Favorable outcomes with tacrolimus in two patients with refractory interstitial lung disease associated with polymyositis/dermatomyositis.

Two cases of progressive interstitial lung disease associated with polymyositis/dermatomyositis are presented. Both patients were refractory to conventional therapy with high-dose corticosteroids, cyclosporine, and intermittent pulse cyclophosphamide, and thus a therapeutic trial of tacrolimus was instituted. Tacrolimus was markedly effective in achieving subjective, laboratory and radiographic improvement in both patients.

Comparison of intra-arterial digital subtraction angiography with conventional angiography after transcatheter arterial embolization for hepatic malignancy.

Twenty patients with hepatic malignancy were treated with transcatheter arterial embolization (TAE) under examination by intra-arterial digital subtraction angiography (IA-DSA) and conventional angiography (CA). Comparison of these two angiographies revealed that the time required for confirmation of the embolized portion of the artery was about four-and-a-half times shorter with IA-DSA than CA. Moreover, IA-DSA revealed the obstructed portion more readily and accurately than CA. In particular, confirmation could not be made by CA in 35% of cases because of residual Gelform sponge containing contrast medium in the artery. The visualization of residual tumor stain after TAE was 40% better on IA-DSA than CA, because of residual Gelform sponge containing contrast medium and overlapping shadow with calcified costal cartilage, excretory pyelography, and original tumor stain. However, CA was better than IA-DSA in visualizing accidental obstruction of nonobjective arteries such as the cystic artery because of the poor spatial resolution and misregistration artifacts of IA-DSA. CA was better than IA-DSA in visualizing surrounding nonembolized hepatic parenchyma because of the misregistration artifacts of IA-DSA. For effective TAE without severe complication, we concluded that TAE for HCC should be performed under a combination of IA-DSA and CA.

Aberrant dendritic development in the human agyric cortex: a quantitative and qualitative Golgi study of two cases.

A quantitative and qualitative Golgi comparison of the visual cortex from two agyric brains and of two age-matched controls is reported. In the camera lucida drawings, most pyramidal cells were oriented vertically to the pial surface in the external cellular layer, frequently with their apical dendrites directed toward the deep layers (inverted pyramidal neurons). The deep cellular layer contained pyramidal and polymorphic neurons normally found in the second to fourth cortical layers. In quantitative analysis of the agyric cortex of a ten-month-old patient, relative immaturity of basal dendritic arborization was apparent together with a bipolar configuration of dendritic development of the pyramidal neurons. The 3-year-old patient had a significant delay in apical dendritic arborization (shorter branch length, decreased number of dendritic intersections) compared with his age-matched normal control. The pathogenesis of the abnormal dendritic development in agyria is discussed.

Influence of 5-hydroxytryptamine on the combination effect of lonidamine or gossypol and hyperthermia on Ehrlich tumour in vivo.

An experiment was conducted using lonidamine and gossypol against Ehrlich tumour in the foot pad of CD-1 mice. These compounds alone were mild antitumour agents, but their cytotoxicity increased when they were combined with hyperthermia. The antitumor effect was further increased by 5-hydroxytryptamine, particularly when combined with lonidamine.

Cockayne's syndrome: report of two autopsy cases associated with neurofibrillary tangles.

Neuropathological findings in two patients with Cockayne's syndrome, a rare familial condition with mental retardation, progressive neurological deficits, progeria, and dwarfism, are described. Both patients showed extensive tigroid demyelination of the white matter and massive calcifications in the striatum, pallidum, and dentate nucleus. In one patient, neurofibrillary tangles were found in neurons in the nucleus basalis of Meynert, locus ceruleus, substantia nigra, and cerebral cortex, while in the other case only a few tangles were seen in the cerebral cortex. Electron microscopy revealed paired helical filaments. The presence of neurofibrillary tangles in Cockayne's syndrome, rarely reported previously, could suggest premature aging of the brain.

Clinical evaluation of serum alpha-fetoprotein (AFP) and protein induced by vitamin K absence or antagonist-II (PIVKA-II) levels in patients with hepatocellular carcinoma following interventional radiology.

Fourteen patients with unrespectable HCC were treated with various interventional radiology (IVR) procedures. The initial therapeutic response was determined using computed tomography (CT) findings, and determinations of serum alpha-fetoprotein (AFP) and protein induced by Vitamin K absence or antagonist-II (PIVKA-II) levels. When CT studies of the initial response to IVR were compared with changes in the serum AFP and PIVKA-II levels, the AFP level was found to correlate more closely than the PIVKA-II levels. The PIVKA-II level correlated more closely than the AFP level in cases with poor response to IVR. Both of these tumor markers should be measured in combination with the diagnostic imagings for follow-up studies of IVR.

Prevalence of human T-cell leukemia virus type 1 (HTLV-I) in adult T-cell leukemia (ATL) in non-ATL-endemic Hokkaido of Japan.

Sera and peripheral blood lymphocytes of 40 adult T-cell leukemia (ATL) patients in non-ATL-endemic Hokkaido were examined for the prevalence of human T-cell leukemia virus type 1 (HTLV-I). All patients had HTLV-I-specific antibodies. When the peripheral lymphocytes were assessed after short-term cultivation, HTLV-I antigens and virus particles were detected. The seroprevalence in 96 cases of non-T-cell leukemias and lymphomas and in 30,056 healthy individuals in Hokkaido were 3.1% and 0.7%, respectively. HTLV-I seropositive inhabitants of Hokkaido can be estimated at about 40,000, and one out of every few thousand HTLV-I carriers is likely to develop ATL.

Body composition measurement by electrical bio-impedance method to establish the effect of daily physical training in adolescents.

As part of an ongoing study on the effect of daily physical training on adolescents, body composition (percentage fat) was measured using the electrical bio-impedance method in a sample of Japanese students aged 15 years (77 sedentary males, 137 active males, 66 sedentary females and 54 active females), who were selected on the basis of their answers in a questionnaire about physical activity in a cardiac study involving 227.361 high school students. Subjects were divided into 4 weight categories (underweight, normal weight, overweight and severe overweight) using an obesity index. ECG and ultrasound cardiography (UCG) were used for all subjects to measure their left ventricular mass (LVM), which was taken as an indicator of the effect of training. Among the 4 weight category groups, a significant difference in the percentage of fat between sedentary and active subjects was found in the normal weight category of males. Correlation between the percentage of fat and the obesity index was significant in both sedentary and active subjects of both the genders, but correlation coefficients were lower for active subjects than for sedentary ones. The percentage of fat tended to be very low in subjects with increased LVM. Additionally, a significant difference in the amount of fat was found between runners and soccer players in active males with normal weight. These results suggest that there were differing degrees of the effect of training on reducing body fat and that this effect is most notable in males with normal weight.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of intralesional injection of cisplatin on squamous cell carcinoma and normal tissue of mice.

Iodized oil as a drug carrier system for intralesional injection of cisplatin was assessed using human squamous cell carcinoma and normal tissues (kidney and small intestine) of BALB/c nu/nu mice. The results were compared to intralesional and intraperitoneal injections of cisplatin dissolved in distilled water. The following were assessed: change in tumor volume, increase or decrease in body weight of mice, and bromodeoxyuridine uptake of tumor, kidney and small intestine. Tissue platinum concentrations were also measured. Results showed that iodized oil may prove to be beneficial as a drug carrier system of cisplatin.

Intra-arterial digital subtraction portography with a blood-isotonic, non-ionic, dimeric contrast medium.

Intra-arterial digital subtraction portography (IA-DSP) with a blood-isotonic, non-ionic, dimeric contrast medium was carried out in 27 patients with hepatocellular carcinoma. It was possible to obtain images of excellent or good quality of the portal vein and its bilateral main branches in all patients. The third-order branches of the portal vein in the right lobe could be identified in all patients, and images of excellent or good quality were obtained in a mean of 80.2% of patients. Images of third-order branches in the left lobe were of lower quality than those of third-order branches in the right lobe; in particular, images obtained were of poor quality for 27.3% of the medial branches of the left lobe. It was impossible to identify the caudal branches in almost all patients. The side effects of IA-DSP, pain and sensations of heat were very mild: only one patient complained of mild pain, while 18 patients (69.2%) complained of no sensations of heat whatsoever.

Enhancement of antitumor effects of new analogue of platinum, cis-diammine (glycolato) platinum (254-S) combined with hyperthermia in vivo.

Cis-diammine (glycolato) platinum (254-S) is a second generation platinum complex with reduced nephrotoxicity. In this study the antitumor effect of 254-S combined with hyperthermia in vivo in mice was studied. On the 6th day after inoculation of the Ehrlich ascites tumour cells, 254-S (15mg/kg) was administered intraperitoneally (i.p.) and hyperthermia was induced by using a circulating water bath at 42.5 degrees C for 45 min. The antitumour effect was evaluated by relative tumour volume. Furthermore, platinum concentration in tumour tissue was determined by using atomic absorption spectrophotometry. The most effective condition was found to be the combination of 254-S with hyperthermia. A significantly higher concentration of platinum in the tumour tissue was observed when treatment with 254-S was combined with hyperthermia, than with treatment using 254-S alone. Our study suggested that the accumulation of 254-S in the tumour tissue, and its retention at a high concentration within the tumour tissue long term was one of the reasons for the enhancement of antitumour effect of 254-S treatment when combined with hyperthermia.

Antitumor effect of carboplatin combined with hyperthermia on Ehrlich-ascites tumor in vivo.

We examined the optimal timing for the enhancement of antitumor effects of CDDP analogues, cis- diammine-1,1- cyclobutane dicarboxylate platinum (II); CBDCA combined with hyperthermia against the CD-1 mice bearing Ehrlich ascites tumor (EAT) cells in vivo. The prolonged tumor doubling time was observed when two modalities were combined. The longest tumor doubling time was obtained by simultaneous administration of CBDCA combined with hyperthermia. The findings indicated that the most effective condition was the simultaneous combination of CBDCA with hyperthermia. An increase in intratumoral platinum concentration was observed by the treatment of CBDCA simultaneous combined with hyperthermia.

Effects of thermochemotherapy [1-hexylcarbamoyl-5-fluorouracil (HCFU) combined with hyperthermia]: a basic study on the most effective timing and sequence in vivo.

1-Hexylcarbamoyl-5-fluorouracil (HCFU) is known to be hydrolyzed to 5-fluorouracil (5-FU) by heating. We have previously reported the enhancement of the antitumor effects of HCFU when combined with hyperthermia (1). This study aimed at determining the most effective timing schedule for HCFU combined with hyperthermia for clinical application. On the third, 6th, 9th, 12th and 15th day after transplantation, HCFU (80 mg/kg) was administered and hyperthermia induced. The antitumor effect was evaluated by the measurement of tumor volume. 5-FU concentration in the tumor tissue was analyzed on the 12th day after transplantation after administering 150 mg/kg HCFU, with or without hyperthermia (43.5 degrees C, 45 min). 5-FU concentration in serum and other tissues (kidney, liver and submandibular gland) were also analyzed. The concentration was measured by a high performance liquid chromatograph (HPLC) following Mori et al 's method (2). An enhanced antitumor effect and a significant increase in 5-FU concentration in tumor tissue were observed in post-heated groups of mice as compared with pre-heated groups and groups given HCFU alone. The results showed that HCFU given 1 to 2 hours prior to hyperthermia was the most effective treatment.

The effect of ulinastatin on cutaneous microcirculation during inhalation of 100% oxygen in a rabbit ear chamber.

This study aims to investigate the effects of ulinastatin, a human urinary trypsin inhibitor, on the cutaneous microcirculation during inhalation of 100% oxygen in the rabbit. Twenty-one rabbits having a rabbit ear chamber (REC), were divided into 2 groups: 11 rabbits without ulinastain treatment (Group C) and 10 rabbits with ulinastatin (Group U). After air inhalation, the inspired gas was changed to 100% oxygen. In Group U, 50,000 units of ulinastatin were administered before changing the concentration of inspired gas. The results suggests that ulinastatin may be useful for maintaining the cutaneous microcirculation during inhalation of 100% oxygen.

Non-X-linked hyper-IgM syndrome with systemic lupus erythematosus.

A 24-year-old woman had suffered from recurrent bacterial infections and clinical manifestations of systemic lupus erythematosus (SLE). Laboratory findings disclosed an elevated level of serum IgM, markedly decreased IgG, IgA, IgD and IgE levels, and low levels of serum complement. Both the CD40 and CD40 ligands appeared to be normally expressed. Assays of in vitro immunoglobulin production by lymphocytes showed that IgM was produced normally and that IgE but not IgG or IgA production was rescued by signaling through CD40 on B cells. The proliferative response of lymphocytes to phobol ester was markedly decreased, suggesting some impairment of signal transduction in the patient's lymphocytes.

Shikonin, an ingredient of Lithospermum erythrorhizon, inhibits angiogenesis in vivo and in vitro.

Angiogenesis is critical for tumor growth and inflammation. Shiunko is a Chinese herbal ointment used for the treatment of burns in Japan. Its main ingredient is the root of Lithospermum erythrorhizon, which had been used for treating tumors and inflammation in China since the 5th century. We report here that shikonin, the main chemical ingredient of L. erythrorhizon is a novel inhibitor of angiogenesis. It inhibited tumor necrosis factor-alpha-induced and B16 melanoma-induced angiogenesis in mice and normal developmental angiogenesis in the yolk-sac membranes of chick embryos. Shikonin also inhibited proliferation and migration of endothelial cells in culture and network formation by endothelial cells on Matrigel in vitro. The dose-responsive study suggests that the mechanism of this inhibitory effect on angiogenesis involves the prevention of network formation by endothelial cells via blocking integrin alpha v beta 3 expression.

Effect of inhalation of nitrous oxide on rabbit ear chamber microvessels.

The purpose of this study is to investigate the effect of inhalation of nitrous oxide (N2O) on peripheral hemodynamics in vivo. We observed changes in the peripheral microcirculation before and during inhalation of 70% N2O -30% O2 for 2 hours, using rabbit ear chamber method in 16 rabbits. In summary, inhalation of N2O causes decrease of the diameters, blood-flow velocities, and blood-flow rates of arterioles; thus participating in the increase of systemic vascular resistance gradually during the first 5 minutes of N2O inhalation. This phenomenon (arteriole vasoconstriction) was maintained for 2 hours.

The effects of omeprazole, a proton pump inhibitor, on early gastric stagnation after a pylorus-preserving pancreaticoduodenectomy: results of a randomized study.

BACKGROUND/AIMS: To determine the effects of Omeprazole, a proton pump inhibitor (PPI), on gastric stasis following a pylorus-preserving pancreatico-duodenectomy (PPPD) by means of a randomized trial of PPPD patients. METHODOLOGY: Forty-two PPPD patients were randomly divided into two groups: Group 1 (n=24) received a PPI through a jejunal tube after PPPD, whereas Group 2 (n=18), serving as controls, received a saline solution through a jejunal tube and no medication after a PPPD. The daily volume and total acidity of the gastric juice, aspirated via nasogastric tube, were measured each day for 7 days following PPPD. RESULTS: In Group 1 the mean daily aspirated volume of gastric juice was 160.2 ml, and the mean maximum volume was 222.8 ml on the first postoperative day. In Group 2, six patients were withdrawn from this study for therapy on the third or fourth postoperative day due to gastric bleeding and/or a large amount of excreted gastric juice (in excess of 2,000 ml). The mean daily aspirated volume of gastric juice in the remaining Group 2 patients was 787.4 ml, and the mean maximum volume was 1,039 ml on the third postoperative day. Gastric secretion was significantly lower in Group 1 (p<0.05). Further, the total acidity of the gastric juice was significantly lower in Group 1 than in Group 2 for each of the 7 postoperative days (p<0.05). CONCLUSIONS: These results indicate that postoperative administration of a PPI significantly suppresses the volume and acidity of the gastric juice after PPPD.
