Comparative studies on fine-needle aspiration cytology with ultrasound scanning in the assessment of thyroid nodule.

The clinical usefulness of ultrasound scanning in the assessment of thyroid nodule was examined. The total diagnostic accuracy for aspiration biopsy cytology (ABC) in the differentiation of benign from malignant thyroid nodules was 92.4% in 26 patients, which is similar to that of previous reports when compared with the findings of pathological examination by surgery. Additionally, the diagnostic efficacy of high resolution ultrasonography was evaluated based on the results of ABC. When the echogram of 90 patients with a thyroid nodule was divided into eight patterns according to Obara's classification, only 64.3% of the carcinoma exhibited the typical malignant pattern of ultrasonography and 21.4% of the carcinoma exhibited the ultrasonographically homogenous nodule with a clear margin, which was often observed in the benign nodules. Therefore, it is necessary to be very careful when differentiating malignant from benign thyroid nodules by ultrasonography. In conclusion, although high resolution ultrasonography provides useful information for the assessment of most thyroid nodules, all ultrasonography (including high resolution ultrasonography) should be combined with ABC for the final diagnosis.

The heterogeneous expression of MAGE-3 protein: difference between primary lesions and metastatic lymph nodes in gastric carcinoma.

We evaluated the expression of MAGE-3 protein in gastric carcinoma by immunohistochemistry, and compared the frequency between primary lesions and metastatic lymph nodes. Primary lesions and metastatic lymph nodes of 38 cases of gastric carcinoma were analyzed using anti-MAGE-3 monoclonal antibody. Expression of MAGE-3 protein was detected in 17 (45%) primary lesions and 12 (32%) metastatic lymph nodes of 38 cases. In 70% of positive cases of primary lesions, the percentage of stained cells with MAGE-3 decreased in metastatic lymph nodes. The heterogeneous expression of MAGE-3 should be considered when we perform antigen specific immunotherapy for patients with gastric carcinoma.

Potential role of heparin in prevention of liver metastasis from colon cancer.

The case of a 59 year-old man with sigmoid colon cancer and synchronous liver metastases is described in this report. Sigmoid colectomy and partial hepatectomy were performed, and hepatic arterial cannulation was done for prevention of hepatic recurrence. Heparin was injected to prevent catheter-related clots, and no anticancer drugs were used. He did well without signs of recurrence for 5 years after the initial operation. After we stopped the heparin administration, recurrence was detected in the liver. The patient underwent repeat hepatectomy, and he is now doing well without recurrence 2 years after the second operation. The clinical course of this case suggests that heparin may prevent liver metastasis of colorectal cancer.

Postoperative complications of repeat hepatectomy for liver metastasis from colorectal carcinoma.

BACKGROUND/AIMS: When a repeat hepatectomy is possible, it is the most effective treatment modality for recurrent colorectal liver metastasis. The aim of this study was to evaluate the surgical risks of repeat hepatectomy for liver metastasis from colorectal carcinoma. METHODOLOGY: Between 1986 and 1996, 60 patients with hepatic metastasis from colorectal carcinoma underwent surgery in the Department of Surgery I, Oita Medical University. Ten of them underwent a repeat hepatectomy. The cases of these 10 patients were studied retrospectively; in particular, postoperative complications and intraoperative blood loss were compared between the initial and second operation. RESULTS: During the second surgery, recurrence was detected adjacent to the hepatic stump in 9 of the 10 patients. During the initial surgery, 6 underwent non-anatomic resections, and 4 had anatomic resection, including 1 extended lobectomy, 1 lobectomy, and 2 segmentectomies. For the second surgery, 3 had anatomic resections, including 2 lobectomies, and 1 segmentectomy, and 7 underwent non-anatomic resections. There were no mortalities during the initial or second operation. There was no morbidity following the initial surgeries and 7 postoperative complications (intraabdominal abscess, 4 cases; biloma, 3 cases) following the second surgeries. Mean blood loss during the second operation (1044 mL) was significantly greater than during the initial operation (561 mL). CONCLUSIONS: The present results show that repeat hepatectomy for recurrent liver metastasis from colorectal carcinoma resulted in significantly greater intraoperative blood loss and postoperative complications than those of the initial surgeries. The blood loss and complications in the second operation, the one for the recurrence, were directly associated with the fact that the recurrence was so close to the hepatic stump. Since the resection line in the second surgery was adjacent to the hepatic hilus, resection of the lesion caused much more injury to the main bile duct and main portal vein than that caused by the.

Systemic lupus erythematosus relapse with lupus cystitis.

A 45-year-old woman who had been diagnosed as having systemic lupus erythematosus (SLE) at the age of 28 years and who had been in remission developed severe urinary frequency, watery diarrhea, vomiting and weight loss. She also developed acute renal failure and her serological examination was consistent with active SLE. She had a markedly decreased urinary bladder capacity of 20 ml with hydroureteronephrosis. Histopathological study of her urinary bladder biopsy specimen showed mucosal edema, infiltration by lymphocytes and granulocytes, and deposition of IgA in the epithelium and submucosal region. We diagnosed this as a case of lupus cystitis. The patient's symptoms were alleviated by bilateral nephrostomy and corticosteroid therapy. In the present episode the patient showed none of the usual symptoms of SLE. This case and others reported in the literature show that lupus cystitis presents with specific signs and symptoms and therefore, this syndrome may represent a specific clinical manifestation of SLE.
