Persistent elevation of serum alanine aminotransferase levels leads to poor survival and hepatocellular carcinoma development in type 1 autoimmune hepatitis.

BACKGROUND: Although the prognosis of type 1 autoimmune hepatitis is generally good with immunosuppressive treatment, the disease progresses in some patients despite the treatment. The prognosis may be determined by the clinical course. AIM: To evaluate the long-term prognosis and assess the predictive factors for a serious event, including the development of hepatocellular carcinoma or death. METHODS: Sixty-nine patients with type 1 autoimmune hepatitis were prospectively followed up regularly, with a median follow-up period of 96 months (49-201 months). RESULTS: During the follow-up period, three patients (4%) developed hepatocellular carcinoma, and two of these three patients died. Another patient died of liver failure. The 10-year survival rate was 98%, and the 10-year hepatocellular carcinoma-free rate was 93%. The four patients experiencing a serious event received higher maintenance doses of corticosteroid during their follow-up periods than those did not. However, serum alanine aminotransferase levels during the follow-up period were higher in these four patients than in the others. CONCLUSIONS: Persistent elevation of serum alanine amniotransferase levels during the follow-up period, rather than factors existing prior to medical treatment is considered to be an important prognostic factor, and it is indicated that poor outcomes may result from the resistance to immunosuppressive treatment.

Clinical characteristics of synchronous colorectal cancer are different according to tumour location.

AIMS: The purpose of this study was to investigate the clinical characteristics of synchronous cancer patients, with particular attention given to variations in tumour location. METHODS: A retrospective evaluation of 249 synchronous cancer cases out of 3061 consecutive colorectal cancer patients. RESULTS: Multivariate analysis of risk factors for synchronous cancer according to tumour location revealed that male gender was a significant risk for synchronous lesions in the left colon only (odds ratio=2.05, 95% confidence interval 1.34-3.13). Meanwhile, aging was a risk factor for synchronous cancer in the right colon only (odds ratio=1.05, 95% confidence interval 1.02-1.08), and in both sides of the colon (odds ratio=1.03, 95% confidence interval 1.01-1.05), but not in the left colon only (odds ratio=0.98, 95% confidence interval 0.97-1.00). In addition, patients with synchronous lesions in the right colon only tended to have adenomas in the right colon, while those with synchronous lesions in the left colon only tended to have adenomas in the left colon (each P value <0.05). CONCLUSION: The risk factors and status of concurrent adenomas of synchronous cancer cases varied according to tumour location, suggesting that the colonic site susceptible to neoplasia varies according to patient characteristics.

Clinical features of Japanese male patients with type 1 autoimmune hepatitis.

BACKGROUND: Recently, unusual patients with autoimmune hepatitis, such as male patients, have increased. AIM: To assess clinical feature of Japanese males with type 1 autoimmune hepatitis compared with females. METHODS: We investigated consecutive 160 patients with type 1 autoimmune hepatitis, who consisted of 20 males and 140 females, with a median age of 55 (16-79) years. RESULTS: Compared with females, males had a lower frequency of definite diagnosis according to the revised scoring system proposed by the International Autoimmune Hepatitis Group (40% vs. 85%) and lower serum levels of immunoglobulin G [1932 (1085-3850) mg/dL vs. 2624 (1354-6562) mg/dL]. However, they were similar in age, form of clinical onset, symptomatic concurrent autoimmune disease, human leucocyte antigen DR status and frequency of cirrhosis at the time of diagnosis. The normalization of serum alanine aminotransferase levels within 6 months after the introduction of corticosteroid treatment was lower in males compared with females (73% vs. 93%). CONCLUSIONS: In male patients, a diagnosis of autoimmune hepatitis should be made carefully. In Japanese patients with a dominant frequency of human leucocyte antigen DR4, gender may affect the response to corticosteroid treatment.

Clinical characteristics of fulminant-type autoimmune hepatitis: an analysis of eleven cases.

BACKGROUND: Although a few adult cases of fulminant-type autoimmune hepatitis have been reported, their clinical features and prognosis have remained uncertain. AIM: To assess the clinical features and prognosis of patients with fulminant-type autoimmune hepatitis. METHODS: Eleven patients (10%) diagnosed with fulminant-type autoimmune hepatitis in accordance with the 1999 criteria of the International Autoimmune Hepatitis Group were analysed. RESULTS: All 11 patients were female, with a median age of 53 years. Five patients survived without liver transplantation, one received a liver transplantation, and five died without liver transplantation. Nine patients (82%) survived for 2 weeks or more following diagnosis, without liver transplantation. Except for the patient receiving a liver transplantation, serum total bilirubin levels measured during the clinical course were significantly higher in non-survivors than in survivors, although the accompanying serum alanine aminotransferase levels measured for the two groups were similar. Most significantly, serum total bilirubin levels in non-survivors worsened during days 8-15, while levels in survivors improved during the same period. CONCLUSIONS: The short-term prognosis for patients with fulminant-type autoimmune hepatitis may be good. However, patients whose serum total bilirubin levels worsen during days 8-15 should be considered for liver transplantation.

[Thymic carcinoid accompanied by elevation of serum gastrin-releasing peptide precursor; report of a case].

A 74-year-old male was admitted with an abnormal mediastinal shadow. Computed tomography (CT) and magnetic resonance imaging (MRI) of the thorax showed an anterior mediastinal mass without invasion to the ascending aorta and pulmonary artery. In addition, serum gastrin-releasing peptide precursor (Pro GRP) was increased (60.6 pg/ml, normal range <46 pg/ml). Video-assisted thoracoscopic biopsy demonstrated that the mass was thymic carcinoid. Therefore, median sternotomy was performed to facilitate thymectomy, including the tumor with partial resection of the left upper lobe and pericardium. The patient received mediastinal irradiation postoperatively. The postoperative serum level of Pro GRP decreased to the normal limit 6 months later. Although a biological relationship between Pro GRP and thymic carcinoid was not proven, it might be useful marker for detecting tumor recurrence.

Genetic characterization of parthenogenic Fasciola sp. in Japan on the basis of the sequences of ribosomal and mitochondrial DNA.

Accurate identification of aspermic Fasciola forms in Japan remains difficult because of their morphological variations. In order to characterize the forms genetically, nucleotide sequences of ribosomal internal transcribed spacer (ITS1 and ITS2) and mitochondrial cytochrome c oxidase I (COI) and NADH dehydrogenase I (NDI) genes in 34 liver flukes from 16 prefectures in Japan were analysed. Two major forms represented by Fsp 1 and Fsp 2 had sequences identical to or closely resembling those of F. hepatica and F. gigantica, respectively, in all the 4 DNA markers and were mainly distributed in northern and eastern-western parts of Japan, respectively. Fsp 1 and Fsp 2 would have been introduced into Japan with infected cattle of 2 distinct lineages via the Korean Peninsula and spread through limited parts of Japan (northern and eastern-western parts) together with the movement of each cattle lineage. The Japanese form (Fsp 1/2), which showed heterozygosity in ribosomal DNA and Fsp 2 haplotype in mitochondrial DNA, may have originated in interspecific cross hybridization between paternal F. hepatica and maternal F. gigantica.

[Endobronchial hamartoma removed by bronchoscopic electrosurgical snaring].

A 61-year-old man was referred to our hospital, presenting with endobronchial mass in the right middle bronchus. Chest computed tomography (CT) showed a polypoid tumor. Bronchoscopy revealed a hard, smooth, whitish and pedunculated tumor obstructing the orifice of the both S4 and S5 segmental bronchi. We successfully removed this polypoid tumor via bronchoscopy using a biopsy forceps and electrosurgical snaring. The histological findings were compatible with chondromatous hamartoma. We recommend the endoscopic electrosurgical snaring to treat endobronchial hamartoma, especially when pedunculated, because this procedure is a minimal invasive technique.

Synthetic sulfonolipids deduced from sulfonoquinovosyl diacylglycerols of sea urchin reduces hepatic ischemia-reperfusion injury in rats.

BACKGROUND: In hepatic surgery and liver transplantation, ischemia-reperfusion (I/R) is an unavoidable process, and protection against hepatic I/R injury is a major unresolved problem. In this study, we investigated whether 3-O-(6-deoxy-6-sulfono-beta-D-glucopyranosyl)-1,2-di-O-acylglycerol bound to saturated C18 fatty acids (beta-SQAG9), which was derived from sea urchin intestines, could reduce this injury. This agent was recently reported to have immunosuppressive effects in allogeneic rat skin grafts. MATERIALS & METHODS: Male Lewis rats were divided into two experimental groups. Group 1 rats were injected with SQAG9 (50 mg/kg) into the penile vein 15 minutes before the induction of ischemia and into the portal vein just reperfusion. The same amounts of normal saline were injected into rats in the control group (group 2). Each experimental groups included six rats. Seventy percent hepatic ischemia (20 minutes) was induced by occluding the blood vessels and bile duct with a vascular clamp. For examination of hepatic function, serum levels of aspartate aminotransferase, (AST) alanine transaminase (ALT), and lactic dehydrogenase (LDH) were measured. In addition, histological examination was also assessed. RESULTS: Three hours after reperfusion, the mean plasma concentration of AST, ALT, LDH in group 1 was suppressed compared with group 2. Six hours after reperfusion, the hepatic damage in group 1 was mild in comparison with that in group 2. CONCLUSIONS: Our data demonstrated that SQAG-9 reduced the warm hepatic I/R injury.

Time-series analyses of mandibular and perioral soft tissue movements during mastication.

Masticatory movements are rhythmically repeated and coordinated movements of the jaw, tongue and facial muscles. Thus, we considered that the elucidation of movements that are specific to perioral soft tissue, as a result of perioral facial muscle activities, should be useful for evaluation of the smoothness of masticatory movements. The aim of this study was to evaluate the smoothness of masticatory movements from the component of movements that are specific to perioral soft tissue during mastication by the application of time-series analysis. The subjects were 15 healthy persons with complete natural dentition. The experimental food used for mastication in this study was sufficiently softened chewing gum. The results showed that the component of movements that are specific to perioral soft tissue during mastication are the equal repetition spatially and stable movements temporally, and that these movements have the same accurate rhythm as that of mandibular movements and cooperate with mandibular movements temporally. Moreover, the results suggested, from the viewpoint of kinematics, that the innervation of the central pattern generator was concerned with the neural basis of rhythm generation of perioral facial muscles. Therefore, the component of movements that are specific to perioral soft tissue during mastication is useful for evaluation of the smoothness of masticatory movements.

Video-assisted total glandectomy and immediate reconstruction for breast cancer.

This paper describes a new surgical technique and our clinical experience with video-assisted endoscopic total glandectomy via a middle axillary incision followed by immediate reconstruction with latissimus dorsi muscle flap (LDMF) performed in 17 patients with bigger, multiple tumors or extensive ductal spread of breast cancer. The novel techniques in this procedure are as follows: (1) By securing patients in a semi-lateral position and suspending the upper extremity, either supine or semi-lateral position can be easily achieved by simply rotating the operating table, resulting in a wider working space from the axillary to hip area. (2) By applying a retractor for skin flap traction, endoscopic glandectomy and reconstruction become safe and reliable. As a result, the mean number and size of tumors were 1.2 and 4.12 cm, respectively. Surgical margins of all the cases were pathologically negative and there were no recurrences observed during 14 months follow-up to date. Esthetic results have been satisfactory and the surgical wounds were not visible from the front in any case. Compared to mastectomy, this procedure shows the same therapeutic results, but offers a greater esthetic and psychological advantage to all the patients.

Specific interaction between an oligosaccharide on the tumor cell surface and the novel antitumor agents, sulfoquinovosylacylglycerols.

Some sulfoquinovosylacylglycerols (SQAG) have been shown to be potent DNA polymerase inhibitors, and to have strong antitumor activity in vivo. In this study, we investigated the mode of action of SQAG with regard to the interaction with the tumor cell surface. Of the SQAG used, the monoacyl forms (SQMG) with C18-, C18:1- or C16-fatty acids (SQMG-alphaC18, -alphaC18:1 or -alphaC16) effectively inhibited cell proliferation of a human adenocarcinoma cell line, DLD-1, but SQMG-alphaC14 and the diacyl forms (SQDG) did not. Analysis of the interaction of SQMG-alphaC18 and -alphaC18:1 on three oligosaccharides of cell surface, sLe(A), Le(X), and SM3, by flow cytometry demonstrated that the most effective interaction was observed on sLe(A). DLD-1 cells bound to SQMG-alphaC18:1-coated plates, and this binding was inhibited by monoclonal antibody against sLe(A) or SM3. However, these cells did not bind to SQMG-alphaC14-coated plates. Moreover the cytotoxic effects of SQMG-alphaC18, -alphaC18:1 on DLD-1 cells was inhibited by monoclonal antibodies against sLe(A) or SM3. Our results suggested that the interaction of SQMGs and sLe(A) plays an important role in suppression of the DLD-1 cell proliferation.

Drosophila mitochondrial transcription factor A: characterization of its cDNA and expression pattern during development.

We cloned a cDNA for Drosophila mitochondrial transcription factor A (D-mtTFA) and characterized the recombinant protein. In Drosophila Kc cells, D-mtTFA was localized in the mitochondria, but not in the nucleus. By repetitive precipitation with His-tag and PCR amplification, the consensus nucleotide sequence for D-mtTFA-binding was determined to be 5'-TTATC/G. The binding sequence was found to be clustered in the A + T region of mitochondrial DNA which is suggested to be a replication origin and promoter region for light strand and heavy strand. We found a DNA replication-related element (DRE)-like sequence located upstream of the transcription initiation site of the D-mtTFA gene and obtained results indicating that DRE-binding factor (DREF) can bind to the DRE-like sequence of the D-mtTFA gene. The data suggest that transcription of the D-mtTFA gene is under control of the DRE/DREF regulatory system. Based on these results, the functions of D-mtTFA were discussed in relation to mitochondrial biogenesis of Drosophila melanogaster.

Two types of replication protein A 70 kDa subunit in rice, Oryza sativa: molecular cloning, characterization, and cellular & tissue distribution.

Replication protein A (RPA), which is comprised of three subunits, is an important factor involved in DNA replication, repair, and transcription. We isolated and characterized 70 and 32 kDa subunits of RPA from rice (Oryza sativa cv. Nipponbare) termed OsRPA70a and OsRPA32. OsRPA70a shows a low level of homology with OsRPA1 which was isolated from deepwater rice (Oryza sativa cv. Pin Gaew 56), previously. We also succeeded to isolate OsRPA70b which is homologue to OsRPA1 from Oryza sativa cv. Nipponbare. OsRPA70a shows only 33.8% sequence identity with OsRPA70b, indicating that two different types of 70 kDa RPA subunits are present in Oryza sativa cv. Nipponbare. These subunits showed differences in their expression patterns among tissues. The transcripts of OsRPA70a and OsRPA32 were expressed strongly in proliferating tissues such as root tips and young leaves that contain root apical meristem and marginal meristem, respectively, and weakly in the mature leaves which have no proliferating tissues. On the other hand, OsRPA70b was expressed mostly in the proliferating tissues. The roles of these molecules in plant DNA replication and DNA repair are discussed.

Strand exchange reaction in vitro and DNA-dependent ATPase activity of recombinant LIM15/DMC1 and RAD51 proteins from Coprinus cinereus.

We previously cloned recA-homolog genes from a basidiomycete, Coprinus cinereus, and obtained the recombinant proteins (Nara et al., Mol. Gen. Genet. 262, 781-789, 1999, see Ref. 1; Nara and Sakaguchi, Biochem. Biophys. Res. Commun. 275, 97-102, 2000, see Ref. 2). The primary purpose of the present study was to characterize the biochemical properties of the recombinant LIM15/DMC1 (CoLIM15) and RAD51 (CoRAD51) proteins. We purified the recombinant proteins, and their molecular masses were 37 and 35 kDa, respectively. Both enzymes showed DNA-dependent ATPase activity and ATP-dependent strand exchange reaction in vitro. CoRad51 was a five- to sixfold stronger DNA-dependent ATPase and showed greater dependency on single-stranded DNA than CoLim15. In meiosis, both enzymes were highly accumulated in the meiotic tissue at leptotene and zygotene stages at which the homologous chromosomes pair, but disappeared just before the pachytene stage at which they recombine. From these and the previously reported results, we discuss here the relationships between the enzymes and meiosis.

Shc mediates ligand-induced internalization of epidermal growth factor receptors.

In order to clarify the physiological relevance of the interaction between Shc and adaptins, components of plasma membrane-coated pit adaptor complex AP2, we investigated the role of Shc in ligand-induced endocytosis of epidermal growth factor (EGF) receptors. In vitro peptide binding assay showed that alpha-adaptin bound to the wild-type peptide corresponding to amino acids 346-355 of Shc, RDLFDMKPFE, but not to the mutant peptide in which both phenylalanines at 349 and 354 were substituted for alanines (FA). Using adenovirus vectors carrying a herpes simplex virus epitope-tagged 52-kDa wild-type Shc and Shc FA, we examined the interaction between Shc, AP2, and EGF receptors in intact cells. Alpha-adaptin bound to wild-type Shc in an EGF-dependent manner, whereas EGF-dependent association of alpha-adaptin with Shc FA was markedly reduced. In addition, EGF increased the amount of alpha-adaptin coprecipitated with EGF receptors in cells expressing wild-type Shc but not Shc FA. These results suggest that EGF stimulates Shc-AP2 complex formation and association of Shc-AP2 complexes with EGF receptors. Internalization assay showed that (125)I-EGF internalization was reduced in cells overexpressing Shc FA. Immunofluorescence study showed that punctate staining along the plasma membrane border as well as punctate pattern characteristic of cytoplasmic vesicles near the plasma membrane was enhanced in cells expressing wild-type Shc. These results suggest, therefore, the implication of Shc in ligand-induced endocytosis of EGF receptors in intact cells.

Selective adsorption of biopolymers on zeolites.

Zeolites adsorb biopolymers on their surface and may be suitable as a new type of chromatographic carrier material for proteins, nucleic acids, and their conjugates. We report here various parameters that influence the adsorption of biopolymers on synthesized zeolites with regard to the Si/Al2 ratio and three-dimensional structure. There are three physicochemical principles that may underly the adsorption: 1) below the isoelectric point (pI), mainly Coulombic attraction similar to ion-exchange chromatography; 2) at pI, hydrophobic interactions (a kind of van der Waals attraction) plus the three-dimensional mesopore structure; and 3) above pI, the sum of the Coulombic repulsion and attraction forces, such as the hydrophobic interaction, and also substitution reaction of water on the Al molecule with a protein amino-base. At high Si/Al2 ratio in the presence of a small amount of Al and with mesopores between the zeolite particles, maximal adsorption was seen at pI and was suggested to be dependent on the number of hydrophobic interaction points on the mesopores, and their morphology. The application of zeolites to biochemistry and biotechnology is also discussed.

Studies on in vitro evaluation for the biocompatibility of various biomaterials: inhibitory activity of various kinds of polymer microspheres on metabolic cooperation.

Gap junctional intercellular communication is a function that plays an important role in maintaining cell and tissue homeostasis and in regulating cell growth, development, and differentiation. Change in this function when contacting fibroblasts with various polymer microspheres was estimated using the metabolic cooperation assay system. When the cells were in contact with the microspheres after their adhesion onto a substrate, the function did not alter. However, when they were in contact with precoated microspheres on test dishes, the function was inhibited as the quantity of microspheres increased. Moreover, the inhibition level increased as the diameters of polyethylene and polystyrene microspheres decreased. However, no inhibition was observed if precoated microspheres were composed from poly(L-lactic acid). These findings suggest that the size and the material of microspheres, and how cells recognize the microspheres, are factors affecting cell function of gap junctional intercellular communication. Therefore, estimating this function may provide valuable information about the biocompatibility of many kinds of materials even in the form of particles.

Chirality transferring [3,3] sigmatropic rearrangement of (1-Acyloxy-2-alkenyl)trianlkylsilane synthesis of optically active vinylsilane-containing alpha-amino acid.

A vinylsilane-containing alpha-amino acid and alpha,alpha-disubstituted alpha-amino acid 2 having two contiguous asymmetric carbon centers at their alpha and beta positions were synthesized in an optically active form by ester-enolate Claisen rearrangement of the alpha-acyloxysilane 1 as the key step, where the chirality of an alpha-acyloxy-TBDMS group was completely transferred to the rearranged product.
