Plasma from seventeen children with arthritis was investigated for the content of lipoxygenase products. Two of the patients had untreated juvenile chronic arthritis, three had a pyogenic joint infection, four had Henoch-Schonlein purpura and eight had reactive arthropathy. After lipid extraction, the lipoxygenase products were identified by high-performance liquid chromatography (HPLC). Chemotactic material coeluting with leukotriene B4 (LTB4) was found in plasma from the arthritis patients. Enhanced chemotactic activity of this compound was found in juvenile chronic arthritis and pyogenic arthritis patients, compared to the chemotactic activity in plasma from patients with Henoch-Schonlein purpura and reactive arthropathy. The findings may indicate that activation of lipoxygenases may play a role during the initiation of the inflammatory reaction in arthritis.

Colchicine was administered to a 12 year old girl with familial Mediterranean fever and progressive renal insufficiency. There was immediate resolution of abdominal attacks together with a dramatic fall in the serum creatinine concentration and the degree of proteinuria. At the same time her severely impaired growth was stimulated.
In a prospective randomized study of 39 consecutive cesarean sections, 20 patients received cimetidine 400 mg intramuscularly as a pre-anesthetic, an 19 control patients were given NaCl. No perinatal effects on the infants were observed by cardiotocography before delivery, and K, Na, pH, PCO2, HCO-3 and glucose values in capillary blood were nearly identical in the two groups 2 hours after birth, the difference being non-significant (p greater than 0.05). No respiratory effects or arrhythmias were observed. In another study comprising 8 elective cesarean sections in patients with uncomplicated pregnancies, cimetidine was found to cross the placenta at a maternal/cord blood ratio of 3:1. The drug could not be detected in any of the infants 2 hours after delivery.

Twenty patients undergoing cesarean section received cimethidine 400 mg intramuscularly as pre-anesthetic approximately 70 minutes prior to gastric aspiration. The average pH was 5.05, as against 2.97 in the control group (p less than 0.01). No significant reduction in the aspirated volumes was observed. In the cimethidine group the proportion of patients at risk of developing Mendelson's syndrome (pH less than 2.5 and volume greater than 25 ml) was significantly reduced, viz. 5% as against 42% in the control group (P = 0.014). No perinatal effects attributable to cimethidine was observed in the infants. Hence, cimethidine is a safe and useful pre-anesthetic for patients undergoing cesarean section, irrespective of indication and, consequently, much to be preferred to oral antacids.

Calf thymus DNA was methylated in vitro with cell extracts of Bacillus subtilis OG3R (r+m+) and S-adenosyl[Me-3H]methionine. After depurination of the [3H]methylated DNA, the analysis of the pyrimidine dinucleotides revealed the following positions of the methylated nucleosides (indicated by an asterisk) within the BsuRI recognition sequence: 5' dG--dG--dC--dC dC--dC--dG--dG 5'.
