Severe thrombocytopenia in extremely low birthweight infants with systemic candidiasis.

The clinical, haematological and microbiological features of 12 extremely low birthweight (ELBW) infants with Candida infection were compared with 36 gestational-age and birthweight matched controls who experienced 42 episodes of bacterial septicaemia. Thrombocytopenia was the feature observed most commonly, occurring in 12 of 12 study infants and 23 of 42 controls (P = 0.004). Minimum platelet counts were below 50 x 10(9)/L in 11 of 12 study patients compared with seven of 42 controls (P < 0.0001). Severe thrombocytopenia may be a useful indicator of systemic Candida infection in ELBW infants.

Interleukin-1 and experimental gastric ulcer healing in the rat.

The involvement of inflammation in peptic ulcer development and healing attracts growing interest. Since lymphokines, in particular interleukin-1 (IL-1), as ubiquitous mediators of inflammation are currently intensively studied in the gastrointestinal tract, we assessed the effect of this cytokine as well as that of a specific IL-1 release inhibitor (IX 207-887 (IX)) on development and healing of experimental gastric ulcers. After a single dose of IL-1, 4 micrograms/kg, i. p., basal acid secretion was almost completely inhibited for 4 hours in conscious chronic gastric fistula rats. In a first study, following induction of a 7 mm wide cryo-ulcer in the gastric corpus, three groups of 24 rats were treated either with a non-acid inhibitory dose of IL-1 (0.4 microgram/kg) or with an antisecretory regimen (4 micrograms/kg) b.i.d. or saline control. Ulcer size did not differ from that of control animals, neither after 24h nor 7 days. Similarly, IX applied daily (20 mg/kg/s.c) from 5 days before ulcer induction and continued thereafter for 15 days had no effect on ulcer development or healing. Despite its anti-inflammatory property IX produced no macroscopically visible damage on the gastric or intestinal mucosa and may therefore offer a higher safety profile within the gastrointestinal tract than conventional non-steroidal anti-inflammatory drugs.

Inflammation, capillary leakage and healing of experimental gastric ulcers in the rat.

Little information is available on the role of inflammation and small vessel alteration in the development and healing of peptic ulcers. We studied the spontaneous healing of experimental ulcers in the rat stomach with a novel radio-isotope technique for the monitoring of capillary damage in ulcer-related inflammation. Following ulcer induction, healing was assessed on days 1, 7 and 15 by macroscopical and histological determinations of ulcer size and lesion area, parietal cell counts in the ulcer margin and nuclear imaging of the lesion area using Nanocoll, a 99mTc-labelled colloid. The ulcers were re-epithelized after 15 days. However, Nonocoll activity was still significantly enhanced in the ulcer area, and regression of the microscopically visible lesion area, which, from day 1 on, occupied a zone considerably larger than the ulcer itself, was delayed and still incomplete at the end of the study. Compared to the intact mucosa, parietal cells were reduced in the scar tissue up to 15 days (24-40%). However, despite macroscopical and histological re-epithelization, small vessel leakage and substantial tissue alteration persisted in the ulcer scar.

Detection of multiple PRL- and GH-like proteins in human pituitary by Western blot analysis.

Surgically removed normal and tumorous pituitary tissues from a prolactinoma patient were analyzed by Western blot techniques for PRL and GH variants. Criteria for identification were the Rf of the bands within the gel, immunologic crossreactivity to specific antisera, and structural verification by tyrosine peptide-mapping of individual bands from the gel. The authors found the tumor-tissue to be characterized by the presence of a PRL band greater in concentration than in the normal tissue and the virtual absence of a GH band. Immunoblotting of the electrophoretically resolved proteins form both types of tissues revealed several new bands crossreactive with human PRL and GH antibodies. Some of the new bands were of Mr greater than the monomeric PRL and GH and others were of lower Mr. Relative of two of the low mobility Mr PRL-immunoreactive bands designated as 16K and 8K corresponded to the Rf of the two fragments of cleaved PRL, which suggested that cleaved PRL occurred naturally in the human pituitary gland. The most conspicuous of the new PRL-immunoreactive bands, a 25,000 Mr protein migrating slightly behind PRL, displayed strong crossreactivity to hPRL antibodies and was present in greater concentration in the prolactinoma tissue than in the normal tissue. These properties suggested that it was related to hPRL and perhaps represented its glycosylated variant. However, its tyrosine peptide map did not resemble that of hPRL. Thus, it is not clear whether it represented G-hPRL or a new pituitary protein that cross-reacts with hPRL antibodies. In addition, two other bands of low Mr, designated as unknown 1 and unknown 2, reacted with hPRL antibodies. Immunostaining with hGH antibodies revealed the 20K-hGH variant, the F1 fragment of cleaved hGH, and a pair of new bands immediately behind GH that could represent glycosylated hGH--possibly a product of Seeburg's variant hGH gene. Both PRL and GH antibodies elicited numerous bands of high Mr by the technique employed, far more than ever observed by Sephadex chromatography. The nature of the high Mr bands remains unknown. Further characterization of these new PRL- and GH-immunoreactive proteins might help in the understanding of the multiple physiologic functions of PRL and GH in man.

[Experiences with intraoperative radiotherapy in inoperable pancreatic carcinoma].

From January 1983 to April 1985 six patients suffering from unresectable pancreatic cancer underwent intraoperative irradiation therapy (IORT) followed by external radiotherapy. All tumors showed T4-stages except one tumor staged as T2N2. From the onset of symptoms to IORT the median time of hospitalization was 26.5 days. After surgery, treatment (external irradiation included) required a median hospitalization period of 43.5 days. Follow up, complications and the terminal hospital stay of the patients who died lead to another median hospitalization period of 30 days. Altogether hospitalization required an average of 90 days which represented 38.5% of the mean life-expectancy of 234 (SE = 57.4) days. 47.3% of the survival time did not show discomfort, in 31.6% there were reversible and in 21.1% irreversible complaints. Therefore, patients suffering from unresectable pancreatic cancer had to pay a heavy price for prolonging life expectancy by IORT and following percutaneous irradiation.

[The reproducibility of A. G. Lauritzen's registrations].

The reproducibility of the positional relationship of mandible models in the Dentatus Articulator was studied. Based on the method of A.G. Lauritzen, the articulator is oriented on a hinge axis. The registration procedure was carried out by two subjects on three patients (5 times each). The deviations in the hinge axis point against a value of zero (initial measurement) were then determined with a face-bow. TMJ and SAM face-bows were used for these controls. The deviations from the mean value of the hinge axis point determined in the articulator were within the range of +/- 3 mm in the sagittal and vertical directions. The results tend to indicate that experience, routine, and knowledge of the varioius possibilities of error are necessary if the registration is to be accurate.

[Reciprocal in vitro actions of Streptococcus mutans, Actinomyces and Veillonella: a simplified model for carbohydrate metabolism in plaque].

Different acids can be found in vitro, if glucose is metabolized in plaque or by different combinations or oral bacteria: 1. Streptococcus mutans and Actinomyces do not show interactions concerning metabolism and formed acids. 2. Streptococcus mutans and Veillonella after short incubation time produce lactate; after long time of incubation acetic and propionic acid are main products and lactic acid diminishes. 3. Actinomyces and Veillonella react similar to Streptococcus mutans and Veillonella. Succinate produced by Actinomyces is fully utilized by Veillonella.

[Mercury concentrations in blood and urine in persons with and without amalgam fillings].

Using flameless atom-absorption spectroscopy, the mercury concentration in the urine and in the blood of 102 persons was determined in a double-blind experiment. The subjects were divided into four groups: persons with and without amalgam fillings as well as persons with and without occupational contact with mercury and amalgam. Mercury concentrations in the blood and the urine were the same in relation to the presence or absence of amalgam fillings. The individual values varied, depending on the nutrition, and did not correlate with the number of amalgam fillings present in the individual case. Persons with occupational contact with mercury and amalgam excreted slightly more mercury in the urine. Even these values however were far below the upper limits of the range of normalcy.

[Functional disorders in juvenile jaws].

A wide variety of symptoms and findings taken from 172 patients between the ages of 10 and 26, who presented for diagnostic examinations of function, were compared. The amazingly high frequency of functional disturbances appearing in this age group demands special study.

[Uses of the occlusal registration instrument (ORI)].

By means of the occlusal recording instrument (ORI), jaw models can be measured three-dimensionally and filed with respect to axes and skull. In clinical trials evaluated with the ORI, it was shown that the position of the maxilla in the skull can be transferred to the articulator with great accuracy. Suggestions are given for improvements to refine the method of measurement. If the third point of reference is also tattooed, the accuracy of the transference of the face-bow is considerably increased. Under these conditions the apparatus is suitable for checking the accuracy of the transference of the face-bow and for photographic documentation.

[Gnathologic aspects in total prostheses].

Using a combination of the all-oral procedure (M. Hofmann), the Lauritzen technique, and the TMJ system, it is possible to construct dental prostheses geared to the individual patient for individuals whose teeth have all been extracted. This rational procedure was introduced and discussed.

Carbon tetrachloride induced proliferation of tight junctions in the rat liver as revealed by freeze-fracturing.

Application of carbon tetrachloride produced a progressive proliferation of tight junctions in the rat liver. This system proved to be rapid and highly reproducable and affords the opportunity for tracing the fate of tight junctions in freeze-fracture replicas, facilitating investigations on their formation and function. Beginning on day one carbon tetrachloride treatments resulted in the progressive loosening and fragmentation of the junctional meshwork. After three to four days the membrane outside the zonulae occludentes was extensively filled with proliferated discrete junctional elements often forming complex configurations. From the fifth day on the zonulae occludentes were restricted again predominantly around the bile canaliculus margins. But the junctional meshwork of the zonulae occludentes remained loosened in comparison to those in the control rats. It could be shown that tight junction proliferation on the lateral surface of the plasmalemma occurred both through de novo formation from discrete centers of growth by addition of intramembranous particles and through reorganization of preexistent junctional strands of the fragmented zonulae occludentes bodies. Whereas the large gap junctions close associated with the zonulae occludentes remained more or less unaffected during the experiments, small gap junctions increased in number after five days and were located at the margin or in the tight junction domain. It is assumed that the degeneration of the tight junctions served as a pool for intramembranous particles which form the gap junctions. The results of these observations are discussed in relation to those obtained in other systems.
