[A case of a male gastric choriocarcinoma with massive mesenteric tumor and review of the literature on male choriocarcinoma].

A 32-year-old man was admitted to our hospital, complaining of an abdominal mass. An operation disclosed a large tumor, the size of a child's head, in the mesenterium of the transverse colon, with a multiple liver metastasis. A tumor resection was performed and, histologically, the tumor was diagnosed as being a choriocarcinoma. Gynecomastia was not observed. The patient died 2 months after this operation in spite of chemotherapy. An autopsy disclosed metastatic choriocarcinomas in the lungs, the liver, the peritoneum, the stomach, the adrenal glands and in the lymph nodes. The testis was found to be normal by a serial histological examination. Primary advanced adenocarcinoma, which is histologically similar to a choriocarcinoma, was found in the stomach. This case was finally diagnosed as being a gastric adenocarcinoma which showed choriocarcinomatous differentiation at various metastatic sites.

[An analysis of 117 cases of patients who died from tumor recurrence or from the progression of ovarian malignancies].

Out of 403 patients with ovarian with malignancies during 1978 to, 1985, 117 (29.0%) died from the progression of a non-curative cancer (56 cases, 47.9%) or from a cancer recurrence (61 cases, 52.1%). The mean survival rate 117 cases was 13.4 months, of which 103 (88.0%) cases concerned patients who died within 2 years and 114 (97.4%) who died within 3 years. A histological analysis revealed that patients with a serous or an endometrial cancer had a longer survival rate than others. Among 61 recurrent cases, 59 (96.7%) fell into recurrence within 2 years. Regarding the relationship between a recurrent of cancer and its prognosis, patients with a recurrence of an ascitic or a metastatic disease had a poorer prognosis than patients with a pelvic or an abdominal mass. An aggressive operation, such as a resection of a recurrent tumor, even if small in volume, led to a better prognostic result than no therapy.

Determination of urokinase in the urine of healthy volunteers and patients with renal diseases.

A sensitive enzyme immunoassay was developed for the determination of urokinase in the plasma and urine. Normal human urine contained 2.68 +/- 0.36 u/ml of UK. Gel filtration of the mixture of normal urine resulted in 2 peaks in the profile, one with molecular weight of 32,000 and the other with molecular weight of 22,000. Patients with glomerulonephritis had higher UK levels. Kidneys with minimal changes and mild type or with the inactive phase of systemic lupus erythematosus (SLE) secreted relatively high levels of UK in the urine.

Immunolocalization of type III collagen and procollagen in cirrhotic human liver using monoclonal antibodies.

Immunolocalization of type III collagen and procollagen in cirrhotic human liver was studied using monoclonal antibody specific for the helical determinant of type III collagen extracted from human placenta. Deparaffinized, trypsin-treated cirrhotic liver sections from 8 autopsy cases were examined by the unlabeled peroxidase-antiperoxidase and immunofluorescence techniques. These techniques revealed the localization of this epitope shared by type III collagen and procollagen not only in the extracellular matrix of hepatocytes and sinusoidal cells but also in the cytoplasm. In hepatocellular carcinoma concurrent with cirrhosis, neoplastic cells were shown to react with this antibody as well. These results are consistent with data obtained using antiserum specific for bovine type III procollagen aminopeptide which appeared in our previous report.

[Embryonal carcinoma (Higuchi, Kato) of the ovary, clinical analysis and recent improvement of treatment using alpha FP as a tumor marker].

Forty-one cases of embryonal carcinoma were analyzed from a clinico-pathological point of view. The clinical stages of the patients were: 21 cases in stage I, 8 in stage II, 11 in stage III and 1 in stage IV. Survival curves were compared in accordance with clinical stages. A statistically significant difference was demonstrated between stages I and III, and a significant difference was demonstrated between stages I and II. Embryonal carcinoma in a narrow sense and yolk sac carcinoma were compared in survival curve but there were no significant differences between the two. Cases were divided into two that were pure embryonal carcinoma and embryonal carcinoma with other constituent mixtures. A comparison of these two showed that the mixed type had a better survival curve than the pure type. Effects of treatment in two different periods were compared patients who were treated in 1971-1979 belong to the 1st period group and those treated in 1979-1985 to the 2nd period group. A comparison of these two showed a much better survival curve in the 2nd period group. This difference can be attributed partly to a VAC combination chemotherapy and its maintenance for two years and partly to improvements in the operating technique such as added omentectomy and or the introduction of a second look operation. By measuring alpha FP over the whole clinical course, alpha FP was shown to be a good tumor marker reflecting the patient's condition and aggravation of the patient's condition frequently occurred around or somewhat after 15 weeks after primary surgery.

[The determination of thymidylate synthetase inhibition for testing fluoropyrimidines--pharmacokinetics and metabolism of carmofur (HCFU) in patients with gynecologic malignancies--Tokai HCFU Study Group for Gynecologic Malignancies].

The pharmacokinetics and metabolism of carmofur (HCFU) were studied. Sixty-six patients were administered 100 mg of HCFU orally, and the plasma levels of the HCFU fraction (HCFUf) and 5-fluorouracil (5-FUra) were determined at 0, 1, 2, 4 and 6 hours. The average half-life of HCFUf and 5-FUra were 1.05 and 1.31 hours, and the average areas under the curves (AUC) of the plasma concentration were 6.51 hr X mcg/ml and 0.46 hr X mcg/ml, respectively. Surgical specimens of the tumors were obtained about three hours after the administration and assayed for HCFUf. 5-FUra fluorodeoxyuridine-monophosphate (FdUMP), deoxyuridine-monophosphate (dUMP), total thymidylate synthetase (TS total), and non-FdUMP-bound free enzyme (TS free). The TS inhibition rate (IR) was calculated by the follow method: IR = (TS total-TS free)/TS total X 100 levels of the TS total varied from not-detected (less than 0.10 pmol/g) to 20.5 pmol/g. The average FdUMP: dUMP ratio was 3.44 X 10(3), However, more than 80% inhibitions of TS were observed in nine cases (21.4%). The correlation indicates between TS IR and tissue FdUMP level or FdUMP: dUMP ratio were 0.57 and 0.62 in ovarian malignancies respectively. No significant correlations were observed between TS inhibition and levels of tissue 5-FUra or AUC of 5-FUra.

Anti-fibrotic effect of malotilate on liver fibrosis induced by carbon tetrachloride in rats.

The effect of malotilate on liver fibrosis of rat induced by carbon tetrachloride (CCl4) was studied. CCl4 was given subcutaneously (1.5 ml/kg) to male Wistar rats twice a week for 11 weeks. Administration of malotilate (100 mg/kg) 5 times a week was started in the 5th experimental week and continued thereafter. The biochemical parameters in serum such as concentration of total protein and transaminase activities were improved by malotilate administration. The livers treated with CCl4 only were atrophic and markedly nodular, and histologically, severe collagen deposition and pseudolobular formation were observed. However, the livers treated with CCl4 and malotilate showed only slight accumulation of collagen fibers although hypertrophic and fatty metamorphosis was observed. Immunocytochemically, type I and type III collagens were stained in livers from rats treated with CCl4 only and rats treated with CCl4 and malotilate, but stainability was rather weak in the latter. The increased amount of hydroxyproline in the liver and that excreted into urine were markedly suppressed by malotilate administration. The levels of protocollagen prolyl hydroxylase activity in the liver were almost the same in CCl4-treated and CCl4 and malotilate-treated rats. However, the levels of collagenolytic enzyme activity were slightly higher in the latter. From these results, anti-fibrotic action of malotilate was clear and involvement of enhanced collagenolytic enzyme activity was suggested.

[Phase II study of epirubicin on gastric cancer--a cooperative study of the Tokai Cancer Chemotherapy Group].

A phase II study of epirubicin, a new anthracycline derivative, was performed in 23 patients with advanced gastric cancer. Epirubicin was administered intravenously at a dose of 20-30 mg/m2/day for two or three consecutive days every two or three weeks. Sixteen cases were evaluable and there were two partial responses and one minor response. Overall response rate (more than PR) was 12.5% (2/16). Leukopenia (less than 4,000/mm3) and anemia (less than 11.0 g/dl) were observed in 71.4% and 69.2% of patients, respectively. No thrombocytopenia was observed. Other toxicities were alopecia (71.4%), nausea and vomiting (42.9%), anorexia (25.0%), stomatitis (12.5%), fatigue (12.5%), fever (6.3%) and tachycardia (6.3%), but these effects were relatively mild in most cases.

[A case of retroperitoneal alveolar soft part sarcoma].

We present the first case of retroperitoneal alveolar soft part sarcoma (ASPS) to be reported in Japan. The patient was a 27-years-old woman. After the retroperitoneal (tumors were) removed, bilateral lung metastasis was found and removed. Shortly thereafter, a recurrent retroperitoneal tumor arose, but because removal of the tumor was impossible, injection of 100 mg of cisplatin and 100 mg of adriamycin into the feeding artery was performed, and radiotherapy was also undertaken. However, the treatments were not effective, and the patient is now being followed on an outpatient basis.

[Effect of immunotherapy using the non-specific immunopotentiator OK-432 in malignant ovarian tumors--selecting appropriate cases for immunotherapy].

Immunotherapy for malignant ovarian tumors has been discussed in a number of recent studies. However, the effects of this therapy are produced by an indirect mechanism, and are, therefore, difficult to evaluated. In order to select appropriate cases for immunotherapy using OK-432 against malignant ovarian tumors, clinical and experimental studies were carried out. Clinically patients with ovarian cancer were divided into two groups; OK-432 treated group and no immunotherapy groups. As a parameter of immunoresponse to OK-432, PHA and Su-PS skin tests were performed periodically. Natural killer (NK) cell activity was also measured before and after administration of OK-432. Peripheral blood lymphocyte subsets were analysed by using monoclonal antibody and a laser flow cytometer. Finally tumor growth velocity was evaluated in tumor transplanted nude mice. The survival rate was used as an index for evaluation. The survival rate of the OK-432 treated groups was better than the control groups in advanced cases, but a statistically significant difference was found between the two groups. Among fundamental studies which were carried out to select appropriate cases, Su-PS skin test, the NK response of OK-432 seemed to be actually utilized.

Prognostic significance of nuclear analysis in invasive carcinoma of cervix patients treated by primary radiation.

The value of microspectrophotometric analysis was studied for the assessment of prognosis in patients with squamous cell carcinoma of the cervix treated with primary radiation therapy. Cellular nuclear desoxyribonucleic acid (DNA) content and nuclear size were measured in smears taken before, during and immediately after radiation therapy. Additional determination of DNA content/nuclear size ratio was made. Changing patterns of DNA distribution, nuclear size and their ratio were classified into Type I, II, III, and IV, according to the mode of their shifts in the course of radiation therapy. The results showed diploid (47%), polyploid (23%) and aneuploid (30%) DNA distribution in pre-treated carcinomas. Particularly, the results showed a distinct correlation between the type of histograms and patient outcome. Patients with Type II histograms (initial shift to the right and posterior shift to the left) showed a better survival rate and no recurrences compared with other types. On the other hand, patients with Type III histograms (initial shift to the left and posterior shift to the right) had poor prognosis and were predominant among dead cases with radioresistant tumors. These results suggested the potential usefulness of cytophotometric analysis for assessing prognosis in advanced cases.

Localization of type III procollagen aminopeptide antigenicity in hepatocytes from cirrhotic human liver.

Immunolocalization of type III procollagen (pro III) in normal and cirrhotic human liver was studied using rabbit antiserum specific for bovine type III procollagen aminopeptide. The material examined was deparaffinized, trypsin-treated hepatic tissue sections from 28 autopsy cases, including 19 cirrhotic and 9 normal liver donors. Immunostaining, performed by the unlabeled peroxidase-antiperoxidase antibody technique demonstrated that extracellular matrices corresponding to perisinusoidal reticulin, collagen in periportal areas, and blood vessel walls were the common sites of pro III antigenicity in both normal and cirrhotic liver. Moreover, in the cirrhotic liver, the fibrous septa of pseudolobules, and cytoplasm of hepatocytes and sinusoidal cells were positive when stained for pro III peptide. The differential counts of pro III positive cells in cirrhotic liver, however, revealed that the average ratio of these hepatocytes to sinusoidal cells was 25 to 1, indicating complete dominance of hepatocytes with respect to stainability for pro III peptide compared to sinusoidal cells. In hepatocellular carcinomas coexisting with cirrhosis, neoplastic cells also displayed pro III antigenicity. These data suggest that hepatocytes of cirrhotic liver and hepatocellular carcinoma cells play a significant role in type III collagen synthesis in vivo.

[A case of cystadenocarcinoma developing from an ovary autografted to the abdominal wall].

A 73-year-old woman developed an abdominal tumor. We performed tumor resection. The tumor, located between the fascia and peritoneum, was removed completely. The histological finding was cystadenocarcinoma. Thirty-eight years earlier she had undergone total hysterectomy and bilateral oophorectomy because of abnormal genital bleeding. At that time, her own ovary had been transplanted to the abdominal wall. We diagnosed this tumor as having developed from the autografted ovary. The complication of ovarian autografting is cystic degeneration. This complication is rare in Japan. Moreover, malignant change is extremely unusual in ovarian autografting.

[Effects of chemotherapy of advanced malignant ovarian tumors].

Malignant ovarian tumor is regarded as the disease with the worse prognosis among obstetrical and gynecological malignancies. In the past, a great number of trials have been done with single or combined treatments. Because of the rather low incidence of malignant ovarian tumor in any one hospital over a short period of time and the numerous factors affecting the prognosis of patients, no definite procedures have been established so far. In order to evaluate the effects of chemotherapy, 460 cases were investigated retrospectively. These cases were treated primarily with surgery from 1974 to 1979 and adjuvant chemotherapy was selected freely by the doctors responsible. Cumulative 5-year survival rates were calculated as 88.8% for a low potential malignant group, 65.9% for stage I ovarian cancer, 40.6% for stage II, 9.7% for stage III, 0% for stage IV and 10% for metastatic tumor from other organs. The effects of chemotherapy were investigated especially for stage III and IV and the metastatic tumor group. Cumulative survival curves were justified by generalized Wilcoxon test. An MFC combination, a double-agent, a single-agent and a no-chemotherapy group were compared by survival curves. There were statistical significances between the MFC and no-chemotherapy group, and the double-agent and no-chemotherapy group. Significant tendency was present between the MFC and single-agent group. Several popularly-used combinations in this period such as MFC, VEM, METT, FAMT, METVFC and a group with other combinations were compared. There were no statistical significances among the 6 combinations, but an MFC combination seemed to be a better treatment for malignant ovarian tumors judging from accumulative survival curves.

[Chemosensitivity test for gynecologic malignancies by in vitro colony assay].

In Vitro Colony Assay was carried out to examine chemosensitivity in 57 specimens from gynecologic malignancies, and following results were obtained. Total of 34 (59.6%) formed adequate colonies for sensitive assay. Among ten anticancer drugs that were tested, Cis-platinum, Adriamycin, 5-FU and Bleomycin were relatively effective for ovarian malignancies. In vivo chemosensitive tests were also performed using five xenograft systems of human gynecologic tumor. These in vivo results were compared with in vitro data, and a 75% true positive and an 87.5% true negative rate were observed. Retrospective correlations between in vitro chemosensitive data and patient responses to chemotherapy were analyzed from 15 evaluable patients, then a 71.5% true positive and a 75% true negative rate were observed. To obtain pharmacokinetic data on anticancer drugs, we measured concentrations of plasma and tumor tissue for Cis-platinum, Adriamycin and Bleomycin in operated patients and xenografts. The in vivo tissue concentrations were higher than the in vitro drug concentrations usually employed in colony assay. By comparing the inhibitory effects on tumor colony growth of 1-hr and continuous drug exposures in colony assay, in vitro continuous exposure schedules were made available for cell cycle specific anticancer drugs.
