The level and the persistence of islet cell antibodies in healthy schoolchildren are associated with polymorphic residues of the HLA-DQ beta chain.

Cytoplasmic islet cell antibodies (ICA) were determined in a group of non-diabetic Caucasian schoolchildren (n = 4208). The prevalence rate for ICA positivity was 1.05 per cent (95 per cent confidence interval: 0.8-1.4 per cent). Analysis of HLA risk factors revealed that HLA-DRB1*03 (p less than 0.01), HLA-DRB1*04 (p less than 0.01) and HLA haplotypes with non-charged amino acids (non-Asp) at codon 57 of the HLA-DQ beta (p less than 0.01) chain were significantly increased when compared to controls. High levels of islet cell antibodies, i.e. Juvenile Diabetes Federation units (JDF units) equal to or greater than 30 JDF units were found to be associated with amino acids other than aspartic acid at codon 57 of the DQ beta chain molecule. Also the persistence of circulating ICA was found to be associated with non-Asp homozygosity of the proband (p less than 0.03).

[Arthroscopic synovectomy of the shoulder and elbow joint].

We reviewed experiences and results of six shoulder and nine elbow joint synovectomies, performed in a time period between 1989 and 1990 by arthroscopic technique. The operation technique is pretentious an requires some expenditure. The postoperative course distinguishes itself by little pain and early increase of range of motion.

[Bacterial growth on carcinomas of the oral mucosa].

Carcinomas of the oral mucosa are colonized by a variety of aerobic and anaerobic bacteria. Apart from the normal flora of the oral cavity also organisms of pathogenic potential are found, which may cause postoperative wound infections. A specific antibiotic prophylaxis regimen in extensive head and neck surgery requires identification of the pathopotent organisms and sensitivity testing followed by perioperative individual antimicrobial chemoprophylaxis management. The results of an additional bacteriological examination of the surrounding pharyngeal flora showed no major difference to the range of organisms found in the carcinoma.

Clinical comparison between membrane and bubble oxygenators in cardiopulmonary bypass.

The clinical course of two similar patient groups was compared in whom, during cardiopulmonary bypass, a membrane or bubble oxygenator was employed. According to our results there is no significant functional difference between the two types of oxygenators as long as the perfusion time does not exceed 90 minutes. Beyond this time limit, the membrane oxygenator has distinct advantages, particularly with regard to hemolysis. We presently prefer the bubble oxygenator. The use of a membrane oxygenator is restricted to complex open heart procedures with suspected technical problems.

Evaluation of the type of overadditive drug interaction under various experimental conditions: differentiation of sequential from functional synergism in dose response studies.

In the assessment of the pharmacodynamic action and interaction of drugs, frequently the influence of one drug is studied in the presence of a synergistic drug, resulting either in an additive or an overadditive response. The latter may be due to independent (functional synergistic) or interdependent (sequential synergistic) interaction. From dose response experiments in vitro with prototypes of synergistic drugs A and B acting in sequence, it can be deduced how under different experimental situations, i.e. known or unknown, possible maximum response, sequential synergism can be ascertained. The procedure described rests on the evaluation of net effects in addition to total effects of drugs. The net effects of A + B, i.e. of A in the presence of B, match with those of A in the absence of B in case of functional synergism. They markedly exceed it in the cases of sequentially interacting drugs, as shown. The latter can therefore be regarded as examples of sequential synergism and illustrate the assessment of this type of overadditive synergism under different experimental situations.

[Postoperative pain control in the Trento community hospital. Perception of pain by the nurses].

The College of Nurses of Trento (Collegio IPASVI) promoted a call for research works done by nurses, to award a prize to those judged worthy. The three best works were respectively on concordance on triage classification between doctors and nurses; on the results of organizational and educational interventions on the reduction of needle-stick injuries; and on the differences on postoperative pain assessment between nurses and patients. The final reports were later discussed as part of a training exercise aiming at improving the skill of scientific writing and they are published here, together with some comments that emphasize areas of improvement in data analysis and interpretation and some broader issues related to nursing research.

Integrated radiation hybrid and yeast artificial chromosome map of chromosome 9p.

A panel of 93 radiation-reduced hybrids have been screened using PCR amplification and oligonucleotide primers for sequence-tagged sites (STSs) specific for 114 single-copy loci mapping to the short arm of chromosome 9. An x-ray dose of 6,000 rads gave an average retention frequency of approximately 23%. We have constructed a framework map containing 31 markers ordered by analyzing coretention patterns, with support for the order greater than 1,000:1. In addition, we have placed the remaining markers which could not be mapped to a single interval with this support to a range of intervals on the framework map. The STS oligonucleotide primers used in the construction of the radiation hybrid (RH) map have been used to isolate and order yeast artificial chromosomes (YACs) assigned to 9p identified from the CEPH megaYAC library. Eighty-nine STS markers have screened positive with at least one YAC. A total of 88 individual YACs (with an average size of 0.9 MB) have been placed on the map in a series of contigs and in some cases mapped cytogenetically by fluorescence in situ hybridization. Additionally, the YAC information has been used in conjunction with the RH framework placements to generate an integrated map containing 65 loci including 51 uniquely positioned markers, with an average resolution of 0.79 Mb.
