[Evaluation of hyperlipidemia in high school student--Part 1].

Recently the risk of ischemic heart disease (IHD) is increasing in young generations. Thus, we evaluated the degree of hyperlipidemia in the students of high school in Miyagi prefecture. At first, we examined the plasma level of total cholesterol in all students as screening. About 10% of all the students showed high level of total cholesterol over 200mg/dl. For second screening, we examined further indexes for hyperlipidemia including lipids, glucose and blood pressure at fasting state and got information about family history, diet and exercise. There was positive correlation between total cholesterol and LDL-cholesterol (LDL-C), but no relation was found among others. Lipoprotein (a) which was an independent factor of IHD showed a broad distribution as described before, and had no relation with other lipid levels. Early education and a familial consultation for protecting from IHD should be followed after the screening of lipids levels.

[High dose nicardipine therapy for delayed cerebral vasospasm after aneurysmal subarachnoid hemorrhage].

The effect of high dose nicardipine on delayed cerebral vasospasm was studied in 33 patients with aneurysmal subarachnoid hemorrhage (SAH). Intravenous infusion of nicardipine started before or immediately after early operation. The dosage was from 4 to 10 mg/h with an average of 7.4 mg/h. The duration was from 2 to 17 days with an average of 12.3 days. Symptomatic vasospasm was observed in only one (3%) out of 33 patients. The remaining 32 patients (97%) showed neither ischemic symptoms nor low-density areas on computed tomography. Severe angiographical vasospasm was diagnosed in 3 patients, who entered hospital on day 3 to 5 after initial attacks. They were operated upon on day 4 to 6 under the administration of nicardipine before or immediately after the operation. No patients showed symptomatic vasospasm and angiographical vasospasm was improved markedly. The present study demonstrated that high dose nicardipine appeared to prevent both brain ischemia and angiographical vasospasm after aneurysmal SAH.

[ELISA tests for detection of Helicobacter pylori antibodies].

The efficacy of commercial ELISA tests for detection of Helicobacter pylori antibodies was investigated. Some investigators reported that six commercial ELISA tests had sensitivities of 81-96%, and had specificities of 29-96%. It is possible that some of false-negative results by ELISA may occur for patients who are in the acute phase of infection. When ELISA tests gave false-positive results, it should be taken into account that there might be some false negative diagnoses for the gastric H. pylori status, arising from sampling errors. Some of the false-positive results by ELISA might indicate the previous infection only.

[Relationship between Helicobacter pylori colonization in the gastric mucosa and the severity of atrophic gastritis].

We studied the relationship between Helicobacter pylori infection and atrophic gastritis. Eighty non-ulcer patients underwent endoscopy with biopsy. Detection and quantitative assessment of H. pylori was made based on smear, culture and tissue section (Gram stain). The extent of atrophic gastritis was assessed endoscopically according to the classification of Kimura and Takemoto. Severity of glandular atrophy and intestinal metaplasia was assessed histologically. H. pylori infection was accompanied by or preceded the extension of atrophic gastritis. In patients with severe atrophic gastritis H. pylori was not detected. In the advanced stage of atrophic gastritis H. pylori might be cleared from the gastric mucosa. Our results suggest that H. pylori may be one of the factors for the extension of atrophic gastritis.

[Epidural buprenorphine under nitrous oxide-oxygen-enflurane anesthesia in elderly patients].

The effects of epidural buprenorphine (Bn) under nitrous oxide-oxygen-enflurane (E) anesthesia in elderly patients were examined. The subjects were older than 65 years (n = 82) and scheduled for abdominal surgery. They were divided randomly into two groups according to epidural drugs used. The first group received epidural Bn 0.1-0.2 mg (group B, n = 41). The second group received epidural Bn 0.1-0.2mg + 1% lidocaine (group BL, n = 41). The enflurane concentration was 1.13% in group B, and it was significantly higher than 0.78% of group BL (P < 0.01). There was no intra-group difference in the highest systolic blood pressure during operation. The lowest systolic blood pressure during operation was 92.0 +/- 2.1 mmHg in group BL and it was significantly lower than 100.2 +/- 2.9 mmHg of group B (P < 0.05). The percentage of patients who needed vasoconstrictor drugs was 12% in group B, and it was significantly smaller than 42% of group BL (P < 0.01). On the other hand the percentage of patients who needed vasodilator drugs was 39% in group B, and it was significantly larger than 17% of group BL (P < 0.05). There was no intra-group difference in arousing time from anesthesia. Concerning the postoperative respiratory state, there was no intra-group difference in the respiratory rate, tidal volume and PaO2. But the PaCO2 value was 44.7 +/- 0.8 (mean +/- SE) mmHg in group BL, and it was significantly higher than 40.7 +/- 0.7 mmHg of group B (P < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[Two cases of giant aneurysms arising from the distal segment of the anterior cerebral circulation].

Giant aneurysm arising from the distal segment of the anterior cerebral circulation is extremely rare, and only 14 cases of anterior cerebral circulation aneurysms have been reported in the literature. This report describes two cases of giant intracranial aneurysms, one arising from the distal segment of the middle cerebral artery, and the other from the azygos anterior cerebral artery. Although both of the patients started with intracranial hemorrhage, their surgical outcomes were good. The growth mechanisms of each aneurysm are discussed based on the clinical and pathological findings. Since the wall of the middle cerebral artery aneurysm showed granulomatous changes with infiltration of many inflammatory cells, the aneurysmal enlargement is considered to have resulted from chronic inflammation of the wall. In the case of anterior cerebral artery aneurysm, hemodynamic stress through the azygos artery may have caused the giant aneurysm. Operative indication for giant intracranial aneurysm is controversial. Since the surgical outcomes of reported cases are definitely better than those of the patients with proximal giant aneurysm, we conclude that giant aneurysm arising from the distal segment of the cerebral artery should be treated surgically.

[Ca-antagonist, K-channel-opener in the treatment of patients with congestive heart failure].

The development of vasodilator drugs that open the K+ channels in blood vessels has been of great academic and practical interest. The K-channel openers (cromakalim, pinacidil and nicorandil) decreased afterload or preload due to their vasodilator action. In addition, K-channel openers produced a cardioprotective effect in lower doses that did not affect coronary blood flow or blood pressure. It is highly expected to be useful for treatment of congestive heart failure.

[Changes in respiration and hemodynamics during open heart surgery without blood transfusion].

The changes in respiration and hemodynamics during open heart surgery were studied in 25 patients undergoing coronary artery bypass grafting without blood transfusion. The respiratory and circulatory parameters were measured at the time of anesthetic induction and after cardiopulmonary bypass (CPB). The values after CPB were compared with those at the time of anesthetic induction. The values of HR, CI, SVI and mPAP increased, and the values of mAP, SVRI and PVRI decreased after CPB. The PaO2, BE and pH decreased but PaCO2, A-aDO2 and QS/QT increased after CPB. Although VO2I and DO2I increased after CPB, OER (VO2I/DO2I) was unchanged. Arterial lactate, pyruvate and cortisol levels increased after weaning from CPB. Hemodynamics during open heart surgery without blood transfusion showed hyperdynamic state after CPB. Hypoxia was not evident in the peripheral tissue. This suggests that the depression of the oxygen delivery with hemodilution is compensated by hyperdynamic circulation. Coronary artery bypass grafting without blood transfusion seems to offer no clinical problems.

[Evaluation of skin temperature of patients under spinal anesthesia during cesarean section].

Temperature changes of the first finger and toe were evaluated in patients under spinal anesthesia. One hundred and thirty female patients who were to undergo cesarean section were selected for this study. During spinal anesthesia the changes in skin temperature were classified in three patterns. The first pattern showed a rise of the first finger and toe temperature just after intrathecal injection of 0.3% dibucaine (hand and foot type, n = 77). The second pattern showed only a rise of the first toe temperature (foot type, n = 55), and the third pattern showed no change in the first finger and toe temperature (unchanged type, n = 3). The results of cold description tests were T5 +/- 2 (mean +/- SD) in hand and foot type, T8 +/- 2 in foot type and L1 +/- 6 in unchanged type. The skin temperature pattern was affected by the position of the patient. When the patients were placed in a head down position, 38 cases of foot type showed a rise of the first finger temperature, and 1 case of unchanged type showed a rise of the first finger and toe temperature. In these cases the operative procedure was done under spinal anesthesia. The remaining 4 cases of foot type and 2 cases of unchanged type needed another anesthetic method. In cesarean section under spinal anesthesia, skin temperature pattern of hand and foot type seems desirable. Simultaneous monitoring of hand and foot (first fingertip temperature) seems to be useful to evaluate the height of spinal anesthesia.

[Measurement of bile CEA levels in patients with colorectal cancer: is it of value for diagnosis of occult liver metastases aiming at prophylactic regional hepatic chemotherapy?].

A preliminary study was conducted to determine whether the measurement of CEA levels in gallbladder bile would be of value for diagnosis of occult liver metastases aiming at prophylactic regional hepatic chemotherapy in patients with colorectal cancer. Forty-fold dilution by normal human sera was necessary to measure CEA levels of gallbladder bile by the enzyme immunoassay (EIA). Bile CEA levels were 27.2 +/- 30.4 (M +/- SD) ng/ml in patients with benign biliary diseases (control), 248.5 +/- 426.3 ng/ml (p < 0.05, vs. control) in Dukes B, C patients, and 3,331.5 +/- 4,473.3 ng/ml (p < 0.01 vs. control, p < 0.05 vs. Dukes B, C) in patients with hepatic metastases from colorectal cancer, respectively. Bile CEA tended to demonstrate the existence of hepatic metastases more accurately than serum CEA. Our results obtained from a small number of patients suggest that patients with either elevation of bile CEA level or with a high ratio of bile CEA/serum CEA may be candidates for hepatic recurrence that requires prophylactic hepatic chemotherapy. However, it may be difficult to determine the bile CEA level at which occult liver metastases are demonstrated.

[A case report: mitral valve replacement for the patient with essential thrombocythemia].

A 62-year-old man with heart murmur and recent cerebral thrombosis was admitted to our hospital. Examination of his heart revealed mitral stenosis and hematological study confirmed the diagnosis of essential thrombocytosis (ET). After 1100 ml pre-operative phlebotomy, mitral valve replacement with a 29 mm Björk-Shiley prosthesis was performed utilizing routine cardio-pulmonary bypass using only his prestored blood and moderate hypothermia. We had no difficulty in hemostasis at the operation. Acute gastric bleeding began suddenly as much as 1500 ml on the 4th postoperative day. As medical care was in vain, emergent distal gastrectomy was performed. As we used urokinase for fear of thrombosed valve formation, residual gastric bleeding was continued on. Then, we began to use Gabexate-Mesilate (FOY) instead of urokinase. Retrospectively, Gabexate-Mesilate was very useful drug for preventing the left atrium and prosthesis from thrombo-formation until the start of oral intake of Warfarin and Busulfan. To our knowledge, this would be the first case report of open-heart valve replacement surgery in a ET patient in Japan.

[Relation between fingertip temperature pattern and epidural puncture site during epidural anesthesia combined with general anesthesia].

We are employing fingertip temperature monitoring to estimate the spread of epidural anesthesia in patients who received combined epidural and general anesthesia. In this study we examined the relation between fingertip temperature pattern and epidural puncture site. One hundred patients scheduled for elective digestive tract surgery were studied. They were divided into three groups according to the epidural puncture sites as follow; Group 1, T7-9 (n = 26); group 2, T9-12 (n = 48); and group 3, T12-L5 (n = 26). Fingertip temperature was determined by Anritsu thermocouples. These probes were attached to the thumb and the big toe (hand and foot fingertip). The changes of these temperatures were classified to four types. In type 1, the hand fingertip temperatures were elevated after administration of the local anesthetics. In type 2, both hand and foot fingertip temperatures were elevated. In type 3, the foot fingertip temperature was elevated. In type 4, the hand and foot fingertip temperatures were unchanged. In group 1, we found 10 cases of type 1, 13 cases of type 2 and 3 cases of type 3. In group 2; 15, 22, 11 cases respectively. In group 3; 1, 16 cases respectively and 1 case in type 4. In group 1 and 2, the type 1 temperature pattern was observed more than in group 3 (P < 0.01). And in group 3, the type 3 temperature pattern was observed more than in group 1 and 2 (P < 0.01). We found no intergroup difference in type 2 pattern. But there were some exceptions.(ABSTRACT TRUNCATED AT 250 WORDS)

Relationship between Helicobacter pylori colonization and acute inflammation of the duodenal mucosa.

A relationship between Helicobacter pylori colonization and acute inflammation of the duodenal mucosa was studied in 75 patients with duodenal ulcer and nine with endoscopically normal duodenal mucosa. A biopsy of the duodenal bulb in each patient was used for both detection of H. pylori and histological assessment of acute inflammation [polymorphonuclear leukocyte (PMN) infiltration and regenerative changes of the duodenal epithelial cells]. Biopsies with regenerative changes showed a marked PMN infiltration, regardless of the duodenal H. pylori status. In biopsies without regenerative changes, H. pylori colonization was closely associated with PMN infiltration. Acute inflammation in the duodenal mucosa surrounding ulcers is caused mainly by acid, but our data suggest that H. pylori is another important factor in the development of PMN infiltration in the duodenal mucosa.

[A study on the integration of fetal behavior and the development of association between parameters evaluated].

To evaluate the development of the fetal behavioral state with reference to the association of the fetal parameters we selected, we simultaneously monitored fetal heart rate (FHR), fetal movement (FM), fetal eye movement (FEM) and fetal breathing movement (FBM). These various parameters were monitored with 2 ultrasonographic real time scanners and a doppler device to monitor fetal movement. We assessed the convergence and integration of these four parameters to evaluate the association rate (AR). FHR with a variation in excess of 3 min. was interpreted as the active phase (A), and reduced variation in excess of 3 min. as the inactive phase (I). When FM and FEM were observed in the 1 min. window of A, we labeled this A3, and when FBM was present in more than 10% of the 1 min. window, we labeled it A4. A3/A = AR-A3% and A4/A = AR-A4% was calculated. A similar calculation was done for I, without FM and FEM in the 1 min. window of I (I3) and when FBM was present less than 10% of 1 min. (I4), allow derivation AR-I3% and AR-I4%. A discrete separation of synchrony in A and asynchrony in I can be seen to develop as the fetus matures, and we feel that this may be a valuable tool in the evaluation of fetal central nervous system development in utero.

[The circulatory and respiratory effects of methylprednisolone associated with catecholamine administration of a few hours].

We studied the circulatory and respiratory effects of methylprednisolone (MPS) associated with catecholamine administration of a few hours after cardiopulmonary bypass (CPB). Forty patients undergoing coronary artery bypass grafting were randomly divided into two groups. Twenty patients in group 1 [MPS (-) group] did not receive MPS and 20 patients in group 2 [MPS (+) group] received MPS 15 mg.kg-1 i.v. 60 minutes after CPB. The circulatory and respiratory parameters were measured 60 minutes after CPB, 30 minutes after MPS i.v. and 60 minutes after MPS i.v. The values in MPS (-) group were compared with those in MPS (+) group. CI and SVI decreased, and SVRI and PQR increased in MPS (-) group, but these values were unchanged in MPS (+) group. RAP, mPAP, PCWP and PVRI were unchanged in MPS (-) group. PCWP increased and the other values were unchanged in MPS (+) group. The blood gases and pulmonary alveolar function were not different between the two groups. The oxygen delivery decreased in MPS (-) group, but it was unchanged in MPS (+) group. The oxygen consumption was unchanged in the two groups. Blood cortisol increased and epinephrine and norepinephrine decreased in MPS (+) group. Methylprednisolone had an effect to improve the decreased CI and the increased SVRI, and no respiratory problems were observed in patients for coronary artery bypass grafting with catecholamine administration for a few hours after cardiopulmonary bypass.

Altered localization of 73-kilodalton heat-shock protein in rat kidneys with gentamicin-induced acute tubular injury.

BACKGROUND: The constitutive 73-kilodalton heat-shock protein (HSP73) has been shown to have various essential functions in cells under both normal and stress conditions. In the present study, we observed serial localizations of HSP73 in rat kidneys with gentamicin-induced acute tubular injury. EXPERIMENTAL DESIGN: Sprague-Dawley rats received gentamicin (80 mg/kg/day) for 14 days, and developed acute proximal tubular injury. The intrarenal immunohistochemical distribution of HSP73 was examined by using a specific antibody against HSP73. In addition, HSP73 content in both isotonic buffer- and detergent-extractable renal fractions were measured by immunoblot analysis. RESULTS: After the gentamicin exposure, HSP73 moved from the nucleus to the cytoplasm, and accumulated in granules that were considered to be expressed within enlarged lysosomes in the injured proximal tubular epithelial cells. These granules started to appear from 36 hours after the first gentamicin exposure, enlarged in size until day 12, and gradually diminished after day 18. At day 27, the HSP73 localization pattern returned to that in the normal kidney. Moreover, significantly increased HSP73 protein bands were detected by immunoblot of detergent-extractable fractions from gentamicin-treated rat kidneys at from 36 hours to day 15 after the gentamicin exposure. CONCLUSIONS: Our findings suggest that HSP73 is rapidly induced as an insoluble form in injured lysosomes of the proximal tubular epithelial cells during gentamicin-induced acute tubular injury.
