Labeled avidin-biotin enzyme-linked immunosorbent assay for detecting antibody to infectious laryngotracheitis virus in chickens.

A labeled avidin-biotin enzyme-linked immunosorbent assay (LAB-ELISA) for detecting antibody to infectious laryngotracheitis (ILT) virus in chicken sera was developed and compared with ordinary ELISA. Purified ILT virus, biotin-labeled anti-chicken IgG rabbit IgG conjugate, and horseradish-peroxidase-labeled avidin were used in the LAB-ELISA. When sera from farm chickens were tested by serum neutralization (SN) and two kinds of ELISA, the correlation rate between SN and LAB-ELISA was 50/50 (100%), and that between SN and ordinary ELISA was 39/50 (78%). In LAB-ELISA, all of the sera that were antibody-negative by SN had low absorbance (A) values (below 0.05), and the A values were closely correlated with the SN indexes. In ordinary ELISA, however, the sera antibody-negative by SN had various A values ranging from 0.06 to 0.32. LAB-ELISA had much lower nonspecific reactions than ordinary ELISA against sera from ILT-negative chickens, even when chickens were 30 weeks old. ILT antibody production after ILT vaccination could be detected by LAB-ELISA. A values peaked 5 weeks postinoculation and were maintained for 17 weeks.

The metabolism of roxatidine acetate hydrochloride. Liberation of deuterium from the piperidine ring during hydroxylation.

The metabolism of roxatidine acetate hydrochloride (RA), a new histamine-2 receptor antagonist, was studied by GC/MS in rats and dogs in vivo. The co-administration of 14C-RA and RA-d10 labeled with deuterium in the piperidine ring expedited the isolation and identification of 15 urinary metabolites. The major metabolites in both animals were M-1, M-8, M-10, and M-11; M-4 could be found only in the rat. The aromatic and piperidine ring-hydroxylated metabolites were found in small amounts in both species. Following the administration of RA-d10 to rats and dogs, oxygenated metabolites on the piperidine ring, such as M-3 and M-4, were isolated and their analysis indicated the unexpected loss of three or four deuterium atoms from the ring. Also, first and second isotope effects were observed on the conversion rate in vivo and retention time in HPLC, respectively.

Significance of postoperative irradiation for stage III lung cancer.

The significance of postoperative irradiation for stage III lung cancer was analyzed in 30 patients. Radiation was given to 15 of the patients and the remaining 15 did not receive any radiation therapy following surgical intervention. A total dose of 40 to 70 Gy was given to the radiation group with a fraction dose of 2 Gy five times a week using cobalt 60 gamma-ray or linac 10 MV X-ray. There was no significant difference of survival time between these two groups. However, in analyzing modes of operation, radiation seemed to improve the survival rate in patients who underwent curative or relatively curative operations (P = 0.1), while the patients who underwent non-curative operations did not receive any benefit from the postoperative irradiation. Some reasons for the ineffectiveness in cases of non-curative operation are discussed.

Role of sodium pump systems to determine sensitivity to mitomycin C in non-small cell lung cancer cell lines.

There are some active transport systems in the cell membrane, such as potassium pump, calcium pump, and proton pump. Although it has been reported that sodium/potassium and sodium/calcium pumps of cell membrane play roles in the intracellular accumulation of anticancer agents, the significance of the active transport channels in accumulation of mitomycin C (MMC), one of the most active agents for non-small cell lung cancer (NSCLC) has been unclear. In this study, we evaluated the role of the potassium pump, calcium pump, and proton pump as determinants of the sensitivity to MMC in vitro by using the selective inhibitors, ouabain, verapamil or AG-2000 (an active metabolite of Lansoplazole), respectively. PC-9 and PC-9/MC4 cell lines which are sensitive and resistant to MMC were used for these experiments. PC-9/MC4 was 9.4-fold more resistant to MMC than PC-9 cells. Relative resistance was not significantly changed by co-incubation with a non-cytotoxic dosage of these inhibitors. From these results, it was revealed that the active transport systems in cell membrane do not play a role in determining the sensitivity to MMC and the acquisition of resistance to MMC in PC-9 cell lines. Intracellular bioactivation may be an important factor to determine sensitivity to MMC in NSCLC cells under aerobic conditions.

Effect of proton pump inhibitor on cell growth and sensitivity to cis-diamminedichloroplatinum(II) in non-small cell lung cancer cell lines.

Recently, the importance of the potassium pump in the cellular accumulation of cis-diamminedichloroplatinum(II) (CDDP) has been reported. In this study we evaluated the role of the proton pump as a determinant of the sensitivity to CDDP in non-small cell lung cancer cell lines in vitro by using a selective proton pump inhibitor, AG-2000. PC-9 and PC-9/CDDP cell lines, which are sensitive or resistant to CDDP, were used for these experiments. PC-9/CDDP was 17.4-fold more resistant to CDDP than PC-9 cells. Relative resistance was not altered by co-incubation with a non-cytotoxic dosage of AG-2000. From these studies, it was shown that the proton pump inhibitor AG-2000 did not enhance the sensitivity to CDDP. However, as AG-2000 is not cytotoxic and does not compromise the CDDP-sensitivity in NSCLC cells at the concentration of clinical use for gastroduodenal ulcer, AG-2000 can be used with CDDP in chemotherapy for lung cancer.

Potentiation of the antidipsogenic action of alpha-human atrial natriuretic polypeptide at the preoptic area in spontaneously hypertensive rats.

The effect of alpha-human atrial natriuretic polypeptides on fluid intake following administration of angiotensin II and 48 hr water deprivation was assessed at the preoptic area in spontaneously hypertensive and normotensive Wistar-Kyoto rats. Angiotensin II (100 ng), injected into the preoptic area, and water deprivation increased the intake of water and of a 2.7% NaCl solution in spontaneously hypertensive as well as in Wistar-Kyoto rats. These intakes were potentiated in the spontaneously hypertensive rats. The fluid intakes evoked by angiotensin II were dose-dependently antagonized by alpha-human atrial natriuretic polypeptide and angiotensin II, administered into the preoptic area, in spontaneously hypertensive but not in Wistar-Kyoto rats. In 48 hr water-deprived rats, alpha-human atrial natriuretic polypeptide also inhibited the fluid intake in spontaneously hypertensive but not in Wistar-Kyoto rats. The results suggest that atrial natriuretic polypeptide and angiotensin II may play an important role in controlling fluid intake at the preoptic area level and that these peptides might be involved in the pathogenesis of spontaneously hypertensive rats.

The feasibility and efficacy of right ventricular assistance without thoracotomy.

Mechanical right ventricular assistance has never been percutaneously instituted. We invented a new method for administering right ventricular assistance without thoracotomy (RAWT) and evaluated its effects in acute experiments using dogs. To institute RAWT, an outflow cannula was inserted into the pulmonary artery from the femoral vein, and an inflow cannula was inserted into the right atrium from the right jugular vein. A Swan-Ganz catheter inserted into the outflow cannula led it to the pulmonary artery under blood pressure monitoring only. A Sarns' centrifugal pump was used as a blood pump. We applied this system to biventricular failed hearts supported by a left ventricular assist device. We could increase cardiac output from 53.3 +/- 31.8 to 77.8 +/- 31.7 ml/min/kg, and mean aortic pressure from 43.5 +/- 12.3 to 57.5 +/- 11.2 mm Hg with RAWT. The right ventricular unloading effect of RAWT was not disturbed by regurgitation through the pulmonary or tricuspid valve.

Use of cold spray for relieving pain from local anesthetic injections in ocular surgery.

To relieve pain from local anesthetic injections, the authors tried lowering the skin surface temperature by spraying a commercially available cold spray immediately before the injections. A noncontact thermometer was used to measure the thermal change of the skin surface. An esthesiometer was applied to determine the surface perception of the skin. Our basic experiments indicated that there was a relationship between the thermal change and the perception. When the surface temperature dropped to 10 degrees C, there was a definite decrease in the perception. Based on these findings, the authors applied the spray to 100 patients undergoing cataract surgery prior to both akinesia and retrobulbar anesthesia at a distance of 10 cm from the skin surface for 2 seconds, to be immediately followed by the injections. We found the spray to be effective in decreasing the pain due to these injections. Furthermore, there has been no major complications of the spray.

Area of paresthesia as determinant of sensory block in axillary brachial plexus block.

BACKGROUND AND OBJECTIVES: Paresthesia is widely considered a useful indicator for locating components of the brachial plexus using the axillary approach. While establishing axillary brachial plexus blocks, the authors attempted to correlate the area of paresthesia with the effectiveness of the sensory block. METHODS: Axillary brachial plexus blocks were established in 222 consecutive adult patients scheduled for elective surgery of the hand or forearm. After paresthesia was elicited in the distribution of one of the three terminal nerves of the hand (median, radial, or ulnar nerve), 30 microliters 1.5% mepivacaine without epinephrine was injected after advancing an indwelling catheter. Sensory block of each terminal nerve was assessed 20 minutes after the injection. RESULTS: The median and radial nerves were blocked with the highest reliability (87% and 93%, respectively) after paresthesia was elicited in the areas supplied by the corresponding nerves. The ulnar nerve was blocked most reliably ( > 85%) after the elicitation of paresthesia in its area of distribution as well as when no paresthesia was induced. The most reliable block of the musculocutaneous nerve (72%) was achieved after paresthesia was induced in the area supplied by the median nerve. CONCLUSION: Eliciting paresthesia at the nerve supplying the area of a planned surgical incision is a reliable determinant of successful axillary brachial plexus block.

Effects of interleukin-6 on in vitro cell attachment, migration and invasion of human ovarian carcinoma.

High levels of IL-6 have been observed in ascitic fluid in patients with ovarian cancer. In this study, we examined the effects of IL-6 on in vitro invasiveness of human ovarian cancer cells. Human ovarian cancer cells, NOM1 and SKOV, were used. Cell attachment to Matrigel, migration, and invasion were examined with or without IL-6. Zymography was performed to analyze gelatinase production by these cells. NOM1 cell attachment was increased by IL-6 (0 to 10 ng/ml). In a migration assay, IL-6 ranging from 0 to 10 ng/ml showed chemotactic and/or chemokinetic activities for the two cell lines. The invasiveness of these cells was significantly enhanced by IL-6 at the above concentrations. However, IL-6 did not modulate the production of either MMPs or TIMPs. IL-6 affects cell invasion through its effects on attachment and migration in human ovarian cancer cells. IL-6 might thus contribute to the progression of this disease.

[Basic study on blood pressure measurements with a digital sphygmomanometers].

In the present study, digital sphygmomanometer (SMmeter)(Omuron Digital 709 Fuzzy and Digital 806F)for family use and 24 hour ambulatory SMmeter were checked in terms of reproducibility (CV%) of blood pressure(BP) measured and comparison of measured BP with Digital SMmeter with those measured with mercury SMmeter, the golden standard. Reproducibility of BP measured with Hg-SMmeter was less than 4%, with each Digital SMmeter less than 9%. Inter-unit reproducibility was less than 5% in each SMmeter. Systolic BP measured with 24 hour ambulatory SMmeter were higher by 2-4% than BP measured with Hg-SMmeter, diastolic BP higher by 4-9%. Systolic BP measured with Digital 709 Fuzzy were higher by 5-10% than Hg-SMmeter in lower range of systolic BP to be actually measured, in 130-170 mmHg both values almost coincident, in more than 170 mmHg Digital 709 Fuzzy lower by 2-7%. Diastolic BP measured with Digital 709 Fuzzy were higher by 10-15% than Hg-SMmeter in lower range of diastolic BP to be actually measured. In more than 80 mmHg both values were almost coincident. Systolic BP with Digital 806F were higher by 1.5-7%, diastolic BP higher by about 10% than the value with Hg-SMmeter. In conclusion, by taking account of different reproducibilities and characteristics of SMmeter, home BP monitoring can be performed by patient or/and his family.

Exposure to sorbitol induces resistance to cisplatin in human non-small-cell lung cancer cell lines.

Cisplatin is the most active anticancer agent for lung cancer. It has been reported that intracellular accumulation of cisplatin is important in determining resistance to cisplatin, which may be modulated by Na+, K(+)-ATPase activity. On the other hand, it is well-known that sorbitol, a metabolite of glucose mediated by aldose reductase, reduces Na+, K(+)-ATPase in diabetic neuropathy. In this study, the effect of exogenous sorbitol on Na+, K(+)-ATPase activity and sensitivity to cisplatin was evaluated using human non-small-cell lung cancer (NSCLC) cell lines. In the NSCLC cell lines, EBC-1, PC-3, and RERF-LC-MS the cytotoxicities of cisplatin were impaired by exposure to sorbitol in these cell lines. Na+, K(+)-ATPase was inactivated and intracellular accumulation of cisplatin was decreased by the exposure. These results suggest that accumulation of sorbitol may induce resistance to cisplatin in NSCLC cells, and diabetes poorly controlled may be one of the determinants of the antitumor effect of cisplatin in NSCLC.

Failed axillary brachial plexus block techniques result in high plasma concentrations of mepivacaine.

BACKGROUND AND OBJECTIVES: Unintentional extrasheath injection causes failed axillary brachial plexus block. We wanted to find out if extrasheath injections produce higher plasma concentrations of local anesthetics compared to intrasheath injections. We also studied the incidence of extrasheath injection with radiographs. METHODS: Axillary brachial plexus blocks were established using a catheter technique. Fifty milliliters of 1.5% mepivacaine without epinephrine mixed with contrast medium was injected through the catheter. An anteroposterior radiograph was used to determine the distribution of contrast medium. Mepivacaine concentrations in arterial plasma were compared when local anesthetic solution was injected unintentionally outside of the axillary neurovascular sheath (n = 6) and when it was injected correctly into the sheath (n = 6). The incidence of extrasheath injection was studied in a different series of 109 patients. RESULTS: Arterial plasma mepivacaine concentrations were higher after extrasheath injection [8.0 (6.3-9.7) vs 5.8 (4.5-7.0), microg/mL, means (95% confidence intervals), P < .05]. Pharmacokinetic parameters such as mean residence time and total clearance did not differ between intra- and extrasheath injections. Extrasheath injection was observed in 3.7% (4/109) of cases. CONCLUSION: Failed extrasheath injection of 50 mL 1.5% plain mepivacaine produces higher arterial plasma concentration in axillary brachial plexus block.

Carcinoid somatostatinoma of the papilla of Vater: a case report.

A 57 year-old Japanese man with a carcinoid somatostatinoma of the papilla of Vater is presented. He was found to have cholecystolithiasis without any symptoms. Physical examination showed no abnormal findings. Routine laboratory data gave normal results, except for glucose intolerance and an elevated somatostatin concentration. A yellowish papillary tumor was found at the papilla of Vater, and histological examination suggested the diagnosis of carcinoid. He underwent a pancreatoduodenectomy in March 1992. The gallbladder contained a single pure cholesterol stone. Histological, immunohistochemical, and electron microscopic studies resulted in the diagnosis of a carcinoid somatostatinoma of the papilla of Vater, without regional lymph node metastases. Post-operative pancreatic juice output from the total pancreatic duct drainage increased to more than 1000 mL/day. Although an anastomotic leakage of the pancreatojejunostomy was noted, the pancreatic fistula closed 8 weeks later. His postoperative somatostatin value was normal. He has been well for 54 months following surgery, without any signs of recurrence.

The different stimulatory effect of normal tissues on the secretion of matrix metalloproteinases and their inhibitors by human ovarian cancer cells.

To investigate the influence of normal tissues on secretion of matrix metalloproteinases (MMPs) and their inhibitors (TIMPs) by ovarian cancer cells, conditioned medium of several normal human tissues (peritoneum, myometrium, and ovary) was added to ovarian cancer cells (NOM1). MMPs were detected by zymography and TIMP-1 levels were assayed by ELISA. The MMP-9 level was significantly higher in peritoneal conditioned medium than in myometrial and ovarian conditioned medium, but the TIMP-1 level was the lowest. Addition of conditioned medium from these normal tissues to NOM1 cells increased the levels of MMPs and TIMP-1 in NOM1 conditioned medium. Peritoneal conditioned medium induced the highest MMP-9 level in NOM1 conditioned medium, while TIMP-1 was the lowest. Conditioned medium from the normal tissues also significantly increased invading cell numbers, with peritoneal conditioned medium being the most potent. These results showed that secretion of MMPs and TIMP-1 is regulated by normal tissues and peritoneum provides the best conditions for ovarian cancer cell invasion.
