Biological approaches to cancer therapy.

Advances in the biological approach to cancer therapy are reviewed. The mechanisms of actions, clinical effects and uses of interferons, the first biological modifiers to be used, are reviewed first. The interleukins and monoclonal antibodies are also mentioned in detail. This review also covers the clinical use and production of lymphokine activated killer cells, which are used in conjunction with interleukin-2. A brief review of tumour infiltrating lymphocytes, which comprise a subset of lymphocytes found within solid tumours is given. In mice, tumour infiltrating lymphocytes have been shown to be more therapeutic than lymphokine activated killer cells. Tumour necrosis factor, a protein released by activated macrophages in response to stimulation by endotoxin, is also briefly mentioned although clinical data are disappointing. Finally, the role which oncogenes may play in cancer therapy and understanding is discussed.

Serum levels of c-myc and c-ras oncogene products in normal subjects and in patients with neoplastic and non neoplastic conditions.

A new, simple and sensitive low pH ELISA system has been developed and used to measure serum levels of c-myc and c-ras oncogene products in healthy blood donors and patients with neoplastic and non neoplastic conditions. Blood donors had significantly lower serum levels of oncogene products than patients with cancer or other pathologies (p-value less than 0.01). There was, however, no difference between patients with neoplastic and non-neoplastic conditions. Although c-myc and c-ras oncogene products in the serum appear to discriminate between healthy state and pathological conditions they do not discriminate between neoplastic and non-neoplastic conditions.

Scheduled and unscheduled combination chemotherapy in the treatment of squamous cell lung cancer (cyclophosphamide, adriamycin, vincristine and bleomycin, with and without cis-platinum).

Fifty four patients with squamous cell lung cancer, treated with Cyclophosphamide (750 mg/m2), Adriamycin (50 mg/m2), Vincristine (1.5 mg/m2) and Bleomycin (30 mg) (CAVB) in a 2 day scheduled regime, showed an overall median survival time (MST) of 32 weeks (28 weeks for non-responders and 46.5 weeks for responders--a non-significant difference). The response rate was 24%, the median duration of response being 23 weeks. Bone marrow toxicity was not a major problem. A further 21 patients received the same drugs but in a single bolus (unscheduled regime). Comparison of their results with the 25 patients randomised to the scheduled regime showed no significant difference in MST or toxicity. The addition of Cis-platinum (50 mg/m2) to the unscheduled CAVB regime (CAVBP) in a further 22 patients did not result in an improvement either in survival or response, but added to the toxicity.

Tumour markers.

The last decade has seen an enormous expansion of the role of tumour markers in the diagnosis, localization, prognosis, staging and monitoring of malignancies. The definition of tumour markers has been broadened to include cytogenetic markers, oncogenes and oncogene products which provide possibilities for developing more specific tests to help diagnose and manage malignant disease.
