[Sleeve pulmonary arterial resection for bronchogenic carcinoma].

Two cases of bronchogenic carcinoma undergone left upper lobectomy (R 3) with bronchoplasty and sleeve pulmonary arterial resection via mid-sternotomy were reported. Both cases were squamous cell carcinoma originated in the orifice of the left upper lobe. Case 1 was stage IIIB (T2N3M0) bronchogenic carcinoma, its postoperative course was uneventful and died of distant lymphatic metastasis thirty-three months after operation. Case 2 was stage II (T2N1M0) bronchogenic carcinoma and its postoperative management was laborious because of hard expectoration of the sputum but is doing well fifteen months after operation. In order to preserve adequate pulmonary function and to maintain reasonable quality of life (QOL) for the patients with impaired pulmonary function, this angioplastic procedure seems to be acceptable. It is still under discussion to perform this procedure for the patients who would be able to withstand undergoing pneumonectomy, therefore we adopt this method only for every patient for whom it is difficult to maintain desirable QOL after pneumonectomy. Namely, for the patient whose predicted one second forced expiratory volume (FEV1.0) after pneumonectomy is less than 900 ml/m2, we'll be likely to try this angioplastic procedure at first.

[Analysis of locally controlled esophageal carcinomas treated with radiotherapy].

Of 227 esophageal carcinomas treated with a radiation dose of 60 Gy or more, 100 patients had no tumor or ulceration (with or without stenosis) of the esophagus after irradiation. We analyzed local control factors of these 100 patients to determine the need for further treatment. The cumulative local control rate at five years was 40% in all cases, 37% in 21 cases without any stenosis of the esophagus and 40% in 79 cases with stenosis. The presence of stenosis of the esophagus after irradiation was not a critical factor in predicting final local control. Local recurrence of tumors with findings of Borrmann III or Borrmann IV by the pretreatment esophageal barium study, tumors controlled after a total dose of more than 80 Gy, tumors without low dose rate telecobalt therapy (LDRT; 1 Gy/hour, 5 to 7Gy/day, a total dose of 12 to 15 Gy) as boost therapy, and apparently controlled tumors with a stenotic ratio of 60% or more or with 5 cm or more length of stenosis of the esophagus after irradiation was significantly higher than that of the others (p < 0.05). Multivariate analysis revealed that findings of pretreatment barium study, total dose, with or without LDRT, and length of stenosis of the esophagus after irradiation were significantly important factors in local control. Members of the high risk group of apparently controlled tumors should undertake surgical treatment or further intensive chemotherapy.

[Interstitial pneumonia associated with mixed connective tissue disease--marked improvement with azathioprine].

A 55-year-old man who complained dyspnea on exertion was given a diagnosis of interstitial pneumonia associated with mixed connective tissue disease, based on symptoms and on physical and laboratory findings. Soon afterward, he experienced an acute exacerbation of the interstitial pneumonia. He was given three courses of steroid pulse therapy and cyclophosphamide, but his condition worsened. After administration of azathioprine in addition to corticosteroids, his respiratory status improved. The dose of corticosteroids was tapered and the patient has been free of disease for two years. Pulmonary involvement in mixed connective tissue disease has been considered to be relatively benign and easily treated with corticosteroids. Recently, however, many cases of corticosteroid-resistant disease with poor outcome have been reported. Azathioprine was very effective against the interstitial pneumonia in this case, but general conclusions about the most effective type of immunosuppressive agent and the timing of its administration must await the results of further study.

[CT and MR imaging findings in a case of cerebral alveolar echinococcosis masquerading as brain tumor].

We report a rare case of cerebral alveolar echinococcosis that was verified by pathological examination. The lesion presented as an intracranial mass which was slightly hyperdense on CT scans and very hypointense with central hyperintensity on T2-weighted MR images. The lesion exhibited marked contrast enhancement on both CT and MR images and was accompanied by extensive white matter edema. Although radiological differentiation from brain tumor was difficult, hypointensity on T2-weighted MR images appeared to be characteristic of lesions of cerebral alveolar echinococcosis.

[Transient heterogeneous enhancement on dual-phase helical CT of liver].

Transient heterogeneous enhancement was seen on the arterial phase of dual phase helical CT of the liver. The shape of the enhancement was appeared wedged or patchy. These phenomena without liver tumor were observed in 23 (2.3%) of 1012 patients with suspected hepatobiliary disease. Plain CT showed no attenuation difference in the liver. Twenty-two of these 23 cases were diagnosed as scarred liver, A-P shunt caused by liver biopsy, acute cholecystitis, liver abscess, liver cirrhosis, or advanced pancreas head cancer. The diagnosis of one case was uncertain. These phenomena were thought to be caused by 1) a regional direct increase in hepatic arterial flow due to arterial-portal (A-P) shunt, or hypervascular tumor ; or 2) a compensatory increase in hepatic arterial flow as a result of decreased portal venous flow caused by tumor invasion or severe liver cirrhosis.

Enhanced proliferation and IL-1 production of mouse splenocytes by low-dose whole-body X-irradiation.

To elucidate the stimulative effect of low-dose whole-body X-irradiation on the immune system, in vivo, we studied its effects on some immune functions of mouse splenocytes. Results show that Concanavalin A (Con A) and phytohemagglutinin (PHA) responses of splenocytes were significantly increased by irradiation of 2.5 cGy and 5 cGy, whereas lipopolysaccharide (LPS) response was significantly depressed by irradiation of 5 cGy. By irradiation of 2.5 cGy, Con A response was significantly accelerated at each concentration of Con A, but the optimum concentration of Con A shifted to a higher value of 4 micrograms/ml from 2 micrograms/ml in the control group. When blood plasma obtained from 2.5 cGy irradiated mice was added into the medium at 0.005-1%, the Con A response of splenocytes in another un-irradiated mouse was significantly accelerated over that where the plasma added came from the sham-irradiated control mice. Furthermore, 2.5 cGy whole-body irradiation also enhanced the biological activity of intracellular interleukin-1 (IL-1) of LPS-stimulated splenocytes.

Decreased incidence of thymic lymphoma in AKR mice as a result of chronic, fractionated low-dose total-body X irradiation.

We have investigated whether low-dose total-body X irradiation could suppress the development of lymphoma in AKR mice. Male mice were irradiated with 5 cGy three times a week or 15 cGy two times a week from 11 weeks of age for 40 weeks. The incidences of lymphoma were 80.5% in sham-irradiated mice, 67.5% in mice irradiated with 5 cGy three times a week and 48.6% in mice irradiated with 15 cGy twice a week. Incidence of lymphoma was significantly reduced by irradiation with 15 cGy twice a week (P = 0.006). The mean survival time was significantly prolonged from 283 +/- 3 days in control mice to 316 +/- 10 days in mice irradiated with 15 cGy twice a week (P = 0.008) and to 309 +/- 14 days in mice irradiated with 5 cGy three times a week (P = 0.040). Because about 80% of male AKR mice die of lymphoma, it is likely that the prolonged life span observed in the irradiated mice is due to the reduced incidence of lymphoma.

[Leigh syndrome: serial CT and MR imaging findings].

Serial CT and MRI findings in five patients (two boys and three girls) with Leigh syndrome were retrospectively reviewed in a follow-up period lasting from six months to 10 years. The two boys were found to have cytochrome c oxidase deficiency and one of the girls to have mitochondrial DNA mutation, while the remaining two girls had no detectable enzyme deficiency. CT and MRI revealed symmetrical involvement of the brain bilaterally in all cases. The focal lesions were found most frequently in putamina and caudate nuclei (four cases each), followed by thalami (three cases), globi pallidi, and midbrain (two cases each). In addition, diffuse white matter and/or cortical lesions were disclosed in three cases. MRI and CT at an early stage of the disease revealed swollen, symmetrical lesions which showed shrinkage in size accompanied by the adjacent brain atrophy on later images. Some lesions were detected only in the early stage and were not shown in later images. Thus, careful reading of sequential changes appears to be required for accurate diagnosis of Leigh syndrome.

[Relationship between the stage of diabetic nephropathy and serum lipoprotein (a) concentrations--influence of hypoproteinemia].

UNLABELLED: Lipoprotein (a) [Lp(a)] has been reported to be an independent risk factor for coronary heart disease. Elevated levels of Lp(a) in diabetic subjects have also been reported, especially in subjects with nephropathy. However, the mechanism and the clinical implication of Lp(a) elevation in diabetics remain obscure. In the present study, to verify the change in Lp(a) concentration with the progression of nephropathy, serum Lp(a) levels were measured in 546 NIDDM patients, 33 hemodialysis NIDDM patients, and 145 non-diabetic controls. RESULTS: 1) The serum Lp(a) levels in diabetics were significantly higher than those in the non-diabetic controls. 2) Serum Lp(a) concentrations in microalbuminuric subjects were significantly higher than those in normoalbuminuric subjects, and were further increased in macroalbuminuric subjects. 3) In macroalbuminuric subjects, serum Lp(a) concentrations were inversely correlated with serum protein concentrations, but not with serum creatinine. 4) Once hemodialysis was started, the decrement of both serum Lp (a) and LDL-C concentrations were associated with the increment of serum protein concentrations. These data indicate that elevation of serum Lp (a) concentration in overt nephropathy could be, at least in part, due to hypoproteinemia.

[MR imaging of carbon monoxide intoxication: evaluation of 13 cases to discuss the relation between MR findings and clinical course].

MR images of the brain were retrospectively reviewed in 13 patients with carbon monoxide (CO) poisoning to examine the relation between the MR findings and clinical course. The patients included 11 men and 2 women whose ages ranged from 6 to 88 years (mean, 38 years). Repeated MR studies of at most five times were performed on 9 patients. Lesions in the bilateral globus pallidi were found in 5 cases showing hyperintensity on T2-weighted images (T2WI) and iso-to hypointensity on T1-weighted images (T1WI). The patients with only pallidi lesions clinically recovered well. Hyperintense white matter lesions were recognized on T2WI in 8 patients, including all 5 with delayed encephalopathy. Sequential studies generally showed a tendency of regression of these hyperintense lesions with improvement of symptoms, indicative of reversible demyelination as documented in the pathological literature. One patient with poor prognosis revealed white matter lesions, that were hyperintense on T2WI and very hypointense on T1WI, which may represent irreversible change. Thus, clinical course seems to be closely related with white matter lesions, but not with pallidi lesions. In cases of CO intoxication, therefore, MR study provides valuable information on patients' clinical prognoses.

[Preliminary study of radiological image transmission with B-ISDN].

We evaluated the utility of image transmission using broad band ISDN (B-ISDN). The B-ISDN network links Tohoku University Hospital (TUH), interfaced with CDDI, and Tohoku University Department of Technology, interfaced with FDDI (including Tohoku University Computer Center interfaced with Ethernet), at 10km distance interfaced to a Unix workstation. Voluminous radiographic images like MRI 3-D images of the pelvic organs digitized at 5-20 megabytes were transmitted at mean data rates of 1.03Mbps (CDDI-Ethernet) and 30.1Mbps (FDDI-CDDI) with no image distortion. Initially the image data are transmitted to the Computer Center & Department of Technology, then processed and relayed to TUH. B-ISDN can provide fast, accurate image transmission.

Expression of ZAP-70 gene in the developing thymus and various nonlymphoid tissues of embryonic and adult mice.

The full length of cDNA encoding murine ZAP-70 was obtained from a cDNA library of embryonic mouse thymus. Northern blot analysis indicated that the expression of ZAP-70 mRNA was most pronounced in the thymus and spleen and also slightly in the brain. Analysis by in situ hybridization revealed that the mRNA was expressed not only in T cells in the thymus and spleen, but also in nerve cells of the brain. In the thymus, the distribution of positive cells was different between newborn and young adult mice. In the newborn thymus, positive signal in thymocytes was localized in the subcapsular layer as well as in the medulla, while in the young adult thymus, it was localized exclusively in the medulla. Furthermore, the in situ hybridization revealed that positive signal was also seen in the liver, intestine, and lung of embryonic mice. These findings have suggested that ZAP-70 plays an important role in growth, differentiation, and function of not only T cells but also nerve cells and several embryonic tissues.

Quantitative EEG in never-treated schizophrenic patients.

To clarify whether patients with schizophrenia still show EEG slowing in the absence of psychopharmacological treatment, EEG was analyzed in 20 acute never-treated schizophrenics and 20 age-matched healthy controls using the computerized wave-form recognition method. Compared to controls, schizophrenics had more fast theta (6-8 Hz) and slow alpha (8-9 Hz) activity, and less fast alpha activity (9-13 Hz). The average EEG frequency at O1 correlated negatively with total and positive symptom scores on the BPRS in the schizophrenic group. These findings confirm that the frequency of alpha rhythm is slowed in schizophrenia and that this slowing is possibly related to the expression of psychopathology in this disorder.
