Atlanto-axial subluxation (Grisel's syndrome) following otolaryngological diseases and procedures.

Grisel's syndrome is a rare condition of uncertain aetiology characterized by atlanto-axial subluxation following an infection in the head and neck region. The condition most frequently affects children and characteristically presents in the post-operative period with cervical pain and torticollis. We describe three cases and discuss the aetiology and pathogenesis. A high index of suspicion may lead to early diagnosis in its natural history thereby preventing potentially serious complications.

Ethmoidal myofibroblastoma.

Myofibroblastoma is an uncommon mesenchymal tumour characterized by spindle cells exhibiting the ultrastructural and immunohistochemical features of myofibroblasts. We report a case of a myofibroblastoma occurring in the ethmoid sinus, which to our knowledge is the first case reported in the literature. The clinical presentation, diagnosis, histology, and management of this lesion is discussed with a review of the relevant literature.

Bilateral acute mastoiditis complicated by lateral sinus thrombosis.

A case of bilateral mastoiditis with subperiosteal abscesses complicating acute otitis media in a two and a half year old girl is presented. Contrast enhanced computerized tomography confirmed the diagnosis of right lateral sinus thrombosis. The aetiology, diagnosis and management of these conditions are discussed.

The role of computed tomography in the management of peritonsillar abscess.

Twenty-four patients who fulfilled the clinical criteria for peritonsillar abscess (PTA) were randomized into two groups over a 6-month period. Group A (n = 13) underwent CT with contrast, whereas group B (n = 11) was managed without radiologic investigation. In group A, CT with contrast enabled differentiation of PTA from peritonsillar cellulitis in all 13 cases (100%) and demonstrated abscesses in 11 patients (85%), thereby allowing drainage at first attempt. Two patients with peritonsillar cellulitis were successfully managed with antibiotics only. In group B, all patients underwent needle aspiration. In seven patients (64%), pus was found after needle aspiration at first attempt and in one patient after needle aspirations at two locations. In three patients (27%), no pus was found after needle aspirations at three different locations. We conclude that CT enhances diagnostic accuracy, obviating unnecessary drainage procedures, and reduces patient morbidity.

Metastatic melanoma of the orbit. A case report.

We report a metastatic cutaneous melanoma of the medial rectus muscle of the right orbit which presented with diplopia, exophthalmos and retrobulbar pain. Radioimmunoscintigraphy using melanoma-specific monoclonal antibody was used to confirm the diagnosis and to show the presence of disseminated disease in other areas. Effective palliation was achieved with external beam irradiation.

Patients' preferences for the site of injection of low-dose subcutaneous heparin in prophylaxis of thromboembolism. Regional acuphobia.

The efficacy of low-dose subcutaneous heparin in the prophylaxis of deep vein thrombosis and pulmonary embolism is now well established. The most common complication of heparin administration is haematoma formation, especially at the site of injection. A number of patients were noted to have injection site haematoma in the deltoid region: after close inquiry it was found that these patients had refused injections in the anterior abdominal wall in the belief that these would be more painful than the upper arm. A study was set up to determine the prevalence of regional acuphobia.

Left external iliac artery dissection and bilateral renal artery aneurysms secondary to fibromuscular dysplasia: a case report.

A case of acute lower limb ischaemia due to spontaneous dissection of the left external iliac artery and coincidental bilateral renal artery aneurysms is reported. Histopathological examination of the resected iliac artery showed features of fibromuscular dysplasia. Dissection of the external iliac artery without involvement of the aorta is extremely rare with only two previously reported cases. The aetiology and management of fibromuscular dysplasia is discussed.

Multiple brain abscesses secondary to bronchiectasis. A case of 34 discrete abscesses in one brain.

Multiple brain abscesses are a rare complication which may occur in a number of conditions including intra-thoracic sepsis. Computerized tomography has had an advantageous impact on the management of this condition by facilitating earlier diagnosis. However, the treatment modalities are inadequate and mortality remains high. A case of multiple brain abscesses in a patient with bronchiectasis is reported which exemplifies this often fatal condition. The management of multiple brain abscesses is discussed.

Multiple brain abscesses secondary to dental caries and severe periodontal disease.

A case is reported in which an intra-oral focus of sepsis is thought to have resulted in the patient developing multiple brain abscesses without the presence of lung infection or cardiac valvular disease. The link between the two sites of infection is supported by the isolation of two Streptococcus viridans species normally associated with dental caries and periodontal disease. The management of multiple brain abscesses is discussed.

A survey of lead pollution in Chhattisgarh State, central India.

Lead (Pb) is of major environmental concern due to its toxicological importance. The anthropogenic emission of Pb is at least 100 times higher than natural emissions. Soil and dust are significant sources of Pb exposure. Lead is generally immobile in soil and accumulates in the upper layers. Lead particles may enter homes via shoes, clothes, pets, and windows. Central India is rich in deposits of natural resource materials such as coal, pyrite, dolomite, and alumina that contain Pb and other heavy metals at the trace levels, and the substantial exploitation of these materials has tended to increased contamination of water and geological formations. Here we present data on Pb concentrations in the water, soil and sediment samples (n=158) collected from 70 locations in Chhattisgarh state, Raipur region. Lead concentrations in the surface water (n=44), groundwater (n=44), soils (n=60) and sediments (n=10) ranged from 6 to 1410, 3 to 52, 12.8 to 545, and 31 to 423 microg g(-1), with mean values of 305, 16, 102 and 190 microg g(-1), respectively. Most of the Pb fractions of >80% can be leached out with the chemical extractants EDTA, acetic acid, and hydroxylamine hydrochloride. Lead has accumulated in the soil clay fraction due to its relatively large surface area and decreases with increasing depth in the soil profile.

Visible spectrophotometric determination of tantalum in soil, sediment, minerals, and alloys.

A new, sensitive, and selective method is described for the spectrophotometric determination of Ta(V). The method is based on the extraction of the Ta(V)-F-CV+ complex (CV+ = crystal violet cation) with a benzene solution of imidoyl derivatives (ID), i.e., N,N'-diphenylbenzamidine (DPBA), N-hydroxy-N,N'-diphenylbenzamidine, and N-(2,5-dimethyl)phenyl-p-tolylimidoylphenylhydrazine, from sulfuric acid solution; DPBA was selected for detailed study. The molar absorptivities of the Ta(V)-F-CV+-ID complexes in the benzene solution were in the range of (1.00-1.65) x 10(5) L/mol x cm at 600 nm. The limit of detection was 7 ng Ta/mL (which had an absorbance greater than that of the blank + 3 standard deviations). The optimization of the analytical variables, the composition of the complex, and the effect of diverse ions in the determination of Ta are discussed. The present method was applied to the determination of Ta in environmental samples, i.e., soil, sediment, minerals, and alloys.

Audit of patient acceptance of nasal surgery as a day case procedure.

A greater emphasis on day case surgery within the health service is seen as a method of improving efficiency and reducing expenditure. We interviewed 90 consecutive patients undergoing nasal surgery who had been preoperatively assessed as being fit for day case surgery. They were randomised into three groups regarding the duration of postoperative nasal packing. All patients stayed overnight following surgery and were interviewed prior to discharge. Some 52% of the overall sample would be happy to have nasal surgery performed as a day case. If the nasal pack was removed after two hours, this figure rose to 67%. This difference in patient acceptance did not attain statistical significance overall, but there was a significant difference in those undergoing submucosal resection. There was no difference in the age, sex distribution or type of surgery performed between each group. The audit commission quotes patient satisfaction with day case surgery at 80%. Nasal surgery was not examined in their report, but was included as one of a set of procedures suitable for consideration. Although day case nasal surgery may be safe, further research regarding patient acceptance is required.
