Systemic lupus erythematosus in Indian children.

Twenty cases of systemic lupus erythematosus (SLE) in prepubertal children (less than 14 years of age) were seen over a period of 14 years. The male:female ratio was 1:2.3, and the mean age at onset was 9.37 years. Fever with joint involvement was the commonest presenting manifestation (60%), followed by nephrotic syndrome (25%). Notable clinical features included a high incidence of renal involvement (75%), significant hypertension (45%) and reversibility of acute renal failure (2 cases). The other organs and systems involved included: mucocutaneous manifestations (60%), cardiovascular system (30%), respiratory system (25%), neuropsychiatric manifestations (45%), and anemia (75%). Raynaud's phenomenon and thrombocytopenia were rare while leucopenia was not seen in a single patient. Immunological abnormalities noted were 100% positivity for antinuclear antibodies, and 87.5 and 75% positivity for antibodies to double-stranded and single-stranded DNA, respectively. Hypocomplementemia was seen in 75% of patients tested.

Maternal-neonatal serum vitamin A concentrations.

Prevention of neonatal vitamin A deficiency is related to the adequacy of maternal vitamin A stores. In this study we investigated maternal and cord serum vitamin A and retinol-binding protein (RBP) values in an Indian population including, for the first time, clinically vitamin A-deficient mothers. Twenty-eight maternal-neonatal pairs were selected from maternal cohorts of high socioeconomic status without clinical evidence of vitamin A deficiency (group I) and low socioeconomic status with conjunctival xerosis and Bitot's spots (group II). Maternal education, caloric and vitamin A intakes, weight, height, hemoglobin, and birth weight were significantly lower in group II. Serum vitamin A levels were significantly higher in group I mothers and newborns as were RBP levels in group I mothers. However, a significant difference between groups I and II in cord blood RBP was not observed. Upon correlation of maternal vitamin A levels with cord blood vitamin A levels, a logarithmic relationship was revealed, suggesting saturable transplacental transport of vitamin A.

Immunoregulatory treatment for minimal change nephrotic syndrome.

Immunological studies were performed in 18 children with minimal change nephrotic syndrome proved by biopsy examination during relapse and in 15 age matched controls. All 18 children showed dysfunction of cell mediated immunity as evidenced by low absolute lymphocyte count, low blastogenesis index in response to phytohaemagglutinin stimulation, and reduced skin sensitivity to dinitrochlorobenzene when compared with controls. All 18 patients had low serum IgG concentrations, while the IgA, IgM, and C3 concentrations in the serum were within normal limits. Based on the evidence of depressed cell mediated immunity, 14 patients with nephrotic syndrome were treated with an immunoregulatory drug l-tetramisole (levamisole) for a period of 20-24 weeks. Six patients went into complete remission within 4-20 weeks of treatment, a further six patients went into partial remission, while two did not respond. On follow up (six to 24 months after stopping levamisole), of the six patients who achieved complete remission, four continued to maintain the state and two relapsed after roughly six months. Of the six patients who achieved partial remission, two went into complete remission, two continued to be in partial remission, and two relapsed.
