Treatment of caroticocavernous fistulae with detachable balloons introduced by percutaneous catheterization.

Five patients with caroticocavernous fistula were treated by new, interventional angiographic techniques. In four patients, latex balloons filled with a silicone polymer were introduced into the cavernous sinus via the transfemoral route and detached; this resulted in complete obliteration of the caroticocavernous fistula and preservation of the internal carotid artery flow in two of the four patients. In the fifth patient, stainless steel coils were introduced into the cavernous sinus via the superior ophthalmic vein which had previously been surgically exposed. These percutaneous, angiographic techniques of intervention should be considered the initial treatment of choice for caroticocavernous fistulae.

Management of carotid-cavernous fistulas by surgery combined with interventional radiology. Report of two cases.

Two cases of carotid-cavernous fistulas were successfully treated by standard interventional radiology techniques after otherwise inaccessible vessels were surgically exposed. In the first case, an internal carotid artery (ICA), which had previously been ligated as part of an attempted surgical "entrapment" procedure, was recanalized to permit passage of a detachable balloon catheter to the fistula, resulting in its obliteration. In the second case, an enlarged superior ophthalmic vein was exposed and isolated to facilitate retrograde catheterization of the cavernous sinus and obliteration of a dural fistula between the ICA and the cavernous sinus by steel Gianturco coils. The methods and complications of both procedures are discussed.

Balloon dilatation in the treatment of early intermittent claudication.

Balloon dilation of atherosclerotic arteries is an established procedure with a high success rate and low morbidity. In the past 18 months, 71 patients underwent balloon dilatation at the Royal Melbourne Hospital, with a success rate of 90%. This technique is most successful in short stenoses or occlusions, and earlier investigation of selected patients with intermittent claudication is recommended.

Direct measurement of chlormethiazole extraction by liver, lung and kidney in man.

1 Chlormethiazole was used as a basal sedative for patients undergoing angiographic procedures. 2 Blood samples were drawn opportunistically to examine chlormethiazole extraction across liver, lungs and kidney. 3 Extraction across liver was typically 70-80% and apparently unrelated to input concentrations. Evidence for extraction across lung and kidney was inconclusive but these could each be approximately 20%. 4 Pharmacokinetics of chlormethiazole derived from compartment models were in accord with previous reports and were characterised by a high total body clearance (1-1.5 l/min). 5 Postural changes associated with the radiological procedures caused fluctuating blood concentrations which appear as noise in curve fitting procedures. 6 Pharmacokinetic properties derived from compartment theory cannot cope with these perturbations because of the restriction imposed by time averaging (i.e. mean clearances, half-lives and volumes are produced). Systematic studies of pharmacokinetic properties of perfusion-limited drugs such as chlormethiazole must be developed in such a way as to allow for independent variation of flow and extraction.

A needle biopsy would be quickest.

A case of a benign intrathoracic thyroid adenoma which was biopsied percutaneously is described. Except for a routine posterior-anterior (PA) and lateral chest X-ray examination, no preliminary investigations were performed. This resulted in several unnecessary procedures.

Percutaneous lymph node aspiration biopsy: experience with a new technique.

Experience with post-lymphographic percutaneous fine needle aspiration lymph node biopsy is described in 13 patients with suspected metastatic malignant disease. All aspirations were performed using an anterior transabdominal approach under local anaesthesia with fluoroscopic guidance. In most patients more than one lymph node was biopsied. Four patients with confirmed metastatic disease had a positive biopsy in the appropriate lymph node. Seven patients with negative biopsies had the absence of metastatic disease confirmed by surgery or follow-up roentgenograms. One patient with a negative biopsy was lost to follow-up. There were two patients with negative biopsies in whom representative lymphoid tissue was not aspirated. No evidence of metastasis was found in these patients at surgery. No serious complications were encountered.

Percutaneous nephropyelostomy in the management of obstructed renal transplants.

Five percutaneous nephropyelostomies were performed on 4 patients with obstructed renal allografts. In all instances the procedure significantly influenced further management of these patients. It allowed pressure measurement within the obstructed collecting system, assessment of renal function after prolonged drainage, and anatomical delineation of the site of the obstruction using antegrade pyelography. Percutaneous pyelonephrostomy and drainage is a useful means to determine whether obstruction or rejection is the major contributor to renal failure following renal transplantation.

Ventricular fibrillation from diatrizoate with and without chelating agents.

The toxicity of Renografin 76% was compared with that of Hypaque 76% by selective injection of each into the right coronary artery of dogs. Renografin contains the chelating agents sodium citrate and disodium edetate, while Hypaque contains calcium disodium edetate and no sodium citrate. Ventricular fibrillation occurred significantly more often with Renografin, suggesting that chelating agents contribute to toxicity in coronary angiography.

Hemorrhagic tension cyst of the kidney.

A case of a benign renal cyst, which was filled with blood under pressure and causing acute flank pain, is presented. The pain was relieved after cyst puncture and aspiration. Although benign hemorrhagic renal cysts are well recognized, this is the first report of a benign renal cyst filled with blood under pressure.

The reliability of echocardiography in the diagnosis of infective endocarditis.

The echocardiograms of 17 patients with proved infective endocarditis were surveyed to assess the reliability of echocardiography in this disease. Vegetations were present on 20 valves (11 aortic, 7 mitral, 2 tricuspid) at surgery or autopsy. Echocardiography correctly identified the abnormality in 11 valves. There were 8 valves with false-negative studies of which only one examination was technically inadequate. One false-positive echocardiogram was found. A negative echocardiogram does not exclude infective endocarditis. The size of the vegetation is not the only consideration in its detection echocardiographically. False-positive and false negative studies are discussed.

Evaluation of a new transthoracic needle for biopsy of benign and malignant lung lesions.

The results of 88 transthoracic needle biopsies of lung lesions using the Rotex biopsy instrument are analyzed. Of 57 malignant lung lesions, 55 were diagnosed correctly. Two patients with false negative biopsies had technically inadequate examinations. All but one of the 28 benign lung lesions were correctly diagnosed as nonmalignant. One false positive diagnosis of malignancy was made. The occurrence of complications was similar to those reported for conventional fine needle aspiration. The high accuracy rate is attributed to the effectiveness of the Rotex instrument in sampling the lesions and to the use of biplane fluoroscopy.
