Two neonates, aged 8 and 18 days, with pulmonary atresia and intact ventricular septum underwent right ventricular outflow tract reconstruction with an autologous pericardial transannular patch. Preoperative cardiac catheterization revealed a tripartite right ventricular morphology with suprasystemic right ventricular morphology with suprasystemic right ventricular systolic pressure (right ventricular to left ventricular peak pressure ratio was 1.2 in case 1 and 2.0 in case 2). Right ventricular volume was 73.5% of normal and 115% of normal respectively. Generous resection of the infundibular myocardium was done and a patch was inserted with the heart kept beating as the perfusate temperature of 32 degrees C. In case 1 a modified Blalock-Taussig shunt was added on the postoperative day 21, because of the insufficient growth of the right ventricle, but in case 2 prostaglandin E1 could be discontinued on the postoperative day 9.
Using surgical specimens from patients with lung tumor, alveolar apertures and alveolar sizes were quantitatively assessed using scanning electron microscopy. Compared with smoking habits, increased apertures, defined as alveolar destruction were observed in patients smoking more than 35 years, whereas alveolar size was increased in patients, with a 15-year smoking history. Experimental studies with exposure to NO2, O3 and H2SO4 mist showed that exposure of NO2 and H2SO4 could induce epithelial injury such as disappearance of cilia in the central airways.
A 57 years old woman was admitted for the investigation of anginal chest pain. Her coronary angiography didn't show any typical finding seen in the aortitis syndrome, but coronary sclerosis and stenosis seen in the usual atherosclerotic patients. The cerebral angiography was performed, too. The right vertebral artery, which was tortuous and dilated, was seen, but the other three cerebral vessels were not seen. These rare findings were reported in this report. We added the discussion about the relation between the aortitis syndrome and angina pectoris.
Two cases of abdominal bronchogenic cyst were reported. In the first case, the cyst was communicated with gastric fundus and existed as gastric abscess. In the second case, the cyst showed high CT number and chemical analysis of the contents revealed high range of Ca, Fe and Amylase levels.
Gas chromatography was applied for determination of the amounts of propionic and butyric acids in the medium produced by Propionibacterium acnes. The organism was incubated in Peptone-Yeast extract-Glucose medium containing 0.017 mEq/ml of tributyrin and various amounts of Coptidis Rhizoma at 37 degrees C for 24 to 96 hr. The amount of butyric acid produced by the lipase was reduced parallel with that of propionic acid resulted from the growth of P. acnes in the medium. These facts were also confirmed by counting the cell numbers of P. acnes. Furthermore, any lipase-negative colonies were not observed on the Kishishita's Lipase Medium. These findings indicate that Coptidis Rhizoma inhibits growth of P. acens and anti-lipase activity of the drug against the organism is resulted from the fact.
Pulmonary artery banding has been a useful palliative procedure for infants with congenital cardiac anomalies associated with excessive pulmonary blood flow. We have experienced some cases that the band was not sufficient enough to reduce the pulmonary artery pressure in complex cardiac anomalies. Therefore, we developed a new adjustable pulmonary artery banding system which can be re-adjusted extrathoracically without reoperation. We used this system in seven infants with congenital cardiac lesions and obtained good results.
The main reasons of recent advances of radiotherapy are considered to be due to improvements of therapeutic techniques and therapy machines, but the contributions of radiobiology are thought to be no so great. In the present paper, the history of radiotherapy is reviewed, and several problems to be solved in order to make more effective radiotherapy taken in radiobiological factors was discussed.
A woman, who had been developmentally delayed since her birth, further deteriorated after she got an episode of high fever of unknown origin at 2 years of age. At the age of 52 years, she died of liver cancer after a long-standing HB virus carrier state. Neuropathological examination revealed brain atrophy, narrowed white matter, myelin pallor, fibrillary gliosis and status marmoratus of the thalamus. These findings suggested the primary cause of the brain pathology in this case to be a developmental destructive process which should have taken place in the early stage of brain development. In addition, neurofibrillary tangles (NFT) were noticed in the hippocampus, locus ceruleus and nucleus basalis of Meynert, but no senile plaques were found anywhere. These NFT changes seemed to be closely resembling to those of the Fukuyama type congenital muscular dystrophy. But the reason why NFT occur in this case is still obscure and probably different from that in the Down syndrome or in physiological senility.
Small airway obstruction in idiopathic interstitial pneumonia was studied in 54 cases of interstitial pneumonia accompanied by restrictive, but not obstructive impairment (%VC less than 80%, FEV1/FVC% greater than 70%). Correlation analysis of %VC showed that small airway obstruction seemed to play a role in the decrease in VC in these cases. Cases were then divided into two groups. In group A, restrictive impairment was caused by the decrease in TLC (%TLC less than 80% & %RV less than 120%) and in group B, it was caused by increase in RV (%TLC greater than 80% & %RV greater than 120). Cases of idiopathic interstitial pneumonia (IIP) were preferentially categorized into group A. Statistical comparison of the lung function measurements revealed that lung parenchyma was altered less severely whereas small airway obstruction was more advanced in group B than group A. Group B was divided into two subgroups, the subgroups of IIP cases and of the other disease cases, and the latter had a larger lung volume and more severely impaired small airway function. Furthermore, IIP cases from group A had less severely altered lung parenchyma than IIP cases from group B. Smoking habits did not seem to be related to these results. The results indicate that in most cases of interstitial pneumonia other than IIP, the lung function was characterized by small airway obstruction rather than decreased lung volume, and some cases of IIP also showed a similar trend. In addition, a long-term study of lung function was made in some cases of interstitial pneumonia and it was shown that small airway obstruction could change in severity after a time interval.
A 59-year-old woman with hypercalcemia due to hyperparathyroidism and 1-year-old boy with cryptogenic hypercalcemia were referred for bone scanning. There were increased accumulations of the technetium-phosphate complex in thyroid, stomach, lung, heart, and kidney. 99mTc-MDP bone scanning was useful for detecting metastatic calcification.
A new screening test to detect a colorectal (CR) cancer (Shamsuddin, 1988) is reviewed. This test (Shams' test) is based on enzymatic detection of beta-D-Gal(1----3)-D-GalNAc (T-antigen) in the CR mucin. The sample and the reagent for this test are stable, the reaction process is simple, and the result is accurate and well reproducible. In contrast to the immunologically-detected fecal occult blood test, the sensitivity and specificity for CR cancers are surprisingly high, the percentage values in using the Shams test having been found to be 100% and 93.1%, respectively (Shamsuddin). In our pilot study using Japanese patients, the sensitivity was more than 80%. In view of these findings, the Shams' test can be considered a suitable replacement for the presently used immunological fecal occult blood test in screening CR cancers.
Data obtained from thirty patients with villous or tubulovillous tumors of the colon were examined in a search for correlations between histological and endoscopic features. There was a good correlation between the two findings. As villous elements became more prominent in the histology, finely granular and/or villous surface patterns, adherent mucus, and a characteristic color (slightly reddish with white spots) were seen more frequently in the endoscopic photographs. These findings are considered to be typical endoscopic features of villous tumor of the colon. Endoscopy using the sprayed dye technique is useful for visualizing the finely granular and/or villous surface pattern.
Among 848 cases of profound mental retardation with motor disturbance admitted to Metropolitan Medical Center of Severely Handicapped in the last 20 years, 98 died. The 94 cases whose cause of death was determined were clinically investigated. There was no difference in sex, and 72% of the patients died before the age of 15 years. Half of the patients died of pneumonia; sudden death occurred in 9, and ileus in 8. These three were thought to be the most important and characteristic causes of death in severely handicapped patients. In recent years, deaths due to pneumonia have decreased and those due to ileus have disappeared, but deaths due to malignant neoplasm have begun to be recognized. There were also some deaths from intracranial hemorrhage in young children, and some deaths from tracheal bleeding in those who had tracheal tubes. These two were also important causes of death in the patients. Sudden death had certain characteristics: most cases were adolescent or young patients with mixed quadriplegia who were sensitive to environmental changes and often showed marked hypertonia by athetosis, and in addition, all of their acute changes occurred between 5 and 8 a.m. or between 6 and 9 p.m.
A mouse monoclonal antibody (mAb) was developed against monohexaosylceramide. This mAb differentially reacted on thin-layer chromatograms with 3 types of galactosylceramide (GalCer) obtained from bovine brain. Structural analysis of the 3 glycolipids revealed that they consisted of the same galactose and sphingosine but of apparently different fatty acids. Among the GalCers, the mAb reacted with teh two GalCers which contained alpha-hydroxy fatty acids, but not with GalCer composed of nonhydroxy fatty acids. These findings suggest that not only that the mAb discriminated the fatty acid composition in the ceramide moiety of GalCer, but also that the ceramide structure defines the immunological epitope as it is known to do for the carbohydrate moiety of glycosphingolipid.
Radiotherapy was applied to 14 esophageal carcinomas with respiratory tract fistulas using various treatment regimens. Closure of the fistulas was observed in four cases after irradiation at a daily dose of 1.5 Gy. The period of closure was transient in two cases but long-term in the other two. One long-term closure case underwent low-dose rate telecobalt therapy (LDRT)(1 Gy per hour, 7 Gy per day; a total dose of 28 Gy) as a boost. His cancer has been well controlled and he is still alive without recurrence at 74 months after fistula formation.
Uncontrollable change of diabetes mellitus (DM) has occurred in one of our patients who had received hepatic arterial embolization (HAE) for hepatocellular carcinoma (HCC). This prompted us to examine the influence of HAE to the diabetic patients with HCC. Thirty-four patients accompanying DM who had received HAE were examined fasted blood glucose (FBG) and the liver function before and after the procedure. HAE was performed using Gelatin Sponge and Lipiodol containing anticancer agents, either alone or combined. Of 34 patients 6 showed increase of FBG level of more than two times after HAE. The FBG level had a tendency to elevate as the grade of DM advanced. The tendency was also recognized on pre-HAE oral glucose tolerance test. However, FBG elevation had no relation to the changes of liver function (GPT, Choline Esterase), the difference of embolic materials and pre-HAE status of DM control. From the results, one must be aware that HAE or Lipiodol infusion to diabetic patients with HCC sometimes may cause uncontrollable change of DM, especially in case of advanced DM patients. Consequently, careful follow-up of HCC as DM is advisable for improvement of the patients' prognosis.
Transcatheter arterial chemoembolization therapy using polylactic acid microspheres containing aclarubicin hydrochloride (ACR) was performed in 62 patients with primary hepatocellular carcinoma. These microspheres were about 200 microns in diameter and contained 10% (w/w) aclarubicin. A single dose of polylactic acid microspheres containing ACR (50-100 mg of ACR) was administered 1 to 8 times with a mean of 2.2 doses (a total of 160 treatments) in 62 patients. Antitumor effects were observed from the decrease in serum alpha-fetoprotein levels (82.1% of the patients) and in two dimensional size of tumor on computed tomography (93.6%). The cumulative survival rate was 54.3% at 1 year, 24.6% at 2 years, and 19.2% at 3 years, respectively, among 59 patients with unresectable tumors. In 3 resected liver specimens, there was a significant accumulation of ACR in the tumor, and severe necroses of the tumors were observed histologically. Systemic toxicity was mild and all patients tolerated this treatment. These results suggest that transcatheter arterial chemoembolization with the use of polylactic acid microspheres containing ACR is a useful tumor-targeting chemotherapy and is effective in the treatment of hepatocellular carcinoma.
