5-1.0 microgram/dl for sulphated 3 alpha- or beta-hydroxysteroids in serum. The present assay method is highly specific, reliable and reproducible and is thus applicable to a clinical study on the metabolism of sulphated 3 alpha- and beta-hydroxysteroids in patients with adrenal or gonadal diseases.
