Teratogenicity of di(2-ethylhexyl) phthalate (DEHP) and di-n-butyl phthalate (DBP) in mice.

Di(2-ethylhexyl) phthalate (DEHP) and di-n-butyl phthalate (DBP) were mixed with diet at graded levels of 0.05, 0.1, 0.2. 0.4 and 1.0 wt-% and given to pregnant ICR mice throughout gestation. Maternal weight gain was suppressed and fetal resorption increased at 0.2, 0.4 and 1.0% levels of DEHP and 1.0% level of DBP. All the implanted ova died early in rats fed 0.4 and 1.0% levels of DEHP. External malformations increased significantly by 0.2% DEHP, and 1.0% DBP showed borderline significance. The major malformations in treated groups were neural tube defects (exencephaly and myeloschisis), suggesting that the phthalic acid esters (PAEs) affect neural tube closure in developing embryos. Treatment with the compounds caused intrauterine growth retardation and delayed ossification with an apparently dose-related response pattern. These results indicate that a high dose of DEHP and DBP might be embryotoxic and teratogenic in mice. The maximum nonembryotoxic doses of PAEs in mice were more than 2000 times the estimated level of human intake through the food chain. Thus it is assumed that the current "normal" exposure level of PAEs dose not pose an imminent threat to human fetal development.

Mycoplasma pneumoniae pneumonia associated with IgA nephropathy.

An 18-yr-old male developed Mycoplasma pneumoniae pneumonia associated with IgA nephropathy. In the biopsied renal tissue, immunofluorescence showed striking deposits of IgA, C3 and C3 activator in the mesangium, and electron microscopic examination showed electron-dense deposits in the mesangial region. When the patient's serum was preincubated with rabbit antiserum to human IgA, the passive hemagglutination titer for M. pneumoniae was much decreased. We suggest that in some cases, IgA nephropathy may follow infection by an organism which stimulates in IgA antibody response, leading to the formation of IgA antigen antibody immune complex deposits in the glomerular mesangium.

Suppurative coxitis due to Salmonella typhimurium in systemic lupus erythematosus.

A 36-year-old woman with systemic lupus erythematosus (SLE) developed septicaemia and subsequently suppurative coxitis due to Salmonella typhimurium. Although systemic treatment with antibiotics eradicated salmonella from the arterial blood, cup arthroplasty and irrigation of the operative wounds with effective antibiotics were needed for eradication of the organism from the affected joints.

The usefulness of thallium-201 myocardial perfusion scintigraphy in the diagnosis of chronic cor pulmonale.

To open up the possibility of exact clinical diagnoses of chronic cor pulmonale, we performed thallium-201 myocardial perfusion scintigraphy on patients with chronic pulmonary disease. From the thallium activity in the left ventricle (plus ventricular septum) and right ventricle counted scintigraphically, we determined the thallium activity ratio of left to right ventricle (TAR). The intra- and inter-observer variations of this ratio were negligible. We compared TAR with pulmonary hemodynamic findings and with left to right ventricular mass ratio measured at autopsy, and obtained the following results. 1) Left to right ventricular mass ratio determined in 11 autopsied patients by the method of Fulton et al. was closely correlated with TAR values (r = 0.90, p less than 0.001). 2) In patients with chronic pulmonary disease, there were highly significant exponential inverse correlations between TAR and pulmonary arterial mean pressure, total pulmonary vascular resistance and right ventricular systolic pressure the the coefficients of -0.75, 0.72 or -0.77, respectively (p less than 0.001). These results suggested the possibility that the severity of pulmonary circulatory disturbance may be ascertained non-invasively by TAR measurement. Thus, thallium-201 myocardial perfusion scintigraphy is able to reveal non-invasively right ventricular hypertrophy in patients with chronic pulmonary disease, and it might be a useful means of clinically diagnosing chronic cor pulmonale. If TAR obtained from scintigrams is smaller than 2, the patient concerned may possibly have right ventricular hypertrophy and he or she may be diagnosed as having chronic cor pulmonale.

Testicular function of actively immunized male rats with LH releasing hormone (LHRH): a possible role of prolactin on regulation of spermatogenesis.

The effect of testosterone propionate (TP) on the recovery of spermatogenesis was compared in 3 types of experimentally induced gonadotropin-deficient rats; 1. hypophysectomized rats, 2. rats actively immunized to LHRH and 3. hypophysectomized and immunized rats bearing a pituitary isograft. In order to immunize animals to LHRH, deamidated LHRH conjugated with BSA (LHRH-BSA) was injected intradermally to male rats 4 times at 2 week intervals and additional booster injections continued once a month. Anti-LHRH titer was the highest on the 12th week and the titer remained at a relatively high levels thereafter. A remarkable decrease in testicular weight and testosterone production was achieved in 10 weeks associated with a drop in serum LH and FSH levels. In long term immunized rat (LIM rats) which had been immunized for more than 3 months, testicular atrophy advanced to a similar extent to that in long term hypophysectomized rats (HX rats). The administration of TP (1 mg/day) sc for 30 days restored spermatogenesis in LIM rats. The simultaneous administration of anti-LH and -FSH sera in addition to TP did not affect the restoration of spermatogenesis in LIM rats. The same TP treatment in HX rats, however, failed to restore spermatogenesis. Spermatogenesis was reinitiated after TP treatment if immunized-hypophysectomized rats received a pituitary isograft. Though the serum PRL level in LIM rats was one fifth of that in normal rats, a 2.5-fold rise in the PRL level was observed after TP treatment. These results suggest that PRL is involved in the process of spermatogenesis.

Treatment of angina pectoris patients with niludipine a new calcium antagonistic drug.

The antianginal effectivity of niludipine (Bay a 7168), a new calcium antagonistic drug, was investigated in angina pectoris patients, selected for admission to the trials by pre-determined strict criteria. Oral administration of from 60--120 mg niludipine daily for 4--8 weeks, resulted in the following: 1. Number of anginal attacks significantly reduced. 2. Nitrate consumption markedly lowered. 3. Investigating physicians assessed globally the clinical results of niludipine treatment on the basis of the reduction of the number of anginal attacks, nitrate consumption, improved physical ability and reports by the patients as to their subjective evaluation of the test drug. Niludipine was judged clinically effective in 21 out of 27 patients (77.8%). 4. Full records of ECGs, a necessary item for assessment, were obtained from 24 out of 27 patients. Ischemic ECGs were improved or normalized in 6 out of 24 patients (25%). 5. In the investigators' final overall assessment, including the evaluation of ECG changes, niludipine was rated effective for 20 out of 27 patients (74.1%). 6. Tolerance to the test drug was excellent. Only one patient complained of transient, mild nausea. After reduction of the daily dose, the patient completed the trial without any further appearance or complaints of side effects. Results suggest that niludipine is a useful antianginal drug in the management of coronary artery disease patients. Further clinical investigations with a larger number of patients should be conducted.

Clinical study on the hypotensive effect of diltiazem hydrochloride.

Diltiazem HCl was administered p.o. at a fixed daily dose level of 180 mg to patients with essential hypertension, and the hypotensive effect of the drug was investigated. The following results were obtained: diltiazem HCl given alone exhibited a hypotensive effect on systolic pressure in 88.9% of the patients and on diastolic pressure in 66.7% of the patients. The concurrent use of diltiazem HCl with trichlormethiazide exerted a hypotensive effect on patients who were nonresponsive to diltiazem HCl alone. As regards side effects, one patient complained of gastric fullness and one of dizziness. However, the symptoms were so mild that further continuation of diltiazem HCl therapy was possible. From the results obtained, it is concluded that diltiazem HCl can effectively be used clinically as a hypotensive drug.

Correlation between steroid myopathy and serum lactic dehydrogenase in systemic lupus erythematosus.

Examination of 27 patients with systemic lupus erythematosus (SLE) before treatment showed an elevation of the serum level of lactic dehydrogenase (LDH) in 15 patients. In these patients, the LDH level fell to normal in response to corticosteroid therapy. In six of 27 patients, steroid myopathy with elevation of the LDH level developed during corticosteroid therapy. At the same time, there was no or only a slight increase in the creatine phosphokinase level, while the SGOT and aldolase levels remained normal. The elevated LDH levels gradually returned to normal as the corticosteroid dosages were reduced and the myopathic symptoms disappeared. We suggest that the measurement of LDH levels is useful for diagnosis and the subsequent treatment of patients with steroid myopathy in SLE.

Ectopic pregnancy and myoma uteri: teratogenic effects and maternal characteristics.

Morphological data from 3,614 well-preserved human embryos derived from artificial termination of pregnancy were used to determine whether ectopic implantation or enlarged myomas could enhance the prevalence of localized malformations of the embryo. The frequency of malformed embryos was 11.6% among 43 recovered from ectopic pregnancies, 6.2% among 97 from myomatous pregnancies, and 3.3% among 3,474 from normally implanted pregnancies not complicated by myomas. Unilateral amelia in the ectopic cases and caudal dysplasia in the myoma cases were significantly increased. Both malformations were quite unusual in the control group. It is argued that spatial restrictions could be a teratogenic agent in human embryos. Mothers of both ectopic and myoma cases were on average much older than mothers of the control specimens, and those with myomas had a higher frequency of previous pregnancy wastages. Ectopic pregnancy was found to be associated with lowered parity, previous ectopic pregnancy, and maternal smoking and drinking. These associations are discussed and interpreted in relation to etiology of each of the two conditions.
