[Retrospective analysis of hip joint development after pericapsular Pemberton ilium osteotomy (modified by Tönnis) in conjunction with intertrochanteric corrective osteotomy].

A review of 45 hips with disorders (congenital dislocation of the hip, subluxation of the hip, hip dysplasia with or without secondary pertheslike changes) treated by pembertons pelvic osteotomy with femur osteotomy is shown. Its a followed-up about two years. Many of them (85%) had satisfactory roentgenographic results and its showed the effectiveness of this procedure. We think the best time for treatment is the age between 2 and 4 years.

[Rapid prenatal diagnosis of familial amyloidotic polyneuropathy using DNA amplification].

We have used in vitro DNA amplification by the polymerase chain reaction for prenatal diagnosis of familial amyloidotic polyneuropathy (FAP) in a German family. Fetus DNA was prepared from chorionic villus sampling and specific prealbumin DNA sequence that may carry FAP type I mutation was enzymatically amplified. The mutant prealbumin gene could be distinguished by direct DNA analysis with Nsi I using amplified DNA. This method is sufficiently sensitive and rapid that prenatal diagnosis of FAP can be done on the same day when fetal DNA is available.

Molecular structures and evolution of mouse isozyme genes functioning in the malate-aspartate shuttle.

To examine molecular mechanisms of transcription of mammalian isozyme genes functioning in the malate-aspartate shuttle and to observe structural and evolutionary relationships, we investigated gene organizations of cAspAT and mAspAT, and cMDH and mMDH, and isolated and characterized cDNAs and genomic DNAs for these isozymes in mice. The deduced amino acid sequences of mouse cAspAT and mAspAT showed about 47%, and those of mouse cMDH and mMDH, about 23% overall homology. Surprisingly, the homology between the mouse cMDH and thermophilic bacterial MDH, as well as the homology between the mouse mMDH and E. coli MDH, markedly exceeds the intraspecies sequence homology between mMDH and cMDH from mice. The first duplication of a common ancestral MDH gene should thus have occurred long before the emergence of the eukaryotic cells, and subsequently, the mammalian mMDH and E. coli MDH genes have evolved from one of the duplicates. The mammalian cMDH and Thermus flavus MDH genes have no doubt evolved from one of the other duplicates. Moreover, structural organizations of the two-pairs of isozyme genes indicated that introns antedate the divergence of these mitochondrial and cytosolic isozyme genes. The 5' ends of all four isozyme genes lacked the TATA and CAAT boxes characteristic of eukaryotic promoters but did contain G + C-rich sequences and multiple transcription-initiation sites. We found several highly conserved regions in the 5' flanking sequences between mAspAT and cAspAT, between mMDH and mAspAT, and between cMDH and cAspAT genes.

[A primary malignant lymphoma of the appendix].

A malignant lymphoma (ML) of the gastrointestinal tract occurs most frequently in the stomach, and ML of the appendix is very rare, with only 16 cases reported in the Japanese literature. Reported is a case of a primary ML the appendix in a 48 year old male, in whom an ileocecal resection was carried out. This case was pathohistologically diagnosed as a diffuse large lymphoma of the B cell type, according to the LSG classification. Therapeutic problems were encountered because of the difficulty in making an early diagnosis.

Variations with age and serum cholesterol level in the topographic distribution of macroscopic aortic atherosclerotic lesions as assessed by image analysis methods.

The topographic distribution of macroscopic atherosclerotic lesions in the aorta was studied by image analysis of aortas taken at autopsy from 155 males aged 10 to 54 yr. In both the thoracic and abdominal aorta, fatty streaks (FS) appear before raised lesions (RL) affecting preferential areas located longitudinally along the intercostal ostia, at the aorta bifurcation, and surrounding the points of origin of branches of the abdominal aorta. Later RL appear in the same areas of the aortic intima affected by FS at younger ages. As for the effect of serum total cholesterol (TC) in the progression of atherosclerosis, the prevalent involvement areas by FS and RL were similarly distributed as indicated by our analyses. However, areas showing frequent occurrence of FS and RL in each aortic segment were distributed more extensively at identical areas in the higher serum TC groups than in the lower TC level group. Thus, it can be concluded that FS are the precursor lesions of RL in the aorta and that the serum TC promotes the progression of atherosclerosis.

[Treatment of radiostomatitis with Salcoat].

Most malignancies occurring in the oral cavity are squamous carcinoma for which radiotherapy as a treatment is the first choice. However, radiostomatitis, which occurs as a concomitant symptom due to radiation, largely disturbs the effect of radiotherapy. Antibiotics or a gargle are used to treat this concomitant symptom, but these preparations are not satisfactorily effective especially in cases in which the symptoms are severe and/or wide spread. Salcoat, a spray-type preparation of Beclomethasone dipropionate that is used to treat stomatitis, has excellent features which compensate for the disadvantage of the previously mentioned therapy. This preparation can be administered in a non-contact manner, and it contains hydroxypropyl cellulose which assures good adhesion to the mucosa, thus covering and protecting the affected area for a long time. In our clinical trials, this preparation was found to be highly useful in relieving pain and dysgeusia due to radiostomatitis, there by maintaining the quality of life.

[Consideration on transfusion in urologic surgery--a retrospective study on homologous and autologous transfusions].

To obtain data for promoting autologous transfusion in urologic surgery, the cases of perioperative homologous transfusions in the past 5 years are reviewed. Although the mean perioperative homologous blood transfusion rate was not so high (10.2%), the rate in common urological surgery was comparatively high (eg. radical nephrectomy 39.4%, simple or palliative nephrectomy 21.6%, total nephroureterectomy 33.3%, renal allotransplantation 82.8%, total cystectomy 96.2%). Mean homologous transfusion volume in these surveys was 982 ml. Two successful cases of autologous blood transfusion by the predeposit method or intraoperative collecting method are presented. Transfusion-associated adverse effects, especially viral infection and immunosuppression, were discussed and the value of autotransfusion was stressed.

Observations on cardiac ultrastructure of the adult Japanese newt, Cynops pyrrhogaster.

Investigation of atrial and ventricular cardiac ultrastructure by transmission and scanning electron microscopy of the adult Japanese newt, Cynops pyrrhogaster, has revealed some unique cell types and features in this amphibian. Typical urodelan-type thin trabeculae composed of myocytes with conventional architecture of loosely scattered myofibrils were observed. Scarce granules of variable size and density were seen in atrial myocytes, and some Purkinje-like fibers were observed. Unique structures included scattered endocrine-like cells with dense cytoplasmic granules in the epicardial spaces, a distinct granular cell found occasionally on the endocardial surface projecting into the lumen of the cardiac trabeculae, and peculiar granulated inclusions in the myoplasm. The function of these possible endocrine cells remains undetermined. It is important to characterize the heart of this species, as it has recently been proven to be an excellent model for prolonged experimental studies in vitro.

Structure of the ornithine transcarbamylase (OTC) gene and DNA diagnosis of OTC deficiency.

Complementary and genomic DNA clones corresponding to human ornithine transcarbamylase (OTC) have been isolated and analyzed. Restriction fragment length polymorphisms (RFCPs) on OTC gene locus were analyzed in Japanese female population. Based on these data, a prenatal diagnosis of OTC deficiency was carried out in a risk family. A C-to-T substitution was found in exon 5 of OTC gene is an another female patient.

Further characterization of sulfated homopolysaccharides as anti-HIV agents.

Fucoidan and dextran sulfate showed anti-HIV activities against mononuclear cells from AIDS patients, and they abrogated HIV reverse transcriptase (RT) activity by interacting with the HIV envelope in the membranes of target cells. Furthermore, they showed a synergistic effect with azidothymidine (AZT).

Retention behavior of cardiac steroids using cyclodextrin in the mobile phase in high-performance liquid chromatography.

The retention behaviour of twenty cardiac steroids and four fluorescent derivatives was examined by the addition of cyclodextrin to the mobile phase in reversed-phase high-performance liquid chromatography. The addition of a suitable cyclodextrin improved the separation of isomeric cardiac steroids. The steroid A/B ring junction is the most important factor in the choice of the optimum cyclodextrin to be added; the C/D ring junction is less important. The hydroxyl group at the 3- or 12-position of the steroid enhanced the changes in retention times of these compounds. The effect of an unsaturated lactone ring at the 17 beta-position on the retention in the presence of cyclodextrin was also determined with cardenolide (five-membered ring) and bufadienolide (six-membered ring) but little difference was observed. Isomeric cardiac steroids, whose separation has not been done by the conventional method, were clearly separated by this method. The fluorescence intensity of 3-(1-anthroyl)-cardiac steroid was enhanced by the addition of cyclodextrin to the mobile phase.
