Ultrasonographic monitoring of the placenta in patients with bleeding during the first and second trimesters.

Thirteen patients with vaginal bleeding in the first and second trimester, in 730 pregnancies confirmed with live fetuses, were diagnosed as having subchorionic hematoma at the perivillous or periplacental region by a real-time B-mode scan device. In 9 of the 13 patients, the hematoma disappeared within 1 to 3 months after detection, while in 2 patients it increased in size. Seven of the 13 pregnancies progressed to full term (53.8%), 3 ended in premature deliveries (23.1%) and 3 in spontaneous abortions (23.1%). In 5 of the 13 patients, PROM occurred. Pathological examinations revealed 5 circumvallate (38.5%) and 2 circummarginate (15.4%) placentas. Since our clinical observation indicates that subchorionic hematoma and marginal separation of the placenta may be important causes of bleeding during pregnancy, monitoring of the placenta and fetus by ultrasound was used to obtain precise information in order to manage patients with bleeding. It might be speculated that extrachorial placenta results from subchorionic hematoma or its absorption.

Specific but differential antagonism by glibenclamide of the vasodepressor effects of cromakalim and nicorandil in spinally-anaesthetized dogs.

1. Whether the vasodepressor effects of cromakalim and nicorandil, potassium channel openers, were differentially antagonized by glibenclamide, a supposed specific antagonist of potassium channel openers, was investigated in spinally-anaesthetized dogs in which arterial pressure was maintained elevated by i.v. infusion of noradrenaline. 2. Cumulative administration of cromakalim (3-100 micrograms kg-1, i.v.), nicorandil (30-1000 micrograms kg-1, i.v.), diltiazem (3-300 micrograms kg-1, i.v.) and nitroglycerin (0.3-100 micrograms kg-1, i.v.) caused dose-dependent decreases in mean arterial pressure. In dogs which received glibenclamide (3 mg kg-1, i.v.), the dose-vasodepressor response curves for cromakalim and nicorandil were located on the right in parallel to the respective curves determined in control dogs, but those for diltiazem and nitroglycerin were not different. ED50% values increased about 6.7 fold for cromakalim and 2.2 fold for nicorandil. 3. The depression of LV dP/dtmax produced by a high dose of cromakalim (100 micrograms kg-1, i.v.) was abolished by glibenclamide and that produced by nicorandil was not only antagonized but converted to an increase. 4. These results suggest that the vasodepressor action of cromakalim is due predominantly to potassium channel opening, but that of nicorandil involves not only potassium channel opening but its action as a nitrate.

Antigenic determinant of a monoclonal antibody: extracellular domain at the M3-M4 junction of the alpha-subunit of Na,K-ATPase.

The binding site of a monoclonal antibody, M45-80, against the alpha-subunit of horse Na,K-ATPase was determined. Various sizes of DNA fragments derived from rat Na,K-ATPase alpha 1-subunit cDNA were cloned into pUC19 expression vector and some fragments of horse genomic DNA were cloned into pUC18. Escherichia coli JM83 cells harboring the plasmids were grown and the cell lysates were used as antigens. An enzyme-linked immunosorbent assay revealed that M45-80 recognizes the hexapeptide Glu-Tyr-Thr-Trp-Leu-Glu (which is identical to the rat and horse alpha 1-subunits) at the M3-M4 junction located on the extracellular side. The ouabain-binding site is discussed in relation to the recognition site of M45-80.

Hemodynamic changes by recombinant erythropoietin therapy in hemodialyzed patients.

Recombinant human erythropoietin therapy was given to 15 patients undergoing long-term hemodialysis with normal cardiac function. None of the patients had hypertension before the erythropoietin therapy and had received no antihypertensive agents. Before and after the erythropoietin therapy M-mode and pulsed Doppler echocardiographic studies, measurements of plasma volume by radioiodinated human serum albumin, and measurements of atrial natriuretic factor were carried out. After 6 weeks of erythropoietin therapy, hematocrit increased from 20.0 to 33.0%. Cardiac output, stroke volume, left ventricular diastolic dimensions, and left ventricular wall stress were all significantly decreased. Total peripheral resistance, interventricular septal thickness, and left ventricular posterior wall thickness were significantly increased. In Doppler echocardiographic studies, the mean velocity of aortic ejection flow and left ventricular acceleration time were decreased. The blood volume derived from plasma volume and hematocrit was not changed, whereas plasma atrial natriuretic factor concentration was significantly decreased. These data suggest that recombinant human erythropoietin administration suppressed the hyperdynamic cardiac state that was required to maintain oxygen delivery to the peripheral tissues in severe uremic anemia.

[Effects of lamprey oil prepared under different deodorization conditions on serum lipids, reticuloendothelial system and antibody production in mice].

Fresh lamprey oil (F-La) is known to contain a large amount of eicosapentaenoic (EPA) and docosahexaenoic (DHA) acids. When F-La was deodorized with steam at 280 degrees C and 1 mmHg for 1 h, the contents of EPA and DHA were reduced and unidentified peaks were detected by gas-liquid chromatography. In male ddY mice, one week feeding of the lamprey oil deodorized at 280 degrees C (H-La) remarkably decreased the serum triglyceride level and increased the liver weight 2.0-fold. H-La also lowered the corrected phagocytic index (alpha), which is an indicator of activities of reticuloendothelial system, but suppressed relatively little the antibody production against sheep red blood cells. It is suggested that H-La contains biologically active substances, which have potent hepatotoxic and serum triglyceride-reducing effects. This biologically active substances appear to be derived from long-chain polyunsaturated fatty acids under thermal deodorization conditions.

Acute toxicity tests of mofezolac (N-22) in mice and rats.

Mofezolac (N-22) is a newly developed analgesic and anti-inflammatory agent. The acute toxicities of N-22 were investigated in ICR mice and Wistar rats in oral (p.o.), intraperitoneal (i.p.) and subcutaneous (s.c.) routes. LD50 values of N-22 in mice were 1528 mg/kg (p.o.), 275 mg/kg (i.p.) and 612 mg/kg (s.c.) for males, and 1740 mg/kg (p.o.), 321 mg/kg (i.p.) and 545 mg/kg (s.c.) for females. Those in rats were 920 mg/kg (p.o.), 378 mg/kg (i.p.) and 572 mg/kg (s.c.) for males, and 887 mg/kg (p.o.), 342 mg/kg (i.p.) and 510 mg/kg (s.c.) for females. The sex difference was not clearly observed in mice and rats, but the species difference was observed in p.o. routes. As an initial toxic sign, the hypoactivity was observed in mice and rats of all routes, subsequently, paleness of skin, anemic conjunctiva, emaciation, stupor and/or coma were observed in mice and rats of p.o. and i.p. routes. In rats of those routes, tonic and/or asphyxial convulsion and dyspnea were also observed. In pathological examination of mice and rats, gastrointestinal disorders were observed in p.o. and i.p. routes, and changes of subcutaneous tissue at the injection site were observed in s.c. route.

[Fifty two-week chronic oral toxicity study of mofezolac (N-22) in rats].

Fifty two-week oral toxicity study of mofezolac (N-22), a new developed analgesic and anti-inflammatory agent, was carried out in Wistar rats with dose levels of 5, 20, 60 and 120 mg/kg/day, and the 5-week withdrawal was followed for recovery study. Hematuria, blanching of the skin and suppression of body weight gain were observed in females given 120 mg/kg, and 9 of these prostrated and died from week 20 to week 52, or were euthanized when moribund. These symptoms were not seen in males at any dose levels. In 120 mg/kg group, increased positive cases of fecal occult blood were observed during the administration period, and the pathological examination revealed gastrointestinal lesions such as erosion, ulcer, hemorrhage and mucosal regeneration in the small intestine. Renal disorder was also involved mainly in females given 120 mg/kg, as shown by increase in urine volume with declined osmotic pressure and specific gravity, serum urea nitrogen, creatinine, inorganic phosphorus, and other related parameters. In addition to enlargement, rough surface and scar formation, dilated tubular lumen and papillary ducts of the kidney were observed as a main lesion. Incidental findings with those disorders, observed mainly in females given 120 mg/kg, included increase in leucocytes with high neutrophils ratio, and enlargement of the spleen, adrenals and mesenteric lymph node. There were some of the similar gastrointestinal and renal changes mainly in females given 60 mg/kg. Anemic findings were noted in both sexes of 120 mg/kg and in females of 60 mg/kg group, and mainly females given 120 mg/kg showed increase in platelets, reticulocytes and fibrinogen, shortening of blood coagulation time, as well as extramedullary or accented hematopoiesis of the liver, spleen and bone marrow. Other serum biochemical changes observed mainly in females given 120 mg/kg were decrease or decreasing trend in total protein, A/G ratio, and transaminase activity. Fine structure of the liver from females given 20 mg/kg or more revealed cisternal dilatation of smooth-surfaced endoplasmic reticulum of hepatocytes, and the increased liver weight was observed in females given 120 mg/kg. Accordingly, non-effective dose level of N-22 in 52-week chronic toxicity study was estimated to be 20 mg/kg for males and 5 mg/kg for females.

Concentrations of lactoferrin and iron in human milk at different stages of lactation.

Lactoferrin (LF) was isolated from human milk by serial procedures of 45% ammonium sulfate precipitation, CM Sephadex C-50 ion-exchange chromatography, Sephadex G-100 gel filtration, and Cu-affinity chromatography, in which 59Fe lactoferrin was used as the tracer. The recovery of LF from human milk was 3.3%. LF from human milk was a single component having a molecular weight of 78k on SDS-PAGE, and showed pI 8.02 by isoelectric focusing on slab gel. The LF concentration was measured by rocket immunoelectrophoretic assay using anti-human LF antiserum in human colostrum and milk, from 1 to 60 days after parturition (125 samples). The LF concentrations in colostrum (1-3 days of puerperium, n = 35), the transitional milk (4-7 days, n = 60), and mature milk (20-60 days, n = 30) were 6.7 +/- 0.7, 3.7 +/- 0.1, and 2.6 +/- 0.4 (mean +/- SEM) g/liter, respectively. Both the LF and total protein (TP) concentrations showed significantly inverse correlations with the days after parturition (p less than 0.001). The lactoferrin/total protein ratio (LF/TP) in the mature milk (16.1 +/- 1.4%) was significantly less than that in the colostrum (20.4 +/- 1.2%, p less than 0.05) and the transitional milk (21.4 +/- 0.9%, p less than 0.05). Furthermore, iron concentration (Fe) in human milk was also measured by the internal standard technique of the spiked method on atomic absorption, and the lactoferrin iron saturation (LS%) was calculated. Neither Fe nor LS% had significant difference among these three stages of the lactation. The means (n = 125) of Fe and LS% were 60.6 +/- 5.4 micrograms/100 ml and 11.8 +/- 1.1%, respectively. However, significant correlation was observed between LF and Fe (p less than 0.005) or between LF/TP and both of Fe (p less than 0.05) and TP (p less than 0.001) in the mature milk. These results suggest that the mechanism stimulating the synthesis and secretion of LF is different from those of other proteins and LF can play variable roles in iron nutrition of babies at the different stages of lactation.

[Perfusion study of the pulmonary hilar region by SPECT--normal cases].

Alveoli in the hilar region comprise the peripheral area containing daughter branches from subsegmental or one more divisional peripheral bronchi. Pulmonary perfusion in hilar region was examined by SPECT (single photon emission CT) in ten normal volunteers. ROI (region of interest) in the axial images were set in the hilar region, the upper, middle and lower lung fields with 10.8 mm thickness. Counts/one pixel (C/P) were calculated in these ROI. There was a tendency of C/P increase from upper to lower lung field. And there were no significant differences in C/P increase between hilar region and other axial fields. In the chronic obstructive pulmonary diseases, however, ventilation studies in the previous reports using Xe dynamic CT or PET (positron emission tomography) showed differences between outer region and hilar region. This method will be expected to evaluate the pulmonary perfusion not only in the whole lung but in different lung areas, including the hilar region in the chronic obstructive pulmonary diseases.

[Studies on pyospermia in male infertility].

Among 670 infertile men, 72 were diagnosed as pyospermia according to our criteria. i.e., WBC greater than or equal to 10/hpf semen. The sperm motile efficiency index (SMEI) which indicates the rate of progressively motile sperms, was significantly low in pyospermic group compared with that of non-pyospermic men (WBC less than 5/hpf semen). From the result of split ejaculation, a major cause of pyospermia was supposed to be chronic prostatitis. The SMEI was decreased immediately after addition of the neutrophils and granulocyte elastase to semen. The mean value of granulocyte elastase in pyospermic group was 2859.6 micrograms/L, whereas that of non-pyospermic men was 131.6 micrograms/L. In summary, granulocyte elastase in seminal plasma may be a cause of inhibition of sperm motility in pyospermic state.

Simultaneous assay for megakaryocyte colony-stimulating factor and megakaryocyte potentiator and its application.

We have devised a simultaneous assay system for megakaryocyte colony-stimulating factor (Meg-CSF) and megakaryocyte potentiator (Meg-Pot) by modifying a quantitative measuring technique for acetylcholinesterase activity (Ach-E) of megakaryocytes by automatic colorimetry using microplates. We cultured murine bone marrow cells treated with diisopropyl fluorophosphate in a serum-free system with serum-free pokeweek mitogen-stimulated spleen cell conditioned medium (PWM-SCM) and an unknown factor, preparing two microplates with the identical culture system. In the first plate, the total number of Ach-E-positive cells induced solely by the factor tested was indicative of Meg-CSF activity and additive increases in this parameter on simultaneous addition of PWM-SCM and the factor tested were indicative of early Meg-Pot activity. Total Ach-E activity (total change at optical density of 414 nm) per well was measured in the second plate to calculate total change at optical density of 414 nm per megakaryocyte, an indicator of late Meg-Pot activity. With this system, recombinant human erythropoietin showed both Meg-CSF and early and late Meg-Pot activities in in vitro megakaryopoiesis. Recombinant murine granulocyte-macrophage colony-stimulating factor possessed weak Meg-CSF and early Meg-Pot activity, whereas recombinant human granulocyte colony-stimulating factor exhibited late Meg-Pot activity and thrombocytopenic serum exhibited early and late Meg-Pot activities. This assay system is useful in screening Meg-CSF or Meg-Pot activities in unknown factors.

[Significance of the PGI2 producing enzyme system in the pathogenesis of pregnancy-induced hypertension].

Characterization of the PGI2 producing enzyme system (P.E.S.) in pregnancy induced hypertension (PIH) was carried out by comparing its conversion from arachidonic acid (A.A.) or PGH2 to PGI2 using enzyme preparations from endothelial cells of the umbilical vein in normal pregnancy (N) and mild (M) and severe (S) types of PIH. 1) The conversion rate from A.A. to PGI2 in N was significantly (p less than 0.05) higher than in M and S. 2) The conversion rate from PGH2 to PGI2 in N was higher than in M and that in M was higher than in S. 3) The apparent Vmax values (nM/mg protein) (mean +/- S.E.M.) for P.E.S. in N (0.88 +/- 0.21) were significantly (p less than 0.05) lower than in M (2.76 +/- 0.71) and S (1.63 +/- 0.18). The apparent Km values (microM) (mean +/- S.E.M.) for P.E.S. in N (0.76 +/- 0.25) were significantly (p less than 0.05) higher than in M (0.29 +/- 0.07) and significantly (p less than 0.05) lower than in S (3.26 +/- 0.78). 4) The apparent Vmax values for PGI2 synthetase (P.S.) in N (0.44 +/- 0.09) and M (0.61 +/- 0.10) were significantly (p less than 0.05) higher than in S (0.11 +/- 0.06). The apparent Km values for P.S. in N, M and S were 0.12 +/- 0.07, 0.13 +/- 0.06 and 0.16 +/- 0.04, respectively. The present study showed that the symptoms in M may be kept from becoming aggravated by the production of large amounts of PGI2, due mainly to activation of cyclooxygenase, whereas progressive disorder of the regulation of blood pressure in S may be caused by low production of PGI2.(ABSTRACT TRUNCATED AT 250 WORDS)

Maxillary growth after two-stage palatal closure in complete (unilateral and bilateral) clefts of the lip and palate from infancy until 10 years of age.

The purpose of the study is to clarify the maxillary growth effects following different types of palatal closure in complete clefts of the lip and palate from infancy to 10 years of age. Lip repair, carried out at 5 months in one stage, was accomplished by Tennison's procedure. These patients were then assigned randomly to each of the 4 experimental groups according to the types of clefts and of palatal closure. One group of 14 patients in unilateral cases (Unil-S) and another group of 8 patients in bilateral cases (Bil-S) received mucoperiosteal palatal push-back procedure in a single stage at 20 months. The third group of 16 patients in unilateral cases (Unil-T) and the fourth group of 7 patients in bilateral cases (Bil-T) received the two-stage palatal closure based on Perko technique in which primary veloplasty was accomplished at 20 months and hard palate closure at 5 year 10 months. Non-cleft subjects were served as Controls. A longitudinal maxillary growth was monitored by the measurements of maxillofacial cast models obtained from each of the subjects. The results showed that the growth in depth and height of the maxilla of the Unil-T showed catch-up growth after primary veloplasty and resulted in no significant differences compared to that of the Control in the later phases, however, the Unil-S did not. The maxillary growth inhibition in height was characteristic in both Bil-S and Bil-T after palatal closure. There were no differences between the Bil-S and Bil-T in any dimensions and observation phases. The results indicate that the employment of the two-stage palatal closure is more beneficial for the unilateral cases, however, careful consideration is needed in bilateral cases.

[A clinical study of traumatic injuries to deciduous teeth. (3). The influence on their permanent successors].

To study the influence of traumatic injuries to deciduous teeth on their permanent successors, we investigated 126 traumatized deciduous incisors of 66 children who had visited our clinic regularly after traumatic injuries, and whose permanent successors had erupted. The results are summarized as follows. 1) Some developmental disturbances on permanent successors, such as concerning their eruption, or crowns, roots and pulp, were found in 72 teeth. Among these, influences on crown formation and calcification were most often found, and white discoloration in the enamel was recorded in 55 teeth, yellow discoloration in 4 teeth and hypoplasia in 11 teeth. 2) One successor erupted one and a half years after the eruption of its opposite, and two teeth erupted in torsiversion. Three successors had root angulation, while arrest of root formation was found in one tooth. Pulp canal obliteration appeared in two teeth before their eruption. 3) Developmental disturbances of the crowns tended to be more severe and had a higher ratio when traumatic injuries occurred at a younger age. The range of the ages of the children at the time of injury was limited in cases of hypoplasia and yellow discoloration. 4) According to the diagnoses at the first visit, the severity of periodontal injury and the dislocational directions of deciduous incisors might relate to the disturbances of crown formation and calcification of successors. 5) According to the type of treatment, enamel hypoplasia was observed more frequently in the observation group and the extraction group. Yellow and relatively large white discolorations were found more frequently in the reposition group than in the fixation group. 6) In the endodontic therapy group, only relatively small white discolorations were found, but white discoloration was observed more frequently in the extraction after endodontic therapy group than in the extraction without endodontic therapy group after first visit. 7) White discoloration in the enamel was most often located in the both upper and lower teeth at the incisal third position on the labial surface.

[A determination of anti-insulin receptor antibody in serum--a radioreceptor assay excluded the influence of insulin and anti-insulin antibody].

We studied an anti-insulin receptor antibody (IRAb) assay that excludes the influence of serum insulin and anti-insulin antibody in patients with anti-insulin antibody and normal subjects. The placental membranes strongly bound with 125I-insulin at 4 degrees C. The insulin specificity of the radioreceptor assay was confirmed by adding excess non-labeled insulin and other human hormones to the assay system. The strong correlation between the receptor binding reactivity (%) and the anti-insulin antibody levels was noted in the conventional direct IRAb assay (r = -0.95), but not in the present IRAb assay (r = 0.46) in 10 clinical samples. The placental membranes were stable as the target insulin receptor for IRAb assay at -80 degrees C for at least 4 months. A significant difference in IRAb levels was found between 10 patients with positive anti-insulin antibody and 20 normal controls (p less than 0.01, student t test). The coexistence of insulin and anti-insulin antibody were removed by absorbance to silicagel and by utilizing the two-step (indirect) assay, respectively. IRAb assay without the influence of serum cofactors showed excellent reproducibilities (CV 4.4% (N = 5) and 12.5% (N = 5) in within and between assay variations, respectively).
