Effect of supplementing low protein diets with the limiting amino acids on the excretion of N1-methylnicotinamide and its pyridones in rats.

We have hypothesized that the ratio of the excreted by-products of niacin metabolism, N1-methyl-2-pyridone-5-carboxamide (2-pyr) + N1-methyl-4-pyridone-3-carboxamide (4-pyr)/N1-methylnicotinamide (MNA), might be useful as an index to assess the adequacy of amino acid intake in rats. The experiment reported herein was performed to test this hypothesis. When a 10, 20 or 40% casein diet supplemented with 0.1, 0.2 or 0.4% L-methionine, respectively, was fed to rats, the urinary excretion of MNA decreased, and that of 4-pyr increased, as the level of dietary casein and methionine increased. Therefore, the ratio of (2-pyr + 4-pyr)/MNA increased with increasing dietary casein and methionine levels. When the limiting amino acids of casein or soy protein isolate were added to a low casein or low soy protein isolate diet, the urinary ratio of (2-pyr + 4-pyr)/MNA also increased. These results indicate that the increased urinary ratio of (2-pyr + 4-pyr)/MNA can serve as a biological marker for adequate amino acid intake.

Comparison of the cytoskeleton fractions of rat red blood cells prepared with non-ionic detergents.

The characteristics of cytoskeleton fractions prepared from rat red cell ghosts with four non-ionic detergents were studied. One percent (w/v) solutions of Triton X-100, Emulgen 911, MEGA-9 (nonanoyl-N-methylglucamide), and octylglucoside solubilized 78, 68, 80, and 92% of the ghost phospholipid, while they solubilized 82, 78, 72, and 62% of the ghost band 3, a transmembrane protein, respectively. There was no correlation between the solubilization percentages of phospholipid and band 3. Phospholipids retained in cytoskeleton fractions were shown to exist as blebs on the surface by electron microscopic observation. The cytoskeleton fraction prepared with octylglucoside retained about two-fold more band 3 than that with Triton X-100 (Triton shells). However, cytoskeleton fractions prepared from p-chloromercuribenzoate-treated ghosts with the two detergents retained almost equal amounts of band 3, less than 5% of that in the ghosts. Under this condition, most of band 2.1, a protein linking band 3 to the spectrin-actin network, was released from the cytoskeleton fractions. The band 3 solubilized with octylglucoside sedimented faster in a linear sucrose gradient and had a larger Stokes' radius than that with Triton X-100, which is known to exist as dimer. These results strongly suggest that octylglucoside does not disturb the association of tetrameric band 3 with the spectrin-actin network, while Triton X-100 dissociates tetrameric band 3 to the dimer, resulting in the difference in the amount of band 3 retained in cytoskeleton fractions. In conclusion, octylglucoside can produce a more native cytoskeleton fraction of red cell membranes than Triton shells.

Effects of epidural anesthesia on cardiovascular response and survival in experimental hemorrhagic shock in dogs.

The purpose of the present study was to assess the effects of epidural anesthesia on cardiovascular responses and survival in experimental hemorrhagic shock in dogs. Thirty mongrel dogs were randomly assigned to one of three groups on the basis of anesthetic technique: the upper-level group (n = 10), receiving general anesthesia plus upper-level (mainly thoracic region) epidural anesthesia; the lower-level group (n = 10), receiving general anesthesia plus lower-level (mainly lumbar region) epidural anesthesia; and the control group (n = 10), receiving general anesthesia alone. After withdrawal of blood, the changes in mean arterial pressure (40 mmHg) and cardiac index were similar in all groups. In the upper-level group, a lower heart rate and systemic vascular resistance than the control group were maintained throughout in the presence of severe hypotension. A significant difference in survival was seen between the upper-level and control groups over the 100-min observation period as a whole (P less than 0.05 by the Generalized Wilcoxon test), since, at the end of the period, only two of the ten animals in the control group survived, whereas nine of ten in the upper-level group survived (P less than 0.001 by the Kaplan-Meier test). This result demonstrates that, in dogs lightly anesthetized with halothane and nitrous oxide, upper thoracic level epidural anesthesia significantly improves survival in experimental hemorrhagic shock (compared with survival in dogs with lumbar epidural or no epidural anesthesia) when the epidural is performed before hemorrhage and when the mean arterial pressure is constant.(ABSTRACT TRUNCATED AT 250 WORDS)

Proliferative cell index in endometrial adenocarcinoma of different nuclear grades.

Proliferative cell indices (PCI) in 20 cases of endometrial adenocarcinoma were obtained by staining DNA polymerase alpha. The PCI ranged from 11.1 to 42.1, averaging 24.7 +/- 8.7. Four cases of stage III all exhibited fairly large PCI (30.4-32.9). In contrast, in 16 cases of stage I the values were spread over a wide range. In 13 cases with histological grade (HG) 1 of stage I, larger PCI were obtained in the nuclear grade (NG) 2 group; the mean PCI values of HG 1-NG 1 and HG 1-NG 2 were 16.5 +/- 4.6 and 25.6 +/- 5.2, respectively. Because of the good correlation between PCI and HG or NG, PCI may be useful as an additional prognostic factor in endometrial adenocarcinoma, especially in stage I cases.

The possible mode of escape of adult T-cell leukaemia cells from antibody-dependent cellular cytotoxicity.

Antibody-dependent cellular cytotoxicity (ADCC) was measured using 51Cr-labelled ATL derived cell lines as the target, peripheral mononuclear cells (PMNCs) from disease-free persons as the effector cells and heat-inactivated serum from patients with ATL or the IgG purified from this. The release of 51Cr was not usually demonstrated in the HTLV-1 non-producing ATL derived cell line (MT-1), but was evident in the HTLV-1 producing ATL derived cell lines (KT 252 and IT 607). The 51Cr-labelled MT-1 cells after induction of HTLV-1 retrovirus by 5-iodo-2'-deoxyuridine (IdUr), showed a remarkable target sensitivity in the ADCC assay. On the other hand, the 51Cr-labelled MT-1 cells after culture with IdUr and ATL patient's serum, had no ADCC sensitivity. The fresh ATL cells immediately separated from ATL patient's blood did not express HTLV-1 virus in the cell and had no ADCC sensitivity as the target. Based on these findings, an antigenic modulation on the ATL cell surface by ATL patient's serum is considered to be the possible mode of escape of ATL cells from ADCC.

Lymphoid blast crisis in a patient with Philadelphia-chromosome-negative chronic myelocytic leukemia.

A 24-year-old man with Philadelphia-chromosome (Ph)-negative chronic myelocytic leukemia (CML) developed lymphoid blast crisis. In the chronic phase, karyotype was normal and the clinical and hematological features were indistinguishable from those of Ph-positive CML. Rearrangement of the breakpoint cluster region (bcr) was observed. In the blast phase, blast cells showed early B-cell phenotype (CALLA+, Ia+, TdT+) with a rearranged immunoglobulin heavy-chain gene joining region (JH). By using an immunoblotting method and antiphosphotyrosine sera, P210bcr-abl protein was detected. The patient responded well to vincristine and prednisolone (VP) therapy. These findings support the concept that Ph-negative bcr+ CML can behave in a very similar fashion to Ph-positive CML, not only in the clinical features of the chronic phase but also in the manner of the blast crisis.

Decrease in neuronal uptake of noradrenaline simply explains the supersensitivity after sympathectomy in the rat iris dilator.

The effect of superior cervical ganglionectomy or long term treatment with guanethidine on the mechanical response of dilator muscle of the rat iris to several stimulants was examined. The dose-response curve for noradrenaline (NA) was significantly shifted to the left 7 days after denervation (ca. x 10) or by treatment with guanethidine (ca. x 10), but those for methoxamine (Meth) and acetylcholine (ACh) were not. Cocaine did not further sensitize the denervated muscle to NA, but it sensitized the normal muscle to an extent similar to that caused by denervation. Cocaine did not affect the sensitivity to Meth in both control and denervated muscles. The maximum response to NA decreased by about 30% or 20% after surgical or chemical denervation, respectively, while those to Meth, ACh and Ca2+ did not alter significantly. These results indicate that in the dilator muscle, the sympathetic denervation caused a purely specific supersensitivity to NA which was simply explained by a presynaptic mechanism: an impairment of the neuronal uptake process.

A key role of the mammillary body in mediation of the antianxiety action of zopiclone, a cyclopyrrolone derivative.

The present study was designed to elucidate the brain site of the anticonflict action of zopiclone (ZOP), a cyclopyrrolone derivative, using the rat conflict procedure. ZOP at 10 micrograms/microliters, bilaterally injected into the mammillary body (MB), produced a significant increase in the punished responses, with no change in the unpunished responses. There were no significant changes in these responses when ZOP was injected into the central amygdala, frontal cortex or dorsal hippocampus. Attention should be given to the possibility that the MB is the site of the anticonflict action of ZOP.

Immunomodulatory activity of the newly synthesized compound TOK-8801 (N-(2-phenylethyl)-3,6,6-trimethyl-5,6-dihydroimidazo[2,1-b]thiazole-2- carboxamide).

The in vitro and in vivo effects of the newly synthesized compound TOK-8801 (N-(2-phenylethyl)-3,6,6-trimethyl-5,6-dihydroimidazo[2,1-b] thiazole-2-carboxamide) on immune responses were investigated in comparison with that of levamisole (LMS). TOK-8801 enhanced the anti-sheep red blood cells (SRBC) plaque forming cells (PFC) response and mitogen-induced proliferative responses in murine splenocytes and thymocytes in vitro at concentrations of 10(-7)-10(-5) M, while LMS augmented these responses at 10(-4) M. The stimulatory effect of TOK-8801 as well as LMS on the antibody production was eliminated by the removal of T cells. TOK-8801 (0.5 mg/kg, p.o.) suppressed the in vivo generation of anti-SRBC PFC in normal mice, but the compound restored the depressed antibody production in restraint-stress mice which was mainly caused by helper T cell defects. From these results, TOK-8801 was shown to have an immunomodulatory activity in the antibody production.

Reinforcement of the anastomosis between the fragile arterial vessel and the prosthetic graft. A headband method.

A new method of anastomosis was devised, intended for the reinforcement of the anastomosed area between the fragile vessel of the patient and the prosthetic graft in cardiovascular surgery. This method, which may be called a headband method, consists of suturing a small prosthetic graft wrapping the anastomosed area together with the arterial vessel in the manner of a headband. Fourteen cases have been evaluated at follow-up periods ranging from 24 to 40 months. All cases proved successful without noteworthy complications.

Coexistence of cerebral aneurysm and renal angiomyolipoma--case report.

We report a late middle-aged female in whom a renal angiomyolipoma was discovered by ultrasound after she developed hemorrhagic shock following surgery for a ruptured cerebral aneurysm. This appears to be the first reported case of the coexistence of a cerebral aneurysm and a renal angiomyolipoma. The associations between cerebral aneurysm and various other pathological conditions are discussed, and attention is directed to diagnostic techniques and characteristic findings.

Phosphotyrosine phosphatase activity prevents the detection of P210bcr/abl protein in mature cells in chronic myelogenous leukemia even by an immunoblotting technique.

P210bcr/abl protein with tyrosine protein kinase has been implicated in the proliferation and differentiation of chronic myelogenous leukemia cells. Using an immunoblotting technique with antiphosphotyrosine (anti-P-Tyr) antibodies, we examined whether P210bcr/abl protein was expressed in chronic phase cells in patients with chronic myelogenous leukemia (CML). We could detect P210bcr/abl protein in blast cells regardless of myeloid or lymphoid lineage but not in chronic phase cells from patients. However, in a patient with both blast cells and chronic phase cells, we could identify the protein only after the enrichment of the blast crisis cells by Percoll gradient centrifugation. When K562 cells were mixed with mature granuloid cells, the P210bcr/abl in K562 cells detected by immunoblotting was decreased. Using phosphotyrosyl proteins in K562 cells as substrates, high phosphotyrosyl (P-Tyr) phosphatase activity was observed, not only in the lysate of chronic phase cells from CML patients but also in the lysate of neutrophils from normal subjects. These findings suggest the possibility that high P-Tyr phosphatase activity prevents the detection of P210bcr/abl in CML cells in the chronic phase. The activity may be characteristic of mature cells and may regulate cellular events through dephosphorylation of P210bcr/abl.

[A case of giant mediastinal cystic lymphangioma].

A 13 year-old boy was admitted because of anterior chest pain, fever up and exertional dyspnea. Chest X-ray film showed a large mass shadow on the middle and lower lung field with positive silhouette sign. Under the ultra-sonic tomogram the mass showed cystic pattern with septum. The bloody fluid collected by puncture suggested hemorrhage into the cyst. Chest CT scan revealed a well defined cystic mass in the left thoracic space. The mass reached to the right thoracic space through behind the Inferior Vena Cava (IVC). At operation, although the tumor adjoined heart, esophagus, diaphragma and IVC, it was removed completely. Pathological diagnosis was cystic lymphangioma.

[Omentum in the management of thoracic surgery: especially its application to tracheobronchial surgery].

Omentum is now not only policeman of the abdomen but also of the thorax. We applied omental pedicle flap in the management of 14 patients with thoracic surgery including chest wall reconstruction, empyema, thoracic skeletal infection and tracheobronchial problems. Especially, tracheobronchial reconstruction using omental pedicle flap for the patient combined with lung cancer (T4N2M0 STAGE IIIB) and asthma under steroid therapy was reported. The case was 71-year-old man with complaint of hemosputum. He had 5-6 year history of bronchial asthma with disturbed pulmonary function of % VC 44%, FVC1.0% 37%. Bronchoscopic study revealed the tumor invading the right side of trachea originating from right upper bronchus with histological diagnosis of moderately differentiated squamous cell carcinoma. Preoperatively, he experienced a heavy asthma attack which was controlled by steroid administration. Following extended right sleeve upper lobectomy, we applied omental pedicle flap around the reconstructed portion for the protection of infection, impaired wound healing due to postoperative steroid therapy and strong tension at anastomoses. Postoperative course was satisfactory. We suggest omental pedicle flap is an effective surgical armamentarium in the management of tracheobronchial surgery for the patient with strong anastomotic tension, immunocompromised condition, preoperative irradiation at bronchial stump and use of drug causing delayed wound healing (steroid, anticancerous drug etc).

[Myocardial protection of temperature].

The effects of myocardial temperature on 120-minute preserved hearts were studied on heterotopically transplanted hearts of rats of isogeneic strain. Rats were divided in the following groups; the control group, the 0 degree C preservation group, the 5 degrees C group, the 10 degrees C group, the 15 degrees C group and the 20 degrees C group. Experiments were divided into the acute stage experiments and chronic stage experiments. The former included electron microscopic studies of the myocardium of heart transplants and serum enzymes of recipients. In the chronic stage experiments, the percentage of fibrosis was compared as an index of success. No hearts of the 20 degrees C group resuscitated, and by the indices of acute stage experiments, the 10 degrees C group and the 15 degrees C group showed considerable impairment; other groups showed less damage of the same degree. The chronic stage experiments indicated that the 0 degree C group was best, followed by the 5 degrees C group in the increasing order of temperature, evaluated by the rate of fibrosis. It may be concluded that hypothermia close to the freezing point has the best protective effects on the myocardium. Cold injury, even if present, may be transient and not harmful when ischemia is about 120 minutes. Comprehensive evaluation of various factors contributing isograft survival should include not only various methods examined in the acute stage but also an over-all evaluation of fibrosis in the chronic stage, since the myocardium of the graft shows a variability of findings in the acute stage.

[Clinical study of hypertensive cerebellar hemorrhage: surgical indication and measurement of volume of hematoma].

There have been a few reports available for determining surgical indications in hypertensive cerebellar hemorrhage based on volume of hematoma on computerized tomography (CT). The authors then studied the clinical results of hypertensive cerebellar hemorrhage, and the surgical indication based on clinical findings and volume of hematoma on CT scan was considered. Forty-five patients with hypertensive cerebellar hemorrhage diagnosed by CT scan who were hospitalized to Matsue Red Cross Hospital from January 1980 up to December 1986 were studied. The 25 male and 20 female patients ranged in age from 52 to 85 years, and 16 of them were operated on. The results were as follows: 1) The Kanaya's neurological grading tended to be high in the patients with cerebellar vermis hemorrhage or a large volume of hematoma (greater than or equal to 30 ml). 2) In patients with grade I or II and a moderate volume of hematoma (15-30 ml), the patient complicated with hydrocephalus should be treated with ventricular drainage. The patients with grade III and IVa should be treated with surgical therapy (suboccipital craniectomy and evacuation of the hematoma). The patients with cerebellar vermis hemorrhage should be treated with surgical therapy. The patients with a large volume of hematoma (greater than or equal to 30 ml) should be treated with surgical therapy. The patients with grade IVb and V should not be treated actively because the prognosis is bad.(ABSTRACT TRUNCATED AT 250 WORDS)
