Second primary cancers in patients with gastrointestinal cancers.

The occurrence of multiple primary malignant neoplasms in 9,191 consecutive cases of primary gastrointestinal (GI) tract cancer from 1962 to 1981 at the National Cancer Center was analyzed. The person-years of the cases totaled 28,399.4: 185 second cancers were diagnosed clinically and 85 were first diagnosed at autopsy. The overall frequency was 2.01% for clinically recognizable cancers and 2.94% including those discovered at autopsy. The overall observed/expected ratio was 0.96 for clinical cancers and 1.33 including autopsy cases. However, a significant association between the first and second cancers was present in the oro-pharynx and esophagus; esophagus, stomach and colon; stomach and all other parts of the GI tract; and the colon and rectum. The mode of appearance was also analyzed to clarify the organ relationship.

Recurrence of thymoma with appearance of myasthenia gravis 18 years after surgery: a case report.

A 59-year-old woman was admitted to the hospital because of recurrence of thymoma with appearance of myasthenia gravis 18 years after excision of an invasive thymoma. Recurrence was observed in the anterior mediastinum, right pleura, pericardium and left supraclavicular lymph node. Hashimoto's thyroiditis, vitiligo vulgaris and leukopenia were also seen. After creation of a pericardial window for pericardial effusion, she received vincristine, then carbazilquinone intravenously as single chemotherapeutic agents based on the chemosensitivity by clonogenic assay of the supraclavicular tumor, but response was not achieved. At one and a half years after radiotherapy to the mediastinum, she is now doing well without symptoms and is receiving a fairly low dose of anticholinesterase medication.

[Progress and prospect in basic and clinical lung cancer researches].

After looking back upon the progress in both basic and clinical lung cancer researches in the past twenty-some years, what is to come in the future was anticipated. In the diagnostic field, interpretation of chest X-ray films became more accurate, scraping cytology for peripheral carcinoma was developed, and then fiberoptic bronchoscope was devised, all of which contributed to more accurate diagnosis of lung cancer. In lung cancer therapy, extended surgery and bronchoplasty were introduced, resulting in an increase in the 5 year survival rate. Chemotherapy for small cell carcinoma was much improved. Pathologically, adenocarcinoma and squamous cell carcinoma of the peripheral type appear to have increased in incidence. In the field of basic research, both K-ras and myc-oncogenes were found to be amplified in a case of giant cell carcinoma. Several human monoclonal antibodies were produced, each of which has some interesting characteristics. In the future, the results of basic researches will be applied to prevention, diagnosis and therapy, hopefully reducing the morbidity and mortality of lung cancer.

[Ultrastructural study of human breast cancer--relation to the basement membrane with invasive breast cancer].

The Japan Mammary Cancer Society's General Rules for Pathological Record prescribe differential diagnosis of the noninvasive or invasive status of human breast cancer upon discovery of basement membrane. We discovered basement membranes not only in normal mammary glands and non invasive breast cancers, but in metastatic breast cancers to the lungs and regional axillary lymph nodes. We concluded that, for such diagnosis, it is necessary to observe the cellular arrangement and condition of the surrounding stromal tissues with light microscopy. Nuclear chromatin patterns should also be examined; they showed predominantly euchromatin in invasive breast cancer and heterochromatin in non-invasive breast cancer with electron microscopy.

Multiple primary cancers in 5,456 autopsy cases in the National Cancer Center of Japan.

Multiple primary malignant neoplasms in 5,456 consecutive autopsy cases from 1962 to 1981 in the National Cancer Center were analyzed. There were 285 (5.2%) double primary cancers, 58 (1.1%) triple or more, and 65 (1.2%) minute cancers of the thyroid or prostate gland. Higher incidence of second cancer is observed in cancers of the oropharynx, intestine, larynx, uterus, bladder, and thyroid. Organ association between the two cancers was present in certain organs; for example, there was a tendency for upper gastrointestinal tract cancers to be associated with lower gastrointestinal tract cancers. Influence of histologic subtypes in multiple cancer cases is discussed for lung cancer.

Bronchoplastic surgery for lung cancer and the results.

Fifty-nine patients with lung cancer underwent bronchoplastic surgery in our institute from September, 1965 to May, 1982. The post surgical stages of disease were as follows: 7 cases of Stage I disease, 7 cases of Stage II, 41 cases of Stage III, 4 cases of Stage IV. The bronchoplastic surgery performed included sleeve resection combined with lobectomy in 35, wedge resection combined with lobectomy in 11, reconstruction of the lower area of the trachea in 5, bronchial sleeve resection combined with pulmonary artery resection in 6 and bronchial wedge resection combined with pulmonary artery resection in 2. There was no case of operative mortality within 30 days after the operation. To prevent postoperative complications, careful techniques are required in suturing and postoperative bronchoscopic suction of intrabronchial secretion is necessary. The use of Dexon or Dexon S, polyglycolic acid sutures, showed good results. Adjuvant therapies were performed on 42 cases, including preoperative infusion of Mitomycin C into the bronchial arteries or postoperative irradiation or both. Twenty-eight of 59 were alive and well from 7 months to 16 years after the operations. The relative 5 year survival rates were 40.5 per cent in total cases and 46.5 per cent in those with squamous cell carcinomas. The prognosis of patients undergoing bronchoplasty was compared with that of patients undergoing lobectomy and pneumonectomy. There was a statistical difference between bronchoplasty and pneumonectomy.

[Electron microscopic study on scirrhous carcinoma of human breast--cellular characteristics of subtypes and prognosis].

Scirrhous carcinoma of the breast was divided into 2 subtypes and studied electron microscopically. One type consisted of severe scleroid infiltration of papillotubular carcinoma into the stromal tissues, the other of scirrhous carcinoma in a narrow sense. In the former type, heterochromatins, light and dark cells, and intercellular lumens were observed. In the latter type, euchromatins, light cells and the absence of intercellular lumens were noted. Many intracytoplasmic lumens were observed in scirrhous carcinoma of the latter type. Thus, the 2 subtypes of scirrhous carcinoma differed greatly from each other electron microscopically due to different cellular activities. In discussing prognosis, the 2 subtypes of scirrhous carcinoma of the breast should be differentiated.

Juvenile thyroid carcinoma with metastasis to the bilateral cervical lymph nodes--a case report.

A case of juvenile thyroid carcinoma with metastasis to the bilateral cervical lymph nodes in a 9-year-old male is reported. The clinical picture of juvenile thyroid carcinoma is characterized by early metastasis to the lungs and cervical lymph nodes. In Europe, there have been many reports of thyroid carcinoma after radiation. However, our patient had received no radiation. Surgery consisted of subtotal thyroidectomy and right modified neck dissection. The tumor was a papillary adenocarcinoma and metastasis was seen in 24 out of 38 lymph nodes removed. The serum thyroglobulin level, determined by radioimmunoassay, was 184 ng/ml preoperatively, but by 8 months postoperatively the level fell to 48 ng/ml. No signs of recurrence have been found to date (30 months after the operation).

[Immunomorphologic responses of regional lymph nodes and prognosis in patients with stage Ia pulmonary adenocarcinoma].

The relationship between postoperative prognosis and the immunomorphologic response of regional lymph nodes was examined in 50 patients with stage Ia peripheral bronchial adenocarcinoma and compared with 22 patients in stage Ib and II. Regarding paracortical lymphocyte hyperplasia, the prognosis of the responsive group in patients with stage Ia was significantly better than that of the non-responsive group; however, no definite relationship was found between follicular hyperplasia and prognosis. Follicular hyperplasia correlated with the size of the primary lesion or the distance from the site of the primary lesion. In stage Ib and II cases with metastasis to the pulmonary hilar lymph nodes, paracortical lymphocyte hyperplasia or follicular hyperplasia had no relation to the postoperative prognosis.

Afterloading technique for interstitial irradiation of lesions in deep-seated organs.

An afterloading procedure using iridium 192 seeds was developed for the control of both localized primary lesions that are difficult to remove and superficial residual lesions that remain after the partial removal of tumors in relatively inaccessible, deep-seated organs. This technique makes possible the delivery of a minimal radiation dose to lesions in deep-seated organs. Fewer complications result, the seeds can be adjusted at appropriate intervals in proportion to the amount of radioactivity desired, and radiation exposure to personnel can be reduced. This paper describes the techniques and apparatus used to place the seeds in the tubing. Primary lesions of the urinary bladder, the prostate gland, and the esophagus, residual lesions in the brain and lung, and metastases in intrapelvic lymph nodes were treated. No technical difficulties or complications were encountered. Radiation exposures to personnel were minimized. The results indicate that the procedure has practical applications.
