[Changes in C100-3, and N14 antibodies in patients with chronic hepatitis C treated with interferon].

In interferon therapy for chronic hepatitis type C, we assayed C100-3 and N14 antibodies, and also determined IgM-C100-3 and IgM-N14 antibodies as well as HCV-RNA. N14 antibody was assayed by an end point titer method. In patients responding to interferon therapy, C100-3 and N14 antibody titers were decreased and became negative. In patients irresponsive to interferon therapy, C100-3 antibody titers were hardly changed, while N14 antibody titers were markedly increased. IgM-C100-3 and IgM-N14 antibody levels were significantly higher in the patients non-responding to interferon therapy than in those responding to interferon therapy. The daily assays of C100-3 and N14 antibody titers are considered useful in judging the effectiveness of interferon against chronic hepatitis type C.

[Neoadjuvant (preoperative) chemotherapy for resectable breast cancer].

Neoadjuvant (preoperative) chemotherapy (NACT) represents a novel approach based on sound theoretical, pharmacokinetic, and experimental principles. The purpose of NACT is to improve control of the primary site by stage reduction and/or to improve control of micro-metastatic disease. We showed in this paper that: 1) Intensive NACT is beneficial to most patients with infiltrative breast cancer. 2) The extent of surgery can be reduced by NACT, and may provide optimal local control with the possibility of breast conservation. 3) There is no evidence that cure or survival is altered by NACT, as compared with standard postoperative adjuvant therapy. 4) Evidence from all of these studies indicates the safety of NACT relative to overall systemic toxicity and local wound healing. In recent years, NACT has been tested in twelve patients with stage III and inflammatory breast cancer in our institute. The objective clinical response was as follows: complete response 0%, partial response 92%, no change 8%, progressive disease 0%. Projected disease-free survival was 51% and projected overall survival was 63% at 7 years. These reported results and our experience emphasize the important role of NACT in the management of breast cancer.

Structure-activity relationships of an endothelin ETA receptor antagonist, 50-235, and its derivatives.

27-O-Caffeoyl myricerone (50-235) is a nonpeptide endothelin receptor antagonist which is highly selective for the endothelin ETA receptor subtype. In order to determine which functional groups in 50-235 are essential for its activity, we examined the potencies of 50-235 and its derivatives to inhibit [125I]endothelin-1 binding and endothelin-1-induced increase in the cytosolic Ca2+ concentration in rat aortic smooth muscle A7r5 cells. The results suggest that the 3-keto, 17-carboxyl and 27-caffeoyl groups in 50-235 are important for ETA receptor blocking activity. Modifications of the catechol ring of the 27-caffeoyl group influenced the affinity and the functional antagonist activity, but the effects were not parallel.

Preparation of lipid-derivatized glycosaminoglycans to probe a regulatory function of the carbohydrate moieties of proteoglycans in cell-matrix interaction.

We have shown previously that chondroitin sulfate, but not heparan sulfate/heparin, linked to either natural core proteins or serum albumin interferes with cell-to-substrate adhesion, provided that the external proteoglycans are topologically immobilized on plastic plates. In order to study the roles of glycosaminoglycan chains (GAGs) as recognition structure, a new assay system is now developed which involves the conversion of free GAGs to reactive lactone derivatives selectively modified at the reducing end. The modified GAGs can be coupled to the amino group of phosphatidylethanolamino (PE) for use as probes on either plastic plates or cell surfaces. Incubation of GAG-PE solutions in polystyrene plates results in a time- and dose-dependent increase of the density of the GAG chains noncovalently immobilized onto the plates. No immobilization is detected with any of the GAG-PE samples that have been treated with phospholipase D. A M(r) 30,000 chondroitin sulfate conjugate to PE (CS-PE), when immobilized onto a fibronectin-coated well for 2 h at an initial concentration of 0.06 microgram/100 microliters/well, inhibits the adhesion of baby hamster kidney (BHK) cells to the substratum by approximately 50%, whereas heparin-, heparan sulfate-, hyaluronic acid-, and dermatan sulfate-PE do not. The effect of CS-PE is abolished by treating the CS-PE-coated plates with chondroitinase ABC. A similar level of inhibition by CS-PE is found when the RGD-containing 120-kDa fragment of fibronectin is used in place of fibronectin. CS-PE in soluble form, once exposed to BHK cells in suspension, can be associated with the cell surfaces, thereby exerting some inhibitory effects on cell-to-substrate adhesion. On a per mol basis, however, the activity of cell-associated CS-PE is far lower than that of substrate-associated CS-PE. Together the results indicate that our GAG-PEs offer useful tools for probing regulatory function of the GAG moieties of proteoglycans and further support the hypothesis that the inhibitory regulation of cell-to-matrix adhesion by large chondroitin sulfate proteoglycans is caused by an interaction between the cell surface and the chondroitin sulfate chains topologically immobilized on extracellular matrices.

Quantitative analysis of distribution and fate of human lung cancer emboli labeled with 125I-5-iodo-2'-deoxyuridine in nude mice.

The chemical and radio toxicity of 125-5-iodo-2'-deoxyuridine (125IUDR) on 870127T human lung cancer (HLC) cells grown in tissue cultures and the quantitative analysis of the distribution and fate of 125IUDR-labeled 870127T HLC cells in nude mice were evaluated. After 870127T HLC cells were plated and 125IUDR was added to the dishes at levels ranging from 0.1 microCi/ml to 5.0 microCi/ml of media, the growth rate of the cells for 24 h was similar to that of non-labeled cells. Nude mice were given intravenous injections of 125IUDR labeled 870127T HLC cells and killed at various intervals ranging from 5 min to 24 h after injection. Organs were collected, processed, and monitored. The lung contained most of the tumor cells at all intervals and the number of tumor cells in the lung decreased gradually post-injection. The tumor cells died rapidly, and only about 1.5% of all cell survived after 24 h post-injection. This study confirmed that very few surviving tumor cells are needed to cause metastasis.

Further study of experimental gastritis in rats: endocrine and neurocrine profile.

We conducted this study to identify the endocrine and neurocrine mechanisms of gastric mucosal protection in rats with experimental atrophic gastritis (erosive atrophic antritis) induced by prolonged exposure to taurocholate. This resulting gastritis was characterized by a significant reduction of parietal cell mass, a decrease in mucosal thickness, decreased numbers of pyloric glands, infiltration by inflammatory cells, and fibrotic proliferation in the gastric mucosa. Mucosal erosions were also prominent. These morphologic and morphometric findings indicate the presence of erosive atrophic gastritis, as previously described. Fasting levels of serum gastrin increased significantly in the rats with gastritis versus controls, whereas the mucosal gastrin levels did not differ significantly from those of controls. Mucosal levels of somatostatin decreased significantly, and vasoactive intestinal peptide (VIP) increased significantly in the pyloric sphincter region. These findings suggest that these peptides and neuropeptides are involved in the induction of this form of gastritis. The peptides may play an important role in the mechanisms of gastric mucosal protection (i.e., gastrin is an aggressive and somatostatin is a defensive factor, and VIP promotes the reflux of bile into the stomach by relaxing the pyloric sphincter).

Primary hepatic carcinoid tumor.

A case of primary carcinoid tumor arising in the liver of a 69 year old woman with no endocrine symptoms is reported. Histopathologically, the tumor was diagnosed initially as a hepatocellular carcinoma in the biopsy specimen, and was shown subsequently to be a carcinoid tumor, demonstrating diffuse positive staining with Grimelius method. Mucin stained with periodic acid-Schiff (PAS), alcian-blue, and mucicarmine, and was shown partially in the glandular structures. Immunohistochemically, most of the tumor cells stained positively for chromogranin-A, epithelial membrane antigen (EMA) and neuron specific enolase (NSE). Ultrastructural examination revealed electron-dense core granules, measuring 40-120 nm in diameter in some of the tumor cells. Intensive and careful searches pre- and post-operatively revealed no other primary source of tumor other than the liver. The patient was reported well with no symptoms 3 1/2 years after the operation. This case is considered to be a primary hepatic carcinoid tumor. The recent literature is reviewed, and the possible histogenesis of hepatic carcinoid tumor is discussed.

[A study on the detection of Helicobacter pylori in children with gastric and duodenal diseases].

Infection of Hellcobacter pylori (H. pylori) in the gastric mucosa is said to participate in the pathogenesis of gastric and duodenal ulcers as well as gastritis. The detection rate of H. pylori in tissue specimens endoscopically taken from adult patients with these diseases was 75.2% (408/542). In affected children (2-15 yrs), on the other hand, 27 biopsies were performed in 22 cases but the initial positive rats was as low as 45.5%. According to the diagnosis, it was 75% for acute gastric mucosal lesion (AGML), 50% for duodenal ulcer, 33% for duodenitis, and 38% in seemingly normal children. When changes in the detection rate of H. pylori was examined after administration of H2 blocking antiulcer agents, it was found to very case by case, e.g. changed in individual disease stages or completely or reoccurrence disappeared.

Immunoglobulin-complexed aspartate aminotransferase.

We report a case of increased aspartate aminotransferase (AST, EC 2.6.1.1; GOT) in a 17-year-old girl which persisted for 3 years. The patient was healthy, but a high level of serum AST was detected during a school health check. Further examination revealed that AST was increased to as high as 259 IU/l while alanine aminotransferase (ALT) was normal. Immunoelectrosyneresis and immunoprecipitation methods revealed that this atypical AST combined with IgG--kappa, lambda globulin and formed macromolecular complexes. Including the present case, 26 cases of IgG-complexed AST have been reported. It is important to be aware of this syndrome, and thereby avoid unnecessary examinations and therapies.

[Effects of nipradilol on the patients of angina pectoris: results from the study of the exercise electrocardiography using HR-ST loop method].

This study was performed to confirm the usefulness of 12 mg nipradilol, a beta-blocker with a vasodilator action similar to that of nitrate-type drugs, in the treatment of effort angina pectoris. It also discusses the mechanism of the action of this drug when using the exercise test heart rate-ST displacement analysis. The symptom threshold multistage graded exercise test was performed using nine patients with chronic stabilized effort angina pectoris after an observation period and an 8-week administration period and HR-ST analysis was performed simultaneously. The number of anginal attacks significantly decreased from the 2nd week, and the heart rate at rest decreased significantly in the prone position although blood pressure did not change. The maximum oxygen intake increased markedly from 13.7 +/- 1.1 ml/kg/min to 19.3 +/- 2.5 ml/kg/min, and the maximum ST decrease improved significantly from 1.72 +/- 0.45 mm to 1.17 +/- 0.19mm. In HR-ST analysis, delta ST/delta HR and delta ST/delta DP tended to improve slightly. Since monotherapy with a beta-blocker causes significant increase in delta ST/delta HR and delta ST/delta DP, it was assumed that a vasodilator action in addition to a beta-blocker action is involved in the improvement of maximum ST decrease brought about by nipradilol.

[Case report of metastatic cutaneous angiosarcoma causing bilateral pneumothorax].

The patient was a 72-year-old man who underwent operation for bilateral pneumothorax due to rupture of cystic lung lesions. Two months after the operation, intrathoracic bleeding redeveloped. The patient developed a tumor on his head. Pathological examination of the scalp lesion revealed cutaneous angiosarcoma. A biopsy of thickened pleura showed that the bleeding was due to pleural metastatic angiosarcoma of the skin. The multiple cystic lesions were considered to be metastases of angiosarcoma. We conclude that pneumothorax or hemothorax in the elderly should be differentiated from malignant metastatic lung tumor. Cystic lung lesions should be considered as possible metastatic angiosarcoma from the skin.

A novel non-peptide endothelin antagonist isolated from bayberry, Myrica cerifera.

A potent non-peptide ET receptor antagonist, myriceron caffeoyl ester (50-235), was isolated from the bayberry, Myrica cerifera. This compound selectively antagonized specific binding of [125I]ET-1, but not of [125I]ET-3, to rat cardiac membranes, ET-1-induced increase in the intracellular free calcium concentration in Swiss 3T3 fibroblasts, and ET-1-induced contraction of rat aortic strips. Thus, 50-235 is the first non-peptide ET(A) receptor antagonist. This compound can be useful for studying the physiological role of endothelin and exploring its role in various diseases.
