Effects of parathyroidectomy on left ventricular mass in patients with hyperparathyroidism.

The effects of parathyroidectomy on left ventricular muscle volume and mechanical performance were evaluated echocardiographically in 24 patients with secondary hyperparathyroidism due to chronic renal failure and in 7 with primary hyperparathyroidism. Intraventricular septum and posterior wall thickness, left ventricular end-diastolic diameters, shortening fraction, ejection fraction, and left ventricular mass index were measured by M mode recording by the parasternal short axis view prior to parathyroidectomy as baseline and repeated 12 months after parathyroidectomy. Serum basal carboxyterminal parathyroid hormone levels in patients with secondary hyperparathyroidism (34.4 +/- 13.7 ng/ml) were significantly higher than in those with primary hyperparathyroidism (3.4 +/- 5.1 ng/ml; p < 0.0001). At 12 months after parathyroidectomy, intraventricular septum and posterior wall thickness, left ventricular end-diastolic diameter and left ventricular mass index were reduced from 11.8 +/- 3.1 mm, 10.9 +/- 1.7 mm, 53.8 +/- 6.3 mm, 200.8 +/- 57.1 g/m2 to 10.0 +/- 2.1 mm (p < 0.05), 9.8 +/- 1.9 mm (p < 0.05), 50.7 +/- 7.2 mm (p < 0.05), 149.6 +/- 38.7 g/m2 (p < 0.0001), in patients with secondary hyperparathyroidism. In patients with primary hyperparathyroidism, all echocardiographic parameters remained in the normal range and did not show any significant changes before or after parathyroidectomy. From this study, parathyroid hormone at extremely high concentrations as seen in secondary hyperparathyroidism appears to be a cardiotoxic substance. Therefore, all patients with secondary hyperthyroidism should be examined by echocardiography and parathyroidectomy should be considered if myocardial hypertrophy is present.

[The role of emergent endoscopic retrograde pancreatography (ERP): its usefulness in the diagnosis of pancreatic injury].

To determine the therapeutic modalities for pancreatic injury (PI), it is important for the pancreatic ductal injury (DI) to be present or not. We performed ERPs in 36 patients who had positive findings in physical examinations, serum amylase levels and CT within about 13 hours after injury to diagnose the DIs early after injury and to avoid negative laparotomy. In 33 successful ERP patients (intraoperatively in 3), 22 had PIs (14 DIs) and 11 had non-PIs. Of 14 patients with DIs, 12 with main DIs and one with branch injury were treated surgically. Another patient with branch injury, who was treated conservatively, died of the complications of PI. Nineteen patients without DIs, including 8 PIs, were treated conservatively or operated for the associated abdominal injuries, who had no complications on PIs in the hospital days. Three unsuccessful ERP patients, having PIs, were also operated upon. Among 15 patients, including these 3 patients and 12 treated surgically during the era before introducing ERPs, 2 (13.3%) negative laparotomies were noted. Among 16 patients, who underwent both of ERP and operation, no negative laparotomies were noted. No complications of ERPs occurred. In conclusion, ERP is a reliable modality to detect DIs and to determine the therapeutic modalities for PIs.

[Clinical evaluation of 99mTc-MAG3 as a radiotracer to assess the function of transplanted kidney: comparison with 123I-OIH].

99mTc-MAG3 (mercaptacetyl glycilglycilglycine) is a newly developed radiopharmaceutical for the functional imaging of kidneys. We evaluated the clinical usefulness of 99mTc-MAG3 in 12 cases (7 men and 5 women, aged from 18 to 57 years old) with transplanted kidney, and compared it with 123I-OIH (o-iodohippurate). Immediately after bolus injection of 300 MBq of 99mTc-MAG3, first pass images (1 frame/3 sec for 1 min) and dynamic renal images (1 frame/60 sec for 30 min) were recorded. In 10 of 12 cases, the same examination was carried out using 37 MBq of 123I-OIH within 2 weeks. Because of its larger dose and suitable gamma-ray energy, 99mTc-MAG3 provided much better images than 123I-OIH, particulary among first pass images. Effective renal plasma flow (ERPF) calculated from the clearance of 99mTc-MAG3 was less (about 70%) than that of 123I-OIH, however, a strong correlation (r = 0.98, p < 0.001) was observed between the ERPF values of the two radiotracers. No adverse effects were caused by 99mTc-MAG3. Our data suggested that 99mTc-MAG3 was a promising radiopharmaceutical with which to evaluate regional dynamic renal function.

[Evaluation of surgical treatment for bone metastatic lesion in renal cell carcinoma].

Ten renal cell carcinoma (RCC) patients with bone metastatic lesions were treated in the Kyoto Prefectural University of Medicine between January, 1977, and December, 1992. Surgical treatment for the metastatic lesion was performed on 4 patients, while conservative treatment was performed on the other 6 patients. Surgical treatment made the patients painless consecutively, resulting in higher activities of daily life. However, recurrence of RCC developed in all the cases between 2 and 7 years later. Surgical treatment for the metastatic lesion was considered to be only a palliative therapy for RCC with bone metastasis and the indication should be limited to patients expected to have a good prognosis.

[Changes in neuropsychological functions following cardiovascular surgery].

This study was designed to elucidate if there is a potential hazard of developing neuropsychological deterioration after major cardiovascular surgery. Neuropsychological functions were studied in 49 patients before and after cardiac (coronary artery bypass, cardiac valve replacement, etc.) and thoracic aortic surgery (aortic aneurysm). Neuropsychological examinations using 5 batteries (Mini-Mental State Examination, Cross Cultural Cognitive Examination, Miyake's Verbal Memory Test, Benton Visual Retention Test and Wisconsin Card Sorting Test) were performed before surgery and subsequently at 3 weeks and 8 months after. Twenty-five of 49 patients (51%) showed a significant deterioration at least in one of the test batteries. There was no significant relation between age or educational levels and post-surgical impairment of cognitive function. The impairment of cognitive function was found mainly in the "memory function". Follow up examinations were carried out on 31 of the 49 patients 8 months after surgery. Among these patients, 13 patients did not show any changes in cognitive function throughout the study period. Eleven of the 16 patients who developed a decrease in cognitive function for a certain period after surgery (3 weeks), recovered to the same level at the follow-up examination that it was prior to the surgery. Cognitive function in five patients did not improve and remained at the decreased level. The other two patients whose function was normal at the first post-surgical examination, were found to have developed impairment of cognitive function at the follow up. Although the patients had some deficits on their neuropsychological function, they scarcely complained about any problems in their daily life.(ABSTRACT TRUNCATED AT 250 WORDS)

[Benign hemorrhagic renal cyst diagnosed by endoscopic biopsy: a case report].

A case of benign renal hemorrhagic cyst in a 73-year-old man is reported. Ultrasonography, computed tomography and magnetic resonance imaging revealed a solid mass in the left renal cyst. Percutaneous puncture of left renal cyst was carried out followed by endoscopic biopsy of the intracystic masses. Specimens showed no malignancy. As we could not rule out malignancy, left partial nephrectomy was performed. The surgical specimen was free from malignancy by pathological examination. Thus endoscopic biopsy of intracystic tumor indicated the correct diagnosis.

[Relationship between retained microbubbles and neuropsychologic alterations after cardiac operation].

We studied the relationship between quantity of microbubble retained in the left heart and neuropsychologic alterations after surgery in 21 patients undergoing cardiac surgery including cardiopulmonary bypass. The neuropsychologic change was evaluated by three kinds of psychological test, which mainly analyzed memory and cognition. The microbubble was continuously monitored by the long axis view of the descending aorta of transesophageal echocardiography and then quantitatively analyzed and graded by the on-line computer. More microbubbles were detected in the valve surgery requiring the intracardiac procedure than in coronary artery bypass grafting and neuropsychologic deterioration, although the relationship did not reach statistical significance. Since most of the microbubbles were detected during the unclamping of aorta and the weaning from cardiopulmonary bypass, a technical improvement of the surgical procedures could reduce them. We think that transesophageal echocardiography is useful for monitoring microbubbles during operation.

Heterogeneous responsiveness of the in situ rat vascular endothelial cells to mechanical stretching in vitro.

En face endothelial cell (EC) sheets from various vessels of the rat vascular tree were stained with rhodamine-phalloidin to examine the response of the ECs to mechanical stretching, especially from the viewpoint of actin stress fiber (SF) dynamics. Arterial ECs were classified into three types according to the distribution pattern of their SFs before and after stimulation. In both ECs with few SFs from arteries near the heart and those having many SFs from arteries distributed in the uterus, ovary or kidney, new SF formation was not induced by stretching. ECs with various amounts of SFs from the abdominal aorta to the femoral artery developed many SFs in response to stretching. On the other hand, the response of venous ECs with initially few SFs was generally low, although active SF formation was observed in ECs from the uterine vein, portal vein and anterior facial vein, which are normally subjected to higher levels of mechanical stress in situ. The amplitude of stretching necessary for SF formation in ECs was about 40% in both vascular systems, although the time necessary for SF expression was 10 min and 30 to 40 min in arterial and venous ECs, respectively. A rapid increase in the number of ECs with SFs for a given amplituding of stretching suggests the presence of a threshold-like value in this reaction system. It was clarified that notable heterogeneity of ECs in their response to mechanical stress exists among the arterial and venous trees.

Histamine depletion in brain caused by treatment with (S)alpha-fluoromethylhistidine enhances ischemic damage of gerbil hippocampal CA2 neurons.

The effect of (S)alpha-fluoromethylhistidine (FMH), a specific inhibitor of histamine synthesis from histidine, on ischemic damage was examined in gerbil brain after forebrain ischemia. Two h after subcutaneous FMH injection, the histamine content of the brain was significantly reduced. Neuronal loss in the CA2 region of the hippocampus 7 days after 3 min ischemia was enhanced by treatment with FMH. These results indicate that depletion of brain histamine aggravates neuronal death of hippocampal CA2 neurons after 3 min ischemia.

Potent stimulatory effect of interleukin-1 alpha on apolipoprotein D and gross cystic disease fluid protein-15 expression in human breast-cancer cells.

To better understand the multiple hormonal control of the expression of apolipoprotein D (apo-D) and gross cystic disease fluid protein-15 (GCDFP-15, also designated prolactin-inducible protein), which are 2 major proteins found in benign breast-disease fluid, we investigated their regulation by interleukin-1 alpha (IL-1 alpha) in the presence or absence of steroid hormones in ZR-75-1 human breast cancer cells. Exposure of these cells to IL-1 alpha decreased basal cell proliferation by half and markedly reduced the mitogenic action of 17 beta-estradiol (E2), the half-maximal inhibitory effect being exerted at 1.5 pM. In parallel, IL-1 alpha stimulated apo-D and GCDFP-15 secretion with a similar potency. The antiproliferative effect of IL-1 alpha was additive to the inhibition of cell proliferation caused by dihydrotestosterone (DHT) or the glucocorticoid dexamethasone (DEX). In parallel, IL-1 alpha-induced stimulation of apo-D and GCDFP-15 secretion was additive to that exerted by DHT or DEX. The sensitivity of the apo-D and GCDFP-15 responses to the stimulatory action of DHT or DEX was not changed by the presence of IL-1 alpha. IL-1 alpha also increased apo-D and GCDFP-15 mRNA levels. The present findings demonstrate the potent stimulatory effect of IL-1 alpha on basal as well as androgen- and glucocorticoid-induced apo-D and GCDFP-15 expression. The present data strongly suggest that IL-1 alpha and steroids may modulate the secretion of these 2 proteins through different transduction pathways.

Functional domain structure of human centromere protein B. Implication of the internal and C-terminal self-association domains in centromeric heterochromatin condensation.

Centromere protein B (CENP-B) is a common centromere DNA-binding protein among mammalian centromeres. CENP-B possesses the specific DNA binding activity to the 17-base pair sequence dispersed in centromeric repetitive DNA sequences. In the previous study, we have shown that its DNA-binding domain exists within the N-terminal 134 amino acid residues. Here, to clarify the whole domain structure, another functional unit required for CENP-B self-association was examined. Recombinant CENP-B was expressed in Escherichia coli. First, a chemical cross-linking reagent, disuccinimidyl suberate, was used to fix the physical association without losing the DNA-binding activity. The complexes with the same molecular weight as homodimer and trimer were identified after a separation by SDS-polyacrylamide gel electrophoresis. With a series of CENP-B deletion constructs, the area responsible for this oligomer formation was located at the internal region. Second, an electrophoretic mobility shift assay was also used to survey the minimum regions required for the CENP-B self-association. Three separate elements were identified by assaying the capacity to form the additional slow-migrating complex, two in the internal region and one in the C terminus. These results suggest that CENP-B molecules interact with each other at the multiple sites to fold the centromeric DNA repeats into a heterochromatin structure.

DNA polymerase alpha associated protein P1, a murine homolog of yeast MCM3, changes its intranuclear distribution during the DNA synthetic period.

We isolated a murine gene for the DNA polymerase alpha associated protein P1, which shares high homology with the budding yeast MCM3 protein, which is a member of a protein family involved in the early event of DNA replication having a putative DNA-dependent ATPase motif. Using a polyclonal anti-P1 antibody raised against a beta-galactosidase-P1 fusion protein, we identified at least two forms of P1 protein in the nucleus of a mouse cell line, an underphosphorylated form that was associated with a particular nuclear structure and a hyperphosphorylated form loosely bound to the nucleus. During progression through S phase, P1 disappeared, first from the euchromatic region, then from the heterochromatic region, apparently in parallel with temporally ordered DNA replication. Thus, it is likely that the underphosphorylated P1 is dissociated from the nuclear structure after DNA replication by cell cycle-dependent phosphorylation. This is the first direct observation of a protein whose behavior is consistent with that of a hypothetical factor which restricts the chromatin to replicate once per cell cycle in higher eukaryotes.

[CT evaluation of extravascular perfusion of contrast medium and its potential to a new method of diagnosis: an experimental study using macro, micro-molecular contrast media].

To evaluate the dynamics of extravascular perfusion, dynamic CT with two different molecular sized contrast media was performed on VX2 tumor of rabbit. The first dynamic CT was performed with a bolus injection of iopamidol (IP:120 mgI/ml, 5 ml). After ascertaining that the tumor attenuation had returned to the pre-contrast level, the second dynamic CT was performed on the same slice with bolus injection of iodoethylated starch (IES:120 mgI/ml). The time-density (T-D) curves of the same tumor area on the images obtained by two contrast media were compared. The T-D curve with IP showed definitely higher level than that with IES. This occurrence can be explained that IP, 13 A in size, has higher permeability distributing not only in the intravascular space, but also into the extravascular space. On the other hand, IES, 200 A in size, will stay mostly in the intravascular space. From this, we consider that the attenuation difference between the two curves will be an indicator for the dynamics of extravascular perfusion, suggesting to become a new method for CT diagnosis.

[Experimental study on vascular permeability and hemodynamics of nonionic dimeric contrast media (Iotrolan): a comparison to nonionic monomer on dynamic CT and angiography].

The difference of vascular permeability and hemodynamics between Iotrolan and Iopamidol was experimentally studied on dynamic CT and angiography. A hemodialyzer with 60 A pores were applied as a phantom of a target organ with capillaries and extravascular space. Dynamic CT scans and DSA of the phantom were performed with injection of each contrast medium and time-density curves obtained from several regions of interest were compared. The dynamic CT scans performed at 40 seconds after the completion of injection, showed that Iopamidol was mostly distributed at the site of inlet, while Iotrolan was mostly at the center of the phantom. Time-density curves on DSA with Iotrolan revealed lower and shorter peak density and time than did Iopamidol. In conclusion, Iotrolan has lower permeability than that of Iopamidol because of its larger molecular size. Iotrolan has slower flow through catheters and capillaries than that of Iopamidol because of its higher viscosity. One should be aware the difference between the two contrast media and be careful to the analysis of time-density curves of dynamic CT and DSA.

MSG5, a novel protein phosphatase promotes adaptation to pheromone response in S. cerevisiae.

Pheromone-stimulated yeast cells and haploid gpa1 deletion mutants arrest their cell cycle in G1. Overexpression of a novel gene called MSG5 suppresses this inhibition of cell division. Loss of MSG5 function leads to a diminished adaptive response to pheromone. Genetic analysis indicates that MSG5 acts at a stage where the protein kinases STE7 and FUS3 function to transmit the pheromone-induced signal. Since loss of MSG5 function causes an increase in FUS3 enzyme activity but not STE7 activity, we propose that MSG5 impinges on the pathway at FUS3. Sequence analysis suggests that MSG5 encodes a protein tyrosine phosphatase. This is supported by the finding that recombinant MSG5 has phosphatase activity in vitro and is able to inactivate autophosphorylated FUS3. Thus MSG5 might stimulate recovery from pheromone by regulating the phosphorylation state of FUS3.

Induction of bcl-2 gene expression by intercellular information from hemopoietic supportive stromal cells to DA-1 cells.

When interleukin-3 (IL-3) dependent DA-1 cells were cultured on hemopoietic supportive stromal cells (MS-5), DA-1 cells survived and proliferated in the absence of detectable IL-3. Although IL-3 was not produced by the MS-5 cells, their production of granulocyte-macrophage colony-stimulating factor (GM-CSF) was increased when they were co-cultured with DA-1 cells. This suggests that DA-1 cells transmit signals to stromal cells that enhance growth factor(s) production. Expression of bcl-2 by DA-1 cells was induced when they were co-cultured with MS-5 cells, suggesting that DA-1 cells express bcl-2 strongly in response to a signal produced by MS-5 cells. These data indicate the existence of a two-way interaction between DA-1 cells and hemopoietic supportive stromal cells.
