With an increasing number of elderly people retaining natural dentition, the number of exposed root surfaces and root surface caries is increasing. A total of 100 people, aged from 38 to 86, were examined for exposed root surfaces and root surface caries. Denture wearers and non-denture wearers were examined. The following results were obtained; 1. The percentages of exposed root surfaces were significantly higher (P < 0.05) in the 60's and 70's age groups. Root surface caries showed a tendency to increase until people were in their 60's. 2. Denture wearers had more exposed root surfaces (P < 0.05) and were more likely to have root surface caries than non-denture wearers. Denture wearers showed a tendency to have more severe grades of root surface caries than non-denture wearers. 3. Concerning denture wearers, there were not significant differences in the percentages of exposed root surfaces of buccal and proximal surfaces facing edentulous space. However, proximal surfaces facing edentulous space were more prone to root surface caries than buccal surfaces (P < 0.05).

We used autotransfusion in valvular heart surgery and evaluated its effect on perioperative homologous blood requirements. Methods of autotransfusion we used were intraoperative blood salvage using Cell Saver 4 and retransfusion (IAT), postoperative autotransfusion of shed mediastinal blood (PAT), and hemodilutional autotransfusion (HAT). The patients undergoing valvular heart operations were divided into three groups; group 1 (control group), group 2 (using IAT and PAT group), and group 3 (using IAT, PAT, and HAT group). Perioperative homologous blood requirements significantly decreased in group 3 compared to group 1 and group 2. Operations without homologous blood transfusion significantly increased in number in group 3 compared with group 1. In conclusion, autotransfusions described above were useful blood conservation techniques in valvular heart surgery.
A new sectional partial denture utilizing lock mechanism (lock denture) has been developed. Utilizing the lock mechanism, two parts of sectional partial denture are locked together easily and tightly. A lock denture has been constructed for an upper removable partial denture, has been worn for over three months and has clinically been proven to obtain good function and esthetics.
To evaluate the efficacy of blood conservation technique at our center, 144 patients undergoing primary coronary artery bypass grafting were studied between Jan. 1988, and Dec. 1990. Combined intraoperative and postoperative autotransfusion technique reduced the requirement of homologous blood to a half and 24% of the patients were without homologous transfusion. With more use of dilutional autotransfusion, 41% of the patients were without homologous transfusion. Intraoperative and postoperative autotransfusion technique is effective and dilutional autotransfusion is also a relatively effective method to reduce homologous transfusion.
The sera from patients with various liver diseases were investigated for the antibody against calmodulin (CaM) extracted from bovine brain by the enzyme linked immunosorbent assay. The specificity and purity of CaM were confirmed by the Western blot technique using anti-CaM antibody (anti-CaM) positive sera. IgA class antibody was frequently detected in patients with hepatocellular carcinoma (HCC), autoimmune hepatitis (AIH) and chronic active hepatitis (CAH). On the other hand, IgG class antibody was very often present in patients liver cirrhosis, AIH and acute viral hepatitis (AVH). Sixty seven percent of patients with AVH in the acute phase were positive for IgM class anti-CaM and 33% of patients with AVH in the convalescent phase positive respectively. In AVH, the titer of anti-CaM reached its peak on 26.3 days after the onset. The titer of anti-CaM in fulminant hepatitis was higher than that in AVH. Seventy percent of type A hepatitis patients were positive for IgM class anti-CaM, 33% of type B and 33% of type non-A non-B. These results suggest that the frequency and titer of anti-CaM may depend upon the type of hepatitis and the degree of liver cell injury.
A 3-year-old girl with congenital choledochal dilatation, prehepatic portal occlusion, liver abscess, and cholelithiasis is reported. Ultrasonography, computed tomography, RI scintigraphy, endoscopic retrograde cholangio-pancreatography, and angiography were performed to get the diagnosis.
In order to study the clinical significance of IAP and ASP, we have measured serum concentration of IAP and ASP in patients with different cancer types including digestive organs, head and neck. Significantly higher correlation (coefficient of correlation = 0.924) was found between IAP and ASP. IAP and ASP levels were elevated in about 60-80% of cancer from digestive organs except hepatocellular carcinoma, and in about 56-100% of head and neck tumor. There was no correlation between IAP, ASP and various tumor markers (CEA, CA19-9, AFP) in all malignancies except pancreatic cancer. There was positive correlation between CEA and IAP, ASP in pancreatic cancer. IAP and ASP levels were significantly higher in patients with cancer having evidence of disease than the patients with cancer who had no evidence of disease, and so the determination of IAP and ASP was clinically useful for the diagnosis of tumor existence. At the time of diagnosis of recurrence, the levels of IAP and ASP were elevated in about 70% of patients with cancer recurrence. There was a definite relation between IAP, ASP and the gastric cancer stage. In later state (stage III and IV), serum IAP and ASP levels were significantly higher than the levels of stage I and II. In hepatocellular carcinoma without liver cirrhosis, the levels of IAP and ASP were higher than the patients with cirrhosis. The determination of IAP and ASP seems to be useful for monitoring clinical course, judgement of therapeutic effects.
In order to study the clinical significance of ASP and IAP, we have measured serum concentration of ASP and IAP in 259 patients with inflammatory diseases, non-inflammatory diseases, autoimmune diseases and liver diseases. Significantly higher correlation (coefficient of correlation = 0.922) was found between ASP and IAP. There were good correlation between ASP, IAP levels and erythrocyte sedimentation rate (ESR), C-reactive protein (CRP). Coefficient of correlation were 0.705 and 0.673 between ESR and ASP, IAP; 0.621 and 0.623 between CRP and ASP,IAP, respectively. There were food correlations between ASP levels and thrombo test (TT) and hepaplastin test (HPT). The positive rate and mean concentration of ASP, IAP in patients with inflammatory diseases were significantly higher than noninflammatory diseases. It is suggested that serum ASP, IAP could be one of the useful indicator for evaluating the clinical course of patients with inflammatory diseases. Serum concentration of ASP, IAP in patients with chronic liver diseases were lower than normal subjects, especially serum levels in patients with decompensated liver cirrhosis and chronic hepatitis (active) were significantly lower than normal subjects. In patients with hepatoma with cirrhosis, serum levels of ASP, IAP were significantly lower than the hepatoma patients without cirrhosis. The determination of ASP, IAP in liver diseases seems to be useful for evaluating the severity of diseases, the effects of therapy and the forecast of prognosis.

We studied contact factors and kinin-kallikrein in normal non-pregnant and pregnant women, FXII deficient toxemia and DIC. The results obtained are as follows: 1. The levels of plasma prekallikrein, high molecular weight kininogen, kallikrein inhibitor, and C-1 INA were gradually decreased at delivery, and the levels of kallikrein like activity and bradykinin were increased during pregnancy and at the time of parturition. These facts indicate that kinin kallikrein systems played important role in uterine contraction. 2. The levels of contact factors (FXII and FXI) were lower at delivery than those of term. 3. In rat uterus, specific binding of bradykinin was observed by the method of radio receptor assay in the pelet of 10,000 X g fetal membranes, and its activity was 38%. 4. A synthetic kallikrein inhibitor (OS-291, MS) and bradykinin antagonist inhibited completely spontaneous uterine contraction of Wistar rats during delivery. 5. In the case of FXII deficiency, the levels of plasma prekallikrein, high molecular weight kininogen were normal, but at delivery, these levels were lower than those of term. The levels of kallikrein like activity which was half of normal parturition level was increased at parturition. 6. In cases of DIC (17) and severe toxemia (22), plasma prekallikrein levels were lower than the normal controls. The decrease was due to consumption of plasma prekallikrein to kallikrein activation.

The statistics of clinical observation at Department of Internal Medicine, Nippon Dental University at Niigata from July 1981 to December 1987 (duration 6.5 years) were as follows: Total number of inpatients: 1,238, Total number of death cases: 106. Findings include: 1) Ratio of male patients to female patients 1.34:1.00. Male deceased patients to female deceased patients 1.52:1.00. 2) Average patients number hospitalized per year was 200. The high percentaged of certain advanced aged groups was reflected by the recent demographic changes in the society in general.; in their 60's 46.0%, in their 70's 24.3%, in their 80's 6.7%. In these age groups, female number is tendency to increase the number of male. 3) The diseases of inpatients were mostly due to the digestive tract, which accounted for 60.4% of the total. Of this percentage, 65% was due to hepato-biliary diseases. 4) The death statistics of malignant tumor was 68.9%; Benign diseases being 31.1%. Male patients died from hepatocellar carcinoma, lung cancer, colon cancer and stomach cancer, in descending order. Females died from cancer of the biliary tract, stomach cancer and hepatocellular carcinoma, again, in descending order. 5) 71.7% of all deaths were caused by the digestive tract, in particular, hepatocellular carcinoma, cancer of the biliary tract, liver cirrhosis and primary biliary cirrhosis, all belonging primarily to the hepato-biliary disease group. 6) As a result of 58 autopsies performed for 106 death cases, 32 cases had complete autopsies and 26 cases had partial organ punctures.
The therapeutic effect and the prognosis of transcatheter arterial embolization (TAE) and one-shot chemotherapy were studied in 90 cases of unresectable hepatocellular carcinoma (HCC). A therapeutic effect, which was assessed by the serum concentration of alpha-fetoprotein, angiography, computed tomography and ultrasonography, was seen in 33 (83%) out of 40 cases treated with TAE, and in 16 (32%) out of 50 cases treated with one-shot chemotherapy. In the cases that received TAE, the cumulative percentage survival rates at 6 months, 12 months, and 24 months were 75%, 48% and 20%, respectively. In contrast, the survival rates in the cases that received one-shot chemotherapy were 30%, 10% and 2%, respectively. In addition, the prognosis on the basis of the degree of tumor invasion of the portal vein was studied. In cases with the same degree of tumor invasion, the survival rate of the cases treated with TAE was significantly higher than that of those treated with one-shot chemo-therapy, except for those cases with tumor invasion of the main portal vein. These results show that TAE should be performed as the therapy of first choice in unresectable cases of HCC.
