Basic fibroblast growth factor (FGF) protein has been recognized as a potent factor for angiogenesis and as a mitogen. The sites of basic FGF in the mammalian retina have varied from report to report. On the other hand, only the inner segments of the photoreceptor cells have been reported to synthesized basic FGF as revealed by the presence of mRNA for basic FGF by in situ hybridization. To define the sites of basic FGF and its mRNA in the human retina, we immunohistochemically localized basic FGF protein and mRNA for basic FGF by in situ hybridization in normal human retinas. Basic FGF protein was found in the ganglion cell layer, the inner and outer nuclear layers, and the basement membranes of Müller cells (the inner limiting membrane), blood vessels, and the retinal pigment epithelium (Bruch's membrane). The mRNA for basic FGF was found in the cells of the ganglion cell layer, the inner nuclear layer, and the outer nuclear layer, and the inner segments of the photoreceptor cells. These findings suggest that basic FGF in the human retina functions in both an autocrine as well as a paracrine fashion.
A prospective randomized study was conducted to measure the serial thickness of the lower uterine segment (LUS) by transvaginal ultrasonography in a control group of 80 women having no history of uterine surgery and in a study group of 43 women having a history of previous cesarean section (C/S). In the study group, more than 2 mm of thickness of the LUS was considered as good healing and less than 2 mm of thickness as poor healing. After serial sonographic examination, the women with good healing were given trial for labor unless an obstetrical indication for C/S existed. The appearance of the LUS during surgery was compared with antenatal ultrasonographic assessment by direct inspection. Twenty two (79%) of 28 women with a well healed scar had trial labor with the result that 46% had a successful vaginal birth without any uterine rupture of dehiscence. Eight women with poor healing all had elective C/S. Seven women with a 2 mm LUS thickness were individually categorized for delivery mode. Two of those women delivered vaginally. The LUS was found to be thin to translucent in these later two groups. Two mm or less as a criterion for poor healing had the sensitivity and specificity of 86.7% and 100% respectively. The positive predictive value was 100% and the negative predictive value was 86.7%. Ultrasonographic evaluation is effective in predicting the quality of a uterine scar and in differentiating the risk group of probable uterine rupture from the non risk group.
BACKGROUND: We previously estimated the turning point of reflux nephropathy irreversibly deteriorating to end stage renal disease, mainly based on findings in renal biopsy (Eur. Urol., 26: 153-159, 1994). In this study, we aim to evaluate clinical parameters which may be closely associated with progression of reflux nephropathy to end stage renal disease. SUBJECTS AND METHODS: Ninety five patients (84 children and 11 adults; 41 men and 54 women) with renal scar and/or reflux (>/ = grade 3), mean aged 9.4 +/- 9.1 years (3 months-53 years) were followed up for 3.7 years +/- 2.7 (6 months-18 years). Vesicoureteral reflux was bilateral in 64 and unilateral in 31; primary in 85 and secondary in 10 patients. Clinical parameters including body weight, height, blood pressure, 24 hour urinary protein excretion, serum creatinine, 99m Tc-DTPA GFR and 99 m Tc-DMSA uptake were monitored over time. All patients underwent antireflux surgery (with or without other reconstructive surgery) and open renal biopsy. Three patients progressing to end stage renal disease underwent subsequent biopsy. RESULTS: Over-5 year observation period, the prevalence of new scare formation and further extension in scar was significantly higher in the group of renal functional deterioration (35%) than in the group of stable renal function (6.0%). Over the same period DMSA uptake decreased significantly (< 0.05) in the group of scar b (Smellie's classification), suggesting most kidneys of scar b eventually resulting in atrophic kidney (scar c). Proteinuria more than 100 mg/day appeared to be a critical level for predicting irreversible deterioration in renal function. Glomerular hypertrophy was closely related to the increase in urinary protein excretion and serum creatinine, contrary to the decrease in DTPA-GFR. In addition, bilateral renal scar b, glomerular hypertrophy (> 2 SD), proteinuria (> 300 mg/day), low GFR (mean: 49 ml/min), and diastolic hypertension seemed to be implicated in the genesis of ESRD. CONCLUSION: Glomerular damage due to either reflux nephropathy or dysplasia may cause proteinuria. Proteinuria of 100 mg/day was significantly (p < 0.01) associated with 2 SD of glomerular hypertrophy on histology and clinical observation (suggesting hyperfiltration). Thereafter, a rapid increase in proteinuria followed by diastolic hypertension appears be significant for predicting ESRD.
The new compound 3-[2-([1,1':2,1"]-terphenyl-4'yl)ethyl]phenoxyacetic acid (F1070) was synthesized and its effects on platelet aggregation induced by thrombin, thrombin receptor agonist peptide (TRAP), ADP and collagen were evaluated in humans, guinea pigs and rats, and were compared with the effects of he thrombin antagonists argipidine and (D)Phe-Pro-Arg-CH2Cl (FPR). F1070 inhibited the platelet aggregation induced by these agonists and was highly selective in its inhibition of thrombin. F1070 inhibited fibrin formation induced by thrombin, but far less effectively than argipidine. In a guinea pig model of extracorporeal circulation thrombosis, F1070 (10 mg/kg p.o.) significantly inhibited the development of a thrombus. F1070 is thus a key compound that should facilitate the development of new orally active antithrombotic drugs that are specific for thrombin.
To determine the increased production and release of vascular endothelial growth factor (VEGF) from the retina in the eye with non-angiogenic retinal detachment in which relative blood supply disturbance may be present, the concentration of VEGF in subretinal fluid and vitreous fluid collected from the eyes was investigated by enzyme linked immunospecific assay. The average concentration of VEGF was 0.5 +/- 1.1 ng/ml (mean +/- standard deviation) in nine samples of vitreous fluid collected from proliferative retinal detachment, and was 1.2 +/- 1.2 ng/ml in eleven samples of subretinal fluid from rhegmatogenous retinal detachment. The concentration of VEGF in six samples of vitreous fluid from angiogenic diabetic eyes (5.0 +/- 2.8 ng/ml) was significantly higher than in the samples from eyes with retinal detachment. The results suggest that the relative ischemic insult to the detached retina increases the release of VEGF into the vitreous cavity and subretinal space. The increased concentration of VEGF does not induce remarkable angiogenesis since the concentration is lower than the biological threshold, or the effect is modulated by inhibitors.
Ultrastructural localization of vasoactive intestinal polypeptide (VIP) was studied in the chicken pancreas by the immunocytochemical method. VIP-immunoreactive nerve endings were found in contact with acinar cells in the pericapillary space or between acinar cells. They also terminated on epithelial cells of arteriolae. In pancreatic islets, VIP-immunoreactive nerve endings were in contact with B-cells, but not with A- or D-cells. VIP-immunoreactive terminals contained many small clear vesicles which indicate the cholinergic feature. Nerve endings containing many small clear vesicles but not showing VIP-immunoreactivity were also detected in both the exocrine and endocrine pancreas. According to these data and the previous studies concerning the cholinergic and VIPergic innervation of the chicken pancreas, it is suggested that VIP-containing cholinergic nerves innervate B-cells and cholinergic nerves not containing VIP innervate D-cell.
A 75-year-old woman was admitted to our hospital because of dyspnea and fever. A chest roentgenogram obtained on admission showed cardiomegaly. An echocardiogram, a computed tomogram, and a magnetic resonance computed tomogram revealed a pericardial tumor and a large pericardial effusion. A tumor biopsy was done under echocardiographic guidance, and sarcomatous mesothelioma was diagnosed on the basis of histological and immunohistological studies of biopsy specimens. Pericardial fenestration followed by tumor resection gave relief from cardiac tamponade. The postoperative course was good, and the patient was discharged from the hospital on the 19th postoperative day. Four months later the pericardial mesothelioma recurred and the patient died of constrictive pericarditis. Palliative resection was useful in this case because it allowed the patient to resume activities of daily living by relieving the cardiac tamponade.
3-([1:1',2':1"]-3'-Terphenyl)propanol (CAS 186835-06-3, F050) and acetylsalicylic acid (ASA) inhibited platelet aggregation induced by CaCl2, arachidonic acid, collagen, adenosine diphosphate (ADP) and thrombin in guinea pigs, rabbits and rats in vitro. However, F050 had a wider spectrum of actions than ASA. Orally administered F050 inhibited platelet aggregation ex vivo. F050 significantly reduced the thrombus formation in the extracorporeal circulation thrombosis model in guinea pigs. It inhibited erythrocyte hemolysis induced by hypotonic NaCl, while ASA did not. F050, but not ASA, inhibited increases in platelet [CA2+]i caused by thrombin in guinea pigs. F050 is a parent compound that will facilitate the development of an orally active drug for the treatment of thrombotic diseases.
During a physical checkup, a 42-year-old Japanese man with liver dysfunction was diagnosed as having beta-thalassemia. Using molecular biological techniques including PCR, we investigated the chemical basis of the hematological disorder. We found that a frameshift attributable to the insertion of a thymidine into or following the TTT sequence of codon 42 transformed codon 43 (GAG) into a stop codon (TGA). We believe that this mutation has not been previously reported.
OBJECTIVE: This study was conducted to investigate biologic responses of the pulp to single-tooth dento-osseous osteotomy and to explore an appropriate level of transverse bone resection for maintaining biologic activity of the pulp in monkeys. STUDY DESIGN: The levels of transverse bone resection were designed at approximately 10 mm (10 mm group) and 5 mm (5 mm group) above the apices. Histologic and histomorphometric changes were evaluated microscopically, and changes in blood flow in the pulp were also measured in terms of vascular responses to a vasoconstrictor (noradrenaline, NA). RESULTS: Vacuolar degeneration in the pulp chamber associated with less number of the pulp cells were more prominent in the 5 mm group, and the response of blood flow to NA was maintained better in the 10-mm group. CONCLUSION: It is shown in this 8-week experiment that biologic activity of the pulp is much better preserved in the 10-mm group than in the 5-mm group, whereas the pulpal response may become similar if observed for a longer period.
We report a rare case of prolapsed ureterocele in an adult female, which had been identified as a small finger-tip sized orthotopic ureterocele 12 years earlier and recently presented a hard neoplastic mass due to strangulation at the urethra and long-standing inflammation.
The vitreous levels of angiogenin, which is a potent blood vessel-inducing protein, were measured to determine their association with proliferative diabetic retinopathy (PDR). Undiluted vitreous fluid specimens were collected from 30 eyes with PDR at the time of vitrectomy. A sandwich enzyme-liked immunosorbent assay was then used to quantitate the levels of angiogenin. As a control we determined the levels in the specimens from 21 patients with proliferative vitreoretinopathy (PVR) and 4 patients with idiopathic macular epiretinal membrane (IERM). The average angiogenin level in the eyes with PDR was 43.7 ng/ml, and no significant difference was observed among PDR, PVR and their reoperation cases. In the category of IERM, the mean concentration of angiogenin was 2.1 ng/ml, which was significantly lower than that of the PDR and PVR cases. Our study thus demonstrated a significant increase in the vitreous angiogenin levels in eyes with PDR, PVR and those undergoing reoperation for these conditions in comparison to eyes with IERM. We therefore postulated that the elevated angiogenin levels thus reflected a breakdown of the blood-ocular barrier in eyes with PDR and PVR.
To enhance the selective delivery of antitumor drugs into regional lymph nodes and cancerous tissues via a hyaluronate (HA) receptor (CD44), we synthesized HA-mitomycin C complex and HA-epirubicin complex. To investigate the specific distribution of HA into regional lymph nodes and to evaluate the HA receptor on lewis lung carcinoma cells, we also synthesized 14C-labelled HA and fluorescent HA (FR-HA). The metabolic studies of 14C-HA and HA-epirubicin complex were performed in rats. The specific distribution of both compounds to the lymph nodes were observed after sc treatment. Internalization mechanisms of HA into carcinoma cells (lewis lung carcinoma) via HA receptor was investigated using fluorescent HA (FR-HA) in vitro. Internalization of FR-HA following binding to the cell surfaces was observed. HA-Mitomycin C (MMC) exhibited potent anti-metastatic effects against lewis lung carcinoma implanted in mice at an extremely low dose (0.01 mg/kg) whereas free MMC had no effects.
3-(1-Methyl-2-hydroxyethyl)-benzophenone (F025) is a close structural analogue of ketoprofen, in which the carboxylic acid of ketoprofen is reduced to a carbinol moiety. The in vitro effects of F025 on platelet aggregation, cyclooxygenase and hypotonic NaCl-induced erythrocyte hemolysis were examined in comparison with those of ketoprofen. Although the inhibitory activity of F025 on collagen, arachidonic acid and ADP-induced platelet aggregation was between 2 and 20 times that of ketoprofen in healthy volunteers, guinea pigs, rabbits and rats, F025 had only one-tenth of the inhibitory action of ketoprofen on sheep seminal vesicle cyclooxygenase F025, but not ketoprofen, inhibited hypotonic NaCl-induced erythrocyte hemolysis in guinea pigs. The substantial antiplatelet activity of F025 may be due to membrane stabilization as well as to cyclooxygenase inhibition.

To determine the association of a hypoxia-induced, vascular endothelial cell selective growth factor: vascular endothelial growth factor (VEGF) on proliferative diabetic retinopathy (PDR), undiluted vitreous fluids were collected from 10 eyes with PDR at the time of vitrectomy. Vitreous fluids from 10 eyes without PDR were used as control. All samples were electrophoresed with a 14% sodium dodesyl sulfatepolyacrylamide gel, and electrically transferred to a Durapore membrane. Immunoblot detection of VEGF was done by standard western blotting using rabbit polyclonal anti-human VEGF antibody. Positive immunoreaction bands were observed on lanes of 7 samples from PDR, and they were comigrated with recombinant human VEGF. No reactions were observed on lanes of all samples without PDR. The presence of VEGF within the vitreous fluids of eyes with PDR was determined, and suggesting the association of VEGF with PDR.
A 48-year-old female was admitted to our hospital because of pancytopenia and pneumonia in February, 1993. The increase of abnormal promyelocytes with t (15; 17) and PML-RAR mRNA was detected in bone marrow aspirate and a diagnosis of acute promyelocytic leukemia was made. She obtained complete remission after the administration of all-trans retinoic acid (ATRA) and following chemotherapy. Then she received peripheral blood stem cell transplantation in September, 1993. However she noticed a umbilical tumor in June, 1995. Abnormal promyelocytes were demonstrated not only in bone marrow aspirate but also in the umbilical tumor. Because of the poor response to ATRA and development of fever, a side effect of ATRA, G-CSF and prednisolone were administrated together with ATRA. After the combined therapy, umbilical tumor disappeared and she obtained complete remission again. These findings suggest that combined therapy of ATRA and G-CSF is effective to the low responder to ATRA and that combined use of ATRA and prednisolone reduces the side effect of ATRA without diminishing the favorable effect on differentiation.
