Molecular cloning of the gene for the human placental GTP-binding protein Gp (G25K): identification of this GTP-binding protein as the human homolog of the yeast cell-division-cycle protein CDC42.

We have isolated cDNA clones from a human placental library that code for a low molecular weight GTP-binding protein originally designated Gp (also called G25K). This identification is based on comparisons with the available peptide sequences for the purified human Gp protein and the use of two highly specific anti-peptide antibodies. The predicted amino acid sequence of the protein is very similar to those of various members of the ras superfamily of low molecular weight GTP-binding proteins, including the N-, Ki-, and Ha-ras proteins (30-35% identical), the rho proteins (approximately 50% identical), and the rac proteins (approximately 70% identical). The highest degree of sequence identity (80%) is found with the Saccharomyces cerevisiae cell-division-cycle protein CDC42. The human placental gene, which we designate CDC42Hs, complements the cdc42-1 mutation in S. cerevisiae, which suggests that this GTP-binding protein is the human homolog of the yeast protein.

Dacron fabric-enveloped hydroxyapatite prosthesis for sternal tumor defect: an autopsy report.

A 38-year-old housewife with solitary plasmacytoma of the manubrium who underwent a subtotal sternectomy treated by resection of the lesion is reported. This was followed by replacement with a Dacron fabric-enveloped hydroxyapatite prosthesis. The Dacron fabric was sutured to the surrounding tissues, and then the clavicle was passed through the cylindrical-shaped Dacron fabric to form a sternoclavicular joint capsule. The patient returned to her daily life 3 months after the operation. She had no trouble in her daily living, without any dislocation of the sternoclavicular joints or any displacement of the artificial sternum. The autopsy examination about 1 year after the operation showed that the Dacron fabric enveloping the artificial sternum became stronger with time. The sternoclavicular joint also was stably fixed, and the Dacron fabric fulfilled its function as an artificial articular capsule and biologic fixation of the surrounding supporting tissues.

Increase in permeability of human endothelial cell monolayer by recombinant human lymphotoxin.

The effect of lymphotoxin (LT) on transendothelial permeability was examined using in vitro human endothelial cell (EC) culture system. To assess permeability, we measured the movement of human IgG labeled with horseradish peroxidase (HRP-huIgG) across an EC monolayer cultured on a fibronectin and collagen-coated membrane. LT increases the permeability dose dependently within 16 hours. Similar results were obtained with tumor necrosis factor (TNF)-alpha. The results suggest that increase in vascular permeability by LT and TNF-alpha plays an important role in initiating hemorrhagic necrosis of tumors in vivo.

[Multiple primary malignant tumors of rhabdomyosarcoma in an upper arm and breast cancer].

Multiple primary malignant tumors of rhabdomyosarcoma in a upper arm and cancer of breast are extremely rare. In September 1970, a female, aged 86 years, underwent the first resection operation performed on a pleomorphic rhabdomyosarcoma of the right upper arm. She underwent radiation therapy (8000 rads at the operative field) postoperatively. In August 1980, a simple mastectomy was performed for a cancer of the right breast. In December 1984, fourteen years after her first operation, a recurrence was detected. The tumor recurred five times during the next two years and was resected each time. The patient died of cachexia sixteen years after the first operation.

[Total femur replacement in osteogenic sarcoma of the femur].

A 14-year-old girl was admitted with a diagnosis of osteogenic sarcoma of the distal femur. Pre-operative chemotherapy was effective, and sclerosis of the lesion made clear the presence of a proximal femoral and pulmonary metastasis. Total femur and knee joint replacement with titanium alloy was made after a total resection of the femur. Her post-operative clinical course was excellent without local recurrence. Now, 14 months after the operation, she can walk without crutches. We wish to emphasize that the presence of a pulmonary metastasis is not absolutely a contraindication as long as the tumor appears surgically resectable and responsive to per-operative chemotherapy.

[A case of Krukenberg tumor associated with elevation in blood testosterone].

One case of Krukenberg tumor associated with elevation in blood testosterone levels was reported. The patient was a 31-year-old woman. She visited the outpatient service of our hospital with the chief complaint of uterine bleeding and infertility. She was diagnosed to have an anovulatory cycle associated with elevation in blood testosterone levels. However, after an examination at our hospital, a tumor of the left ovary was pointed out and she underwent bilateral ovariectomy. Postoperatively, tarry stool was passed, for which she received an examination at the department of internal medicine. Roentgenography of the stomach and examination by a gastric camera revealed undifferentiated carcinoma of Borrmann's type III in the greater curvature of the angular area. This lesion was diagnosed to be Krukenberg's tumor and a gastrectomy was performed. On the other hand, the blood testosterone level which had shown elevation before resection of the ovaries was restored to the normal range after the operation. Various reports have been published concerning the endocrinological activity of this tumor. It remains to be elucidated whether it is due to metastasis of the carcinoma or if it originates from other ovarian parenchyma. However, it has been reported that elevation in endocrinological activity is apt to occur when the tumor originates from the digestive tract.

[Surgical treatment of varices of the lower extremities (author's transl)].

In the last 6 years we have treated 132 patients with varices of the lower extremities and operated on 65 of them. We came to the following conclusions: 1. Stripping of the saphenous vein with subfascial ligature of the incompetent venae communicantes is essential for the complete treatment of primary varicosity with or without ulcer. 2. Pelvic phlebography is necessary to exclude secondary varicosities in both bilateral and unilateral varices. 3. The removal of secondary varices has a definitely negative result if recanalisation is incomplete; i.e. they ought not to be extirpated if the outflow from the great saphenous vein is directed centrally on walking.

Recurrence of the nephrotic syndrome in the renal isograft: A preliminary report.

The fifth case of kidney transplantation between monozygotic twins in Japan is presented. Immediately after the transplanation the original disease, nephrotic syndrome, recurred but it was treated with steroids successfully. There have been few cases reported in which recurrence of the original disease in the isograft was treated successfully.

Effects of lumbar sympathectomy on thromboangiitis obliterans.

The clinical response to lumbar sympathectomy has been reported. The incidence of improvement was as follows: Coldness 52 percent, ulcer 58 percent, rest pain 64 percent, and claudication 31 percent, respectively. The blood flow in the anterior tibial muscles did not increase within 5 weeks postoperatively. There were correlations between the effects and the clinical stages, the interval from the onset to the operations, the abuse of tobacco, and the degree of reactive hyperemia, but not relation was observed between as to the site of obstruction. In the experiments, the influence of acetylcholine on blood flow increased after the operations and the duration of vasoconstriction following administration of noradrenalin became shorter. On the basis of the clinical and experimental facts, postoperative improvement seems to be due to increased blood flow to the peripheral skin and to relief of vasospasm.

Long-term follow-up results of the iliofemoral venous thrombosis.

During the past 7 years 116 patients with acute and chronic iliofemoral venous thrombosis were treated surgically and medically. Exploration was undertaken mainly by clinical assessment and routine venography for follow-up was not considered as a requisite. Thrombectomy was performed 18 times in 17 patients with excellent or good results in 66.6%. Palma's operation was carried out 17 times in 17 patients with satisfactory results in 82.3%. These were contrasted with 34.2% satisfactory results among the medical patients. These lead to the following conclusion: 1. Venous thrombectomy within 14 days after the onset is effective. 2. The usefulness of delayed thrombectomy is uncertain. 3. Palma's operation offers satisfactory results for chronic iliofemoral venous thrombosis. 4. In Japan, in contrast with America and Europe, pulmonary thromboembolism is exceedingly rare and ligation or interruption of the inferior vena cava or the femoral vein is not performed routinely.
