About 3 percent of live borns on average need intensive care. This has been proved to be highly effective in reducing neonatal mortality and morbidity provided it is incorporated in a graded system of regional perinatal health services. Its concept is based upon selection and weighing of risks and distribution on different care levels according to individual needs. It depends on close interdisciplinary regional cooperation. Although high quality conditions could be established at single places, there is yet, in general, lack of understanding of and of proper facilities for functionally adequate neonatal care.
This report contains three sections: A general definition of aims and tasks of neonatal intensive care is followed by the description of four regions and four neonatal units of different structure with some relevant statistics. According to this inquiry about 1.5 cots for maximum neonatal care and 4 cots for intermediate care per 1000 live births are needed. Considering efficiency and effectivity the smallest independent intensive care unit should "cover an area" of 4000 deliveries per year; it should be closely connected to a least one obstetrical unit. Where and how this neonatal unit may be integrated into a childrens hospital can be derived from the four examples given. It should be recognized that 1) high quality intensive care depends on the availability of additional pediatric services, 2) comprehensive care of newborn infants necessitates regional organization including a newborn ambulance system. Thus, close contacts with obstetrical, but also with pediatric units offering better facilities should be established. In yearly intervals results are to be self-controlled by statistical means. The following incidences of normal survival of infants referred to childrens hospitals can be gained currently [2]: Birth weights below 1000 g: above 20%; 1000--1499 g: above 85%; above 1500 g: better than 95%. Less favorable figures call for analyses of reasons and measures to abolish them.
Antibodies were measured in 172 unselected neonates against mixed polyvalent antigens derived from E. COLI, Klebsiella, Ps. aeruginosa and Proteus spp. respectively, using a modification of the indirect haemagglutination technique according to Neter. 120 of these neonates with no evidence of infection served as controls. 6 out of 8 children with proven E. coli septicaemia had either raised titres or significant rises in titre. Five further neonates with septicaemia, four due to Klebsiella and one to Pseudomonas, had pathologically raised titres as well. 8 out of 12 neonates with clinically suspected septicaemia in whom no pathogen was isolated from the blood or cerebrospinal fluid had raised antibody titres. Following premature rupture of the membranes 2 out of 3 neonates who subsequently developed proven E. coli septicaemia had raised antibody titres against E. coli even on the first day of life. These results suggest that the measurement of antibody titres can be a valuable aid to the diagnosis of neonatal septicaemia.

Factor VIII activity and factor VIII associated antigen were determined in healthy term newborns. On the first day of life factor VIII activity was higher than in adults. The activity reached the adult level at about the tenth day. The factor VIII associated antigen was found higher than the activity and did not decrease in the first ten days. Studies of the factor VIII associated antigen of newborns by the two-dimensional immunoelectrophoresis showed the same pattern as compared to adults.
Based upon scanty literature we discuss, whether unexpected fatal apneic spells of two full-term, apparently vigorous newborn infants at an age of 3 and 37 hours, respectively, may be related to Pethidine (+ Levallorphane) given to their mothers shortly before birth.
We report on a newborn male infant suffering from anhidrotic ectodermal dysplasia. This x-linked recesive disorder has a high letality during the first year of life. Survivors are psychologically grossly impaired. This necessitates identification of carrier females. Characteristics of heterocygotes (e.g. palmar ridge flattening, paucety of pores, dermoglyphic pattern) are described. In pregnancy amniocentesis and chromosome analysis for sex determination are to be recommended.
A newborn infant with a large hepatic hemangioma developed congestive heart failure, consumption coagulopathy, microangiopathic hemolytic anemia, and obstructive jaundice. The patient was mildly heparinized (250 units per kg and day) and underwent successful resection of the tumor without lobectomy at the age of 3 days. Blood volume increased from 93.9 ml/kg at the age of 5 h to 124.2 ml/kg prior to surgery. Red-cell mass simultaneously decreased from 53.8 to 39.4 ml/kg. The increase of blood volume is explained by congestive heart failure, the decrease of red-cell mass by intravascular coagulation within the tumor resulting in formation of thrombi and microangiopathic hemolytic anemia. A review of the literature on infants with symptoms caused by an intrahepatic hemangioma during the first month of life confirms that surgical intervention is the treatment of choice for infants with giant solitary hemangioma of the liver.

In 107 infants aged 3 hours to 9 months, hemoglobin concentration, 2,3-DPG concentration and GOT activity in red cells, and reticulocyte count were determined in order to evaluate whether the previously observed decrease in red cell 2,3-DPG [1] during the first 30-40 days of life related to red cell ageing. A significant positive correlation of 2,3-DPG vs. GOT, an indicator of red cell age, is found. The decrease in 2,3-DPG over a period of 25-30 days is finally followed by a sharp rise in reticulocyte count, suggesting that an alteration in blood oxygen affinity might trigger the onset of erythropoiesis in normal infants in this age group.
