High concentration and retained amidation of fecal bile acids in patients with active ulcerative colitis.

Fecal bile acid profiles of 14 patients with ulcerative colitis in the active phase were analyzed to study the potential significance of bile acids in the pathophysiology of this disease, and the results were compared with those in 12 healthy controls. The excretion levels of total bile acids (mean +/- SD) in patients were higher than in controls, 445.1 +/- 392.1 vs 215.5 +/- 148.0 mumol/day, 3.1 +/- 1.7 vs 1.6 +/- 1.0 mumol/g wet feces (P less than 0.05), and 17.2 +/- 9.2 vs 12.4 +/- 13.3 mumol/g dry feces. Fecal profiles of individual bile acids showed higher levels of primary bile acids (52 +/- 27%) in patients compared to those (26 +/- 21%) in controls. Proportions of glycine and taurine conjugates in patients (26 +/- 24%) were higher than in controls (5 +/- 2%) (P less than 0.05), whereas proportions of unconjugates and sulfates were lower in patients than in controls. Accordingly the extent of deconjugation and dehydroxylation of bile acids was lower in patients than in controls. These trends were prominent in patients with more severe disease activity. A high concentration of bile acids in the intestine may have a significant role in the pathophysiology of ulcerative colitis at active phase.

Chondrosarcoma arising from the falx cerebri.

A chondrosarcoma arising from the falx cerebri in a 15-year-old boy is reported. The patient had been suffering from convulsive seizures and weakness of the limbs on the left side for about 7 months prior to admission. A high-density mass attached to the falx was demonstrated by computed tomographic scan. Clinically it was very similar to a falx meningioma, and the final diagnosis was obtained from histopathological examination. The tumor was completely excised at operation, which is extremely important to prevent both local recurrence and distant metastasis.

Recurrent cystic meningioma.

The authors report a case of recurrent meningothelial meningioma with a large cyst. At the first operation, the mural nodule was totally resected and the cyst wall was removed as much as possible. This cystic tumor recurred six years later at the site where the first operation was performed. Histologically, the cyst wall contained islands of the same neoplastic cells as were previously found in the mural nodule. The authors discuss the preoperative diagnosis and mechanism of cyst formation, and emphasize the importance of complete removal of the cyst wall for permanent cure.

Effects of quinidine on plateau currents of guinea-pig ventricular myocytes.

Effects of quinidine on membrane currents forming the plateau of action potentials were studied using an isolated single ventricular cell from guinea-pig hearts. Quinidine (5 mg/l) produced a fall and shortening of the early part of the plateau, and delayed its later part and final repolarization, without changes in resting membrane potential. Application of quinidine caused a reversible depression of the peak Ca2+ current by about 30% of the control. Delayed outward K+ current, iK, also decreased to less than 20% of the control. Thus, an outward tail current upon repolarization to -40 mV from depolarizing voltage steps of the plateau ranges became inward. Current values at the end of 200 ms pulses in response to voltage steps to -60-0 mV were always positive and were not changed by the drug. The inward current elicited at potentials negative to resting potential level, also, decreased by 13% to 23% of the control in the presence of the drug, but the effect was not reversible upon wash-out of the drug. These results suggest that quinidine causes a non-specific depression of inward rectifier K+ current, iK1, with minor degree but has little effect on the window sodium current. Therefore, changes in the action potential repolarization produced by quinidine can be explained by its effects on both calcium current and delayed outward K+ current.

Cardiovascular effects of a new inotropic agent, denopamine (TA-064); with reference to it's effects on cardiac hemodynamics and metabolism.

A new inotropic agent, denopamine (TA-064) was shown to have a strong positive inotropic effect. Its effect on cardiac hemodynamics and metabolism was evaluated by using wall stress and direct measurement of myocardial oxygen consumption. With plasma concentration of denopamine (21-29 ng/ml on average), attainable by one single oral dose (10 mg), the positive inotropic effect was evident by the significant increase in peak (+)dp/dt (+15% increase from control), and shortening velocity of the left ventricle (+39%), when heart rate or blood pressure was not altered significantly. End diastolic stress and end systolic stress of the left ventricle, defined as indices of preload and afterload, respectively, were reduced significantly. The reduction of preload (-51%) was the result of improved left ventricular filling, and the reduction of afterload (-23%) was due to the increased contractility. Neither coronary sinus blood flow not aortocoronary AV O2 defference was changed. Consequently, myocardial oxygen consumption remained unaltered. When the dose is chosen properly, denopamine is able to exert salutary effects in patients with severe heart failure.

Enhanced expression of a 37,000-dalton protein in human lung cancers.

When proteins in the tissue extracts of lung cancers were analyzed by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (PAGE), a 37,000-dalton protein (37K) was detected as a prominent form in all cases thus far examined. This protein was barely detectable or its amount was very small in the normal lung tissue extracts. Two-dimensional PAGE of lung tumor extracts showed 37K as a major spot with an isoelectric point of 7.6. An antibody against 37K was raised in a rabbit by injecting purified 37K. Immunoblot analysis of normal lung and lung tumor tissues revealed that the affinity-purified anti-37K antibody specifically recognized 37K and that the intensities of immunostaining corresponded to those of Coomassie brilliant blue staining.

[Studies on the pathogenesis of congenital hypoplastic anemia (Diamond-Blackfan syndrome)].

CFU-E growth from fractionated bone marrow cells of five patients with Diamond Blackfan syndrome was studied. In all patients, CFU-E growth was reduced in mononuclear cell rich fraction. In two patients out of five, CFU-E growth was returned to normal by the depletion of E-rosette forming cells of monocytes from mononuclear cell rich fraction. In the patient whose CFU-E growth returned to normal by the depletion of E-rosette forming cells, cocultivation between bone marrow buffy coat cells and autologous bone marrow E-rosette forming cells resulted in a significant decrease of CFU-E growth, and there was a significant increase in CFU-E growth by the treatment of OKT 4. In other three patients out of five, there was no significant increase in CFU-E growth by the depletion of monocytes and/or E-rosette forming cells. It is concluded that immunologic causes such as cellular factors may play a role, at least in part, on the pathogenesis of Diamond-Blackfan syndrome.

Human corticosteroid-binding capacities in normal, high-risk or pregnancies with an abnormal outcome.

To determine whether or not the serum corticosteroid binding capacity (CBC) level is related to the outcome of pregnancy or fetal well-being, a total of 124 determinations of CBC were performed in 96 pregnant individuals, using hydrophobic resin adsorption methods. The serum CBC values were elevated with progression of gestation. On the other hand, high-risk pregnancies or those with an abnormal outcome were among the low CBC group, below the mean value (p less than 0.001), whereas normal pregnancies were more often in the high CBC group, over the mean value, in comparison with the low CBC group below the mean value (p less than 0.001), after 28 weeks of gestation. In addition, the ratios of birth weight/placental weight and maternal weight/placental weight were slightly but not statistically high in the low CBC group, in comparison with the high CBC group. Thus, various events related to placental growth might be important factors regulating the serum CBC level. While there are many determinants of corticosteroid binding globulin production and excretion, assessment of CBC levels seems to be a reliable indicator of the outcome of pregnancy, including high-risk pregnancy, in the 3rd trimester.

[Farmer's lung in the east of Hokkaido. A follow-up study in 1985].

The follow-up study of 16 patients with farmer's lung and 35 farmers having antibody to thermophilic actinomyces was performed. The following results were obtained. Three patients out of 16 patients with farmer's lung were readmitted to the hospital. The patients out of 11 believed that the dust respirator is useful for prevention against the development of farmer's lung. Any new patient was not found out from farmers having antibody to thermophilic actinomyces in 1985.
