Heterotopic gastric mucosa accompanied by aberrant pancreas in the duodenum.

An operated case of a heterotopic gastric mucosa, accompanied by an aberrant pancreas tissue, in the bulb of the duodenum was reported. A 40-yr-old man complained of epigastralgia and was diagnosed to have a protruding lesion in the duodenum by roentgenological and endoscopic examinations. Partial excision of the duodenal wall including the mass was performed. Cut surface of the specimen showed a yellowish-whitish mass in the submucosal through the subserosal layers, which was covered by a papillary elevated mucous membrane. Histologically the mass was an aberrant pancreas of the Heinrich I-type and the mucous membrane covering the mass was a heterotopic gastric mucosa, consisting of completely developed fundic glands.

Alagille's syndrome. A case with a hamartomatous nodule of the liver.

A male case of Alagille's syndrome associated with a hamartomatous nodule of the liver is described. The patient developed jaundice soon after birth, and was diagnosed as the syndrome with signs such as paucity of the intrahepatic bile ducts, pulmonary stenosis and embryotoxon in the cornea at 15 years of age. The liver was examined in recurrent biopsies and other tests. However, no evidence of liver cirrhosis was confirmed until his 15th year. The patient died of hepatic dysfunction when he was 17 years old. At autopsy, a large hamartomatous nodule was found in the liver showing biliary cirrhosis. Morphology of the nodule resembled that of focal nodular hyperplasia. Abnormalities of the large vessels were noted around the liver. Vascular abnormalities were also seen in the mass. The relation of these vascular abnormalities to etiological background of the syndrome and occurrence of the nodular lesion is discussed.

A case of clear cell adenocarcinoma of the uterine cervix in pregnancy.

An extremely rare case of clear cell adenocarcinoma of the uterine cervix in pregnancy is reported. The primary lesion was first found at 34 weeks of pregnancy in a 34-year-old patient, and was cytologically suspected to be malignant. At 39 weeks of pregnancy, the patient underwent abdominal cesarean section concomitantly with removal of huge right ovarian tumor (3050 g) and delivered a normal male baby weighing 3590 g. Histological findings for cone biopsy at 22 days postpartum revealed invasive adenocarcinoma, and abdominal radical hysterectomy and pelvic lymphoadenectomy were performed at 36 days postpartum. Microscopically, the tumor tissue was composed of clear and hobnail-type cells. It showed a considerable amount of PAS-positive diastase-labile glycogen but was only weakly positive for immunoperoxidase staining of carcinoembryonic antigen.

Fc receptor(s) induced by human cytomegalovirus bind differentially with human immunoglobulin G subclasses.

The IgG subclass specificity of Fc receptor(s) induced on cells by infection with human cytomegalovirus (HCMV) was studied in a binding assay by using infected cells and purified iodinated IgG of various subclasses from HCMV seronegative healthy adult donors. All four human IgG subclasses bound to HCMV-infected cells, with the following relative magnitudes: IgG1 greater than or equal to IgG4 greater than IgG2 greater than IgG3. The IgG subclass specificity of the Fc receptor was further analysed in an inhibition assay by using fragments prepared from purified human IgG by papain digestion, and using unlabelled subclass proteins. Fc but not Fab fragments inhibited the binding of 125I-labelled human IgG to HCMV-infected cells. The biological role of the Fc receptor in HCMV infection is discussed.

[Pineocytoma--a case report].

A report on a rare case of pineocytoma is presented. A 27-year-old woman visited our clinic because of a 3-month history of intermittent headaches and nausea. A CT scan revealed the presence of a marked obstructive hydrocephalus and mass without any contrast enhancement in the pineal region. Immediately, V-P shunting was performed and resulted in relief of all symptoms. Ventriculography showed a complete occlusion at the aqueductus Sylvii and filling defect at the posterior part of the 3rd ventricle. The patient was operated on in the prone position via infratentorial supracerebellar approach by suboccipital craniectomy on November 9, 1982. A grayish red-colored, well-defined solid tumor located at the pineal region was removed partially. The histopathological appearance of this tumor resembled the pattern of the normal pineal gland. Many cells exhibited a polar form, eosinophilic cytoplasm with the process often being directed toward a blood vessel. The cells around the central areas occupied by pale eosinophilic material were arranged like a "rosette". Combined chemo-radiotherapy was carried out after surgery. That is, a total dose of 4,825 rads to the whole brain was irradiated, and ACNU 140 mg and VCR 6 mg in total were administered intravenously and intermittently. After irradiation therapy, the tumor increased in size producing a ring-like enhancement effect as shown on repeated CT scans. During this time, she started to complain of blurred vision with Parinaud's sign. A second operation via interhemispheric approach by right parietal craniotomy was undergone, and the tumor was partially resected again on March 29, 1983.(ABSTRACT TRUNCATED AT 250 WORDS)

[Sclerosing stromal tumor of the ovary].

A 20-year-old woman had the complaints of a tumor in the lower abdominal region and irregular menstruation. A spherical tumor measuring 7 X 6 X 3.5 cm in the great dimension was removed from the left ovary. The cut surface showed a lobular pattern and cystic degeneration in some areas. Histological examination revealed edematous areas and highly cellular areas. The cells were variable in size and shape. Round cells resembled signet-ring cells in places. By PAS reaction and alcian blue stain, these cells were found to be negative, but by Sudan III stain, positive. Postoperatively, the patient complained of genital bleeding for five days. Subsequently, her menstrual pattern returned to normal.

[Advanced gastric cancer successfully resected following cisplatin therapy. A case report].

A case of unresectable advanced gastric cancer (portion A, Min, S3 infiltrating into pancreas, N3, por., Borrmann III-type, hen's egg size) was treated with cis-dichlorodiammine platinum (CDDP) at a dosage of 30 mg/day for five days every month following surgery. After three courses of administration, examinations revealed that the tumor had disappeared and an evaluation of complete response was made. Four months later, a second-look operation was performed. It was found that the cancer was less than thumbsize and was able to be resected radically. No significant side effects were observed during CDDP therapy.

Immunohistochemical study of placental alkaline phosphatase in primary intracranial germ-cell tumors.

Indirect immunoperoxidase staining by the peroxidase-antiperoxidase (PAP) technique was carried out on 23 human primary intracranial germ-cell tumors (17 germinomas, one embryonal carcinoma, one yolk-sac tumor, three teratomas, and one teratoma with embryonal carcinoma) and on six human primary pineal non-germ-cell tumors (one pineocytoma, two pineoblastomas, two astrocytomas, and one glioblastoma multiforme). The technique used specific rabbit antisera against placental alkaline phosphatase (PLAP), alpha-fetoprotein (AFP), and human chorionic gonadotropin (HCG). Thirteen of 17 intracranial germinomas (76.5%) showed positive staining for PLAP mainly on the tumor cell membrane. In six primary intracranial non-seminomatous germ-cell tumors, there was weak positive staining indicating the presence of PLAP in only a few cells of one embryonal carcinoma, and in some glandular epithelial cells of one teratoma; this staining was limited to the cytoplasm. None of the other six primary pineal non-germ-cell tumors showed any positive PLAP reaction. From these results, PLAP was shown to be very useful in histopathology as a diagnostic tumor marker of intracranial germinoma. Positive AFP staining was seen in several yolk-sac tumor cells and a few embryonal carcinoma cells. However, no intracranial germinomas and non-germ-cell tumors of the pineal region showed positive reaction. As for HCG, only one suprasellar germinoma and one pineal embryonal carcinoma among 29 specimens contained a few positive-staining tumor cells.

[A case of intracranial multiple fibrous histiocytoma. Case report and review of literature].

A case of intracranial multiple fibrous histiocytom is arising from the dura mater and both lateral ventricles is presented. To the authors' knowledge, this is the ninth case of intracranial fibrous histiocytomas reported in the literature. A 10-year-old girl was admitted with a 6 month history of cerebellar ataxia. Radiological examination including CT scan revealed a large mass in occipito-suboccipital region. On November 1, 1978, subtotal resection of the tumor was performed. Yellowish hard tumor, weighing 195 g had wide attachment to the dura mater, but did not invade the brain tissue. Postoperative CT scan, three months later revealed the other tumors in the left lateral ventricle and in the suprasellar region in addition to the lesion that had been operated on. In the course of hospitalization, the suprasellar tumor enlarged rapidly despite of 60Co radiation therapy, then led her to hypothalamic dysfunction. In January, 1981, she had paraplegia and paresis of the right upper extremity. Spinal lesion at the level of C-5 was presumed, but further examination was not performed because of her poor condition. On February 19, 1981, she expired. Autopsy of the head revealed four independent lesions in both lateral ventricles, the suprasellar region and the residual lesion which had been resected subtotally at the operation, respectively. Entire falx cerebri was invaded continuously by the tumor in occipito-suboccipital area. Microscopically, all tumors were identical in histology and were composed of spindle shaped cells with storiform pattern, foam cells and rare giant cells. Any mitotic figures were not detected. Clinical behavior was aggressive but histological figures suggested fibrous histiocytoma.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical studies of sulbactam/cefoperazone in the field of obstetrics and gynecology].

Sulbactam (SBT), a new beta-lactamase inhibitor, in combination with cefoperazone (CPZ) was studied for the clinical evaluation in the field of obstetrics and gynecology. SBT/CPZ was given to 6 cases with the following infections; 4 of pyometra, 1 of Douglas-abscess and 1 of pelvic cellulitis. Five cases were old patients accompanied by cancers, and 2 cases were refractory to other antibiotics therapy. The clinical efficacy was excellent in 1 case, good in 3 cases, poor in 2 cases. No side effects nor abnormal laboratory findings associate with the agent were observed.

[A family of von Recklinghausen's disease associated with bilateral acoustic neurinomas and falx meningiomas].

A familial or inherited occurrence of von Recklinghausen's disease has been reported hitherto, some of which were associated with intracranial tumors such as gliomas, neurinomas, meningiomas or others. Authers report a family (case 1: father, case 2: son, case 3: daughter) of von Recklinghausen's disease associated with both bilateral acoustic neurinomas and falx meningioma. Case 1. A 41 year-old male was admitted to our hospital with complaints of bilateral hearing disturbance, headache and loss of visual acuity. On admission, neurological examination revealed left papilledema, right cataracta, impairment of bilateral cranial nerves V, IX, X, bilateral deafness, cerebellar ataxia, motor weakness of lower extremities and sensory disturbance of peripheral region of extremities. Twelve subcutaneous nodules were noted over the entire body and Café-au-lait spots were found slightly to be scattered over the chest and the back. Craniogram and vertebral angiography demonstrated bilateral CP angle tumors, and the diagnosis of bilateral acoustic neurinomas was made. Subtotal removal of bilateral tumors and Torkildsen's shunting were carried out. Histological diagnosis was neurinoma. Post-operative course was good. But 6 years later, he complained of gait disturbance. Computed tomography revealed falx meningioma in the frontal lobe and recurrence of bilateral acoustic neurinomas. So meningioma was totally removed. Eight months after surgery he died of aspiration pneumonia aged 47. Case 2. A 15 year-old male, son of case 1, was admitted with complaints of bilateral deafness, gait disturbance and visual disturbance. Neurological examination disclosed left papilledema, right congenital cataracta, impairment of bilateral cranial nerves V, IX, X, ataxic gait and sensory disturbance of right lower extremity.(ABSTRACT TRUNCATED AT 250 WORDS)

CNS dysplasia in dysencephalia splanchnocystica (Gruber's syndrome). A case report.

A macrosomic male infant with multiple malformations survived for 4 days. His external dysplasias comprised macrocephalus, cheilopalatoschisis, auricular anomalies, and unilateral hexadactyl; his internal dysplasias included cysts of kidneys and pancreas, and a patent foramen ovale. The child had frequent generalized convulsions and died of bronchopneumonia. Chromosomal analysis was normal. The main neuropathological findings were a cleft foramen magnum, micropolygria and heterotopia of the neocerebrum, hypoplasia of the vermis and central white matter of the cerebellum, diffuse heterotopia of Purkinje cells, and unique heterotopic gray matter in the central cervical cord. The infant's disorder was classified as Gruber's syndrome, and this report may be the first detailed description of CNS malformations in this syndrome which, however, are probably not specific for this syndrome. The neuropathological findings were compatible with a heterochronic pathogenesis. this and the familial occurrence of malformations suggest a genetic nature of the syndrome.

Adult metachromatic leukodystrophy. IV. Ultrastructural studies on the central and peripheral nervous system.

Ultrastructural studies on the central and peripheral nervous system of 2 patients with adult onset metachromatic leukodystrophy (MLD), dead at the ages of 46 and 51 years, showed MLD-specific inclusions, tufaceous and prismatic structures, a wide spectrum of membranous arrangements within lysosomal residual bodies, and the intimate admixture of sulfatides and other membranous material with lipopigments. Oligodendrocytes and Schwann cells were foremost affected but membranous inclusions could also be verified in neuronal perikarya and astrocytes. The varying ultrastructural spectrum of lysosomal residual bodies in adult onset MLD and the association with lipopigments, chiefly in nerve cells, exceed the fine structural observations on late infantile and juvenile MLD and may reflect morphological differences between these subtypes of MLD that are also known from clinical and biochemical observations.
