Proliferation of double-negative (CD4-CD8-) T cells bearing T-cell receptor-alpha beta in a haemophiliac with human immunodeficiency virus type 1 infection and factor VIII inhibitor: functional properties of double-negative T-cell receptor-alpha beta+ T cells.

We present a patient with haemophilia A showing human immunodeficiency virus type 1 (HIV-1) infection and factor VIII inhibitor in whom a novel T-cell subpopulation, double-negative (CD4-CD8-) T cells bearing T-cell receptor (TCR)-alpha beta, proliferated polyclonally in the peripheral blood. An interleukin-2-dependent T-cell line with a CD4-CD8-TCR-alpha beta+ phenotype was established from the peripheral blood lymphocytes of the patient, and its biological functions were studied. It was found that the CD4-CD8-TCR-alpha beta+ T cells possessed both HLA-unrestricted cytotoxicity and helper function for immunoglobulin production by B cells. In addition, these T cells were found to produce interferon-gamma and interleukin-2 following activation via CD3-TCR complexes. These data demonstrating the multifunction of these newly defined CD4-CD8-TCR-alpha beta+ T cells thus suggest that these cells play an important role in protection against HIV infection. The mechanism of production of factor VIII inhibitor in the present case is also discussed focusing on the CD4-CD8-TCR-alpha beta+ T cells.

Symptoms and social adjustment of schizophrenic patients as evaluated by their family members: a cross-cultural psychiatric study between offshore islands and an urban city.

We conducted a comparative study of symptoms and social adjustment of schizophrenic patients on offshore islands and one urban region in Japan. In the evaluation of symptoms and social behavior by family members, no differences were observed between the two regions. Looking at social adjustment in the two regions, no differences were observed by family members either in the performance level or in the expectation level of socially expected activities. However, on the offshore islands, the expectation level of socially expected activities by family members was significantly higher than the level of self-accomplishment set by schizophrenic patients themselves. As for leisure activities, both family members and patients on the islands evaluated the level of performance to be significantly higher than in the urban region, and the level of satisfaction of the patients themselves was also higher on the islands. Considering both these findings and Japanese traditional views of work and leisure, it is believed that on the islands patients are required to engage in more productive activities and less leisure activities than in the urban region. Further it seems that patients on the islands compared to patients in the urban region may have greater difficulty in social adjustment.

Pharmacokinetics of FK-506, a novel immunosuppressant, after intravenous and oral administrations to rats.

A pharmacokinetic study of FK-506 (FK), a novel immunosuppressant being about hundred times more potent than cyclosporin A (CyA) in the in vitro experiment, has been performed in rats after intravenous and oral administrations at two doses, 1.0 and 5.0 mg/kg. As compared with CyA, plasma, bile, urine and lymph FK levels were determined by a fluorescence high-performance liquid chromatographic method with chemiluminescence detection. Non-compartment pharmacokinetic parameters were calculated by area/moment analysis. After the i.v. injection of FK at 1.0 and 5.0 mg/kg, the total plasma clearance, CLtot, was 7.028 +/- 0.076 (mean +/- S.E.) and 7.651 +/- 0.755 ml/min, steady-state distribution volume, Vd.ss, was 4623 +/- 28 and 4201 +/- 529 ml/kg, elimination half-life at the terminal elimination phase, t1/2 beta, was 2.36 +/- 0.25 and 2.54 +/- 0.29 h, and the volume of the initial distribution space, V1, was 1336 +/- 150 and 1065 +/- 115 ml/kg, respectively. By comparing the pharmacokinetic parameter with CyA, the CLtot of FK is about three times greater than that of CyA. There is not a significant difference on t1/2 beta between CyA and FK. The V1 and Vd.ss of FK are 4-5 times greater than that of CyA. Therefore, higher clearance of FK is ascribed not only to the faster elimination from the rat body but also to the greater distribution space in the body. The mean percentage of FK transferred into the thoracic lymphatics over 6 h were 0.09 +/- 0.02% (1.0 mg/kg) and 0.16 +/- 0.02% (5.0 mg/kg), respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of pH, dietary proteins and trypsin inhibitors on the hydrolytic rate of human granulocyte colony-stimulating factor (G-CSF) by rat digestive enzymes.

The effects of pH, dietary proteins and trypsin inhibitors on the hydrolytic rate of recombinant human granulocyte colony-stimulating factor (G-CSF) by digestive enzymes were studied by in vitro incubation experiments. The bile obtained by cannulation into the rat common bile duct and the eluates obtained by infusing distilled water into the gastrointestinal tract were used as sources of digestive enzymes. Some proteins including dietary proteins such as casein and ovalbumin, and two kinds of trypsin inhibitors, chicken egg white and soybean ones, were used to examine the protective action on the hydrolysis of G-CSF by digestive enzymes. With an experiment using the digestive enzyme fluids obtained after centrifugation of bile by ultra-filters with a molecular weight (Mr) cut off of 30000, 10000 and 5000, the proteolytic activity to G-CSF decreased as the cut off Mr decreased. The enzyme solution obtained with a membrane with a Mr cut off of 5000 still had an enzyme activity against G-CSF. The protease activity was mainly ascribed to the pancreatic fluid, but the hepatic bile still had an enzyme activity. The hydrolytic rate of G-CSF was dependent on the pH of the enzyme fluid, and the intestinal fluid. The hydrolytic rate of G-CSF was studied at pHs of 1.68, 4.01, 6.86 and 9.18. Especially, as the pH decreased to 1.68, the hydrolytic rate of G-CSF considerably decreased. Some proteins including dietary proteins also affected the hydrolytic rate of G-CSF. The strength of the inhibitory effect of the proteins is the following order; ovalbumin greater than casein greater than mucin greater than keratin.(ABSTRACT TRUNCATED AT 250 WORDS)

Reticuloendothelial system-potentiating and alkaline phosphatase-inducing activities of Plantago-mucilage A, the main mucilage from the seed of Plantago asiatica, and its five modification products.

Five kinds of chemically modified products were prepared from Plantago-mucilage A, the representative mucous polysaccharide isolated from the seed of Plantago asiatica L., and their reticuloendothelial system-potentiating and alkaline phosphatase-inducing activities have been investigated. Both activities were markedly enhanced when the mucilage was de-O-acetylated. The products obtained by periodate oxidation, controlled Smith degradation, and partial acid hydrolysis under the two conditions were not effective. Structural features of the partial hydrolyzates were elucidated, and it was shown that these products lost all O-acetyl groups, all xylose branches and many hexuronosyl arabinose side chains.

Changes in convulsion susceptibility of lidocaine by alteration of brain catecholaminergic functions.

Influences of the manipulation of brain catecholaminergic neuronal activity on the incidence of lidocaine-induced convulsions in mice were studied and compared with those of pentylenetetrazol (PTZ)-induced convulsions. alpha-Methyl-p-tyrosine (alpha-MPT) decreased both brain noradrenaline (NA) and dopamine (DA) levels, and disulfiram decreased the NA level and increased the DA level. The incidence of lidocaine-induced convulsions was decreased by treatments with alpha-MPT and disulfiram, while that of PTZ was increased by either treatment. The incidence of lidocaine-induced convulsions was slightly, but not significantly increased by L-dihydroxyphenylalanine (L-DOPA), although the brain DA level was increased by L-DOPA. Methamphetamine and desipramine increased the incidences of lidocaine-induced convulsions. These results may suggest that brain catecholaminergic neurons, differing from their role in inhibiting control of PTZ-seizure, act to facilitate lidocaine-induced convulsions.

Isolation and purification of bacteriocin from Prevotella intermedia (Bacteroides intermedius).

Thirty-three out of 46 strains of Prevotella intermedia (Bacteroides intermedius) isolated from gingival deposits of patients with adult periodontitis possessed the ability to inhibit growth of P. intermedia strain ATCC 25611, and 6 of the 33 showed strong inhibitory activity. Thirteen isolates did not have the ability to inhibit the growth of P. intermedia strain ATCC 25611. An inhibition factor from one of the isolates, P. intermedia TH14 which had strong inhibition, was purified and characterized. Bacteriocin TH14 activity was effective against only P. intermedia ATCC 25611 and Fusobacterium nucleatum, but not against other crevicular bacteria, including P. intermedia ATCC 33563, Porphyromonas gingivalis (Bacteroides gingivalis), Capnocytophaga sputigena and Actinobacillus actinomycetemcomitans. Streptococci, actinomyces, and veillonellas were not inhibited by bacteriocin TH14. Bacteriocin TH14 was heat sensitive, proteinaceous with molecular weight of 1,700 daltons and inhibited the growth of strain ATCC 25611 in 15 minutes. The growth inhibition of P. intermedia ATCC 25611 by bacteriocin TH14 was found to be due to the bactericidal activity of the bacteriocin.

Coexistence of intracranial and spinal meningiomas--report of two cases.

The authors report two rare cases of multiple meningiomas in both the intracranial and spinal regions. A 64-year-old female presented with a right sphenoidal ridge meningioma and a cervical extramedullary meningioma. Tumor histology was transitional and vacuolated types, respectively. The tumors were removed successfully in two stages, craniotomy then laminectomy 3 months later. A 62-year-old female presented with a right sphenoidal ridge meningioma (meningotheliomatous type) which was totally removed. An extramedullary spinal meningioma became symptomatic 33 months later, which was also removed totally. The meningiomas in the first case had different subtypes, but immunohistochemical characteristics including microcyst formation were similar. The second case had meningiomas of the same subtype with similar characteristics, but different fibrous septum development. Multiple meningiomas, even in different compartments of the central nervous system, may have common characteristics.

[Combined chronic toxicity/carcinogenicity test of tris(2-chloroethyl)phosphate (TCEP) applied to female mouse skin].

Tris(2-chloroethyl)phosphate (TCEP), a widely used flame retardant, was tested for its skin chronic toxicity/carcinogenicity using female Slc: ddY mice. TCEP (5 and 50%) dissolved in ethanol was applied to the shaved skin twice a week for 79 weeks. The control group received ethanol under similar condition. In addition, 5 animals in each group were killed at 6 and 12 months and used for the chronic toxicity study. In body weight, food consumption and survival rate, there was no significant difference between the control and treated groups. Spleen weight was decreased in the 50% group. No significant difference in the incidence of tumors and other non-neoplastic lesions of the skin and other organs was found between the control and treated groups. The results indicate that under the conditions of the present study, TCEP has no carcinogenicity and toxicity for the skin.

[Acute and subacute toxicity studies of Bis(2,3-dibromopropyl) phosphate magnesium in rat].

Acute and subacute oral toxicity tests of Bis(2,3-dibromopropyl) phosphate magnesium (Bis-BP.Mg) were carried out in Wistar rats. In the acute toxicity test, Bis-BP.Mg suspended in arabic gum was administered orally to a group consisting of 10 male and 10 female rats, and they were observed for 14 days. LD50 values of male and female rats were 283 (253 approximately 314) mg/kg and 261 (219 approximately 310) mg/kg, respectively. As toxic symptoms, eyelid closure, crouching, shivering and staggering gait were observed in the treated groups of both sexes. In gross findings, hypertrophy, discoloration and necrotic change of the liver, and hypertrophy and discoloration of the kidney were observed in the treated groups. In histopathological examination, necrosis, desquamation, large nuclei formation of the tubular epithelium, and tubular dilatation of the kidney and necrosis of the liver cells were observed in the treated groups. In the subacute toxicity test, groups of rats consisting of 5 males and 5 females were fed a commercial diet containing 0, 30, 100, 300 and 1000 ppm Bis-BP.Mg for 45 days. In body weight and food consumption, there were no significant difference between the control and treated groups. Significant increases were observed in the liver and kidney weights of male rats fed 1000 ppm Bis-BP.Mg. Histopathologically, desquamation, swelling, and large nuclei formation of the tubular epithelium and tubular dilatation of the kidney were observed, but they were much less frequent than those in the acute toxicity test. It was concluded that Bis-BP.Mg has apparent renal toxicity.

[Therapeutic efficacy of imipenem/cilastatin sodium on respiratory tract infections in lung cancer patients].

Imipenem/cilastatin sodium (IPM/CS) was used to treat respiratory tract infections (RTI) in 54 patients with lung cancer. Out of the 54 patients studied, 53 were evaluable for the utility of IPM/CS; 42 had pneumonia, 9 had obstructive pneumonia, 1 had a lung abscess and 1 had acute bronchitis. The efficacy rate was 71.7%. Seventeen causative organisms were isolated from 14 patients. They included Staphylococcus aureus 5 strains, Staphylococcus epidermidis 4 strains, Staphylococcus sp. 2 strains, Enterococcus faecalis 1 strain, Pseudomonas aeruginosa 2 strains, Pseudomonas fluorescens 2 strains, Acinetobacter sp. 1 strain, and the eradication rate was 81.8%. Clinical adverse effects (nausea and vomiting) were observed in 1 patient. Abnormalities in laboratory test results were observed in 3 patients. They disappeared or returned to normal values after completion of therapy or discontinuation of IPM/CS administration. IPM/CS appears to be a useful antibiotic for RTI in patients with lung cancer.

[Flow chart of the diagnosis and treatment of a case of catamenial pneumothorax].

A 44-year-old female had sudden cough during menstruation. She visited a clinic and mild right pneumothorax was pointed out on chest X-ray. She was admitted to our hospital for continuous pneumothorax and tube drainage was performed. Although pneumothorax improved within a week after drainage, catamenial pneumothorax was suspected because her serum CA 125 showed a high value of 109 U/ml. Therefore diagnostic pneumoperitoneum was performed during the next menstruation, and right pneumothorax occurred. Laparoscopy revealed pelvic endometriosis and thoracoscopy revealed a few small holes in the right diaphragm. The case was diagnosed as catamenial pneumothorax and hormonal treatment was given. She had been well for one year without recurrence of pneumothorax. The authors made a flow chart for diagnosis and treatment based on this case and discussed its value in relation to cases described in the literature.

Lymphatics and pre-lymphatics of the rabbit pericardium and epicardium with special emphasis on particulate absorption and milky spot-like structures.

The lymphatics and pre-lymphatic connective tissue of rabbit pericardium and epicardium were examined by light and electron microscopy under normal conditions and after the injection of India ink and latex particles into the pericardial cavity. A characteristic lattice structure of connective tissue was present between the small mesothelial cells and the submesothelial lymphatic capillaries in the basal region of the pericardium, but not in the epicardium. Milky spot-like structures bulging toward the pericardial cavity were found in the pericardium, similar to those in the omentum and mediastinal pleura. Within 60 minutes after injection, carbon and latex particles were directly absorbed through the intercellular clefts of the adjacent small mesothelial cells into the submesothelial layer particularly at sites of characteristic lattice structure. Carbon particles were already present in the lumens of lymphatic capillaries at this time. Macrophages in the pericardial cavity and submesothelial layers of the pericardium engulfed both carbon and latex particles. Our results suggest two possible routes of drainage of particulate matter from the pericardial cavity into the lymphatics: direct absorption and indirect absorption after phagocytosis by macrophages. Macrophages probably migrate from the milky spot-like structures described in this study. Epicardial lymphatics, in contrast, drain tissue fluid primarily from the myocardium.

[Application of Y-chromosome specific DNA analysis for detection of mixed hematopoietic chimerism after allogeneic bone marrow transplantation].

We report a case involving mixed hematopoietic chimerism after an allogeneic bone marrow transplantation (BMT) from a sex mismatched donor. A 31 year-old-man who was diagnosed as having chronic myelogenous leukemia in the accelerated phase received an allogenic BMT from his HLA-identical sister in March, 1989. To determine the mixed chimerism we used the Y-chromosome specific repeated sequence of DNA using a specific probe (PHY 10). The donor's DNA 3.5 kb band appeared in 1-10% of male DNA by Southern blot hybridization in the peripheral blood 21 days after BMT. The Y-chromosome DNA band decreased day by day, and disappeared 110 days after BMT. The Y-chromosome DNA band could be detected, even though few metaphases were obtained immediately after BMT. Thus this method is very sensitive for determining which cells contain the Y-chromosome, and is therefore useful for detecting mixed chimerism after sex-mismatched BMT. Using this method the clinical significance of mixed chimerism can be assessed.

[Blastic form of acute erythremia: report of an autopsy case].

We report an autopsy case of acute erythremia which terminated in generalized infiltration of immature blasts similar to proerythroblasts. A 61-year-old man was admitted because of general fatigue and fever in June, 1990. Mild anemia and severe thrombocytopenia were noted. The bone marrow was hypocellular with 25.5% blasts similar to proerythroblasts and 36.5% erythroblasts, many of which were polynuclear and megaloblastoid. The blasts were cytochemically negative for POX, but positive for PAS staining. Therefore he was diagnosed as having acute erythremia. Partial remission was achieved by BHAC-EV therapy. But three months later, his marrow was replaced by 52.7% blasts as seen in admission. Those blasts were negative for lymphoid, myelocytic, megakaryocytic markers and antiglycophorin A, but positive for OKT 9. Electron microscopy revealed that some of blasts had characteristics of immature erythroblasts. In spite of low dose Ara-C therapy, he died of sudden gastrointestinal bleeding in December, 1990. The autopsy disclosed widespread infiltration of blasts, involving liver, spleen, lung, kidney and stomach. It was interesting that dysplasia had been confined to erythroid lineage throughout his clinical course. He seemed to be a rare case of blastic form of acute erythremia which should be distinguished from erythroleukemia.

[Effect of L-carnitine in patients with ischemic heart disease].

To assess the protective effects of L-carnitine (LC) infusion on ischemic heart disease, 30 patients who had angina and ischemic ECG changes during exercise were evaluated by bicycle ergometry. They were categorized in LC and non-treatment (NT) groups. There were no significant differences in age and sex between the 2 groups. Before exercise, 15 patients (9 males and 6 females) received 60 mg/kg LC and the results including hemodynamics, coronary circulation, and cardiac metabolism at rest and during exercise were compared with those of the NT group studied in the same protocol (50 watts x to cycle, 15 min). At the end of 30 min LC drip infusion, the arterial carnitine content (LC (a)) reached 1,980 +/- 257.3 microM and then was maintained at 1,212.7 +/- 136.2 microM during exercise. There was no correlation of LC (a) with the coronary arterio-venous difference nor with myocardial uptake of LC. Although there was no significant difference in coronary blood flow (CBF: mliters/100 g/min) between the LC and NT groups at rest (LC: 92.1 +/- 29.0 vs NT: 88.0 +/- 26.5), CBF during exercise increased significantly in the LC group compared with the NT group (LC: 230.4 +/- 113.8 vs NT: 139.1 +/- 52.7; p < 0.05). In the NT group, there was no significant change in coronary arterio-venous oxygen difference ((a-cs) O2: vol %) during exercise, but in the LC group (a-cs) O2 increased significantly from 10.2 +/- 1.3 to 11.5 +/- 1.9 (p < 0.01). Furthermore, although there was no significant difference in myocardial oxygen consumption (MVO2: mliters/100 g/min) at rest between the 2 groups (LC: 9.30 +/- 2.96 vs NT: 9.71 +/- 3.09), it increased significantly in the LC group compared with the NT group during exercise (LC: 25.11 +/- 9.98 vs NT: 15.55 +/- 6.09). MVO2/LVWI (LVWI = left ventricular work index) and MVO2MT (MT = myocardial tension) did not significantly differ at rest between the 2 groups. However, these 2 indices decreased significantly during exercise (p < 0.05) in the NT group, and remained unchanged in the LC group, showing a significant difference between the 2 groups (both p < 0.05). In myocardial energy substrates, the myocardial uptake ((a-cs) x CBF) of free fatty acid (FFA: muEq/100 g/min) increased significantly in the LC group compared with that of the NT group (LC: 10.16 +/- 13.26-->31.88 +/- 27.58* vs NT: 16.02 +/- 27.92-->18.11 +/- 31.00;* = p < 0.05, LC vs NT).
