Cytologic findings in microglandular hyperplasia of the cervix.

A case of florid microglandular hyperplasia of the uterine cervix that contained cells with severe atypia suggesting malignancy on a Papanicolaou smear is reviewed. Findings from 25 additional smears from patients with microglandular hyperplasia are reported. These sometimes contained glandular cells with varying degrees of atypia, but demonstrated no features specific for microglandular hyperplasia. The distinction between markedly atypical endocervical cells sometimes exfoliated from microglandular hyperplasia and those of adenocarcinoma is discussed.

Primary clear cell carcinoma of the peritoneum.

A case of primary clear cell carcinoma of the pelvic and abdominal peritoneum which occurred in a 67-year-old woman and with histological characteristics of Mullerian derivation is presented. To our knowledge, this is the first report of such a case. Although clear cell carcinomas have been previously described in peritoneal or retroperitoneal locations, these have been mass lesions thought to arise from endometriosis. All other cases of diffuse primary peritoneal adenocarcinomas have been of the serous type.

Error in histologic dating of secretory endometrium: variance component analysis.

OBJECTIVE: To characterize the extent and sources of imprecision in histologic dating of the endometrial biopsy. DESIGN: Duplicate endometrial biopsies from 25 women were dated by five evaluators on two separate occasions to evaluate the overall precision of the measure. Using variance component analysis, estimates of intrauterine, intraevaluator, and interevaluator variability were determined. SETTING: Samples were obtained during outpatient fertility testing. Evaluators were colleagues at the same institution. PATIENTS, PARTICIPANTS: Women presenting with infertility undergoing routine evaluation. INTERVENTIONS: None. MAIN OUTCOME MEASURE: Variability in histologic dating of the endometrium. RESULTS: Inconsistencies between evaluators accounted for 65% of the observed variability, whereas 27% was because of inconsistencies in duplicate readings by the same evaluator. Regional differences in the uterus accounted for only 8% of the total variability. CONCLUSIONS: The overall error from these sources have the potential to result in a substantial false-positive rate for diagnosis of luteal phase defect.

Recurrence in noninvasive endometrial carcinoma. Relationship to uterine papillary serous carcinoma.

In a series of 227 consecutive operable clinical stage I endometrial carcinomas, there were 28 recurrences. In seven of these cases, no myometrial or vascular invasion was demonstrable. Recurrence in these seven noninvasive cases was strongly associated with papillary serous carcinoma, even when present only focally or manifested by typical cytological features in the absence of well-formed papillae. An association with foci of clear cell carcinoma was also seen, as was direct evidence in two cases of synchronous ovarian serous carcinoma, one in situ.

State-of-the-art digital radiography.

Technologic advances in digital radiography have improved the ways in which radiographic images are acquired, displayed, transmitted, recorded, and archived. With computed radiography, performed with storage phosphor plates and interactive high-resolution workstations, radiation dose is reduced and repeat exposures necessitated due to technical errors are eliminated. Digital fluorography allows reductions in dose, procedure time, and film costs. These digital imaging modalities have been well accepted clinically and are equal in diagnostic accuracy to conventional methods. Teleradiology has advanced with the development of laser film digitization, fiberoptic networks, and dial-up circuit switching technology. Laser film printers yield improved hard copies of transmitted images, but further work is needed to faithfully reproduce the images displayed on high-resolution work-stations. Although the capacity for archiving digital image data has increased (260,000 examinations or 23,500 Gbytes can be stored in a six-unit optical disc library), higher capacity storage media are needed. Further technologic advances in the speed of image transmission and storage capacity are anticipated.

[Natural killer cell activity in Naegleria fowleri infected mice].

The natural killer (NK) cell activity of splenocytes and recycling capacity of NK cells were observed by combining the 51Cr-release cytotoxicity assay and single cell cytotoxicity assay against YAC-1. The ICR mice were infected intranasally with Naegleria fowleri, that is a pathogenic free-living amoeba. The mice infected with 1 x 10(5) trophozoites showed mortality rate of 76.7% and mean survival time of 12.9 days. The cytotoxic activity of NK cells in infected mice was significantly higher than that of non-infected mice during the period between 12 hours and day 3 after infection, and highest on day 1. The target-binding capacity of NK cells in infected mice was not different from that of non-infected ones. Maximal killing potential and maximal recycling capacity were remarkably increased in infected mice as compared with the control. The results obtained in this observation indicated that elevated NK cell activity in mice infected with N. fowleri was not due to target-binding capacity of NK cells but due to the increased activity of NK cells and increased recycling capacity of individual NK cells.

Epidemiological studies of digenetic trematodes in Yongyang County, Kyungpook Province.

The present study was undertaken to determine the endemicity of trematode infections in Yongyang County, Kyungpook Province, Korea. One hundred and seventy-seven out of 955 residents examined were found to be infected with one or more kinds of helminths. The prevalence rate was 18.5%. Among them, 70 were found to be infected with Clonorchis sinensis, 74 with Metagonimus sp. and 24 with both flukes. The eggs of Fasciolidae were demonstrated from two specimens. The prevalence rate of clonorchiasis in males was 12.6%, while that in females was 3.6% (p less than 0.05). The prevalence of metagonimiasis in males was 12.0% and was also significantly higher than 6.1% in females (p less than 0.05). The intensity of both infections was significantly heavier in males than in females. The prevalence of both flukes was higher in residents of over 30 years of age than in those below that age. No correlation was found between the intensity and the age group. Among eight species of the fresh-water fish collected at the Panbyon River, Zacco temmincki was found to be the most frequently and heavily infested with metacercariae of Metagonimus sp., and Gnathopogon atromaculatus alone was found to be infested with metacercariae of Clonorchis sinensis. The present results suggest that Yongyang area still remains endemic with Clonorchis sinensis and Metagonimus sp. infection.

Modification of human placental alkaline phosphatase by periodate-oxidized 1,N6-ethenoadenosine monophosphate.

Oxidation of 1,N6-ethenoadenosine monophosphate (epsilon AMP) with periodate cleaved the cis-diol of the ribose ring and resulted in the formation of a dialdehyde derivative (epsilon AMP-dial). At room temperature epsilon AMP-dial was unstable and underwent beta-elimination to give 4',5'-anhydro-1,N6-ethenoadenosine dialdehyde acetal (A epsilon Ado-dial). These nucleotide analogues were found to inactivate human placental alkaline phosphatase in a time- and concentration-dependent manner. epsilon AMP-dial was shown to be an affinity label for the enzyme on the basis of the following criteria. (a) Kinetics of the enzyme activity loss over a wide range of epsilon AMP-dial concentration showed a saturating phenomenon. Removal of the phosphate group made the reagent (A epsilon Ado-dial) become a general chemical modifying reagent. (b) The artificial substrate p-nitrophenyl phosphate gave substantial protection of the enzyme against inactivation. (c) epsilon AMP-dial was a substrate and a partial mixed-type inhibitor for the enzyme. Results of the inhibition and protection studies indicated that the reagent and substrate could combine with the enzyme simultaneously. Besides the phosphate-binding domain, an induced hydrophobic region is proposed for the substrate-binding site for human placental alkaline phosphatase.

Management of epithelial ovarian neoplasms using a platinum-based regimen: a 10-year experience.

One-hundred and twenty-four patients with primary advanced (n = 103) and recurrent (n = 21) ovarian carcinoma completed a course of platinum-based chemotherapy (cyclophosphamide/doxorubicin/cisplatin or cyclophosphamide/cisplatin) or developed progressive disease while on therapy and were evaluated. All patients were treated between August 1, 1977 and December 31, 1987. The 5-year survival for patients with primary disease was 27% for stage III (n = 73) and 7% for stage IV (n = 30). The 5-year survival based on residual disease was 91% for microscopic disease (n = 13), 24% for disease less than 2 cm (n = 27), and 8% for disease greater than or equal to 2 cm (n = 64). The 5-year survival for the patients treated with recurrent disease was 5% (n = 21). Borderline tumors have been excluded. Long-term toxicity, including cardiac toxicity, renal toxicity, and a 5% incidence of second primary tumors, is evaluated.

Oxidant effects on rat and human lung proteinase inhibitors.

This project tested the hypothesis that inhaled oxidants could cause lung damage by inactivating the proteinase inhibitors that normally protect the lung from proteolysis. Rat alpha-1-proteinase inhibitor (alpha 1-PI)2 was purified from blood plasma, and antibodies to this inhibitor were prepared. The activity of alpha 1-PI in lung lavage fluids from rats was measured by elastase inhibition, and the immunological concentration of alpha 1-PI was quantified in an enzyme-linked immunoassay. The ratio of the amount of active alpha 1-PI relative to its immunological concentration was examined as a measure of the inhibitor's functional activity. This ratio and the ratio of the immunological concentration of alpha 1-PI to the total protein concentration were determined in lung lavage fluids from rats exposed to air, 10 parts per million (ppm) nitrogen dioxide, and diesel emissions (3.5 mg/m3 particulates) for 12, 18, and 24 months. Only diesel exposures resulted in a statistically significant reduction in the functional activity of alpha 1-PI of 30 percent (p less than 0.05). Similar studies were performed on rats exposed to nitrogen dioxide (0.5 ppm background with peaks of 1.5 ppm) and ozone (0.06 ppm background with peaks of 0.25 ppm) for 12 and 18 months. No statistically significant effects were observed in the functional activity of alpha 1-PI or its immunological concentration. In other protocols, rats were acutely exposed to 0.8 ppm or 1.2 ppm ozone for two, four, or eight hours, and to 0.5 ppm or 0.8 ppm ozone in conjunction with 8 percent carbon dioxide for two or seven hours. Although these acute exposure conditions did not reduce the functional activity of alpha 1-PI, the immunological concentration of alpha 1-PI and the elastase inhibitory activity, relative to other proteins, were significantly increased in relation to the total amount of ozone inhaled. The functional activity of alpha 1-PI also was measured in the bronchoalveolar lavage fluids of human subjects exposed to nitrogen dioxide (0.05 ppm with 2 ppm peaks, or to 1.5 ppm continuously) for three hours and to ozone (0.4 ppm) for two hours during exercise. These exposures did not result in significant changes in the functional activity of alpha 1-PI or its immunological concentration.(ABSTRACT TRUNCATED AT 400 WORDS)

Evidence of a new metabolic pathway of 5-fluorouracil in Escherichia coli from in vivo 19F-NMR spectroscopy.

Two distinct metabolic pathways of 5-fluorouracil are proposed in Escherichia coli. The first metabolic pathway is a reductive degradation with the formation of dihydrofluorouracil as the first metabolite. The second metabolic pathway is shown to be a hydroxylating degradation, possibly with the formation of 5-hydro-6-hydroxy-5-fluorouracil as the first metabolite. The metabolites of both pathways undergo subsequent hydrolytic degradation with fluoride ion as the common final product. The chemical structures of these metabolites were partially identified by 19F-NMR. The results show a close resemblance between these two metabolic pathways with in vivo pyrimidine biodegradation. The reductive degradation has been proposed by several laboratories, whereas the hydroxy degradation has not been reported before. Both the reductive and hydroxy pathways are demonstrated in this report, to be independent reactions.

Digital venography of the lower extremity.

A digital 33-cm fluoroscopic system equipped with conventional spot-film and digital or video hard-copy capabilities was evaluated for its usefulness in diagnosing deep venous thrombosis of the lower extremities. The impact of different fluoroscopic field sizes, spatial resolution, and contrast variance was measured by using phantoms. The results indicate that the physical characteristics of the digital system are acceptable for lower-extremity venography. Digital fluoroscopic hard copy was compared with conventional spot films in 22 examinations. The digital examinations were as accurate as the conventional examinations. Procedure time, exposure to radiation, film costs, and repeated injections of contrast medium because of errors in exposure were reduced with the digital method. The need for extra technologists was eliminated. The advantages of digital radiographic displays, postprocessing, storage, and transmission were maximized. Digital fluoroscopic examinations were as accurate as conventional spot films and were found to have many advantages.

Hardcopy recordings of analog and digital images.

The increase in the use of various imaging modalities demands higher quality and more efficacious analog image recordings. Laser electronic recordings with digital array prints of 4K X 5K X 12 bits using laser-sensitive film or paper are being evaluated. Dry silver paper recordings are being improved and evaluated. High resolution paper dot printers are being studied to determine their gray scale capabilities. The authors have conducted a study to evaluate the image quality, costs, clinical use, and acceptability of CT, MRI, DSA, digital radiography, and radionuclide images recorded by the seven different printers (three laser, three silver paper, and one dot) in comparison with conventional film recording. This paper describes the technical developments and instrumentation of digital laser film and analog paper recorders and presents the results of our study.

Effect of superoxide dismutase plus catalase on Ca2+ transport in ischemic and reperfused skeletal muscle.

Cytotoxic oxygen metabolites may contribute to skeletal muscle damage associated with ischemia and reperfusion. This study utilized a rat hindlimb ischemia model to investigate the effect of pretreatment with oxygen free radical scavengers superoxide dismutase (SOD) and catalase (CAT) on skeletal muscle Ca2+ uptake by sarcoplasmic reticulum (SR) in limbs subjected to periods of ischemia and reperfusion. SOD and CAT were conjugated to polyethylene glycol to prolong their half lives. Anesthetized rats (ca. 350 g) received an iv injection of either conjugated SOD (2 mg/kg) plus CAT (3.5 mg/kg) (n = 6, Treated Group) or 0.9 saline (4 ml/kg) (n = 6, Control Group) 5 min before unilateral hindlimb tourniquet ischemia of 3 hr duration. After 19 hr of reperfusion, muscle from each lower leg was excised and homogenized. Skeletal muscle SR was isolated by differential centrifugation. ATP-dependent Ca2+ uptake by the SR was then measured with dual wavelength spectrophotometry and used as an index of muscle function. Pretreatment with SOD and CAT maintained higher rates of Ca2+ uptake by SR of skeletal muscle from postischemic reperfused limbs (Treated Group 2.29 +/- 0.21 vs Control Group, 1.61 +/- 0.06 mumole Ca2+/mg protein/min). These results implicate cytotoxic oxygen metabolites in the pathogenesis of ischemic reperfusion skeletal muscle injury.
