[The hidden intrauterine device].

The hidden intrauterine device (IUD) may result because of disapperance of the tails into the uterus, unobserved loss of the device and perforation. The localization can be estimated by sonography, x-ray and probing of the uterine cavity. In most of the cases the device can be removed without hospitalization, surgical procedures are rarely neccessary. If pregnancy did occur the hidden intrauterine device cannot be removed without termination of the pregnancy in most cases.

[The validity of HPL-determinations in prolonged pregnancy].

Till now there is no unequivocal diagnostic tool for the discovery of prolonged pregnancy. Therefore the present study was performed to evaluate whether HPL-values correlate with the presence of postmaturity syndrome in coherence with other known symptoms. 86 cases, partly with signs of postmaturity (n = 12), partly without such signs (n = 74), were compared with a non-postmature group. In prolonged pregnancy HPL-values were distinctly lower than at term. The more severe the condition, the lower were the HPL-values. There was a significant incidence of higher rates of meconium stained amniotic fluid beyond 42 weeks of gestation. After careful delivery the babies were generally in good condition. There was a slight shift to lower Apgar-values, but base excess and actual pH were found to be normal in umbilical artery. Birth-weights of the postmature infants did not show any differences to those born at term.

[Hemoccult tests of the stool at the time of preventive gynaecological examination (author's transl)].

From January 1, 1977 on, the test for occult blood in the stool becomes, by law, part of routine preventive checkups in order to detect carcinomatous and precancerous lesions of the colon. The effectiveness and practical use of the hemoccult test in the routine gynaecological annual checkup was tested. From October 1975 to June 1976 the test was done in 1,017 women at the time of the preventive gynaecological examinations. The patients received three hemoccult test sachets. They contained guayac resin on filter paper. The stool sample is placed on the filter paper and later tested with peroxide. The patients were informed about the test. The high interest of the patients in the test was exemplified by the high return rate of 80.7%. In 29 patients at least one hemoccult test was found to be positive for a rate of 3.53%. In one of the 821 patients who returned the test a carcinoma of the rectum was discovered which was too high for digital rectal examination. An optimal test is obtained when a stool sample is placed on the filter paper on three consecutive days. When a test is positive the patient is subjected to rectal digital examination, sigmoidoscopy and air contrast barium enema. If the source of bleeding cannot be detected in this manner a coloscopy is done. If the patient followed a diet prior and during the test the rate of positive tests of 3.53% was not increased compared to the group without dietary measures. A special diet prior and during the test is therefore not necessary. The above study shows that the inclusion of the hemoccult test in preventive examinations is a rational extention of preventive testing. Combined with gynaecological preventive examinations, the additional testing for precancerous and carcinomatous lesions of the colon is of additional preventive value for the health of the women.

[The activity of L-cysteine aminopeptidase (Oxytocinase) during premature labour and tocolytic treatment (author's transl)].

The normal levels of L-Cysteine aminopeptidase in the maternal serum from 20 to 41 weeks gestation were tested in 240 samples of serum. The normal levels rise from 0.82 mU/ml at 24 weeks to 4.63 mU/ml at term. In 14 patients with premature labour the enzyme levels were tested during the treatment to inhibit labour with fenotero-hydrobromide. There was no decrease of oxytocinase as a cause of the onset of premature labour. During treatment to inhibit labour the L-Cysteine aminopeptidase showed no deviatiameter of placental function even in patients undergoing treatment to inhibit premature labour.

[Use of seat belts by pregnant women].

There are almost no pregnant women who never use a motorcar. The safety in crashes is diminished by mistakes in contruction and handling of the safety-equipment. Only every fourth woman can be regarded as properly protected. It should be the task of the physician to instruct his pregnant patients in the use of the seat belt.

[Seat belts and pregnancy].

The best safety for the mother and her unborn child while driving a car is the proper use of a seat belt. Only a correctly constructed and fitted lap-shoulder safety harness should be used. The instruction to wear a seat belt and to wear it correctly should be part of the pregnancy-care-program for each physician.

[Functional results of tracheoplasties].

There is a direct correlation between airway resistance and residual tracheal lumen in tracheostenosis. Airway resistance has been recorded in 185 patients with tracheostenosis using body plethysmograph. High grade stenosis of the trachea is better diagnosed with body plethysmograph compared with direct inspection, a pertinent observation mainly in bordering cases. Tracheoplasties were performed in 74 patients using the body plethysmograph. Postoperative courses and functional results were observed. The average tracheal diameter was 8-9 mm at the end of our study with corresponding airway resistance of 24-34 mm H2O/l u. sec. These functional results reflect a good tolerance to average labour impound.

[Report of clinical testing of BCG vaccine (Strain "1331 Copenhagen") for vaccination of newborns against tuberculosis (author's transl)].

BCG vaccination with the vaccine "1331 Copenhagen" in various concentrations was carried out in 129 newborns. Control examinations were conducted to establish the complication rate and the percentage of tuberculin-positive children. Complications, either locally or in regional lymphatic areas, did not appear. The conversion rate increased with the number of viable units pro dose of vaccine. A vaccine with a concentration of about 250,000 units of vaccine pro dose is recommended for additional clinical testing.

[Ariway resistance in intensive care patients (author's transl)].

With the modified interrupition technique airway resistance and tidal volume of bed-ridden patients can be measured simultaneously. This objective measure causes no distress to the patient. The effect of Noleptan on tidal volume and airway resistance has been studied in 28 patients (most of them have been unconscious). Tidal volume was improved significantly leading to an increases of arterial pO2. The bronchial airway resistance was uneffected. Our findings underline the postive experiences with Noleptan in clinical trial of intensive care.

[Comparative x-ray and ultrasound examination in infantile sinusitis (author's transl)].

59 children without symptoms of sinusitis or URI, aged 6 to 8 years had positive radiologica findings of sinus pathology in 32%. Correspond ultrasonographic findings were positive in 22%. A correlation was detected in 67,8%. In non correlating cases, x-ray had a tendency to more pathological findings in 63%. When correlating x-ray and ultrasound in a diagram of 118 maxillary sinuses only 6% had changes worth treatable. The classical radiological signs os sinusitis in children are therefore questionable.

[Pregnancy with an intrauterine pessary in the placenta].

A case of a 23 year old patient which had become pregnant with a copper-T-IUD in situ is presented. After a pregnancy without many complications a healthy child was born. The copper-T-IUD was partially embedded in the placenta. Usually the delivery of a living and healthy child will not take place if a foreign body is situated in the placenta. If a woman becomes pregnant in spite of an IUD in situ, the IUD should be removed, even if this removal should terminate pregnancy. The risk of septic abortion in a pregnant woman with an IUD in situ is 1 in 15 to 1 in 30.

The indirect measurement of nasal airway resistance.

This is a comparative study of two indirect methods for measuring nasal airway resistance. 1. In 499 patients, nasal airway resistance was studied with the alternating pressure principle. Using mathematical and computer analysis, nasal resistance averaged 18,7 and 15,4 mm H2O/1/sec., respectively. This correlates well with literature data of nasal resistance, as defined by different methods. 2. In 760 patients nasal airway resistance was studied with the body plethysmograph. The mean value was 11,5 mm H2O/1/sec. We believe, that there is a methodical failure due to leakage of the mask during exspiration against a mechanical closing mechanism of the body plethysmograph.
