[Developmental changes in taurine and other sulfur-containing amino acid concentrations in fetal and neonatal rats].

The concentrations of taurine and other sulfur-containing amino acids in the fetal, neonatal and maternal liver, placenta, and whole fetal body between the 15th day of gestation and the 14th day after birth were determined using an automatic amino acid analyzer. In the fetal liver and placenta, the taurine concentration was the highest among all ninhydrin positive compounds. In these tissues the concentration of taurine increased significantly with the number of gestational days. Moreover, the total amount of taurine per fetus increased markedly up to term after the 15th day of gestation, and reached almost the same value as the total amount for adult rat liver. A striking difference in the changes in taurine concentrations during gestation between the fetal and maternal rat liver was observed: In contrast to the increase in the fetal liver, a significant decrease in the maternal liver was observed near term. But in these organs, no significant change in methionine, cystathionine and cysteine concentrations was observed during the perinatal period. These results suggest that taurine is supplied to the developing rat fetus from the maternal liver throughout gestation.

[Functional saving in the treatment of cervical cancer].

The radicality of Okabayashi's radical hysterectomy for treatment of cervical cancer has been well-known. However, its radicality also brought up the disturbance of fertility, endocrine and sexual function as well as the dysfunction of micturition and defecation. As early cervical cancer cases have increased yearly, and social rehabilitation became more necessary nowadays, functional saving is important and must be considered as the responsibility of the oncologists. This report denotes the preservation of ovarian function, sexual rehabilitation as well as the fertility in the treatment of cervical cancer.

[Problems in the treatment of cervical carcinoma].

With improved cytologic diagnosis and popularization of mass screening examination, early diagnosis and early treatment of cervical cancer have become possible, and the salvage has much been improved. However, some problems stil remain unsolved. The principle treatment of cervical cancer now consists of surgery and radiotherapy, which have produced excellent treatment results. Nevertheless the complication of injury to the pelvic organs, such as urinary tract and the rectum, is somewhat inevitable on applying surgery and radiotherapy. Since the early cancer cases of young women become increasing nowadays, functional conservation of the ovary and the reproductivity should also be considered. On the other hand, in the cases of advanced carcinoma and the recurrent carcinoma, the treatment by surgery and radiotherapy has its deficit and is considered to be inadequate. Therefore, a combination therapy including surgery, irradiation and chemotherapy has to be tried in order to improve the result.

[Treatment of cervical cancer in the aged].

The incidence of elderly patients in cervical cancer cases at The Department of Obstetrics & Gynecology, Okayama Medical School has been increases to 7.8% during the recent 5 years (1975-1979), comparing to that of the past period 1.9% (1934-1962). According to the statistics of The Ministry of Health and Welfare, the mortality of cervical cancer has been decreased in all the ages except the elderly patients, in which the mortality showed no marked fluctuation or somewhat elevation. According to the report of The Ministry of Health and Welfare, among the 2,014,843 cases of mass screening examination of cervical cancer, the incidence of elderly patient over 70 year old was 0.3% only, and the suspiciously malignant rate was 4.58% in this group. This means that in the elderly group, the incidence of mass screening examination was very low, and the morbidity was relatively high. Reviewing the elderly patients of the recent period, we found that about half of the patients were belong to the Stage III or IV, and even in the early cancer cases, the general diseases were complicated so much, that the main therapy was radiation rather than operation. Reviewing the prognosis and the irradiation dose in the radiation group, we found that the prognosis of the cases who received 4,000 rads external irradiation were better than that of the cases who received 5,000 approximately 6,000 rads. However, sometimes the complication of irradiation were too severe to continue, resulting in an incomplete treatment and bad prognosis. Considering about this, we select the cases to operate according to the physical state but not the age as indicated up to date, and the result was that, even in the aged group, the prognosis of the operable cases were comparable to that of the young aged. As a conclusion, the following can be said: 1) As the mean age has been prolonging, the incidence of the elderly cancer cases will increase progressively, and this will influence the treatment result of cervical cancer. 2) In the aged patients, the prognosis were influenced by the stage rather than the age, therefore, consideration of the treatment according to the stage is important.(ABSTRACT TRUNCATED AT 400 WORDS)

[Case report of large endometrial stromal sarcoma].

A 56-year-old-married woman, gravida 3, para 2, was admitted with chief complaints of atypical genital bleeding, large abdominal and genital tumors, dysuria and abdominal pain. All laboratory examinations were noncontributory. Abdominal total hysterectomy with bilateral salpingo-oophorectomy was performed. The tumor measured 30 X 11 X 8 cm and weighed 1,600 g. It showed polypoid growth and most of the tumor was necrotic. The tumor cells resembled endometrial cells and 20 mitotic figures per 10 high-power fields were noted. Ten months postoperatively, the patient was in good condition with use of PSK.

[Clinical studies of ifosfamide for gynecological malignancies].

This study was performed to evaluate the clinical effect and the side effect of ifosfamide in the treatment of gynecological malignancies. A total of 18 cases were treated with ifosfamide: 11 of them were treated as a single agent and the remaining 7 cases were treated as a combination chemotherapy. A daily dose of 1.5-2.0 grams were given for 5 consecutive days. The number of course varied from one to five. Six (35.3%) out of 17 evaluable cases showed partial response. Three were treated with ifosfamide alone and the another three were with a combination chemotherapy. The side effect such as leukopenia, gastrointestinal disturbance and general malaise were frequently observed. Chemical cystitis (hematuria) could be prevented by adequate water intake.

[Testosterone-estradiol binding globulin and sex steroids during pregnancy and at delivery].

Testosterone-estradiol binding globulin (TeBG), testosterone, estradiol-17 beta, index-T (testosterone not bound to TeBG) and index-E2 were determined during pregnancy, at parturition, in the umbilical artery and vein, and during postpartum periods. TeBG showed a rapid increase during the early pregnancy and then it reached a certain level at the mid pregnancy. At parturition, it decreased to about half of the late pregnancy. In the umbilical artery and vein the level was extremely low. Testosterone gradually increased from the early to the late pregnancy. However, index-T reached its lowest level during the 5th month of pregnancy. In the umbilical artery and vein testosterone exhibited a slightly lower level in comparison with the maternal blood but in contrast to this index-T exhibited five times higher level in the umbilical artery and vein than in the maternal blood. Estradiol-17 beta exhibited a linear increase from the early to the late pregnancy but index-E2 showed a sudden increase during the late pregnancy. The levels in the maternal blood at the parturition and in the umbilical artery and vein were compared: The estradiol-17 beta levels were maternal blood greater than umbilical vein greater than umbilical artery and the index-E2 levels were maternal blood .=. umbilical vein greater than umbilical artery.

Alterations of red blood cells' polyamines during pregnancy and neonatal period.

Red blood cell polyamines were measured using high-performance liquid chromatography. Spermidine and spermine continued to increase with pregnancy and retained a high level during 24-35 weeks of gestation, then decreased at term; putrescine, however, kept the low nonpregnant level during pregnancy and puerperium. At delivery, umbilical blood showed a significantly higher polyamine level than maternal blood (p less than 0.005 or p less than 0.001). In the neonatal period, each polyamine showed a slight increase on the 1st day after birth, then spermidine and spermine decreased day by day, but putrescine kept the same level. Alterations of polyamines seemed to have some relation to the erythropoiesis in bone marrow and rapid cell proliferations.

[Laboratory and clinical studies on cefotaxime in obstetrics and gynecology (author's transl)].

We have conducted basic and clinical examination of a new cephalosporin derivative, cefotaxime (CTX). The average level of transfer measured at various locations in uterus tissue and adnexa 15 to 105 minutes (53.4 minutes in average) after intravenous administration of this drug ranged from 1.5 mcg/g (myometrium) to 3.3 mcg/g (portio vaginalis). It was distributed in cervix uteri and portio vaginalis at highest concentration, followed by oviduct, tunica serosa and ovary, and to myometrium at lowest concentration. The same pattern of distribution was observed in transfer ratio of various location in uterus tissue to uterus arterial blood. The drug was transferred at very high level in pelvic cavity fluid after total hysterectomy throughout 3 postoperative hours. Five cases of gynecological infections receiving in total 10 to 42 g of CTX demonstrated 'excellent' results in 2 cases, and 'good' in 3 cases. Out of these, 3 cases did not respond to other antibiotic. Neither side effect nor clinical test abnormality was observed, except for one case, in which mild increase of GOT and GPT occurred. Based on the results of basic and clinical studies as described above, this drug is considered to have excellent efficacy and safety.

Estimation of gestosis of pregnancy (EPH-gestosis), relationship between fetal development and gestosis index.

Gestosis index score at delivery was compared with factors reflecting the fetal development in a series of 95 patients with EPH-gestosis collected in Okayama University Medical School in 1975-1979. The prediction of small for date (SFD) was performed by means of multivariate analysis of 10 variables, i.e. gestosis index, maternal body weight, height, uterine fundal length, abdominal circumference, maternal age at delivery and urinary estriol within one week prior to delivery. The result was that gestosis index was as useful as uterine fundal length in predicting SFD. The more gestosis index score increased, the more markedly the fetal development ws disturbed. Especially in the cases with scoring above 4 and in those with hypertension and proteinuria, the incidence of SFD increased obviously. No correlation between gestosis index and neonatal asphyxia was noticed. From the growth pattern of uterine fundal length and BPD, intrauterine growth retardation (IUGR) in pregnancy with EPH-gestosis occurred mainly within the third trimester of pregnancy. The functional development of the fetus with EPH-gestosis was evaluated with the use of urinary estriol level and fetal heart rate (FHR) monitoring. In the cases with EPH-gestosis scoring above 4, extreme disturbance of functional development of the fetus was observed.

[The clinical significance of determination of red blood cell and urinary polyamines in gynecologic malignancy (author's transl)].

The polyamines spermidine (spd) and spermine (spm), and their diamine precursor, putrescine (put) were separately determined in red blood cells (RBC) and 24-hour urine samples of gynecologic malignant and non-malignant cases using high-pressure liquid chromatography (HPLC) technique. The results are as follows: 1) Patients with malignancy (49 cases) had the polyamine levels of RBC-spd; 5.85 +/- 3.32 (mean +/- SD), RBC-spm; 7.00 +/- 5.53 (nmol/ml packed RBC), urinary put; 3.92 +/- 4.13, urinary spd; 2.17 +/- 1.74 (mg/day) in pre-therapeutic state. These levels were greatly higher than those of control cases. 2) The diagnostic rate was 100% in the fourth stage of uterine cervical cancer, and 80% in the recurrent cases. 3) Patients with long-term abnormal polyamine levels had the tendency of poor prognosis. 4) In many cases, RBC polyamines and urinary polyamines showed somewhat different fluctuations in the course of treatment. In conclusion, it is suggested that the measurement of polyamines is very useful in the diagnosis of progressive cases and recurrent cases, and can be helpful in the evaluation of therapeutic effects and prognosis.

[Lipid metabolism in pregnancy, metabolism of intravenously injected fat emulsion (author's transl)].

The triglyceride concentration increased gradually after the 10th day of gestation and it increased continuously until the time of delivery. Then it decreased immediately after birth. The phospholipid concentration also increased with gestational age, but cholesterol did not. The fatty acid components of fat emulsion (Intralipids) were labeled with carbon-14 and administrated into pregnant rats, then the fate of these fatty acids in blood was followed with time. The plasma half life was 74 minutes in pregnant rats and 58 minutes in non-pregnant rats. The disappearance was slower in the pregnant rat. This results agree with human studies. The uptake of radioactivity by the fetal liver was only 1/10 that of the mother. The radioactivity level in the expired 14CO2 was examined. 18% of radioactivity appeared in expired CO2 during 5 hours in pregnant rat. While, 30% was collected in non-pregnant animals, showing slower lipid metabolism during pregnancy.

[Changes of red blood cells' polyamines during fetal and neonatal periods (author's transl)].

Polyamines are ubiquitously distributing amines in living organs, and have close relation to nucleic acids and cell proliferation. But there are few reports about polyamines in the obstetric field. So we measured blood polyamines, i.e. putrescine, spermidine and spermine in pregnancy, delivery, puerperium and neonatal period, using high-performance liquid chromatography (HPLC). We discuss about red blood cell polyamines because RBCs contain about 80% blood polyamines. Spermidine and spermine concentration begin to increase with pregnancy, and rapidly increase from the 5th lunar month and keep high levels during the 7-9th lunar month, the decrease at term. In puerperium, spermine, as opposed to spermidine, takes higher value than that of the term. Putrescine, however, keeps almost the same level as non-pregnangs during pregnancy and puerperium. At delivery, the umbilical blood contains significantly higher polyamines than the maternal blood, but there are no differences between umbilical artery and vein. In neonatal period, each polyamine shows slight increase at the first day after birth. Thereafter, putrescine keeps the same level till the 7th day, but spermidine and spermine gradually decrease day by day. These data suggest that polyamines are reflecting the function of bone marrow erythropoiesis and reproduction.

Lipid peroxidation and antioxidants in the rat lung during development.

The extent of lipid peroxidation and the levels of its antioxidants such as superoxide dismutase (SOD), catalase and glutathione peroxidase (GSH-Px) were determined on lung tissues of the fetal, newborn and adult rat. Lipid peroxide formation was slight in the fetal period but augmented after birth reaching a peak at about 10 days after birth. The peroxide concentration then gradually declined with development and the adult level was found comparable to the fetal level. In the examination of the developmental defensive mechanism on the basis of assays for the aforementioned antioxidant enzymes in lung tissue, the SOD activity was low in fetuses reaching approximately 90% of the adult level at 10 days of life. Catalase was extremely low in concentration at all times, and age-related variations could not be definitely obtained. GSH-Px was also measured low in the fetal period and during 20 days after birth, but a subsequent gradual rise resulted in threefold greater activity in adults than in fetuses.

Steroid 16 alpha-hydroxylase from human fetal liver; inhibition by steroids.

The 16 alpha-hydroxylase system in fetal liver which used dehydroepiandrosterone (DHA) or pregnenolone as substrate, was apparently inhibited by various endogenous and synthetic steriods: DHA, pregnenolone, their sulfates, androstenediol, androstenetriol, estrone, estradiol-17 beta, ethynylestradiol and chlormadinone-acetate. The inhibition constants (Ki) towards DHA were as follows; pregnenolone 22 muM, DHA-sulfate 13 muM, pregnenolone-sulfate 21 muM, androstenediol 16 muM, androstenetriol 53 muM estrone 32 muM, estradiol-17 beta 74 muM, ethynylestradiol 22 muM and chlormadinone-acetate 27 muM. The Ki values towards pregnenolone were DHA 6.3 muM, DHA-sulfate 8.3 muM, pregnenolone-sulfate 3.9 muM, androstenediol 8.7 muM, androstenetriol 14.7 muM, estradiol-17 beta 15.4 muM and ethynylestradiol 16.0 muM, respectively. The reaction products, 16 alpha OH-DHA and 16 alpha OH-pregnenolone, showed little inhibitory effect upon the 16 alpha-hydroxylase.
