Myocardial contractility was estimated in 27 patients with hyperthyroidism (10 men and 17 women, aged from 16 to 38) by echocardiography. Echocardiographic examination was performed before and on the 7th day after the subtotal thyroidectomy. Thyroid functions (PBI, T3, and T4) decreased significantly after the operation (p less than 0.01). Heart rate and systolic blood pressure showed a slight decrease (N.S.), while diastolic blood pressure showed a slight increase (N.S.), while diastolic blood pressure showed a slight increase (N.S.). Mean AWVs decreased from 35.8 +/- 7.0 mm/sec to 27.6 +/- 5.7 mm/sec and correlated wtih T3 (r=0.4008, p less than 0.05). Mean PWV's also decreased from 53.2 +/- 12.2 mm/sec to 45.2 +/- 11.4 mm/aec and correlated with T3 (r=0.5237, p less than 0.02). DDR decreased from 108.0 +/- 36.8 mm/sec to 85.7 +/- 31.7 mm/sec and correlated wtih T3 (r=0.4008, p less than 0.05). Above results suggest that enhanced myocardial contractility and elevated rate of ventricular filling in hyperthyroidism are suppressed after the operation, and that echocardiography is useful for the evaluation of the clinical course after the operation.

The fourth heart sound was clearly recorded by using the signal averaging method. Furthermore, the simultaneous recording of the Doppler signal with the fourth heart sound was performed. Thus, it was demonstrated that the Doppler signal started after the P wave and followed by the fourth heart sound. In 2 cases, the effect of the double Master's test and taking a bath was studied on the computer averaged phonocardiogram. After the exercise, the amplitude of the fourth heart sound was increased, whereas after taking a bath it was almost unchanged. The above results show that the simultaneous recordings of a computer averaged phonocardiogram and Doppler signal that is associated with cardiac activity can greatly facilitate the interpretation of small vibrations such as the fourth heart sound.

Cultured fibroblasts derived from rat carrageenin granuloma were treated with bleomycin and the synthesis of hexosamine-containing substances was compared with that in control cells. Four day treatment with o.1 mug bleomycin/ml resulted in a significant increase of the production of these macromolecules by the cells, though DNA synthesis was remarkably inhibited at this dose of bleomycin. The stimulatory effect could be seen as early as the second day of bleomycin treatment, and was enhanced with increasing treatment time. Further fractionation of the hexosamine-containing substances revealed that synthesis of acidic glycosaminoglycans was more sensitive to bleomycin than that of glycoproteins, i.e., acidic glycosaminoglycans increased by 80% and glycoproteins by 53% after four day treatment with 0.1 mug bleomycin/ml. The increased components of acidic glycosaminoglycans included not only hyaluronic acid but also sulphated glycosaminoglycans. Collagen synthesis was increased by 23% by the same dose of bleomycin. N-Acetyl-beta-glucosaminidase, one of the degradation enzymes for acidic glycosaminoglycans released into the cultured medium, was decreased significantly by bleomycin.

Hemodynamic studies (using (131)I-labeled albumin [RISA]) Were performed before and 5 and 42 weeks after the oral administration of pindolol (av. 30 mg/day), oxprenolol (av. 216 mg/day), propranolol (av. 75 mg/day) or bufetolol hydrochloride (av. 30 mg/day) in 40 patients with essential hypertension. Responders to the antihypertensive actions of short-term (5 weeks) pindolol or bufetolol showed a reduction in total peripheral resistance (pindolol, from av. 2622 to 2022 dyne-sec-cm-5-m2; befetolol, from av. 3301 to 2620, p less than 0.05), without significant changes in cardiac index, while hypotensive actions of propranolol or oxprenolol appeared to be due mainly to a decrease in cardiac output (propranolol, from av. 4.03 to 2.99 L/min/m2; oxprenolol, from av. 3.97 to 3.29 L/min/m2), although the decrease in cardiac output was not significant. In long-term (42 weeks) oxprenolol therapy, antihypertensive effects seemed to be related to reduced cardiac output and a readaptation of peripheral resistance to chronic reduction of cardiac output was not always observed. Circulation time was determined in 9 patients with oxprenolol therapy and 8 with pindolol therapy by the measurement of the arrival time in the cerebral hemisphere of the intravenously injected radioisotope. The patients with oxprenolol therapy showed significant prolongation in circulation time (short-term administration, av. 6.6 to 8.4 sec; long-term administration av. 6.6 to 9.2 sec, p less than 0.05), while no prolongation was observed in pindolol therapy. These results suggest that hemodynamic responses to beta-blocking agents are not uniform and that the antihypertensive actions of beta-blockers depend on the effects on both cardiac output and peripheral vascular resistance.

Osteochondroma is one of the most common benign bone tumors, but it is rare in the spine. A woman aged 54 was admitted for investigation of left nuchal pain with radiation to the occipital region for one and a half years duration. Physical and neurological examinations revealed no objective abnormalities. The cervical spine film showed the presence of a calcified mass in the left articulation between atlas and axis. Myelogram outlined a left sided filling defect at the level of a calcified mass. The tumor was totally removed using operating microscope technique through transcervical approach. This calcified mass was confirmed as the benign osteochondroma by histological examination. The patient became completely symptom-free after the operation.

A case of Cushing's syndrome due to huge adrenocortial multinodular hyperplasia who was shown to be hyperresponsive to ACTH administration, unresponsive to metyrapone administration and resistant to dexamethasone high dose suppression was reported. After two years' duration of his symptoms, the multinodular adrenals weighing 161 g in total were removed by bilateral adrenalectomy which abolished his symptoms. Postoperatively, plasma ACTH rose gradually to above normal levels, suggesting the presence of primary disorder in the hypothalamus-pituitary axis.
