Extensive maxillary sinus pneumatization.

AIM: To determine the incidence of variations of maxillary sinus pneumatization especially when it is extensive and the associated anomalies. STUDY DESIGN: Two hundred consecutive direct coronal paranasal sinus computed tomography (CT) scans were reviewed retrospectively. Extensive maxillary sinus pneumatization (EMSP) was defined as one in which the largest horizontal and/or vertical dimension of the maxillary sinus equalled or exceeded 90% of the corresponding diameter of the orbit. Further subtype I, II & III were defined depending on whether the pneumatization was extensive in one dimension (horizontal or vertical), two dimensions (horizontal & vertical) and by the presence of sphenomaxillary plate, intermaxillary plate or extension into frontal recess. RESULTS: EMSP was found in 8%, of these 7% were bilateral and 1% was unilateral. (Subtype I, II and III constituting 1%, 3% & 4% respectively). CONCLUSION: EMSP has been defined as a group and a classification proposed. EMSP will result in an atypical clinical picture, has a role in the pathogenesis of frontal sinusitis in some cases and may predispose injury to the orbit during endoscopic sinus surgery (ESS).

Carotico-cavernous fistula following septorhinoplasty.

Septorhinoplasty is a very common operation in otolaryngological practice. We report the second case of a carotico-cavernous fistula following septorhinoplasty. This case presented with very severe epistaxis before the appearance of the typical pulsating exophthalmos, ophthalmoplegia, headache and engorged veins on the right side of the face. Our case was treated by endovascular thrombosis with electrolytically detachable coils.

Utilisation pattern and managerial issues--a study of emergency medicine department at Nizam's Institute of Medical Sciences, Hyderabad.

The emergency medicine department (EMD) is one of the youngest department in the Nizam's Institute of Medical Sciences (NIMS). The study was conducted for a period of six months from it's inception, since November 01st 1992. The physical facilities, staffing, policies and procedures, work load and cost analysis were studied, and conclusions and suggestions were given. Total patients attended at EMD in the six months were 6,884, out of whom 3,484 were admitted into the hospital for further treatment. Cost analysis showed a net gain of Rs. 78.80 per patient.

Hibernoma in the parotid region.

Hibernoma is a benign tumour, arising from vestigial remnants of brown adipose cells, which have persisted from embryonic life. Although it is a well recognized entity, it is an extremely rare tumour. Recent reports estimate 81 cases in the world literature, only seven being in the neck. To our knowledge, it has never been reported in the parotid region. We report such a case and discuss the historical background of a hibernoma, along with its pathological and clinical behaviour.

Primary radiotherapy for treating all laryngeal cancer: a district hospital experience.

We report our experience of long-term follow-up of all patients treated during a ten-year period with primary radiotherapy irrespective of the disease stage of their laryngeal carcinoma. This policy was adopted in order to allow all patients the opportunity to save their larynx and to preserve their voice. In the present literature, surgery is favoured for treating advanced disease.
