[Significance of high normal blood pressure in occupational health care].

In order to elucidate the significance of high normal blood pressure in occupational health care, a 5 year follow up survey was performed on 874 men with normal blood pressure and 225 men with high normal blood pressure. The major results of this survey were as follows. 1) Sixty-seven out of 874 with normal blood pressure (7.7%), and 78 out of 225 with high normal blood pressure (34.6%) became hypertensive in 5 years. 2) High normals started with a higher frequency of obesity, hypercholesterolemia, hyper gamma-Glutamyl transpeptidasemia compared with normal blood pressure at the start of the survey. 3) Logistic regression analysis showed that age, SBP, DBP were risk factors of developing hypertension from normal blood pressure, while only the amount of alcohol drinking applied for high normal blood pressure. 4) Logistic regression showed that high normal blood pressure, drinking, obesity and were significantly related to the development of hypertension. These results suggested that interventive activities for high normal blood pressure should be included in occupational health care because of a high tendency of underlying poor life style and a high risk of getting hypertension.

Immunohistochemical localization of surfactant protein A in N-bis (2-hydroxypropyl) nitrosamine-induced lung tumors in rats.

Surfactant protein A (SP-A) is most abundant protein associated with pulmonary surfactant which is synthesized by alveolar type II cells in the alveoli. In this study, we localized SP-A in experimentally induced pulmonary hyperplasias and tumors in rats, by immunohistochemistry. When rats were given a single intraperitoneal injection of N-bis (2-hydroxypropyl) nitrosamine (BHPN) followed by exposed to mixture gases of O3 and NO2, hyperplastic alveolar type II cells stained with the antibody against SP-A were located in the alveolar walls near the alveolar ducts. Adenomas and adenocarcinomas were stained with the anti-SP-A antibody in the lung parenchyma. These immunohistochemical findings suggested that the lung tumors induced in rats treated with BHPN and additionally exposed to mixture gases of O3 and NO2 are derived from mainly alveolar type II cells.

[Concentration and fiber size of asbestos in lungs of residents living close to the serpentinite area].

Concentrations and fiber size distribution of asbestos in the lung tissue of 16 ( 8 males and 8 females) rural residents living close to the serpentinite area (neighboring group) were analyzed by low temperature ashing-analytical transmission electron microscopy, and were compared with those of 5 (5 males) rural and 53 (34 males and 19 females) urban residents living away from the serpentinite area (reference group). The results are as follows; 1. Pulmonary asbestos fibers were found in all rural residents. The types of asbestos fibers were chrysotile, amosite, crocidolite, actinolite, and tremolite. These findings were very similar to results for urban residents. 2. In males, the geometric mean concentration of actinolite-tremolite in the neighboring group (1.78 x 10(6) fibers/g dry lung) was significantly higher than those in the reference groups (rural: 0.32 x 10(6), urban: 0.31 x 10(6), p < 0.01). In females also, the geometric mean concentration of actinolite-tremolite in the neighboring group (0.59 x 10(6)) was significantly higher than that in the reference group (urban: 0.16 x 10(6), p < 0.05). There were no significant differences in the actinolite-tremolite concentrations between males and females in the neighboring group. 3. The serpentinite in this area have contained not only chrysotile but also both actinolite and tremolite. There was no significant relationship between actinolite-tremolite concentration and occupational history in the neighboring group. It is possible that actinolite and tremolite are liberated from the serpentinite to the atmosphere and inhaled by the residents near the serpentinite area which increases pulmonary actinolite-tremolite concentration. 4. The difference in the geometric mean concentration of chrysotile between the neighboring and the reference group was not statistically significant for both sexes, possibly due to the poor durability of chrysotile in the lung tissue. 5. There were no significant differences in the geometric mean of fiber length and diameter of actinolite-tremolite between the neighboring and the reference group.

[Natural history of cloudy zone of pulmonary adenocarcinoma on HRCT].

Seven pulmonary adenocarcinomas with cloudy zone (CZ), which was defined as a lower density zone in the pulmonary lesion than pulmonary vessels in the surrounding normal lung on high resolution computed tomography (HRCT), were followed up for at least 3 months. There were three tumors with homogeneous CZ, one of which showed no change when rescanned after three months, the others showed enlargement with or without replacement by high density areas on rescanned images obtained after more than one year. Two tumors containing some high density areas on the initial scans showed slight decreases in size on rescanned images obtained after 1 to 3 months. However, on rescanned images obtained after more than 1 year and 9 months, these tumors also showed enlargement of CZ. Pathologically, CZs corresponded to well-differentiated adenocarcinoma of the bronchioloalveolar type with no or mild interstitial change, whereas high density areas mainly corresponded to papillary adenocarcinoma with severe interstitial fibrosis. In conclusion, long-term careful follow-up of CZ is necessary to detect enlargement or the appearance of high density. Unchanged CZ in short-term follow-up should be considered a finding suggestive of malignancy.

Endometrial carcinoma in women 65 years of age or older: a clinicopathologic study.

The purpose of this study is to clarify the clinical and pathologic features of endometrial carcinomas in women 65 years of age or older in Japan where the incidence of endometrial carcinoma is still low. Between 1979 and 1990, 24 of 164 patients with endometrial carcinoma were 65 years old or over. In addition, 41 patients 50 years of age or younger were also analyzed, comparing clinicopathologic features with those of older age group. Seventeen of 24 (71%) showed deep myometrial invasion (more than a half of the myometrium). Endometrial carcinomas in older age groups have deeper myometrial invasion, and higher histologic grade than those of younger age groups. The adjacent endometrium is usually atrophic and the presence of hyperplasia and/or metaplasia was significantly less frequent in the older group as compared with that in the younger group. Nine of these 24 patients died of disease in a relatively short follow-up period. The prognosis for the older group (expected 5-year survival 69.2%) is significantly worse than for the younger group (expected 5-year survival 94.3%) (P = 0.006). Endometrial carcinomas in patients over 65 years of age are usually of high histologic grade, and have deep myometrial invasion and an unfavorable prognosis.

[Infantile onset chronic inflammatory demyelinating polyneuropathy with clinical course of 23 years; a case report].

We report a 23-year-old female with infantile onset chronic inflammatory demyelinating polyneuropathy. Muscle weakness was noticed when she was 1 year old, and, together with sensory disturbances, gradually progressed in an asymmetric manner. Nerve conduction studies disclosed slowing of conduction velocities, temporal dispersion, and decreased amplitude of compound muscle action potentials, the degrees of which were markedly different amongst different nerves even in the same limbs. The sural nerve biopsy showed various degrees of nerve fiber amongst different fascicles. Marked inter-nerve and intra-nerve differences of involvement and clinical improvement after steroid therapy supported the diagnosis of chronic inflammatory demyelinating polyneuropathy. It should be noted that even after a long clinical course of 23 years, her clinical symptoms remained asymmetrical and multi-focal lesions could be clearly demonstrated.

[Evaluation of occult atherosclerosis in the left main coronary artery in patients with acute anterior myocardial infarction].

Intravascular ultrasound was performed in 30 patients with acute anterior myocardial infarction to evaluate occult atherosclerosis in the left main coronary artery which could not be detected by coronary angiography. The intravascular ultrasound catheter was positioned in the left main coronary artery after successful intracoronary thrombolysis, and cross-sectional coronary artery scanning was performed. Percentage area stenosis (%AS) was calculated as (plaque cross-sectional area/vessel cross-sectional area) x 100. Images of crescent-shaped eccentric plaque were obtained in all patients. The mean vessel cross-sectional area was 28.6 +/- 8.0 mm2, and mean plaque cross-sectional area was 7.7 +/- 4.2 mm2. Mean %AS was 27 +/- 15% (range from 6 to 58%). In five patients, intravascular ultrasound was repeated 3 months later, which showed increased %AS more than 15% in two patients. These findings indicate that occult atherosclerosis is present in the left main coronary artery in almost all the patients with acute anterior myocardial infarction and should be treated with caution when coronary intervention is performed.

[A case of an old woman with Sjögren's syndrome associated with insulin-dependent diabetes mellitus].

Insulin-dependent diabetes mellitus (IDDM) is thought to result from the autoimmune destruction of the insulin-dependent beta cells of the pancreas. Sjögren's syndrome is also an autoimmune disease characterized by the destruction of the lacrimal and salivary glands that leads to keratoconjunctivitis sicca and xerostomia. But it is a very rare case that a patient with Sjögren's syndrome developed IDDM. We present a case of a 65-year old woman with Sjögren's syndrome who developed diabetic ketoacidosis due to IDDM. Recent studies have revealed that there was molecular mimicry between glutamic acid decarboxylase (GAD) and coxsackievirus. Furthermore, some reports have shown that sialadenitis was represented in IDDM model mice. This case shows the possibility that a causal relationship between IDDM and Sjögren's syndrome may exist.

Isolation and characterization of a novel perchloric acid-soluble protein inhibiting cell-free protein synthesis.

We found a novel protein in the postmitochondria supernatant fraction of rat liver, which is soluble in 5% perchloric acid and strongly inhibits protein synthesis in a rabbit reticulocyte lysate system. The protein extracted from the supernatant fraction with 5% perchloric acid was purified by ammonium sulfate fractionation and CM-Sephadex chromatography. The protein was shown to consist of two identical subunits with a molecular mass of 14 kDa. By immunoscreening with the rabbit antisera against the protein, a cDNA encoding the protein was cloned and sequenced. The cDNA contained an open reading frame of 411 base pairs encoding a 136-amino acid protein with a molecular mass of 14,149 Da. The deduced amino acid sequence was completely identical with that constructed from all of the above peptides. Interestingly, the perchloric acid-soluble protein inhibited cell-free protein synthesis in the rabbit reticulocyte lysate system in a different manner from RNase A. The protein is likely to inhibit an initiation stage of cell-free protein synthesis. Among the rat tissues tested, the protein was located only in liver and kidney. These findings are the first report on a new inhibitor that may be involved in the regulation of protein synthesis in those tissues.

Efficient regulation of gene expression by adenovirus vector-mediated delivery of the CRE recombinase.

We have constructed an E1-defective adenovirus (Ad) vector designated AdCAG-Cre containing the Cre recombinase gene derived from bacteriophage P1 under control of the cytomegalovirus immediate early enhancer-chicken beta-actin hybrid (CAG) promoter. We examined the Cre-loxP-based recombination by this Ad vector in C2C12 cells bearing a reporter gene construct CAG-CAT-Z, which directs expression of the E. coli lacZ gene upon Cre-mediated excision of the CAT gene located between the CAG promoter and the lacZ gene. Nearly 100% of these cells were shown to start to produce beta-galactosidase after infection with the AdCAG-Cre vector at MOI 100. On the basis of this result, we discuss the possible use of the AdCAG-Cre vector to manipulate the gene expression in mammalian cells.

Central administration of saikosaponin-d increases corticotropin-releasing factor mRNA levels in the rat hypothalamus.

Saiko agents, Chinese herbal drugs, stimulate corticotropin-releasing factor (CRF) release from the hypothalamus, adrenocorticotropin (ACTH) secretion and proopiomelanocortin (the precursor for ACTH) gene expression in the anterior pituitary. In the present study, the effect of intracerebroventricular injection of saikosaponin (SS)-a and -d, two of the main components of saiko agents, on hypothalamic CRF gene expression was examined in pentobarbital-anesthetized rats. Administration of SS-d, 0.2-2.0 micrograms/kg body wt, increased plasma ACTH levels, proopiomelanocortin mRNA levels in the anterior pituitary and the CRF mRNA level in the hypothalamus in a dose-dependent manner, whereas SS-a failed to have an affect on these levels. These findings indicate that SS-d stimulates both CRF gene expression and CRF release, which in turn increases ACTH release and proopiomelanocortin gene expression in the anterior pituitary. Therefore, SS-d is believed to have an important role both in saiko agent-induced CRF release and CRF gene expression in the hypothalamus.
