[Local administration of recombinant interferon-beta to patients with cancer-associated body cavity fluids].

The effects of local administration of interferon-beta produced by recombinant DNA technology (GKT-beta, Kyowa Hakko) on malignant effusion were studied in cancer patients and evaluated on the basis of the Criteria for the Evaluation of the Clinical Effects of Solid Cancer Chemotherapy (effects on malignant effusion). Out of 13 cases of peritoneal effusion, the effects were evaluated in 9 cases. Four of these cases remained free from ascites for 4 weeks and were evaluated as complete response (CR). Administration of GKT-beta alone was performed in 4 of 7 cases of pleural effusion, and CR was obtained in 1 case each of breast cancer, gastric cancer and malignant mesothelioma. In cases of gastric cancer, CR was obtained in 5 of 10 cases. The above results suggest that GKT-beta is a very effective drug against malignant effusion.

The enhancement of the immune response by pain stimulation in mice. I. The enhancement effect on PFC production via sympathetic nervous system in vivo and in vitro.

Effects of catecholamines and osmotical and physical stimuli on the induction of anti-sheep red blood cells (SRBC) plaque-forming cells (PFC) were investigated in (C57BL/6 X BALB/c)F1 mice in vivo and in vitro. The anti-SRBC PFC from mice immunized with 5 X 10(7) SRBC was markedly increased by daily s.c. injections of epinephrine. The enhancement of PFC by epinephrine was completely blocked by preadministration with propranolol and hexamethonium, but not with phentolamine. The PFC was increased by osmotic and physical stimuli given once a day for 4 days after immunization with SRBC. The enhancement of PFC by these stimuli was completely blocked by preadministration with propranolol and hexamethonium. The enhancement of PFC by physical stimuli was observed in nonimmunized mice when spleen cells from stimulated mice were cultured with SRBC in vitro. In normal mice, the enhancement of PFC was observed 2 hr after one physical stimulation. However, spleen cells from mice given two physical stimuli did not show the enhancement of PFC after treatment with anti-Thy-1.2 antibody and complement, nor after removal of nonadherent cells. Next, the serum obtained from mice 30 to 60 min after a physical stimulation enhanced PFC of normal mice spleen cells in vitro, but the enhancement was abolished by the addition of propranolol. The enhancement of anti-SRBC PFC by s.c. injection of epinephrine suggested that the autonomic nervous system, especially the sympathetic nervous system, was activated by a local stimulus effect of the injection. This enhancement of anti-SRBC PFC appear to be due to the activation of antigen non-specific helper T lymphocytes by the beta-actin of endogenous catecholamines from the adrenal gland.

Inhibition of metabolic response of polymorphonuclear leukocyte by biscoclaurine alkaloids.

Effects of biscoclaurine alkaloids on the various stimulus-responses of PMN, especially on the O2-. generation of PMN, were investigated. Results obtained were: cepharanthine inhibited various metabolic responses of PMN, its biological action probably being due to its membrane modifying action. Inhibition of O2-. generation by cepharanthine was stronger than any other inhibition of metabolic responses of PMN. The inhibitory effect of various biscoclaurine alkaloids on the O2-. generation of PMN was the descending order of tri-, di- and mono-ether type; the coclaurine type showed only a weak effect.

Novel experimental models of human cancer metastasis in nude mice: lung metastasis, intraabdominal carcinomatosis with ascites, and liver metastasis.

The ability of RPMI 4788 cells, a human colon cancer cell line, to produce experimental metastases in the lung, intraperitoneal cavity, and liver was studied in nude mice. Injection of 2 X 10(6) tumor cells into the tail vein of nude mice produced metastatic lung tumors, and an intraportal injection of 5 X 10(6) cells produced metastatic liver tumors. An intraabdominal carcinomatosis with ascites was formed after an i.p. injection of 5 X 10(6) tumor cells. The nude mice with lung metastasis or intraabdominal carcinomatosis always died within a few weeks. Macroscopic observation showed that the number of lung metastatic nodules on day 21 after tumor inoculation was 311.3 +/- 78.2 (mean +/- SD) in BALB/C nude mice, and 187.5 +/- 26.7 in ICR nude mice. In survival experiments, the mice with intraabdominal carcinomatosis showed a mean survival of 29.0 +/- 1.7 (mean +/- SD) days in BALB/C nude mice and 43.6 +/- 6.1 days in ICR nude mice. These novel experimental models of metastases in nude mice produced by injection of RPMI4788 cells had high reproducibility and may be useful not only for the study of the metastatic process but also for testing anticancer drugs.

Analysis of tissue lymphocytes by double fluorescent staining--gastric cancer tissue and regional lymph nodes.

An immunohistochemical study was performed on human lymphocytes in the tissue of gastric cancer, and also in the regional lymph nodes, by double fluorescent staining, using monoclonal antibodies. Leu3a+8+ cells (inducer T cells) which consist about 30 per cent of Leu 3a+ cells were seen in the tissue surrounding the gastric cancer. The other 70 per cent of Leu 3a+ cells were Leu3a+8- cells (helper T cells). In the lymph nodes they were noted in T cell areas in almost the same proportions, while in germinal centers, only Leu3a+8- cells were found. On the other hand, OKT8+Leu15- cells (cytotoxic T cells) were noted in a large number, while OKT8+Leu15+ cells (suppressor T cells) were few. Further, an increase of OKT8+Leu15- cells was seen around gastric cancer or metastatic cancer in lymph nodes. These immunohistochemical findings suggest that cytotoxic T cells are the main component in the tissue of gastric cancers and the regional lymph nodes. Increases in inducer T cells and helper T cells are probably required to induce cytotoxic T cells around the cancer tissue.

Natural killer activity of peripheral blood lymphocytes and its relation to histopathological factors of lung cancer.

This investigation was intended to determine whether the natural killer (NK) activity of peripheral blood lymphocytes (PBL) correlated with the histopathological factor, which is thought to be a result of a balance between tumor aggression and host resistance. The NK activity of PBL from 60 patients with lung cancer was measured by the lysis of 51Cr-labelled K562 target cells. The activity was significantly decreased with advancing stages of the disease, and inversely correlated with increased immunosuppressive substance levels of the serum. Histopathological factors, such as low grade pleural invasion of the tumor and abundant lymphoid cell infiltration around the tumor, were significantly associated with the high NK activity of PBL. These results show that a decrease in NK activity may play a role in identifying those individuals with a greater risk of cancer development.

Synergism between human tumor necrosis factor and human interferon-alpha: effects on cells in culture.

The cytostatic and cytotoxic effects of highly purified natural human tumor necrosis factor (HuTNF-alpha) and natural human interferon-alpha (HuIFN-alpha) on 23 cell lines were studied in vitro. Natural HuTNF-alpha showed cytostatic and cytotoxic effects on PC-9, KHG-2, HT-1197, KG-1 and L-929 cells, and HuIFN-alpha showed both effects on KHG-2 and Daudi cells. A mixture of HuTNF-alpha and HuIFN-alpha (1:1, by unit) showed cytostatic and cytotoxic effects on HuTNF-alpha- or HuIFN-alpha-resistant cell lines such as KB, KATO-III, HEp-2, P-4788, as well as on HuTNF-alpha- or HuIFN-alpha-susceptible cells. Thus, the combined preparation of HuTNF-alpha and HuIFN-alpha expanded the spectrum of sensitive cells. The dosage of the mixed preparation required to produce 50% inhibition of cell growth was less than 20% of that of HuTNF-alpha or HuIFN-alpha alone. These results indicate that the cytostatic and cytotoxic effects of HuTNF-alpha and HuIFN-alpha are synergistically enhanced when they are administered together.

Effects of serum fractions obtained from cancer patients by double-filtration plasmapheresis on tumor growth and metastasis in mice.

We administered serum fractions obtained from cancer patients by double-filtration plasmapheresis (DFPP) to cancer-bearing mice to examine the effects on tumor growth and metastasis. Fraction 1 (whole plasma), fraction 2 (a plasma fraction containing substances with higher particle size), fraction 3 (a plasma fraction containing substances with smaller particle size) and saline were administered intravenously to cancer-bearing mice for 10 days following the inoculation of tumor cells. The tumor growth and metastasis in mice administered fraction 2 was far more rapid than that in the control mice. On the other hand, tumor growth in mice administered fraction 3 was significantly delayed compared with that in mice injected with fraction 2. These results suggest that factors in the higher particle-size fraction of cancer patients' sera promote the growth and the metastasis of tumors in mice, and that DFPP, which remove these factors, is an effective therapy against cancer.

[Study of quantitative evaluation of liver volume and accumulation rate of radioactive agent with single photon emission computed tomography].

The quantitative evaluation on measurement of liver volume and the hepatic accumulation rate of the radioactive agent were studied with respect to body constitution, background activity and splenic consumption rate, using single photon emission computed tomography (SPECT). The optical cut-off level for liver volume determination was proved to be reasonable as 35%. The background less than 5% had no influence to their clinical estimation with the error of less than 1.6%. The estimation of liver volume was mostly influenced by the splenic activity. When the splenic activity was increased two times of the hepatic activity, the liver volume was determined as 4.2% larger. Calculated counts by SPECT was well correlated with the hepatic activity. This study proved that SPECT was useful for the quantitative evaluation of the liver volume and the hepatic accumulation rate.

[A case of alpha-fetoprotein, carcinoembryonic antigen and human chorionic gonadotropin producing renal cell carcinoma].

A 51-year-old woman was admitted with a complaint of a left hypochondralgia and an abdominal mass. Laboratory testing showed a markedly increased alpha-fetoprotein (AFP) level (234,700 ng/ml). The enlargement of the left kidney and the adrenal gland was noted by CT scan and a tumorous thrombus was shown by inferior caval venography. The patient underwent a left nephrectomy, a combined resection of the spleen, tail of the pancreas and the removal of a tumorous caval thrombus. The patients AFP level gradually decreased after surgery. In a histological study, the tumor was diagnosed as a renal cell carcinoma and the presence of AFP, CEA, and HCG in the cancer cells was demonstrate by means of the "PAP" method.

[An immunohistochemical study using a double staining method for regional lymph nodes in gastric cancer].

We have performed a double immunofluorescent staining of two monoclonal antibodies, obtained from gastric cancer patients, that were directed against lymphocyte surface antigens on the regional lymph nodes. Through this method we were able to identify the lymphoid cell subset in detail. As a result of our double staining, most OKT8+ lymphocytes were also found to be Leu 15- lymphocytes and these OKT8+ Leu 15- lymphocytes were regarded as being cytotoxic effector cells or precursors. A few OKT8+ Leu 15+ lymphocytes (suppressor T lymphocytes) were obtained. More Leu 3a+ Leu 8- lymphocytes (helper T lymphocytes) were observed than Leu 3a+ Leu 8- lymphocytes (suppressor inducer T lymphocytes). To investigate whether these T lymphocytes were activated, we used an OKT 9 monoclonal antibody directed against the transferrin receptor and an OKIa1 monoclonal antibody directed against the HLA-DR framework. Such double stained lymphocytes (activated T lymphocytes) were few.
