[A case of cerebrospinal fluid otorrhea 6 years after temporal bone fracture].

A 16 year-old female was admitted to our hospital because of left temporal bone fracture and brain contusion in the left temporal lobe. CSF otorrhea was also present for a week. Four years later, when she was 20 years old, she was noticed to have a white mass in the left external auditory canal. One and a half year later, left CSF otorrhea reappeared, and she was readmitted to our hospital. High resolution coronal CT revealed that the left temporal bone fracture had opened widely and brain tissue had herniated into the left external auditory canal. Metrizamide CT cisternography showed that CSF was leaking around the mass. Operative findings have shown that there were two dural defects, both of which were 2 cm in diameter, and brain tissue surrounded by arachnoid membrane had herniated through these defect. The herniated brain tissue was removed, and both dural- and osteo-plasty were successfully carried out. This case indicates that dissociation of temporal bone fracture and laceration of dura mater may cause herniation of brain tissue and CSF otorrhea years later.

Left atrial myxoma with an unusual tumor vascularity demonstrated by angiography--report of two cases.

We describe two cases of left atrial myxoma, demonstrating an unusual tumor vascularity as revealed by coronary angiograms. Both cases had suffered typical episodes of transient ischemic attack (TIA). Coronary angiography revealed tumor blood supply from coronary arteries in both cases, and also a leakage of contrast medium from the surface of the tumor blood vessels to the left atrium in one case. In the medical literature available in English reviewed so far, no report of such a leakage has been reported. Selective coronary arteriography can be a useful diagnostic method of delineating left atrial myxoma and its blood supply.

Antibody produced against isolated Rh(D) polypeptide reacts with other Rh-related antigens.

Rh(D) antigen-containing polypeptide was prepared by immune precipitation of intact cDE/cDE erythrocytes by using a high-titer preparation of polyclonal anti-D. when isolated Rh(D) polypeptide was administered to rabbits, antibody was produced that was unresponsive toward Rh-positive and -negative cells but reacted strongly with the immunogen in enzyme-linked immunosorbent assay-type immunobinding and Western blot immunostaining assays. Rabbit antibody also immunostains isolated Rh(c) polypeptide as well as the Rh antigen-containing components of sodium dodecyl sulfate-polyacrylamide gel electrophoresis-separated membrane proteins from Rh(D)-positive (cDE/cDE,CDe/CDe), Rh(D)-negative (cde/cde,Cde/Cde), and -D-/-D- cells. It does not react with any membrane protein from Rh-null regulator type cells, thus indicating a specificity for Rh-related proteins. We have also been able to demonstrate that polyclonal and monoclonal anti-D preparations that do not immunostain isolated Rh(D) polypeptide will react with it in our immunobinding assay.

Isolation of ketoses or nonreducing sugars from a sugar mixture containing aldoses by aniline treatment.

A method for the isolation of ketoses and nonreducing sugars from sugar mixtures by the addition of aniline, in the presence of acetic acid as a catalyst, is described. The aldoses react readily to form aniline derivatives, which are then removed using standard procedures: extraction with organic solvents, and treatment with activated charcoal and with a cation-exchange resin. The ketoses and nonreducing sugars are retained in aqueous solution.

Plasma amino acid patterns in hepatocellular carcinoma.

Plasma amino acid levels were determined in 23 patients in comparison with 16 normal subjects and 17 patients with liver cirrhosis. Patients with hepatocellular carcinoma had elevated levels of the aromatic amino acids and lowered levels of the branched-chain amino acids, as seen in liver cirrhosis; however, they had lowered levels of alanine and glutamine as compared with normal subjects and with liver cirrhosis patients. Following treatment with intraarterial chemotherapy and/or transcatheter arterial embolization, plasma levels of alanine and glutamine recovered. These results suggest that the consumption of alanine and glutamine increase in hepatocellular carcinoma.

Effects of murine tumor necrosis factor on Friend erythroleukemic cells.

Tumor necrosis inducing factor (TNF), a 140,000 molecular weight glycoprotein present in the serum of Corynebacterium parvum endotoxin-treated mice, was cytotoxic toward Friend virus-transformed erythroleukemic cells (FELC). These cells grow in culture as undifferentiated pro-erythroblasts but can be induced to differentiate in a limited fashion along the erythroid pathway to orthochromatic normoblasts by various agents such as dimethylsulfoxide (DMSO). Partially and highly purified preparations of TNF were cytotoxic toward logarithmically growing FELC whereas a comparable serum protein fraction from C. parvum treated mice or endotoxin from E. coli had no effect upon FELC viability. DMSO-induced cells were more sensitive to the action of TNF requiring only about half the concentration needed to produce 50% kill in noninduced cells. Inhibition of hemoglobin formation was TNF dose-related and could be decreased by 94%. TNF was also cytotoxic toward DMSO-induced cells in stationary phase and mitomycin C treated noninduced FELC. Neuraminidase modification of the surface of FELC increased the cytotoxicity of TNF by 50%. These results demonstrate that TNF destroys FELC whether they are nondividing, dividing or partially differentiated and suggest that TNF may accomplish this by affecting cell metabolism after internalization.

Antitumor activity of normal human globulin: effects on murine and human erythroleukemic cells.

The effect of the antitumor fraction isolated from the alpha 2-globulin region of normal human serum (NHG-I) upon murine (FELC) and human (K562) erythroleukemic cells in vitro was determined. NHG-I inhibited the growth of both actively growing FELC and K562 cells in a dose-dependent manner. However, it has no effect upon mitomycin C treated FELC which were unable to divide nor upon dimethylsulfoxide-induced FELC in stationary phase. These results indicate that NHG-I has a cytostatic effect upon cell growth and suggests that its action may be dependent upon DNA synthesis. This is in marked contrast to TNF, the alpha 2-globulin factor obtained from murine serum which is also not species-specific but whose action upon these cell lines is cytotoxic.

[Intra-arterial infusion chemotherapy using a subcutaneously implanted silicone reservoir--with reference to the chemotherapy protocol].

Sixty patients with advanced cancers were treated with intraarterial infusion chemotherapy using a subcutaneously implanted silicone reservoir. Forty-five patients received a low dose-intermittent intraarterial infusion chemotherapy employing either combination MFC (MMC 4 mg, 5-FU 500 mg, Ara-C 40 mg) at intervals of two weeks or MMC 4 mg weekly. The results were as follows. (1) Among 30 cases, where objective evaluation of the initial response was possible, partial responses (PR) were observed in 15 cases (50%). (2) No severe complication was observed using this protocol. Only one case out of the 45 needed to discontinue the medication because of bone marrow suppression. (3) Among 28 cases, where follow-up data were available, the average hospital-free interval was 207 days. The average ratio of hospital-free interval to survival period was 56.4%. The results indicate that our method is beneficial as a maintenance chemotherapy for patients with advanced cancers, because it is effective, safer and improves the quality of life.

[Intra-arterial infusion chemotherapy using a subcutaneously implanted silicone reservoir--with special reference to the method].

Sixty patients with advanced cancers were treated with intraarterial infusion chemotherapy using a subcutaneously implanted silicone reservoir. Forty-nine patients were catheterized from a branch of the left subclavian artery. The results were as follows. (1) In spite of some technical difficulties, catheterization from a branch of the left subclavian artery was minimally invasive, and catheter trouble using this route was less frequent than that occurring when the profound femoral arterial route was employed. (2) Implantation of the catheter end using a silicone reservoir liberated the patients from restrictions in their daily lives, and with this method, intraarterial infusion chemotherapy could be carried out easily when required without hospitalization. We conclude that this procedure is a beneficial method of intraarterial infusion chemotherapy for patients with advanced cancers.

Association of HLA-B40 and DRW9 with Japanese alcoholic liver cirrhosis.

Seventy-seven chronic alcoholics with liver disease were studied to evaluate the HLA antigen association. There were no significant differences of HLA antigen phenotype frequencies (PF) between the patients and controls regarding A and C loci, (62 healthy Japanese). Prevalences of HLA-B40 complex (B40 . 48 . 13) and DRW9 tended to increase among chronic alcoholics. When chronic alcoholics were divided according to whether they had liver cirrhosis or not, the cirrhosis group (42 cases) revealed a significantly higher frequency of HLA-DRW9 (chi 2 = 10.88, p less than 0.001, corrected p less than 0.05, relative risk (R.R.) = 4.17) as compared to controls. There was also a tendency of B40 complex to increase in frequency (chi 2 = 5.51, p less than 0.05, R.R. = 2.65) in the cirrhosis group. Haplotype frequency and linkage disequilibrium parameters of HLA-B40 . 48-DRW9 were significantly higher than those of controls. Moreover, the increased frequency of DRW9 in the cirrhosis group was similar to that in autoimmune disease like ulcerative colitis or SLE. These data suggest that HLA-DRW9 and/or HLA-B40-DRW9 might be closely associated with susceptibility to developing alcoholic cirrhosis and that autoimmune mechanisms might be involved partly in its etiology.

Enzymatic modification of the surface carbohydrates of Friend erythroleukemic cells.

Surface carbohydrates of Friend erythroleukemic-cells were modified by treatment with the exoglycosidases, alpha-galactosidase, beta-galactosidase, and neuraminidase without affecting cell growth and viability either in the presence of absence of 1.8% DMSO as inducer. When cells were incubated with a combination of alpha-galactosidase and neuraminidase and then induced, they showed an increased rate of differentiation as measured by the formation of benzidine-positive cells. These enzymes used singly, or beta-galactosidase treatment alone, or in combination with neuraminidase, did not change the rate of differentiation. Cell-surface labeling and electrophoretic separation of the glycoconjugates revealed that two regions of approximate molecular weights of 195,000 and 185,000 were neuraminidase-sensitive and one other of molecular weight of about 75,000 was sensitive to alpha-galactosidase. Both untreated and the combined alpha-galactosidase, neuraminidase-modified cells exhibited the same rate of uptake of carbon-14 DMSO, ruling out the possibility that the observed increased rate of differentiation was due to faster penetration of DMSO into enzyme-treated cells. On the other hand, the decrease in the rate of uptake of rubidium-86, an analogue of K+, by treated-induced cells was significantly enhanced over that observed with untreated-induced cells, suggesting that alpha-galactosidase plus neuraminidase modification of the cell surface was affecting at least one of the early events occurring in the Friend erythroleukemic cell differentiation program.

[A method to retain subcutaneously the catheter end connected to a new silicone reservoir in intraarterial infusion chemotherapy].

A method to retain subcutaneously the catheter end connected to a new silicone reservoir was developed in intraarterial infusion chemotherapy and applied to 12 patients. Its advantages were discussed. The method can be performed with only minor invasion; in addition, the retained catheter and reservoir needs no aseptic care. Accordingly, it liberates patients from restrictions in the daily life thus far experienced in the conventional method. The intraarterial infusion therapy can be done more easily and without hospitalization by this method. Thus, the method is considered to be quite beneficial in taking care of advanced cancer patients.
