Muscle pathology in Marinesco-Sjogren syndrome: a unique ultrastructural feature.

The light- and electron-microscopical findings in the skeletal muscle of a patient with Marinesco-Sjogren syndrome (MSS) are presented. Muscle biopsy specimens showed myopathy with slightly dystrophic changes including variation in fiber size, muscle fiber necrosis, regeneration and rimmed vacuole formation. Fiber type analysis with myosin ATPase staining showed a mild increase in type 2C fibers. Electron microscopy revealed autophagic vacuoles with numerous myeloid bodies, and a unique dense membranous structure associated with nuclei. We consider that this unique membranous structure is an important feature in the muscle pathology of MSS.

Clinical usefulness of electrically elicited pain-related evoked potentials.

In 14 patients with sensory disturbance, the clinical usefulness of electrical pain-related evoked potentials (EPREPs) were studied. In four patients with deep sensation disturbance, conventional somatosensory evoked potentials (SEPs) were abnormal. However, EPREPs were abnormal only in one of the patients. In nine patients with pain-temperature sensation disturbance, SEPs were normal except for only one patient and EPREPs were abnormal in seven of the nine patients. In a patient with herpes zoster myelitis, EPREPs elicited by electrical stimulation below the disturbed spinal level were delayed. After symptomatic improvement, EPREPs recovered. EPREPs are useful in the clinical field.

The lack of dopaminergic effects on the motor responses produced by magnetic cortical stimulation in patients with Parkinson's disease.

The effects of levo-dopa administration on the responses in the abductor pollicis brevis muscle after magnetic cortical stimulation, and on the auditory effects on the motor responses produced by magnetic cortical stimulation, were examined in eight patients with Parkinson's disease (PD). Levo-dopa administration did not change the amplitudes of the motor responses or the auditory effects on the motor responses produced by magnetic cortical stimulation. Not only the dopaminergic system, but also other neurotransmitter systems, may play a role in the motor disturbance of PD patients, at least advanced PD patients.

Reduced late component of exteroceptive reflex in patients with Parkinson's disease.

The exteroceptive reflexes in the masseter muscles in patients with Parkinson's disease (PD) were examined. The reflex produced by 3 Hz mental nerve stimulation consisted of four negative peaks and three positive peaks in the unrectified recordings. The latencies of the peaks in patients with PD were the same as those in control subjects, except for the P2 component in the right masseter muscle. The amplitudes of N3-P3 and P3-N4 were smaller in patients with PD than those in control subjects. Interpeak latency of N3-N4 was shorter in PD patients. Exteroceptive suppression in the masseter muscles was also examined. The early component (ES1) of so-called exteroceptive suppression was normal in patients with PD. The duration and degree of the late component (ES2) of exteroceptive suppression decreased in patients with PD. According to the latencies, N1 and N2 speaks seem to be the onset and end of ESI; N3 and N3 peaks the onset and end of ES2, respectively. The early component did not show any significant abnormality. Reduced late component of exteroceptive reflex in patients with PD might be related to the abnormal motor control system of the facial muscles.
