Effect on birth weight of smoking in pregnancy.

Maternal cigarette consumption during pregnancy, and infant birth weight were assessed in 427 consecutive births at BMH Rinteln. Smokers comprised 46.1% of the mothers. Babies born to mothers who smoked were significantly lighter than those born to non-smoking mothers, regardless of birth order. Smoking mothers were disproportionately represented in the low birth weight group. (19 out of 24) There are clear implications for military obstetric and paediatric practice.

Generalization and maintenance of treatment gains of behaviorally handicapped students from resource rooms to regular classrooms using self-evaluation procedures.

This study provided six behaviorally handicapped elementary school students with a short-term resource treatment to bring their behavior under the control of a combination of treatment procedures emphasizing self-evaluation. Once acceptable levels of appropriate behavior were maintained with only minimal external reinforcement and students were accurately self-evaluating their own work and behavior, generalization and maintenance of behavior gains were sought by introducing a reduced form of the self-evaluation procedures in the students' regular classrooms. A multiple baseline across pairs of subjects design was used to examine individual student's behavior. Analysis of the results of the study indicated that students transferred and maintained high levels of appropriate classroom behavior in their regular classrooms, once self-evaluation procedures were extended into those settings. For four of the six students, all extra-training components were faded. Only two students required a modified form of the original intervention to maintain behavior gains in their regular classrooms.

Acute pancreatitis in pregnancy: two case reports.

Two cases of acute pancreatitis in pregnancy with apparently different etiology are reported and the literature is reviewed. In 1 case, continued fetal growth during a period of hyperalimentation is shown. Although acute pancreatitis is a rare complication of pregnancy, there is evidence that both maternal mortality and fetal loss should be low. The main risk to the fetus is from the effects of premature birth. Experimental evidence suggests that pregnancy may be an etiologic factor, but the condition is probably not more common in pregnancy. Although termination of pregnancy may cause prompt regression of symptoms, it should not normally be necessary. Indications for surgical intervention are not affected by the pregnancy, nor are indications for mode of delivery affected by the pancreatitis.

Standardized classroom management program: Social validation and replication studies in Utah and Oregon.

A comprehensive validation study was conducted of the Program for Academic Survival Skills (PASS), a consultant-based, teacher-mediated program for student classroom behavior. The study addressed questions related to: (a) brief consultant training, (b) subsequent teacher training by consultants using PASS manuals, (c) contrasts between PASS experimental teachers and students and equivalent controls on measures of teacher management skills, student classroom behavior, teacher ratings of student problem behaviors, and academic achievement, (d) reported satisfaction of participants, and (e) replication of effects across two separate school sites. Results indicated that in both sites significant effects were noted in favor of the PASS experimental group for (a) teacher approval, (b) student appropriate classroom behavior, and (c) four categories of student inappropriate behavior. Program satisfaction ratings of students, teachers, and consultants were uniformly positive, and continued use of the program was reported a year later. Discussion focused upon issues of cost-effectiveness, differential site effects, and the relationship between appropriate classroom behavior and academic achievement.

Tissue uptake of isotopically labeled aprotinin in early myocardial ischemia.

The tissue uptake of 3H-aprotinin was studied in anesthetized cats during acute myocardial ischemia (MI) 1 an 2 hr after injection of the tracer. Several tissues exhibited a rapid uptake of the protease inhibitor. Kidney, lung and liver demonstrated the greatest uptake with tissue/plasma ratios of 1.6 to 4.8. Spleen, adrenals, intestine, heart and pancreas exhibited tissue/plasma ratios of 0.28 to 0.58, whereas abdominal aorta, skeletal muscle and omentum had tissue/plasma ratios below 0.17. The rate of clearance of 3H-aprotinin from cat plasma was unaltered by myocardial ischemia. Although ischemic myocardial tissue took up less aprotinin than the non-ischemic myocardial tissue in the same hearts, ischemic tissue accumulated significant amounts of aprotinin relative to the non-ischemic region (82%). These data show that aprotinin reaches ischemic myocardial tissue during the first 2 hr of acute ischemia and would be available to antagonize some of the proteases which may be liberated during these critical early hours of the ischemic process.

Localization of glucocorticoid uptake in normal and ischemic myocardial tissue of isolated perfused cat hearts.

We studied the uptake of labeled dexamethasone (3H-Dex) or methylprednisolone (3H-MP) in isolated perfused cat hearts during the first hour of acute myocardial ischemia. Considerable amounts of 3H-Dex and 3H-MP were taken up by the plasma membrane (F1) fraction in control, border zone, and ischemic myocardial tissue. Lesser amounts were incorporated into the remaining cell fractions. A gradient of glucocorticoid uptake was observed that decreased from control tissue to ischemic tissue in all subcellular fractions (i.e., F1 to F5). Accordingly, supernatant fraction (S) to particulate (P) ratios of labeled glucocorticoid uptake increased from control to ischemic tissue, indicating that myocardial cell damage resulted in a decrease in glucocorticoid-binding capacity in subcellular fractions obtained from ischemic tissue. The activity of 5'-nucleotidase (5'ND), a plasma membrane marker in myocardial cells, also decreased from normal to ischemic tissue. Furthermore, we found that uptake of 3H-MP and 3H-Dex was associated with the retention of 5'ND activity in F1 fractions of both border zone and ischemic tissue. Similar protection of plasma membrane integritg occurred in the supernatant fraction as determined by changes in S/P ratios of 5'ND activity. These data provide support for the concepts that (1) plasma membrane changes occur soon after acute myocardial ischemia, and (2) the mechanism by which glucocorticoids exert a protective effect in myocardial ischemia may be related to membrane stabilization.

Wegener's granulomatosis upper respiratory tract and pulmonary radiographic manifestations in 30 cases with pathogenetic consideration.

Although the radiographic manifestations and pathologic features of Wegener's granulomatosis have been well described, their relationship to two variants of anti-neutrophil cytoplasmic antibodies (c- and p-ANCA) remains unclear. The purposes of this study are to analyze the radiographic and pathologic findings in 30 patients with Wegener's granulomatosis and to correlate c- and p-ANCA with such findings. Major histologic findings included parenchymal necrosis, necrotizing vasculitis, and granulomatous inflammations. Nodular lesions were more often associated with arteritis whereas patchy fluffy lesions associated with hemorrhagic capillaritis. ANCAs, particularly c-ANCA, instead of p-ANCA, were present in a high percentage of patients tested. These findings suggest that c-ANCA could play an important pathogenetic role in Wegener's granulomatosis which showed dual pathologic processes: vasculitis and granulomatous inflammation manifested radiographically as either nodular or pneumonia-like lesions, whereas p-ANCA was more often associated with patchy fluffy lung lesions as the consequence of hemorrhagic capillaritis.
