Myelopathy, ascites and pleural effusion in systemic lupus erythematosus.

A 12-year-old girl with transverse myelopathy, massive pleural effusion and ascites as the presenting features of systemic lupus erythematosus (SLE), is described. All these features developed within a week, but other manifestations of SLE such as malar rash and arthritis were not seen during the illness. Investigations revealed positive direct Coombs' test, high titer of antinuclear antibodies, elevation of serum anti-DNA antibodies, depressed complement activity, thrombocytopenia and proteinuria, indicating that she had SLE. A week after starting treatment with prednisolone 35 mg/day, the thrombocytopenia improved, and following an increase in dosage to 60 mg/day the pleural effusion and ascites diminished after two weeks, and the serological abnormalities improved after two months. The neurological disturbances were unchanged in spite of injections of methylprednisolone 1 g/day for three days from the 89th day of her illness. Review of other cases reported to be improved neurologically, suggested that the earlier initiation of corticosteroids in higher dosage might be beneficial in transverse myelopathy in SLE.

[Assessment of occupational exposures to industrial hazardous substances. III. On the frequency distribution of daily exposure averages (8-h TWA)].

A method based on interday fluctuation of contaminant concentrations for evaluating employee's exposure averages (8-h TWAs) was proposed in our previous report. The method was established on the assumption that daily exposure averages of the workers are lognormally distributed in actual workplaces. The study was conducted to elucidate whether the distribution of daily exposure averages is statistically lognormal or not and to examine the relationship between sample geometric standard deviation (sg) of worker's daily exposure averages and its estimate (sg2) calculated by measurements for two consecutive days. These are critical for our proposed method. The data on daily exposure concentrations over five to six weeks were collected from workers exposed to cobalt, acetone, n-hexane, toluene, xylene, ethylbenzene and ethylacetate. The data on organic lead, inorganic lead and mercury exposures reported by Cope et al. and Lindstedt et al. were also used for the study. The result can be summarized as follows: 1. Lognormal distribution of daily exposure averages was confirmed by plotting on normal probability paper and chi 2-test. 2. Median of sg2 on daily exposure averages obtained from individual worker was smaller than sg. 3. sg calculated by a set of measurements for two consecutive days in every worker can be corrected by the equation: (sg2) 1.48 in obtaining a better estimate of sigma g. 4. Statistical analysis on daily exposure averages of all workers showed that median of sg2 was smaller than that of sg, and 88% upper limit of sg2 was equal to that of sg. 5. Therefore, in evaluating TWA obtained by only single day's measurement using the proposed method, median of sg representative of industrial hazardous substance exposure workplaces could be also corrected by the equation described above. However, correction of 90% upper limit of sg2 is not necessary.

[Assessment of occupational exposures to industrial hazardous substances. IV. A proposed method based on one-sided tolerance limits].

Occupational exposures to potentially hazardous substances may vary considerably because of interday environmental behavioral fluctuations in the contaminant concentration. Such occupational exposures including those of non-monitored days can be theoretically evaluated by the following three ways: 1) assessment of geometric mean and geometric standard deviation, 2) assessment of arithmetic mean, and 3) assessment of upper limits of daily exposure distribution. In our previous report, an evaluation method on 95% upper limit or arithmetic mean of exposures was proposed. The method is useful, particularly, in case where only one or two days are being monitored, but may provide an approximate estimate because of statistical assumption. A sampling and decision scheme using one-sided tolerance limits (OTL) proposed by Tuggle (1982) can precisely evaluate the upper limits of exposures. However, many cases would be evaluated as "no decision," unless the sample size is extremely large in number. We developed a revised method based on OTL for assessment of occupational exposures. The characteristic features of this method can be summarized as follows: 1. Upper limits of lognormally distributed 8-h exposure concentrations can be evaluated in comparison with an established standard. 2. A third OTL factor was introduced into Tuggle's scheme in which two OTL factors were used. A comparison between the upper limits of exposures and the standard can be made at 50% confidence level with the factor. The factor was calculated using non-central t-distribution. 3. The usefulness of the third OTL factor in the assessment of occupational exposures was confirmed by examining the performance characteristics of the method.(ABSTRACT TRUNCATED AT 250 WORDS)

Functional atrial natriuretic peptide receptor in human adrenal tumor.

The effects of synthetic human atrial natriuretic peptide (ANP) on the release of catecholamines, aldosterone, or cortisol were observed in human adrenal tumors obtained surgically from patients with pheochromocytoma, primary aldosteronism, or Cushing's syndrome, respectively. Each tumor tissue or adjacent normal cortical tissue was sectioned into slices, which were incubated in medium-199 in the presence or absence of adrenocorticotrophin (ACTH) and ANP. The amounts of epinephrine, norepinephrine, aldosterone, or cortisol released into the medium were measured. Existence of ANP receptors on the adrenal tissues was examined by binding assays, affinity labeling, and immunohistochemistry. Release of catecholamines from pheochromocytoma tissues was inhibited by ANP, and the presence of the ANP receptor on pheochromocytoma was further demonstrated by both binding assays and affinity labeling; Scatchard analysis revealed a single class of binding sites for ANP with a Kd of 1.0 nM and a Bmax of 0.4 pmol/mg of protein and the molecular size was estimated as 140 and a 70 kDa under nonreducing and reducing conditions, respectively. The presence of ANP receptors in pheochromocytoma was demonstrated by immunohistochemistry. ANP inhibited both basal and ACTH-stimulated aldosterone secretion in the slices of normal cortex, and localization of ANP receptors in zona glomerulosa cells was also demonstrated. However, ANP did not inhibit basal and ACTH-stimulated aldosterone and cortisol secretion in both tissue slices from aldosteronoma and Cushing's adenoma. Consistent with these observations, the absence of ANP receptors in adenoma tissues was determined by binding assays, affinity labeling, and immunohistochemistry.(ABSTRACT TRUNCATED AT 250 WORDS)

[PVB therapy for advanced testicular cancer].

Twelve cases of advanced testicular cancer, including 5 cases of seminoma, 3 cases of teratocarcinoma, 1 case of yolk sac tumor, 1 case of embryonal carcinoma and 2 cases of mixed cell type, were treated with cisplatin-vinblastine-bleomycin (PVB) therapy. Among them, 10 cases had measurable metastatic lesions and the objective response rate was 80%. Three cases showed complete response. Ten cases showed nonexistent disease after PVB therapy and salvage operation. Though PVB therapy was useful for the treatment of advanced testicular cancer, a few cases having poor prognostic factors showed no response to the therapy.

[Electrophoretic analysis of urinary glycoproteins in diabetic nephropathy using peroxidase-lectins].

We examined the clinical usefulness determined by polyacrylamide gel electrophoresis, followed by reaction with peroxidase-coupled lectins using urinary glycoproteins for diabetic nephropathy in 20 patients with diabetes mellitus. Lectins used were Triticum vulgaris (WGA), Phaseolus vulgaris (PHA-E4), Dolichos biflorus (DBA), and Lens culinaris (LCA), which have high affinity for beta 1----4N-acetyl-D-glucosamine (GlcNAc beta 1----4GlcNAc), N-acetyl-D-galactosamine (GalNAc), alpha-galactosamine (alpha-GalNAc), and alpha-mannose (alpha-Man) residues, respectively. Electrophoretic patterns of urinary glycoproteins clearly showed the presence of lectin-reactive glycoproteins with molecular weights lower than that of albumin. The molecular weight of the main bands reacted with WGA, PHA-E4 or LCA were 50,000 and 38,000, and increased with the progress of diabetic nephropathy. WGA reacted strongly with many glycoproteins having a wide range of molecular weights. LCA and PHA-E4 reacted preferentially with glycoproteins of molecular weights glycoproteins of molecular weights lower than 50,000, but no reaction was observed by DBA. These results suggest that low molecular urinary glycoproteins have abundant carbohydrate residues such as GlcNAc beta 1----4GlcNAc, GalNAc, and alpha-Man. The excretion of low molecular weight glycoproteins with high affinities for some lectins suggests functional impairment in diabetic nephropathy.

Chronopharmacological study of bunazosin hydrochloride in healthy subjects.

Bunazosin, an alpha-1 adrenoreceptor blocking agent, was given orally to six healthy subjects in the morning or in the evening in order to examine the time-dependent variations in the pharmacological effects of the drug. The study was carried out on four occasions in a placebo controlled cross-over design. No significant change was observed in blood pressure either after the morning or the evening dosage. Finger skin blood flow (FSBF) assessed by laser Doppler flowmetry increased significantly after the morning dosage, but not after the evening trial. The plasma concentration of bunazosin in the morning in general had a tendency to be greater than that in the evening. A significant correlation was observed between the plasma concentrations and the increments in the FSBF. These results indicate that the pharmacological effects of bunazosin are greater following morning dosage. The present study supports the concept that the time-dependent differences in the effect of bunazosin are, at least, caused by the time-dependent changes in plasma drug concentration.

[A case of retroperitoneal neurinoma].

A case of retroperitoneal neurinoma is reported. The patient was a 34-year-old female without symptoms. CT scan showed a large low density mass, a part of which was enhanced. US showed a lobulated cystic mass. Angiography showed avascular mass and portal vein was shown with displacement and stenosis. Pathological diagnosis was benign retroperitoneal neurinoma (Antoni A and Antoni B).

[Barbiturate therapy in traumatic cerebral vascular disease: report of two cases].

We report two cases of traumatic cerebral vascular disease which were treated successfully with barbiturate. The first case sustained blunt trauma to the bilateral vertebral arteries, resulting in complete occlusion of both arteries. After ligation of the injured vertebral arteries, multiple cerebral infarction appeared. Cerebral angiography revealed dissection and stenosis of the bilateral internal carotid arteries. We treated this case with barbiturate (Thiamylal) in combination with administration of heparin. The second case sustained cerebral contusion and traumatic subarachnoidal hemorrhage as a result of a motor cycle accident. This patient deteriorated and cerebral angiography showed diffuse cerebral arterial vasospasms. When this was treated with induced hypertension, he developed recurrent subarachnoid hemorrhage. In order to protect the brain from ischemia without elevating blood pressure, we employed barbiturate therapy and the patient recovered without major neurological deficit. The condition of severe head injury with cerebral ischemia is complicated. Therefore it has been hard for neurosurgeons to cure the patient with this condition. But we treated it with barbiturate successfully. Barbiturate therapy in severe head injury with cerebral ischemia may decrease the mortality in that group of patients considered difficult to treat with the usual therapeutic modalities.

[Detection of precancerous lesions of the bladder by contact-micro-cystoscopy].

Contact-micro-cystoscopy is a new method to examine the bladder epithelium by micro-endoscope after staining. According to this method, normal epithelium was stained slightly and showed smooth surface and small, sparse nuclei. Neoplastic lesions were stained deeply and showed irregular surface and large, dense nuclei. One hundred and sixty nine biopsy specimens of cystoscopically normal bladder mucosa were analyzed. The diagnostic accuracy of neoplastic lesions by this method was 75.9%. In vivo mapping of the bladder epithelium was also possible using this method in association with random biopsy. Contact-micro-cystoscopy was thought to be useful to detect precancerous lesions or flat carcinoma of the bladder which was difficult to be detected by conventional cystoscopy.

[Evaluation of rapid injection immunotherapy by antigen specific IgE and IgG4 antibodies].

Ten children, aged 3 to 12 years old (mean 8), with house-dust-mite sensitive bronchial asthma were hospitalized and administered a course of rapid injection immunotherapy with house-dust (HD) antigen. In order to evaluate rapid injection immunotherapy, we examined total IgE, anti-HD IgE and IgG4 antibodies, anti-mite (Dermatophagoides farinae: DF) IgE antibodies, and anti-DF IgG4 antibodies in their sera serially from 56 to 80 weeks after rapid injection. The results were as follows: 1) All of their clinical courses were markedly improved by this immunotherapy. 2) Total IgE levels were unchanged. 3) Anti-HD IgE antibodies were decreased 5 weeks after rapid administration (p less than 0.01). And anti-DF IgE antibodies were also decreased (p less than 0.05). 4) Anti-DF IgG4 antibodies were increased more than twofold from 2 to 5 weeks after rapid administration (p less than 0.02). Rapid injection immunotherapy produced positive effects in some of the asthmatic children. This seemed to be attributable to the production of antigen specific IgG4 antibodies (said to be blocking antibodies) and the reduction of antigen specific IgE antibodies.

[Aphasia and size of hematoma in patients with left putaminal hemorrhage].

Presence or absence of aphasic syndrome and its duration were related to the volume of hematoma calculated from CT in 27 consecutive non-surgical cases with left putaminal hemorrhage. In the present study, patients classified into three groups regardless of the types of aphasia; i.e. non-aphasics, transient aphasics and persistent aphasics. Transient aphasics were arbitrarily defined as cases who had presented with more or less evident aphasic syndrome in the acute stage, but almost completely recovered in the chronic stage. Volume of hematoma in non-aphasic patients ranged from 2 to 17 ml, with a mean of 7.7 ml and those in transient cases were from 11 to 23ml with a mean of 17ml. All but one case with persistent aphasia showed the hematoma volume larger than 25ml. This exceptional case was a 83 year-old female who showed marked cortical atrophy on CT. Consequently, the presence of aphasia and its persistence seem to be largely dependent on the size of hematoma in patients with left putaminal hemorrhage. Persistent aphasia may be caused by organic damage of subcortical white matter, when the volume of hematoma exceeds around 25ml.

Reconstruction of neglected Achilles tendon rupture with Marlex mesh.

Reconstruction of the neglected Achilles tendon rupture was performed with marlex mesh on six patients. Marlex mesh was folded into three layers and sandwiched between both ruptured ends that were divided horizontally into two layers in moderate tension. All of the patients showed satisfactory function with a mean follow-up period of 3 years (range, 2.4-3.7 years). The operative procedure is simple because there is no need to harvest healthy tendon or fascia as the donor and there is less adhesion with neighboring tissues and minimal signs of foreign-body reactions.
