[Incidence of tricuspid regurgitation in normal subjects according to a new Doppler echographic criterion].

To develop a Doppler echocardiographic criterion for tricuspid regurgitation (TR) and to determine the incidence of TR in normal subjects, we examined 357 apparently normal subjects ranging in age from five to 95 years using pulsed and continuous wave Doppler echocardiography. A Doppler transducer was placed over the right ventricular apex, with the Doppler beam directed parallel with TR flow. TR was judged to be present when a holosystolic abnormal flow pattern with the peak velocity greater than 1.5 m/sec was recorded at the tricuspid valve orifice. This was based on the theory that in TR systolic pressure gradient across the tricuspid valve should be 10 mmHg or greater in normal subjects. TR was detected in 87 (24%) of 357 subjects and the mean peak velocity of the TR was 2.1 +/- 0.2 m/sec (mean +/- SD). It was more frequent in women (28%) than in men (19%) and differed significantly among the young, middle and old age groups. It was 79% in the first decade, 30% in the second, 10% in the third, 21% in the fourth, 5% in the fifth, 19% in the sixth, 14% in the seventh, 23% in the eight, 37% in the ninth and 40% in the tenth. The minimum dimension of the tricuspid annulus was significantly enlarged (p less than 0.001) in subjects with TR (2.2 +/- 0.3 cm/m2) compared to that of subjects without TR (1.8 +/- 0.3 cm/m2).(ABSTRACT TRUNCATED AT 250 WORDS)

[General pharmacological studies on isepamicin sulfate (HAPA-B)].

General pharmacological properties of isepamicin sulfate (HAPA-B), a new aminoglycoside antibiotic, were studied in animals and the results obtained were summarized below. Intramuscular injections of HAPA-B at doses of 500 mg/kg inhibited the writing response induced by acetic acid, and at doses of 1,000 mg/kg, caused muscle relaxation, respiratory depression, suppression of spontaneous motor activity and prolongation of thiopental anesthesia. Anticonvulsive action and the effect on the rectal temperature were not observed. Intravenous Intravenous HAPA-B showed no significant effect on the general behavior and the function of the central nervous system at doses of 100 mg/kg. Intravenous injections of HAPA-B to anesthetized dogs resulted increases in the femoral arterial blood flow at doses of 12.5 mg/kg, decrease in the blood pressure and increase in the respiratory rate at doses of 25 mg/kg, and increase in the carotid arterial blood flow at doses of 50 mg/kg. Apparent changes were not recognized in the heart rate and electrocardiograms. In conscious rabbits, intravenous HAPA-B produced increases in the heart rate without significant changes of the blood pressure and electrocardiograms at doses of 100 mg/kg. Spontaneous beatings of isolated atria were depressed by HAPA-B in concentrations of 3 X 10(-4) to 10(-3) g/ml. The HAPA-B inhibited the gastric secretion at intramuscular doses of 500 mg/kg or intravenous doses of 100 mg/kg, and depressed charcoal transport through small intestine and the spontaneous movement of isolated ileum at intramuscular doses of 1,000 mg/kg and at concentrations of 3 X 10(-4) to 10(-3) g/ml, respectively. No irritative effect was found on the gastric mucous membrane. Intravenous HAPA-B inhibited the response of nictitating membrane to pre and post ganglionic stimulations of cervical sympathetic nerve at doses of 100 mg/kg. In in vitro test, HAPA-B inhibited nonspecifically the constrictive responses of trachea, aorta, stomach, ileum and vas deferens to various agonists in concentrations of 3 X 10(-4) to 10(-3) g/ml. Spontaneous movements of uteri of estrous or pregnant animals were depressed by HAPA-B at intravenous doses of 50 to 100 mg/kg and in in vitro at concentrations of 10(-4) to 3 X 10(-4) g/ml. Antidiuretic effect was also observed at intramuscular doses of 250 mg/kg. HAPA-B increased the length of the whole blood clotting time and raised the plasma glucose level at intramuscular doses of 1,000 mg/kg and inhibited the platelet aggregation induced by ADP in vitro at concentrations of 10(-3) g/ml.(ABSTRACT TRUNCATED AT 400 WORDS)

Pancreas cancer associated antigen from normal colonic mucosa (PCAAc) assayed with monoclonal antibody.

We prepared monoclonal antibodies against pancreatic cancer associated antigen (PCAA) from normal colonic mucosa (PCAAc) and prepared an assay system to detect the circulating PCAAc in sera. 200 patients with cancer including 85 pancreatic cancer and benign disease were compared with 40 normal healthy individuals. Diagnostic rate of pancreatic cancer was 64/85 (75%) but other malignancy also showed elevated PCAAc: 18/19 (94.7%) of hepatoma and 19/37 (51.4%) of gastric cancer. In pancreatitis, 8/17 (47.1%) showed elevated PCAAc. Compared with our previous reports of PCAA assay, these results indicated that diagnostic sensitivity of pancreatic cancer was increased but specificity of it was decreased.

An important role of the central amygdaloid nucleus and mammillary body in the mediation of conflict behavior in rats.

The present study was designed to elucidate the functional role of central amygdaloid nucleus (ACE) and mammillary body (MB) in the mediation of behavioral suppression using rat conflict punishment procedure. Lesion of ACE produced a significant and long-lasting increase in the punished responding during the experimental period. Rats with lesion of MB also showed a significant increase in the punished response 10-14 days after brain lesioning. These results demonstrated the important role of ACE and MB in the mediation of behavioral suppression such as conflict behavior.

Involvement of septal and striatal dopamine D-2 receptors in yawning behavior in rats.

A behavioral study was performed in an attempt to understand the neuronal mechanisms involved in yawning behavior in rats. Subcutaneous injections of low doses of apomorphine (0.05-0.25 mg/kg) or piribedil (0.2-1.0 mg/kg), which preferentially activate presynaptic dopamine autoreceptors at those doses, evoked yawning. Marked yawning responses were also elicited by both 3-PPP (5-20 mg/kg, SC) and TL-99 (1-2 mg/kg, SC). SK & F 38393, a dopamine D-1 receptor agonist, at doses ranging from 0.1 to 8.0 mg/kg (SC) induced neither yawning nor stereotypy. However, bromocriptine (0.5-32.0 mg/kg, SC), a dopamine D-2 receptor agonist, induced yawning for which the dose-response curves showed a bell-shaped form. After a higher dose of 32 mg/kg (SC) bromocriptine, some rats occasionally showed sniffing and sawdust chewing. Yawning responses induced by systemic injection of apomorphine, piribedil, 3-PPP or bromocriptine were wholly suppressed after treatment with sulpiride (10 mg/kg SC), a dopamine D-2 receptor antagonist. Bilateral injections of apomorphine (20 micrograms/side X 2), piribedil (100 micrograms/side X 2) or 3-PPP (50, 100 micrograms/side X 2) into the striatum or septum also elicited marked yawning. The results indicate that low doses of apomorphine, piribedil, 3-PPP, TL-99 or bromocriptine elicit yawning by stimulating dopamine D-2 receptors and striatal and septal dopaminergic systems may be related to the occurrence of yawning behavior.

Primary malignant thymoma in a 6-year-old boy.

Although not uncommon in adults, thymomas are the least common mediastinal tumors in children. The behavior of these tumors in children is partially distinct with a much more rapid course and a poor prognosis. A symptom-free 6-year-old boy was treated for malignant thymoma detected incidentally on a chest X-ray in a school mass examination. At operation, the tumor was found to have already invaded the surrounding tissue. Complete removal at the base of the invasive tumor is the treatment of choice.

Primary liposarcoma of the mediastinum--a case report and review of the literature.

Liposarcoma of the mediastinum is a rare disease. A 59 year old woman was seen with the complaint of dizziness. A chest radiogram taken during the course of investigation for hypertension revealed a large mass shadow 24 X 18.5 X 12 cm located in the anterior mediastinum. The tumor of the mediastinum was diagnosed as a benign lipoma when it was first resected, but subsequently proved to be a liposarcoma when it re-appeared 2 years and 10 month later. Based on the WHO classification, the mixed type of liposarcoma was diagnosed. The salient and pathological features of mediastinal liposarcoma are reviewed and compared with those nine cases reported in Japan and fifty in North America and European countries. Treatment by simple enucleation or shelling out of the tumor should be discouraged, since this seems to be the main cause of local recurrence. Surgical wide en bloc excision is the treatment of choice.

Insulin autoimmune syndrome with insulin-resistant diabetes at the incipient stage prior to hypoglycemic attacks.

Insulin autoimmune syndrome is characterized by spontaneous hypoglycemia, glucose intolerance, hyperinsulinemia and insulin-binding antibodies in serum without previous immunization. A 31-year-old man with Graves' disease developed insulin autoantibodies after therapy with methimazole. The patient was unique in that persistent hyperglycemia with polyuria and polydipsia had continued for several days before frequent hypoglycemic attacks appeared. We were able to extract a huge amount of immunoreactive insulin (116,000 microU/ml) with acid-ethanol from his serum obtained in the diabetic stage, and serum C-peptide immunoreactivity was as high as 268 ng/ml. The insulin-binding activity of his serum was quite potent, and when 1:5,000 diluted serum was incubated with 125I-porcine insulin, 71.2% of the label could be precipitated by polyethylene glycol. The insulin-binding protein was identified as mainly IgG with kappa light chains. Insulin-binding activity was not detected in serum obtained before methimazole therapy, suggesting that the drug was responsible for the induction of antibodies in this patient. The antibodies recognized porcine, sheep, bovine and horse insulins as well as human insulin. The mechanisms by which the antibodies produced hyper- and hypoglycemia have also been discussed.

Phosphorothioate analogues of phosphoribosyldiphosphate are substrates of quinolinate phosphoribosyltransferase.

Phosphorothioate analogues of phosphoribosyldiphosphate (P-Rib-PP) can act as substrates of quinolinate phosphoribosyltransferase. The order of effectiveness as substrates, with Mg2+ as the activating metal ion, was P-Rib-PP greater than P-Rib-PPbeta S greater than P-Rib-PP alpha S(Sp) greater than P-Rib-PP alsph S(Rp). Cd2+ was more effective than Mg2+ with the Rp diastereommer.

The digital plethysmogram as an index of ageing grade.

In order to determine whether the digital plethysmogram (PTG) can be used as an index of ageing or not, the PTG was recorded in 89 males and 110 females in a pilot study. Of the parameters which were calculated from the PTG, up-stroke time corrected by pulse rate (SXp time) showed the strongest correlation with age. The PTG was recorded in 1066 males and 761 females. There was a significant correlation between age and SXp time in both sexes. The change of SXp time with age in the younger age group was more rapid than in the old age group. From these results, SXp time of the PTG can be used as an index of ageing, especially in the young to middle-aged group.
