Foscarnet 5 versus 7 days a week treatment for severe gastrointestinal CMV disease in HIV-infected patients.

In a randomized open trial foscarnet 90 mg/kg b.i.d. 5 days for 3 weeks was compared to 90 mg/kg b.i.d. daily in severe gastrointestinal cytomegalovirus disease in HIV-infected patients. Thirty-eight patients were randomized, 36 were evaluable (all male, age 24-54 years, median 40 years; CD4/microliter 0-150, median 10). Treatment efficacy was evaluated based on a score consisting of symptoms, endoscopic and histologic examination. In the 5-day treatment group 10/16 (62%) patients responded to treatment, in the 7-day treatment group 13/20 (65%), with symptoms resolving in most patients after 1 week. Side effects and adverse events were seen in 13 patients in the 5-day treatment group and in 15 patients in the 7-day treatment group. Laboratory abnormalities were common in both groups, in one patient reversible renal insufficiency developed. Efficacy and safety of treatment 5 days a week was comparable to the standard regimen.

Induction of apoptosis in human microvascular endothelial cells by divalent cobalt ions. Evidence for integrin-mediated signaling via the cytoskeleton.

Wound healing following implantation is characterized by an acute inflammatory reaction and a subsequent reorganizing phase in which angiogenesis is involved. Endothelial cells (EC) participate in both inflammation and angiogenesis. Thus, the effects on functions of EC exerted by implanted materials could affect the progression of wound healing. The corrosion of metallic implants can cause high concentrations of heavy metal ions in the peri-implant tissues. The purpose of the present study was to test the effects of possible corrosion products on the function and viability of human EC in vitro. Long-term exposure of EC to CoCl2 and NiCl2 (3 days, 0.7 mM) leads to a decrease of cell number and changes in cellular morphology. However, the morphological changes between CoCl2- and NiCl2-treated cells differ significantly. The changed morphology of CoCl2-treated EC and the fragmented DNA pattern indicates apoptosis. Nickel-treated cells demonstrated necrosis. The activity of integrins was tested by an assay of cellular adhesion on collagen-coated surfaces. It was shown that the number of adherent cells significantly decreased upon exposure to CoCl2. Our studies suggest that induction of cell death in EC upon exposure to CoCl2 could be attributed to impaired integrin signaling, which leads to a damaged cytoskeleton and culminates in apoptosis.

[Ovarian epidermoid cyst and endometrioid carcinoma: do they share their origin?].

A case of a well differentiated endometrioid adenocarcinoma of the ovary in combination with an epidermoid cyst is described. A typical epidermoid cyst without skin appendages could be observed. In direct neighborhood, there was a typical endometrioid carcinoma with some foci of squamous metaplasia. There was no continuum between the squamous metaplasia of the carcinoma and the wall of the epidermoid cyst. In addition, the benign cyst and the carcinoma shared some of their immunohistochemical profile. This case highlights the still unsolved question about the origin of epidermoid cysts and adds to the hypothesis that they arise from pluripotent coelomic epithelium.

Increasing clinical use of pulse oximetry.

Changing the system from measuring blood gases through invasive measures to using noninvasive pulse oximetry is a challenge in the Critical Care Unit where invasive techniques are taken for granted. The authors report a project that was successful in increasing the use of noninvasive monitoring techniques by critical care nurses in a Surgical Intensive Care Unit. This clinical project became an important aspect of incorporating the staff in a change to more extensive use of pulse oximetry. This study defines the change in nursing practice with the use of pulse oximetry. The authors discuss three areas: (1) demonstration of the correlation between O2Sat as measured by the pulse oximeter and arterial blood gas saturations; (2) introduction of the pulse oximeter as a reliable alternative to ABGs when monitoring oxygenation; and (3) the establishment of guidelines for using pulse oximetry within the clinical setting.

Radiographic evaluation of pulmonary fibrosis following mantle field irradiation in Hodgkin's disease.

Sixty-nine patients given mantle field radiation therapy for Hodgkin's disease and considered relapse-free and possibly cured were re-examined for pulmonary fibrosis as observed on antero-posterior and lateral chest radiographs. A method is described for the systematic evaluation of these late stage changes, which are observed following irradiation, in a numerical form. The method gave reproducible results in this series, the correlation coefficient being 0.72/0.81 and 0.73 respectively for the intra-observer and the inter-observer variations. The method thus may facilitate comparison between different patient materials.

Sources of attrition in a church-based exercise program for older African-Americans.

Among 123 older African Americans recruited into a church-based exercise program, 43% had dropped out within four months. Compared to those who did not drop out, drop outs had lower levels of education, energy to do activities, energy to exercise, and self-ratings of health, all based on measures taken before the class. Over half of those who dropped out cited non-exercise related health problems, and 17% caregiver responsibilities. Of those who dropped out, half said they would continue to exercise and 32% said they intended to start within the next six months.

The correlation of c-erbB-2 oncoprotein and established prognostic factors in human breast cancer.

BACKGROUND: The c-erbB-2 (HER2/neu) receptor is a transmembrane phosphoglycoprotein associated with multiple signal transduction pathways. Its overexpression in breast cancer tissue has been correlated with poor prognosis. We report preliminary data of an ongoing study in invasive breast cancer patients exploring c-erbB-2 protein overexpression in relation to established tumor characteristics of prognostic value. MATERIALS AND METHODS: In primary breast carcinoma samples from 115 women undergoing surgery in our department in 1999, a polyclonal rabbit antibody to human c-erbB-2 oncoprotein was used for immunohistochemical assessment of the c-erbB-2 expression in formalin-fixed paraffin-embedded material. The data were statistically correlated with classical histopathological parameters. RESULTS: In the studied collective of mainly postmenopausal women (75%) with a high rate of early stage breast cancer (88% pT1 + 2), there was no significant relation between c-erbB-2 overexpression, classified as positive in 42% of the samples, and lymph node involvement, tumor size and grade, or hormone receptor status. CONCLUSION: Using the presented highly sensitive method, no association between c-erbB-2 expression and established prognostic factors was found. These data are in line with reports that the value of HER2/neu determination is not fully clarified for the preadjuvant evaluation of newly diagnosed breast cancer patients.

Feline mammary carcinomas as a model for human breast cancer. I. Sensitivity of mammary tumor cells in culture to cytostatic drugs. A preliminary investigation of a predictive test.

Feline mammary carcinomas were found to maintain well in short-term cultures. Principally the same types of nuclear DNA frequency distribution histograms were recognized in feline mammary carcinomas as in human mammary carcinomas. However, the more abnormal histograms are less frequent in feline than in human mammary carcinomas. Feline mammary carcinomas appeared, at least in vitro, most sensitive to Doxorubicin and 5-fluorouracil. Preliminary, thymidine incorporation studies indicate both cytotoxic and cytostatic effects of Doxorubicin and 5-FU. Methotrexate was found to stimulate thymidine incorporation.
