Modification of batroxobin with activated polyethylene glycol: reduction of binding ability towards anti-batroxobin antibody and retention of defibrinogenation activity in circulation of preimmunized dogs.

Amino groups of batroxobin (Bothrops atrox thrombic protease) were modified with 2,4-bis(O-methoxypolyethylene glycol)-6-chloro-s-triazine (activated PEG2). The modified batroxobin had the reduced binding ability towards anti-batroxobin antibody but retained its enzymic activity in vitro and in vivo. Administration of modified batroxobin in which 29% of the total amino groups in the molecule had been modified, to beagle dogs preimmunized with native batroxobin gave rise to a marked reduction of the fibrinogen level in plasma, accompanied with an increased level of fibrinogen (fibrin) degradation products, FDP. On the other hand, no reduction of fibrinogen level was observed when native batroxobin instead of modified batroxobin was injected to immunized dogs.

[The clinical effects of a new antiviral 9-(2-hydroxyethoxymethyl) guanine (aciclovir) against herpes virus infections].

The clinical effects of a new anti-viral 9-(2-hydroxymethoxymethyl) guanine (Aciclovir) against Herpes virus infections have been investigated. The patients had malignant tumours or auto-immune disease complicated by shingles and chicken pox due to Vaicella zoster virus (VZV) (43 cases), Herpes simplex virus (HSV) (10 cases) and 9 cases which were clinically diagnosed as Herpes, though the virus was not confirmed as the causative agent. As a general principle the dosage of Aciclovir was 5 mg/kg, t. i. d. for 5 days by slow intravenous infusion. The clinically effective rate against VZV was 93%, being excellent in 42% and against HSV it was 80%, being excellent in 40% and when the results of the cases of unknown origin were included it was excellent in 40% and the cumulative effective rate was 88%. Concerning the efficacy in reduction of pain, swelling, disappearance of vesicles and new scab formation, the effect was most noticeable after the third day of treatment. Treatment given early in the disease is likely to provide better results. Concerning side effects, one of 62 patients had proteinuria and the other had a drug rash and an abnormal liver function test. It is likely that the combination of treatment and the primary disease had some influence, but the cause/effect relationship to Aciclovir treatment is not clear.

[Nodal psammoma body in thyroid cancer as an indicator of cancer metastasis to the lymph node].

A 64-year-old woman underwent subtotal thyroidectomy and cervical lymphadenectomy for thyroid carcinoma. In 5 of 15 resected nodes, histologic examination revealed metastasis. The other 10 nodes showed no metastasis, though psammoma bodies were seen in 3 nodes. To clarify the relationship between psammoma bodies and metastasis, 10 cancer-negative nodes were serially sectioned (4 microns in thickness). This disclosed cancer cells and psammoma bodies in 2 to 3 psammoma body-positive nodes. We suggest that the presence of psammoma bodies in the lymph node may indicate metastasis.

[Multiple primary malignancies associated with thyroid cancer].

The characteristics of multiple primary carcinomas associated with thyroid carcinoma were studied in 35 patients (35/362; 9.7%) who met the clinicopathological criteria of Warren and Gate. The frequency of the association was similar in papillary and follicular thyroid carcinomas. The average patient age at the time of initial carcinoma detection was 60.3 years for males and 51.5 years for females. The most frequent sites of the associated cancers were breast, uterine cervix and uterine body in females, and stomach and larynx in males. Patients should be followed closely for recurrence and second cancer detection in the early stage.

[New oral cephem antibiotic, cefroxadine capsules, against superficial suppurative disease in the field of surgery].

Cefroxadine was administered at dose level of 750 mg/day to 21 cases of superficial suppurative diseases and the following results were obtained: The effective rate determined by the treating doctor was 9/21 (42.9%). The effective rate by the evaluation standard was 12/21 (57.1%). The negative-conversion rate was 11/11 (100%) in the 11 cases in which bacteriological effects were clarified. No side effects were observed in all of the 21 cases.

[A case of sudden gross hematuria caused by an iliac artery-ureteral fistula].

A 51-year-old male patient, who had been treated for nonspecific angitis with pulselessness in the upper extremities, was found to have stenosis of the left common iliac artery. Patch grafting was performed for the stenotic lesion of the artery. Sixteen months after the operation, the patient developed a sudden massive hematuria for which he was immediately admitted. He required blood transfusions and bed rest. Pyelography and arteriography revealed almost complete impairment of the left kidney function. During subsequent left retrograde ureterography , the patient again developed massive hematuria and fell in shock. The ureterogram revealed left hydronephrosis and the adhesive left ureter to the site of the patch graft. Extraluminal outflow of contrast medium into the artery was also noted. These findings were considered to indicate the hematuria being an extravasation of blood from an arterio-ureteral fistula formed in the patch graft region. An emergency operation was performed. Contamination was avoided by construction of the femoro-femoral arterial bypass graft to the left lower extremity, prior to the radical removal of the potentially infected fistula and left kidney. The surgical procedures have saved the patient and his lower extremities.

[Glucagon-producing carcinoid of the rectum--report of a case].

A 22-year-old male who had been receiving insulin therapy to treat uncontrollable hyperglycemia, was diagnosed to have a carcinoid in the lower rectum. The excised tumor was 1.5 cm in diameter, and showed a yellowish-white cut surface. It did not invade into the proper muscle. Numerous glucagon immunoreactive cells were found. The tumor cells contained many rounded neurosecretory granules. After tumor excision, the patient's hyperglycemia did not return in the absence of insulin administration. We suggest that the tumor may have secreted glucagon which may have acted as an antagonist against insulin in the circulation.

[Clinical and pathological studies of multiple primary carcinoma of the colon and rectum].

During a 5-year period from 1976-1981, we clinicopathologically studied 21 patients with 45 lesions of multiple primary colorectal carcinomas and compared our findings with those made in 288 single carcinomas of the large intestine. The frequency of multiple primary colorectal carcinoma was 6.8%. The pathological findings suggested an interrelationship between colorectal adenoma and carcinoma. Patients with multiple carcinomas of the large bowel had coexisting adenomas more than patients with single carcinoma. We suggest that it is necessary to do a thorough preoperative examination of cancer of the large intestine to detect double cancers or other adenomatous polypoid lesions with synchronous association.
