Esophageal carcinoma metastatic to the retina. Diagnosis of a case by cytologic examination of intraocular vitreous washings.

BACKGROUND: The most common primary tumors of the eye are melanomas in adults and retinoblastomas in children. Although generally not recognized, metastases to the eye are more frequent than primary tumors. RESULTS: Occult esophageal carcinoma metastasized to the retina in a 51-year-old woman. The cytologic diagnosis of carcinoma was established on intraocular vitreous washings. Further workup disclosed an adenocarcinoma of the esophagus, confirmed by brushings and biopsy. CONCLUSION: The cytologic features of the intraocular aspirate allowed a rapid and reliable diagnosis that led to further investigation.

Cytologic features of poorly differentiated "insular" carcinoma of the thyroid. A case report.

A case of poorly differentiated "insular" carcinoma of the thyroid is presented. Fine needle aspiration cytology suggested the differential diagnosis of poorly differentiated carcinoma versus anaplastic carcinoma. Histologic examination of the resected specimen confirmed the diagnosis of insular carcinoma. The cytologic features of this uncommon primary thyroid malignancy are described and discussed with reference to the literature.

On the creative process and the human capacity to construe.

A discussion of "The Creative Process and the Narcissistic Personality Disorder," by Ben Rubenstein, Ph.D., and Morton Levitt, Ph.D. The human capacity for construal is emphasized as a basic process characterizing mental activity. Motivational and developmental phenomena can be better understood when construal is emphasized. The development of the capacity to construe is intimately linked in feedback fashion with the development of the subjective self which uses enactive, imagistic, and lexical modes of conceptualization. Construal is characterized as conflictual and as concomitantly involving powerful affective components. Examples are given in which the role of construal in artistic work, action, acting out, anxiety and object relatedness are emphasized. The basic creative act is the gradual construal of the subjective self on which all later creativity will have to be based.
