[Clinical, cytogenetic and immunological studies of 2 cases with B-cell acute lymphocytic leukemia. Comparison between B-ALL and Burkitt's lymphoma reported in Japan].

Two patients with acute lymphocytic leukemia of B-cell phenotype (B-ALL) are described. They were 77-year-old female and 34-year-old male. One patient presented with marked splenomegaly, and the other with rupture of spleen on admission. Leukemic morphology revealed a typical L3 profile by FAB classification system in both cases. Immunologic analysis showed the presence of surface immunoglobulins in both cases, and one phenotype was IgM kappa, whereas the other was IgG kappa. Cytogenetic study revealed the typical translocations (8; 14) in both cases. Following chemotherapy, complete remission was achieved in one case, but the other died 36 days after admission. Including our two cases, we studied 12 cases of B-ALL and 77 cases of Burkitt's lymphoma reported in Japan, investigating the clinical prognosis as well as the biological features. We concluded that there are no significant difference of survival between B-ALL and Burkitt's lymphoma. We estimated it is due to oncogenesis from the same original cell in despite of the difference in main tumor site between two diseases.

[A case of mesenteric panniculitis of the transverse colon].

A case of mesenteric panniculitis in a 55-year-old female was reported. Image diagnosis of this rare clinical entity, which is characterized by a nonspecific inflammatory process involving the adipose tissue of the mesentery, was considered. We treated this patient conservatively and this led to remission.

[Phenytoin induced agranulocytosis: direct and T cell-mediated mechanisms].

A 57-year-old male was admitted to our hospital because of high fever and dyspnea. He had convulsive attacks after the operation for subarachnoid hemorrhage. He received an anticonvulsant, Hydantol F (Phenytoin + Phenobarbital), for 20 days before hospitalization. The white cell count on admission was 900/microliters without any neutrophils. Pseudomonas was demonstrated from blood cultures. After the anticonvulsant was discontinued and potent antibiotics were administered, his symptoms improved. Neutrophils appeared in the peripheral blood. We investigated the effect of phenytoin on the granuloid precursors using in vitro colony assay (CFU-C) after recovery from agranulocytosis. There was substantial decrease in CFU-C colony numbers when phenytoin or peripheral T-lymphocytes incubated with phenytoin was added in the culture. The addition of sera from acute phase did not affect the colony formation. These results suggested that direct toxic effect of the drug and cellular immunity played an important role in phenytoin induced agranulocytosis.

Cancer superimposed on adult T-cell leukemia.

Forty-three patients with adult T-cell leukemia (ATL), admitted to the hospital from 1982 to 1987, were studied. Five of those were found to have additional cancer; two of the five had two additional kinds of cancers besides ATL. During the same period, in contrast, the authors encountered only three patients with some superimposed cancer among 155 cases of hematologic malignancies which had no association with human T-cell lymphotropic virus type I (HTLV-I). These facts strongly suggest that HTLV-I infection is not only associated with ATL, but it is likely to increase the incidence of other types of malignancy as well.

Differences in prognostic factors between leukemia and lymphoma type of adult T-cell leukemia.

Prognostic factors affecting the survival of adult T-cell leukemia (ATL) patients were analyzed in three groups: total cases, leukemia type cases, and lymphoma type cases. Factors found to be important overall, i.e. for total cases, were leukocyte count, ATL cell ratio in the peripheral blood, serum calcium levels and lactate dehydrogenase (LDH) level. Of those, LDH level proved not significant when evaluated separately for leukemia type or lymphoma type cases. Leukocyte count and ATL cell ratio were significant in leukemia type patients, whereas it was serum calcium level that was significant in lymphoma type; there were mutually exclusive sets of factors for the two groups. Thus, prognostic factors for ATL patients should be considered separately for each type of the disease.

Antigenic variation of human RSV strains isolated in Japan.

Antigenic variations of respiratory syncytial virus (RSV) strains were analyzed using a collection of nine, seven, two, and one monoclonal antibodies (MAbs), respectively, raised against the fusion protein (F), large glycoprotein (G), nucleoprotein (NP), and phosphoprotein (P) components of the Long strain of RSV. Competitive binding assay by these MAbs demonstrated eight, four, and two distinct epitopes on F, G, and NP components, respectively. Comparison of prototype Long with ten field strains isolated in Sapporo, Japan, during a 9-year period from 1980 to 1988 by radioimmunoprecipitation (RIP), immunofluorescence (IF), and enzyme-linked immunosorbent assay (ELISA) test revealed four different patterns of reaction to these MAbs. Thus, prototype Long reacted to all 19 MAbs. Six field strains have shown a different reactivity to one of nine anti-F and to one of seven anti-G antibodies (subgroup A). Three of the remaining isolates failed to react with three of nine anti-F and with all of seven anti-G antibodies (subgroup B). One strain (58-104) isolated in 1983 was similar to subgroup A except for a lack of reaction with two anti-G antibodies. All field strains reacted with two anti-NP and one anti-P antibodies. The numbers of altered epitopes in subgroup A were 1/8 and 1/4; in subgroup B, 3/8 and 4/4; and in 58-104, 1/8 and 2/4 on the F and G components, respectively. No other variations have been observed among field isolates tested.

Development of cell-mediated cytotoxic immunity to respiratory syncytial virus in human infants following naturally acquired infection.

With virus-infected autologous and allogenic mononuclear cells as specific targets, the development of cell-mediated cytotoxic reactivity to respiratory syncytial virus (RSV) was studied in peripheral blood lymphocytes (PBL) in groups of infants with acute RSV infection and in other control groups of subjects during a community outbreak of RSV infection. No RSV-specific cellular cytotoxicity was observed in cord blood lymphocytes and in other uninfected controls. The PBL of infants with acute RSV infection exhibited significant cellular cytotoxic response. The activity peaked early, usually within 1 week after infection. The response appeared to be age-dependent. Over 65% of infants 6-24 months of age and about 35-38% of infants under 5 months of age exhibited cellular cytotoxicity to RSV. Cellular cytotoxic reactivity was observed against autologous and less frequently against allogenic RSV-infected target cells. These findings suggest the appearance of virus-specific cell-mediated cytotoxic immune response after acute RSV infection. The development of cellular cytotoxic responses may play a role in the mechanisms of protection against or the pathogenesis of RSV infection in man.

[Changes of muscle activities in neck and shoulder region during sustained isometric contractions of jaw muscles].

Eight healthy participants performed horizontal isometric jaw movements with maximum voluntary contractions of the jaw muscles until they could not tolerate the task. Electromyographic activities bilaterally recorded from the superior and inferior regions of the sternocleidomastoid muscle and the nuchal and shoulder regions of the trapezius muscle were observed. The results obtained were as follows: 1. In more than 94% of 32 trials, the amplitude of EMGs during the task were higher than those at rest, and in more than 79%, temporal changes in EMG burst were synchronized with the onset-offset of the exercise. 2. In more than 72% of all trials, small amplitude EMG lasted after cessation of the task. 3. Muscle activities of the lateral cervical region during the exercise were superior to the activity of the nuchal and shoulder regions, and amplitude of EMGs in the neck and shoulder regions tended to be the highest during isometric retrusion. 4. These results don't contradict with our hypotheses that occlusal habits might be one cause of the unpleasant sensation in the neck and shoulder regions and that sustained isometric retrusion might have heavier effects on it.

[von Recklinghausen's disease associated with malignant lymphoma].

A 59-year-old man was admitted to the Saga Medical School Hospital in November 1986 because of unconsciousness and lymph node swelling. He had café au leit spots and multiple neurofibromas since his thirties. The leukocyte count was increased. The level of serum LDH and Ca were extremely elevated. His bone marrow aspiration showed the invasion of lymphoma cells. Lymph node biopsy from his left neck and the study of surface markers were performed. And then, his illness was diagnosed as non-Hodgkin's lymphoma (diffuse, medium sized cell type, B cell type, stage IV B.) following von Recklinghausen's disease. After the admission, his general condition improved by chemotherapy, but he had a relapse 6 months later. And he died in November 1987. The case of von Recklinghausen's disease associated with malignant lymphoma in rarely reported. We reviewed the literature and discussed von Recklinghausen's disease associated with malignant lymphoma including this case.

[Acute biphenotypic leukemia followed by two color flow cytometry].

In this paper is reported a case of acute biphenotypic leukemia who was treated by chemotherapy and pursued its effect by two color flow cytometry. A 33-year-old male patient was admitted due to fever and general fatigue and diagnosed as acute leukemia by hematological findings. Surface markers were investigated to find positive reaction of Leu 12 (CD19), J 5 (CD10), My 7 (CD13) and My 9 (CD33), in which Leu 12 and My 9 were simultaneously expressed on the same blast cells by two color flow cytometry. He was treated with daunorubicin, enocitabine, mercaptopurine, vincristine, and prednisolone to obtain partial remission. Then, he was administered L-asparaginase, doxorubicin, vincristine and prednisolone to reach complete remission. The effect of chemotherapy was investigated by not only bone marrow puncture, but also by two color flow cytometry. From the findings in this case, the two flow cytometry was proved to be a useful tool for not only diagnosis of acute mixed leukemia, aut also the judgement of the effect of treatment.

[Acute myeloblastic leukemia associated with chronic thyroiditis].

A 57-year-old woman who suffered from acute myeloblastic leukemia during the course of chronic thyroiditis, is described. The patient was diagnosed as having chronic thyroiditis in 1984 when she was 53 year-old, and was treated with L-T4.Na. She admitted in July 1988 because of general fatigue, fever, cough and sore throat. On admission, hematological examination in the peripheral blood showed marked anemia and increased leukocytes with 20.5% leukemic cells positive for peroxidase staining. Bone marrow aspiration showed 38.8% leukemic cells. She was diagnosed acute myeloblastic leukemia. She reached complete remission after combination chemotherapy. The case of acute myeloblastic leukemia associated with chronic thyroiditis is rarely reported. We reviewed the literature and discussed acute myeloblastic leukemia associated with chronic thyroiditis including this case.

[Urinary cytodiagnostic abnormality in a patient with malignant lymphoma].

A 69-year-old man was admitted due to chronic subdural hematoma. He had lymphadenopathies in the neck. The diagnosis of non-Hodgkin lymphoma (diffuse, large, B cell type) was made by a cervical lymph node biopsy. The renal failure by obstruction of the urinary tract developed gradually, and the tumor cells were found by urinary cytodiagnosis. He was treated immediately with chemotherapy. After the treatment, the urine volume increased and the general conditions improved. It was indicated from this case that the urinary cytodiagnosis is useful for the detection of urinary tract infiltration by lymphoma cell. The urinary cytodiagnostic abnormalities in patients with non-Hodgkin lymphomas were discussed from the literature, including this case.

The relationship between Ga-67 accumulation and cell cycle in malignant tumor cells in vitro.

Previously we reported that the deposition of 67Ga into malignant tumor may be a sensitive index of proliferative activity in tumor cells. For the purpose of elucidation of this hypothesis, we investigated the relationship between the accumulation of 67Ga into malignant tumor cells and the cell cycle in vitro. We discovered that the uptake of both 67Ga and 59Fe into synchronized mouse tumor cells reaches a peak at the G2 stage which precedes cellular proliferation. Both iron and transferrin are specifically required by cells in culture for cell division, and the fact that 67Ga and 59Fe uptake into tumor cells peaks at the G2 stage of the cell cycle suggests that there is a correlation between 67Ga uptake and the rate of cellular proliferation in malignant tumor cells.

Hypothalamic digitalis-like substance is released with sodium-loading in rats.

Role of the hypothalamic digitalis-like substance (EDLS) on the hypertension associated with an excess intake of sodium and the releasing mechanism were investigated. The blood pressure in rats fed with a sodium diet increased significantly after 4 weeks of the treatment compared to the control rats fed with a regular diet, which was accompanied by increased urinary output of the EDLS. Electrical lesions of the AV3V area in the hypothalamus significantly decreased both the urinary EDLS level and the blood pressure elevated by the sodium-loading. With the immunohistochemistry using digoxin antibody, the immunoreactives were localized in the neurons of paraventricular and supraoptic nuclei and some other hypothalamic areas, and were also seen in the nerve fibers distributed in the basal hypothalamus, infundibulum, and pituitary posterior lobe. Assuming that the CSF sodium is responsible for the release of EDLS, hypertonic NaCl (2.5 M) was infused into the lateral ventricle for 30 minutes. Blood pressure increased gradually, attaining peak rises about 30 minutes later. The plasma content of the EDLS was significantly greater in the hypertonic NaCl group than the control group treated with either the artificial CSF or 2.5 M of urea solution. On the other hand, the hypothalamic content decreased with the infusion of the hypertonic saline. Furthermore, the continuous intracerebroventricular infusions of the hypertonic NaCl with osmotic minipumps in conscious rats significantly increased the arterial pressure after 6 days. Thereby, the plasma level of the EDLS was significantly greater than the control rats that received only the artificial CSF.(ABSTRACT TRUNCATED AT 250 WORDS)

Endogenous digitalislike substance in an adult population in Japan.

Possible involvement of an endogenous digitalislike substance (EDLS) in blood pressure regulation was investigated using a Japanese population. Mean arterial pressure (MAP) significantly correlated with urinary excretion of the EDLS, age, and the obesity index. The plasma EDLS correlated with urinary EDLS. Urinary EDLS excretion well correlated with the inhibitory activity on Na+,K+-ATPase, and also with the urinary excretion of NaCl. Obesity index correlated with the Na+,K+-ATPase inhibition and arterial pressure. Although plasma content of atrial natriuretic polypeptide correlated with the urinary Na+,K+-ATPase inhibition, it did not correlate with the rest of all parameters. Plasma vasopressin level did not correlate with these parameters either. These results clearly indicate that the circulating EDLS (ie, Na+,K+-ATPase inhibitor) is implicated in the hypertension associated with an excess intake of sodium, aging and obesity.

Vasopressor responses and hyperthermia by intracerebroventricular injections of penicillin, a pyrogen, in rats: involvement of brain angiotensin II.

Because hypothalamus is a center for both the blood pressure and thermoregulation, we assumed that there is a close interrelationship between them. To prove this hypothesis, effects of intracerebroventricular (ICV) injections of a pyrogen, penicillin-G (Pc), on the blood pressure and rectal temperature (RT) were investigated. The ICV injections increased the mean arterial pressure (MAP), heart rate (HR) and the abdominal sympathetic activity (SNA) dose-dependently in urethane-anesthetized rats. In conscious rats, Pc (100 U) elicited the two components of rises in MAP (+19 +/- 1 and +20 +/- 1 mm Hg), and both the RT and HR increased corresponding to the second phase of MAP responses (+1.0 +/- 0.1 degree C and +60 +/- 7 beats/min). ICV injections of an angiotensin II (ang II) antagonist significantly decreased the MAP (-30 +/- 4 mm Hg), HR (-106 +/- 13 beats/min), and the RT (-0.9 +/- 0.1 degree C). ICV injections of Pc elicited drinking behavior (the vehicle group; 1.1 ml/90 min vs Pc group; 8.6 ml) in rats water-deprivated for 24 hours. In spontaneously hypertensive rats (SHRs), ICV injections of Pc elicited augmented responses in the first phase but diminished in the second pressor phase. ICV injections of an ang II antagonist elicited augmented effects in both the MAP (-39 +/- 1 mm Hg) and RT (-0.8 +/- 0.1 degree C) in SHRs. Thus, RT fluctuated corresponding to the blood pressure level in the latter phase, where a brain ang II mechanism may be involved.(ABSTRACT TRUNCATED AT 250 WORDS)
